<p>ペランパネル(PER)は、AMPA受容体を非競合的に阻害する新規の抗てんかん薬である。日本国内の使用実績の報告は少ない。今回、知的障害者や12歳未満の小児を含む難治性てんかんの33例について、PERの治療効果と副作用を検討した。発作が50%以上減少した症例を「有効」とし、両側性けいれん性発作への進展を含む焦点発作(Fs)と全般発作のうち強直、間代発作(GTCS)に対する有効率を検討した。FsおよびGTCSに対しては50%の症例で有効であった。全体では52%の症例に有効であった。12歳未満でも12歳以上と同等の有効率が得られた。併用薬剤別では、有意差は得られなかったものの、KBrを併用した2症例でともに有効であった。CBZやPHTといったCYP3A4を誘導する薬剤との併用例の有効率はそれぞれ30%、18%と低い傾向があった。副作用の出現率は55%で、情緒・行動面の異常が30%、傾眠・眠気が18%、めまいが15%であった。若年者や知的障害者では情緒・行動面の異常が出やすく、注意を要する。</p>

指紋認証には大きな弱点がある。それを採られる人が感じる抵抗感だ。人は逮捕されれば指紋を採られる。前科者の証だ。それと同じことをするのは何となくイヤ。指紋は生活していればベタベタ残るものだから,どこで照合されるか分からない。 自分の携帯電話,パソコンといった"閉じた"システムなら問題はない。指紋データは手元の機器から出ていかないからだ。

A series of 120 unselected radiographs of the larynx without organic laryngeal disorders was analysed from the standpoint of laryngeal calcification or ossification. The special patterns of laryngeal ossification were described in the lateral X-ray using low kilovoltage technique of the larynx.1) Normal “figure 8” configuration of the ossified thyroid cartilage angle was characterized in men (37/45, 82%) but rare in women (10/75, 13%).2) A horn-like shadow of the ossified arytenoid cartilage was characteristic in women (43/75, 57%) as compared with men (7/45, 16%).3) A crescent-shaped shadow of the ossified top of the cricoid cartilage was observed in 42% of the total cases.These special configurations of the laryngeal ossification were also discussed from the viewpoint of laryngeal functional surgery.

ドット絵は低解像度ディスプレイでは効果的な表現手法であるが,手作業によって作成するために制作時間がかかる.この制作時間短縮の要望と計算機の高性能化によりポリゴンが使われることが多くなった.しかしポリゴンによる表現よりドット絵が有効な場合も多い.制作に手間がかからなければ,ひとつの表現手法として利用範囲も拡大すると考えられる.本研究の目的は,実写画像をもとにドット絵の輪郭線を描画する手法,特に,ドット絵サイズに縮小した輪郭線の描画手法を提案することである.まず,ドット絵の特徴を明らかにするために描画手順を,および実際に描かれたドット絵の輪郭線の接続方向を調査した.輪郭線は幅1pxの直線や曲線で描かれることが多いことが明らかになった.既存の輪郭線描画手法では幅1pxの輪郭線が描画できない.また輪郭線を既存手法で縮小した場合,輪郭線と背景の2色だった画像がグレイスケール画像に不鮮明化したり,2色を保存しても輪郭線の情報が失われたりした.この課題を解決するために実写画像から抽出した輪郭線画像の縮小手法を提案する.この結果,ドット絵に求められる2値化輪郭線画像を描画できた.

Audit failures are often caused by the inability of auditors to maintain independence. The lack of independence is an enemy of systematic and efficient audit operations. How can auditors secure independence? To answer this question, we need to discuss a wide variety of issues, including not only an appropriate organization of auditors, quality control and governance but also the necessity of auditors' rotation and the whereabouts of an authority to select auditor members or determine remuneration for auditors. This paper focuses on audit quality control, which is an essential task in ensuring reliable audit results, and discusses present situations and problems regarding audit quality control systems from the viewpoint of audit firms, JICPA ( Japanese Institute of Certified Public Accountants) and CPAAOB (Certified Public Accountants and Auditing Oversight Board). We also discuss whether or not the size of an audit firms is relevant to its ability to conduct appropriate quality control of audit operations, since we consider it important to clarify such relevance or irrelevance for identifying desirable quality control systems for respective audit firms. We conducted a questionnaire survey on both large audit firms and small and medium-size counterparts to find and analyze the status of their respective quality control systems, and this paper explains the survey results.
