This study investigates the factors that determine the difficulties of a doze test for Japanese learners of English. A total of 196 Japanese university students who were studying English in Japan were chosen for this study. The passage used in this study was consisted of 220 words. Every 7th word from the beginning was deleted and this passage was made into a 31-item doze test. The participants were asked to fill in the blanks with appropriate words, and their responses were scored based on the exact-answer scoring method. The results of this study indicate high reliability estimates (Cronbach α= .78, KR-21= .78) and high criterion-related validity. Correlation coefficient between the doze test and the TOEIC[○!R] scores is .82. Each item is analyzed based on Item Response Theory, and the results show that six items are too difficult to be included in the test and one item is excluded from the analysis because no participant answered it correctly. The article concludes with discussion of the implications of these findings.

マネジドケアの統一した定義は存在しないが,基本的な構造は次の通りである。1)医師-保険者関係:[1] 医療の標準化とモニタリングとインセンティブによる医療への介入,[2]医師・病院のネットワーク化,2)患者-保険者関係:[1] 患者の医療機関選択の制限,[2]患者(保険加入者)の組織化。マネジドケアとは,これらの仕掛けを通じて,医療システムに見られる2種類の非効率性([1]医療需要が社会的に過剰になる非効率,[2] 医療資源を空間的,時間的に最適配分できない非効率)を改善させる制度的イノベーションである。アメリカのマネジドケアのパフォーマンスを見ると,[1] 入院率の低下,[2] 入院期間の低下,[3] 高額な治療や検査の抑制,[4] 予防サービスの増加,[5] 医療の質に対する満足度の低下,[6] 費用に対する満足度の増加,という傾向が見られる。一方,[1] 医療の質の低下,[2] 高リスク者の排除,[3] 良好な医師-患者関係の崩壊という問題点も指摘されている。マネジドケアの概念と手法をわが国の公的医療保険制度に導入することを考える際,[1] エビデンス・べースの標準化を行うための診療情報とコスト情報を有機的に結ぶ情報インフラの整備[2]「標準」のコンセンサス形成のためのシステム作り,[3] 保険者が患者の利益の代理人として行動するための制度的担保の確立,が必要である。これらの課題が解決されないまま拙速な導入が行われれば,マネジドケアの副作用の方が主作用を上回ることも懸念される。

The linear hashing is a well-known dynamic hashing algorithm designed for internal main memory as well as external secondary memory. Traditional analysis of this search algorithm has been proposed under the assumption that all keys are uniformly accessed. In this paper, we present a discrete analysis of the average search cost of the linear dynamic hashing algorithm for internal main memory in consideration of the frequency of access on each key. In the proposed discrete analysis, the number of probes itself is regarded as a random variable and its probability distribution is derived concretely. Furthermore, the evaluate formula derived from the proposed analysis can exactly evaluate the average search cost in conformity with any probability distribution of the frequency of access. The proposed analysis is compared to the traditional one provided that the frequency of access on each key is uniform, and the differences are discussed.

症例は80歳の男性.2001年10月24日から食欲不振と尿濃染を自覚し,近医を受診.CTで肝内胆管の拡張と総胆管内の腫瘤像を認め,当院へ紹介.ERCP,MRCP,PTBDチューブからの胆管造影で中部胆管に境界明瞭な乳頭状の腫瘍を認め,PTCS直視下生検で低分化腺癌と診断.以上より中部胆管の乳頭型胆管癌と診断し胆管切除術・リンパ節郭清を行った.病理組織学的には核/細胞比が高い小型の細胞がシート状,充実性に増殖する腫瘍であり,chromogranin A染色陽性で,small cell carcinoma (pure type)と診断した.術後合併症なく,第19病日に退院したが,術後9か月目の2002年9月,肝転移のため再発死亡した.胆管小細胞癌の報告例は欧米などの報告を含め11例と極めてまれである.しかし,術前の画像診断で乳頭型の胆管癌を認め,生検で低分化腺癌を認めた場合には本症を考慮する必要があると思われた.
