目的:閉塞性睡眠時無呼吸症候群(obstructive sleep apnea syndrome:OSAS)のリスク評価における日本語版STOP-Bangテストの有用性を検証する.方法: OSASのスクリーニング目的でパルスオキシメトリー検査を受けた内科入院患者を後ろ向きに検討した.結果:受検者144人のうち,3%酸素飽和度低下指数≧10/hrの精査対象者57人に精密ポリグラフィ検査を施行し,17人を中等症,29人を重症のOSASと診断した.受信者動作特性曲線分析の結果,STOP-BangテストのBMIカットオフ値は従来の35 kg/m2よりも30 kg/m2にした場合に良好な診断能を示した.STOP-Bangテスト値3点以上を陽性とした場合,中等症―重症SASを検出する感度は95.7%,特異度は42.9%だった.結論: STOP-Bangテストは簡便にOSASリスクを評価できる.

インターネット上では、従来のマスメディア中心型の環境よりも、情報・ニュースの選択的接触が生じやすく、それによって人びとの意見の極性化が生じ、世論や社会の分断を招くのではないかという懸念が、多くの研究者や評論家から表明されている。本稿では、日本の「ネット右翼」やアメリカの“Alt-Right”に見られるようなネット上の排外主義に着目しつつ、2016年に日本とアメリカで実施したウェブ調査のデータから、ネットでのニュース接触が排外的態度の極性化(二極分化)傾向との連関について検証する。分位点回帰分析の結果、日本ではPCを用いたネットでのニュース接触頻度がユーザの排外的態度の二極化傾向と有意に連関していたが、アメリカではむしろ専ら反排外的な方向のみに変化させることが確認された。このことは、ネットにおける態度・意見の極性化の生起が、社会・政治・文化的コンテクストによって左右されることを示唆している。

After "The Intentional Fallacy" (1946/54) by W. K. Wimsatt Jr. and M. Beardsley, the role of the artist's intention in the interpretation of artwork has been one of the central topics in analytical aesthetics. Recently, this issue has been mainly debated between moderate actual (MAI) and hypothetical intentionalism (HI). In this paper, I demonstrate some difficulties of MAI, comparing it with HI. First, I survey Carroll's version of MAI and point out its main arguments: accessibility to "actual intention" and the reliance on private documents. I examine them and show that the discrimination of MAI from HI is unclear. Additionally, I insist that, against Hans Maes' arguments, MAI has no advantages in interpreting contemporary art, because MAT overlooks the distinction between "semantic" and "categorial" intention, which Levinson draws. Since contemporary art employs diverse materials which have no code to mean something, semantic intention cannot play any role in making such artwork. Instead, categorial intention is the precondition for making it art and tells us how it is to be conceived or approached. Through this examination, I argue that there is no reason to maintain MAT, at least in Carroll's version.

平成29年度は、遠江国井伊谷と彦根藩等の交流を中心に分析を進めると同時に、新たに確認した井伊谷町中井家文書(個人蔵)の調査を中心に実施した。前者については、とくに近世後期の彦根系譜方の動向に着目し、彼らが井伊谷でどのような歴史調査を実施していたのかについて具体的に明らかにしていった。そのなかで、井伊谷だけではなく、周辺の地域(引佐地方を中心とする)の旧家の協力を得て、遠江井伊氏についての調査が進められていった点が確認されたことには一定の意味があった。つまり、こうした旧家のなかには、地元に史料を残すものがあり(東光院・中井家など)、彦根藩側の史料と地元にのこる史料の双方から裏づけることができた。彦根藩の系譜方による由緒向調査が、遠江国井伊谷周辺(旗本近藤氏領)の地域社会に少なからず影響を与えている点は注目される。後者の点については、新たに確認された井伊谷町中井家文書(引佐町史収録分とは別)の目録作成を進めることができた。この史料群は近世中期の史料が大半を占めており、なかには日記などのまとまった記録もみられ、今後の研究の進展が大いに期待されるところである。とくに、これまでほとんど確認されていなかった中井家が井伊谷の本陣をつとめていた際の史料がみつかった点は重要である。中井家は、近世後期になると遠江井伊氏の歴史研究を進めていくが、その背景にどのような要因があったのか、引き続き、この史料群の調査を進めていく予定である。平成29年度は、ひとまず、こうした研究を進めていくうえでの基礎となる目録作成作業を重点的におこなった。

