Cytomegalovirus (CMV) remains the most important pathogen following solid organ transplantation and is the major cause of recipient morbidity and mortality during the first 6 months posttransplantation. To prevent CMV infection and/or to prevent symptomatic CMV disease, immunoglobulin (Ig) G including hyperimmune CMV IgG are used alone or in combination with antiviral medications. The CMV IgG titer, however, has a wide range and frequently depends on the company supplying the Ig preparation even if the preparations come from the plasma pool of a national blood donation agency. In the present study, we therefore simultaneously measured and evaluated the CMV IgG titers in various Ig preparations using two common methods: the neutralizing antibody (NT) and enzyme immunoassay (EIA). The CMV IgG titer in the present study indicated different values using both methods among Ig preparations that were made from the plasma pool of a national blood donation agency (about 3.5- or about 1.7-fold difference using the NT or EIA methods, respectively). Furthermore, there were no correlations in the CMV IgG titer between our findings and published data from the manufacturers, or between the two methods tested here. These findings suggest the importance and necessity of a standard method and/or sample for the measurement and assessment of CMV IgG in Ig preparations.

The present study evaluated and compared the oxidative stress status of dogs undergoing laparoscopic or open ovariectomy. Twelve healthy female dogs were divided into two groups according to the type of the surgical procedure, laparoscopic or open ovariectomy. Plasma total oxidant status (TOS), total antioxidant status (TAS) and oxidative stress index (OSI) levels for the evaluation of oxidative stress were determined. Increases in plasma TOS and OSI levels and decreases in TAS levels were observed in both groups after surgery. The TOS level was significantly lower in the laparoscopic ovariectomy group compared with the open surgery group. Laparoscopic ovariectomy is a safe and beneficial surgical alternative to traditional ovariectomy with respect to oxidative stress status in dogs.

Vascular endothelial growth factor (VEGF) was identified in 1980s as a protein that increases vascular permeability and induces endothelial cell-specific mitosis. VEGF plays an important role in angiogenesis during the embryonic stage and in angiogenesis and in increasing vascular permeability during postnatal life, both physiologically and pathologically. Great progress has been made in studies of VEGF, mainly in the field of oncology, and VEGF-targeted therapy has been successfully used to treat patients with cancer. In research related to chronic inflammation, several reports concerning rheumatoid arthritis or retinopathy and VEGF have been published. In the lower respiratory tract, increased levels of VEGF have been detected in biological samples from patients with asthma. However, VEGF has not been studied in detail in upper-airway diseases, such as rhinosinusitis. This review article focuses on VEGF and allergic rhinitis to advance studies of VEGF in chronic inflammation of the upper respiratory tract. VEGF levels in nasal secretions and nasal lavage fluid were higher in perennial allergic rhinitis than in nonallergic rhinosinusitis, after, rather than before, the antigen provocation test. The major VEGF isoforms were confirmed to be VEGF121 and VEGF165 in allergic rhinitis. Expression of VEGF mRNA was higher in serous versus mucous acini. In allergic rhinitis, serous acini produced significant quantities of VEGF, which was hypersecreted after antigen provocation. VEGF seems to play an important role in the pathophysiology of allergic rhinitis. Modulation of VEGF function seems to contribute to the successful treatment of conditions with airway inflammation such as allergic rhinitis.

日本における原発性脳腫瘍の頻度は,人口10万人あた14.1人(男性11.6人,女性16.4人)と報告されており,米国の10万人あたり19.3人という数字とほぼ同じであることが2011年に初めて明らかになった.発生頻度も,(1)髄膜腫(36.8%),(2)神経膠腫(19.5%),(3)下垂体腺腫(17.8%),(4)神経鞘腫(9.9%),(6)中枢神経系悪性リンパ腫(3.6%)と,米国における頻度と同じである.グリオーマの年間の発生頻度は国内ではおよそ4,000〜5,000人程度と推定される.グリオーマは希少癌ではあるが,この40年間にグリオーマを含む悪性脳腫瘍の予後は改善したものの,依然として膠芽腫の治療成績は5年生存率が10%以下で,あらゆる癌の中でも最も予後不良である.予後が不良の原因のーつは使用できる薬剤が少ないことである.国内でのグレード2の神経膠腫に対する標準治療は手術+放射線治療で,グレード3・4の神経膠腫に対しては手術+放射線治療+テモゾロミドの投与であるが,照射の方法,照射量,化学療法プロトコールなどさまざまな治療が国内で行われている.神経膠腫の治療成績を改善するために,脳外科医はこれまでに蓄積されたエビデンスを理解して治療にあたることが必要であり,国内外で行われるさまざまな臨床試験を迅速に行うことも重要である.

自然縮小した髄膜腫3例を報告する.症例は72〜79歳のいずれも高齢の男性で,2例は小脳梗塞で発症,1例はめまいで施行のMRIで無症候性の髄膜腫が傍矢状洞部,中頭蓋窩,嗅窩部に認められた.経時的MRIで4年半,6年,6年半で腫瘍の縮小(それぞれ6.1cm^3から1.1cm^3,7.2cm^3から2.4cm^3,16.0cm^3から4.0cm^3:縮小率82%,53%,75%)が認められた.腫瘍体積はCavalieriの法則Volume=4/3×π×a/2×b/2×c/2(a,b,cは3方向の径)で計測した.これまでに髄膜腫の自然縮小の報告はない.腫瘍縮小の機序は不明であるが,高齢者無症候性髄膜腫では自然縮小する可能性もあり,慎重に経過観察をするべきである.

当院では2010年に,がん化学療法中の入院患者の給食として8日間サイクルの化学療法食を導入し,現在までに258名に提供した。今回,2011年10月~2012年3月の6か月間に化学療法食を提供した23名 (肺癌8名,急性骨髄性白血病5名,悪性リンパ腫3名等) に食事のアンケート調査を行ない,その結果を基に幾つかの改善を行なった。 食べ易さに対する回答では,96%の患者が食べ易いと評価した。食事量については,朝食について29%の患者が少ないと回答し,昼食と夕食については83%の患者がちょうどいいと回答した。食べ難かった料理では,「冷たい茶碗蒸し」と回答した患者が7名と最も多く,その他の料理では魚料理が食べ難い傾向がみられた。 アンケート結果を基に,朝食量を増量すると共に,「冷たい茶碗蒸し」の献立を「温かい茶碗蒸し」に変更した。また,魚の臭いを抑えるために「煮付け」と「蒸魚」の献立を「蒲焼き」と「あんかけ」に変更した。今後も調査を継続し,より多くの患者に対応できる化学療法食を目指したい。

両側の耳硬化症と診断した42歳女性に右アブミ骨手術を行つた. 興味深いことに, すべての耳小骨は固着し, 病理学的に硬化病変が証明された. 初回のアブミ骨手術の際には, ツチ骨頭部・キヌタ骨短脚を上鼓室より剥離し, テフロンピストンをキヌタ骨長脚と卵円窓に留置した. しかし, 聴力の改善がなかつた. 2回目の手術でツチ骨頭部. キヌタ骨を摘出し, テフロンピストンを鼓索神経にかけ卵円窓に留置したところ, 聴力は改善した.
