A paleomagnetic study of 24 lava flows from Pagan Island (18.1°N, 145.7°E) in the Mariana Islands was conducted to determine the geomagnetic paleosecular variation in the Western Pacific. All flows are normally magnetized, and attempts to obtain radiometric ages for these flows were unsuccessful, and paleointensity determinations by the ‘Thellier method’ were unreliable. The mean virtual geomagnetic pole, (88°N, 110°W) is not significantly different from the rotation axis, suggesting that the lavas erupted over a period that spans at least several thousand years. The between-site angular dispersion SB, is used as a measure of the secular variation. SB=6.0° with respect to the mean VGP, and SB=6.2° with respect to the axis of rotation with a 95 percent confidence interval for the latter of 5.2°<SB<7.8°. Possible interpretations of the data include: (1) the sites studied represent an inadequate coverage in time; (2) the Marianas should be included in the Pacific dipole window; (3) there is a belt of average low angular dispersion encircling the earth between (roughly) 0° and 40°N.

In 1934, Stiasny described cubomedusan specimens from Station 279 of the Discovery Expedition 1925–27 in the Gulf of Guinea, West Africa as Tamoya haplonema. He also presented line drawings of the habitus that raised some doubts on the identification of the species. In the collections of the Natural History Museum of London, the described specimens were still available and in good condition due to their storage in formalin. More specimens could be found in other European museums. The formalin made a genetic analysis impossible but as all anatomical structures were excellently preserved, a direct comparison was possible with the structures in Tamoya haplonema and Tamoya ohboya from the Americas. Due to the morphological comparison with the American Tamoya species it could be confirmed that the African Tamoya specimens are an up-to-now undescribed new Tamoya species, Tamoya anacamori sp. nov., described herein.

A 2-day-old Japanese Black male calf that presented with opisthotonos with spastic extension of all four limbs and nystagmus was presented. Evaluation of cranial neurology revealed a horizontal slow nystagmus and absence of menace response in the left eye. Necropsy revealed a mass located between the posterior margin of the cerebrum and anterior margin of the cerebellum, and continuously with the cerebellar lesion. The brainstem was severely compressed by those lesions. Original structures of the cerebellum were mostly replaced by grayish-white and brownish tissues. Those lesions were diagnosed as presumed cerebellar medulloblastoma by histopathological and immunohistochemical examination. As neuron-specific enolase in the cerebrospinal fluid which is a biomarker for neuronal damage was increased compared with healthy calves.

To assess the physiotherapist's exposure to radio frequency electromagnetic fields (RF) leaking from short wave diathermy equipment, we conducted on-site measurements of stray electric and magnetic fields (27.12MHz) close to continuous wave (CW) short wave equipment. The results show that the operator's knees may have the highest exposure level for both electric field (E-field) and magnetic field (H-field) in the normal operating position, i.e., behind the device console. The whole-body E-field exposure normally does not exceed the 1992 IEEE recommended limit during a normal treatment session. On the other hand, the operator's whole-body exposure to H-field was barely below the recommended limit. Our data suggest little chance of immediate harmful effects of RF leakage from the diathermy. Nonetheless, physiotherapists should still be advised to remain at a distance of at least 20cm from the electrodes and cables to avoid possible overexposure.

Long-term combination treatment with lenalidomide and low-dose dexamethasone is important to achieve a curative effect in patients with multiple myeloma (MM). In this study, the plasma concentration of lenalidomide was measured at 3 h after oral administration, when the drug is in the elimination phase and can be easily measured in outpatients, to identify factors that may lead to the discontinuation of this combination therapy. Patients were assigned to continuation or discontinuation of therapy groups, and the baseline characteristics of patients, lenalidomide concentration, and concentration/dose (C/D) ratios reflecting oral clearance were compared between the two groups. The efficacy and severity of adverse events were also compared. The results showed that patients who discontinued or modified treatment had low plasma concentrations of lenalidomide and C/D ratios, indicating high oral clearance of lenalidomide. The estimated creatinine clearance rate was negatively correlated with the C/D ratio. The plasma concentrations of lenalidomide were independent from kidney function and differed significantly among patients. Taken together, the results indicate that low plasma concentrations of lenalidomide and low C/D ratios may lead to discontinuation of combination therapy in patients with MM. This suggests that early measurement of lenalidomide plasma continuation would help to prevent discontinuation of therapy or a delay in modifying the dose of lenalidomide.