“Economics was primarily an Italian science until the last quarter of the eighteenth century”. Thus, an oracle of Schumpeter has inspired a few Japanese scholars to investigate Italian precursors of modern economic theory, with special regard to F. Galiani. But the cynical Galiani was also, as Ajello points out, a machinist of ‘illuminated’ absolutism by which Queen Maria Carolina corrupted every serious effort of Neapolitan Philosophes and transformed them into a ‘mimic’ of reform.The aim of this paper is to verify, under such circumstances, the Fortuna of the socalled Genovesian tradition (by Di Battista), which had been celebrated by the ‘first’ chair of political economy in the world (1754).In the European-wide circulation of economic thought, Antonio Genovesi had chosen Great Britain as a model of civilized commercial society, though he criticized its tendency toward imperialism and foretold a postcolonial crisis. His Economia civile was an idealization of England in which property rights should be ensured by the equity of justice and everyone's industry can be stimulated by the domestic free trade, so that the development of a balanced national economy might be carefully protected by ‘mercantilist’ policies. Moreover, this idealization looked something like an Arcadia of virtuous gentries and independent free-holders. The reality of Southern Italy, however, did not permit such an eclogue. Therefore, the quest for a missing middle class (ordine mezzano) became the primary theme of the Genovesian tradition.Among Genovesi's followers, young G. Filangieri expected a utopia of ‘American Liberty, ’ while old G. Palmieri, as president of the Supreme Council of Finances, pursued the transformation of feudal nobilities into modern landowners. His scheme, however, resulted in a kind of ‘cynic economy’ that imposed abstinence on the poor people.Why the precocious ‘institutionalization of economics’ was ultimately fruitless (see the tragedy of 1799) remains mysterious. Soon after Genovesi's death in 1769, the Neapolitan Enlightenment had become gloomy, and there we may find in F. Grimaldi another example of exalting the Diogenean lifestyle, which sheltered freemasons and canalized them into the Italian Jacobinism. Regardless, further research is required with regard to the vast economic literature produced by the contemporaries of Genovesi and by the first generation of his disciples: G. B. M. Jannucci, N. Fortunato, F. Villano, F. Longano, M. Torcia, and so on.During the Napoleonic era we finally encounter the solitary exception among the mediocre successors of Genovesian professorship: L. d. S. Cagnazzi was considered to be a pioneer of Smithianism in the Kingdom of Naples and a founder of the Italian school of statistics. However, Cagnazzi's substantive liberalism with regard to the macroeconomy was coexistent with his insistence on the minutest interventionism in the microeconomy, and “the role of a entrouvable [sic] bourgeoisie came to be occupied by a new class of bureaucratic-intellectuals” within such ‘administrativism’ (Salvemini). We therefore cannot easily conclude that there was a significant break nor a seamless continuity with the Genovesian tradition.According to Robertson, who emphasizes the Venturian concept of the Enlightenment ‘above national context’, the Scottish and Neapolitan economists were exemplars “of the achievements of the political economy of Enlightenment.” Free trade and protectionism were two different but complementary alternatives that derived from a common intellectual framework of the cosmopolitan century. However, it must also be considered that the 19th century economic theories were influenced by the international diffusion of a classical political economy. As the honeymoon of cosmopolitanism

After the chytrid fungus was first discovered in Japan, studies have revealed the following facts,1)Chytrid fungus is present naturely in Japan.2)Japanese native amphibians are resistant to chytrid fungus.3)Wild chytrid fungus in Japan has many haplotypes. In other words, this organism has surprising diversity in Japan. The chytrid fungus that originates from the Giant salamander shows a unique genetic cluster of haplotypes.On the basis of these facts, we propose a new hypothesis, i.e., “Chytrid fungus has coevolved with native amphibians as a natural host in East Asia, including Japan.”. In other words, in the long process of the evolution of chytrid fungus and amphibians in East Asia, an equilibrium has formed between the hosts (amphibians) and the pathogen (chytrid fungus) which has continued up to the present. This hypothesis may explain the present state of chytrid fungus in Japan.To prove this hypothesis, it is necessary to survey and analyze the genetic and ecological characteristics of chytrid fungus around the world, and research in East Asia is a priority.

嗅覚障害の治療としてステロイド薬の点鼻療法が一般的に行われているが, 治療が長期にわたる症例も多くその副作用が懸念される. ステロイド薬点鼻療法長期連用に関してその安全性を有用性と比較して検討した報告はない. そこで今回は当科嗅覚味覚外来で同療法を施行した患者を対象にこの比較検討を施行した.0.1%リン酸ベタメタゾンナトリウム液 (リンデロン液®) の点鼻療法を施行した62例中42例 (68%) に点鼻開始後1~2カ月で血清ACTHまたはコルチゾール値の低下が出現したが, 異常な理学的所見や自覚的症状は認められなかった. 点鼻療法を中止した8例は全例1カ月後にそれらの値が正常範囲内に回復した. 一方, 同療法を継続した34例中4例で開始後2~5カ月で自覚的な顔面腫脹感, 顔面の濃毛化というステロイド薬のminor side effectが出現したが, 中止後1カ月ですべての症状が消失した. 同療法のみを3カ月以上継続した23例の治療効果は, 自覚的嗅覚障害度, 基準嗅力検査上ともに統計学的に有意な改善がみられ, 日本鼻科学会嗅覚検査検討委員会制定の嗅覚改善評価法でも78%例で何らかの改善判定が得られた.ステロイド薬点鼻療法の長期連用は軽度で可逆的な副作用を生じ得る. 一方, 嗅覚障害の治療効果は高い. よって同療法は有用な嗅覚障害の治療法であり, 臨床的必要性に応じて十分な注意の下に長期連用することは可能と考えられる.

本論文は、地震と洪水の複合災害を対象として、被害低減対策ごとの被害低減効果の特性について分析し、対策検討上の留意事項を抽出することを目的とする。分析に当たっては、国土交通大臣直轄管理河川を念頭に設定した延長約60kmのモデル河川において、地震・洪水規模、両者の生起間隔、対策ケースを変化させた計120ケースについて直接被害額及び死者数を試算した。本成果を活用することにより複合災害時の被害低減対策についてより具体的に検討できるようになると著者らは考えている。

To be a creative city is now one of the most popular policy goals for urban governments, although the concept is ambiguous and critics are skeptical about its ability to regenerate urban areas. This paper examines the role that this concept plays in urban governance. First, I explain the political economic process called ‘glocalization’, which has enhanced inter-urban competition since the 1970s. Then I describe how urban politics has been transformed, taking on an entrepreneurial form, and how cultural policy is used to reconstruct an urban image to attract investment and human capital. Although rebuilding a city's image is a contested process, the ambiguity of the term ‘creative city’ as a policy issue is utilized to mobilize various political and cultural actors for urban growth.

本研究ではポリマー物性として屈折率 (refractive index, RI) およびガラス転移温度 (glass transition temperature, Tg) を対象にして,モノマーの化学構造から計算される構造記述子 X とそのモノマーを重合して得られるポリマーの物性 y との間で,高分子データベースを用いて物性推定モデルを構築し,構築されたモデルを用いて高い RI かつ高い Tg をもつポリマーを重合できると考えられる新規モノマー構造の設計を行った.モノマー構造の前処理方法・構造記述子・回帰分析手法を検討したところ,RDKit で構造記述子を計算し,support vector regression で回帰モデルを構築したときに,推定性能の良好な RI および Tg 推定モデルが得られた.RI と Tg がともに高いポリマーを達成するモノマーを設計するため,breaking of retrosynthetically interesting chemical substructure (BRICS) により仮想的な化学構造を生成し,モデルの適用範囲により推定値の信頼性を評価した後に,モデルを用いて RI および Tg を推定した.その結果,多様な化学構造が得られ,中には良好な RI および Tg の推定値をもつ化学構造が存在することを確認した.提案手法により複数の目標物性のある高分子材料の開発が促進することを期待する.

Neuerdings habe ich einen Fall von Urethralfremdkörper beobachtet.Krankengeschichte; Ein 15 jähriger Schüler. Familiäre Anamnese zeigt nichts besonders.Seit 10 Jahren bekam der Patient Miktionsschmerz, Harndrang, und Haematurie nach körperlichen Überanstrengungen. In den letzten Zeiten haben sich diese Beschwerden in dem Masse vermehrt, dass er nur tropfenweise urinieren konnte und dass er den Miktionsscnmerz, nicht mehr zu ertragen vermochte, so besuchte er am 15. Febr. 1935 einen Arzt und wurde von ihm katheterisiert, unter der Diagnose von Striktura Urethrae. Bei dem Herausziehen des Katheters aus der Urethra brach die Kathetherspitze ab und blieb in der Urethra zurück.So wurde er zu unserer Klinik geschickt. Allgemeinbefinden; Ein gut gebauter aber etwas anämischer, leidend aussehender Jungling. Guter Puls und normale Atmung, Innere Organe belanglos. Wa. R. (-) M. T. R.3 (++) Murata's R. (-) PIRQUET's Reaktion negativ, Blutsenkungsgeschwindigkeit 46.5m.m Harn: blutig getrübt, enthält reichliche polynucläre Leukocyten, mässig viele Erythrocyten und Epithelzellen, keine Mikroorganismen. Lokalbefund: Die äussere Urethralmündung, Hoden, Hodensack und Nebenhoden sind normal.In der Dammgegend konnte ich einen elastischharten strangartigen Fremdkörper und einenkleinfingerspitzgrossen steinharten Frempkörper palpieren. Deshalb führte ich sogleich Urethrotomia externa unter lokaler Anästhesie aus und aus der Schnittwunde exstirpierte ich ein ca. 8cm langes Stück von Nelaton's katheter und einen grubhöckrigen 1.5cm lang, 1.0cm breit und 0.9gr. schweren Stein.