[Purpose] The purpose of this study was to identify the effects of trunk stability exercise using proprioceptive neuromuscular facilitation with changes in chair heights on the gait of stroke patients. [Subjects and Methods] The subjects of this study were 11 stroke patients. The intervention method was trunk stability exercise using proprioceptive neuromuscular facilitation with different chair heights (50, 60, and 70 cm). These exercises were performed 5 times per week for 6 weeks. Gait velocity, cadence, stride length, gait cycle, and stance phase duration were used to measure gait function. [Results] Significant changes in gait velocity, cadence, and stride length were observed on the affected side. However, no significant changes in gait cycle and stance phase were observed on the affected side. [Conclusion] These results indicate that trunk stability exercise using proprioceptive neuromuscular facilitation with change in chair heights were effective in improving gait velocity, cadence, and stride length on the affected side. However, in this study, no significant changes were observed in gait cycle and stance phase on the affected side. Therefore, various interventions for stroke patients should be investigated in further studies.

Since its introduction, the reward prediction error (RPE) theory of dopamine has explained a wealth of empirical phenomena, providing a unifying framework for understanding the representation of reward and value in the brain. According to the now canonical theory, reward predictions are represented as a single scalar quantity, which supports learning about the expectation, or mean, of stochastic outcomes. In the present work, we propose a novel account of dopamine-based reinforcement learning. Inspired by recent artificial intelligence research on distributional reinforcement learning, we hypothesized that the brain represents possible future rewards not as a single mean, but instead as a probability distribution, effectively representing multiple future outcomes simultaneously and in parallel. This idea leads immediately to a set of empirical predictions, which we tested using single-unit recordings from mouse ventral tegmental area. Our findings provide strong evidence for a neural realization of distributional reinforcement learning.The RPE theory of dopamine derives from work in the artificial intelligence (AI) field of reinforcement learning (RL). Since the link to neuroscience was first made, however, RL has made substantial advances, revealing factors that radically enhance the effectiveness of RL algorithms. In some cases, the relevant mechanisms invite comparison with neural function, suggesting new hypotheses concerning reward-based learning in the brain. Here, we examine one particularly promising recent development in AI research and investigate its potential neural correlates. Specifically, we consider a computational framework referred to as distributional reinforcement learning.

Objective No research on infective endocarditis (IE) concerning populations of more than 40 patients from all departments of the hospitals they may have visited in Japan has been conducted since 2000. The present study clarified the status quo of IE in a university hospital in Japan. Methods Data of inpatients of Saga University Hospital with definite IE from September 2007 to August 2017 were retrospectively analyzed. Patients Records of inpatients with diagnosed IE admitted to any department were scrutinized; those with "definite IE" according to the modified Duke's criteria comprised the study cohort. Results The study cohort was 74 patients with a median age 66.5 years old. Symptoms within 2 months before the first visit to our hospital included a fever (73.0%), general malaise (33.8%), disturbance of consciousness (24.3%), and dyspnea (24.3%). High-frequency causative microorganisms were Staphylococcus aureus (28.4%), followed by Streptococcus viridans (18.9%) and Enterococcus spp. (6.8%). The most frequently involved valves were the mitral valve (48.6%), followed by the aortic valve (25.7%) and multiple valves (14.9%). Patients without cardiac murmurs accounted for 37.8%, and those without or with only mild valvular disease accounted for 32.4%. The incidence of complications was 93.2%, and high-frequency complications were central nervous system disorder (60.8%), followed by glomerulonephritis (45.9%) and extracranial embolism (36.5%). Conclusion The incidences of IE without cardiac murmurs and IE without or with only minor valvular disease were higher than those values previously reported in 2000 in Japan. When IE is suspected clinically, clinicians must check thoroughly for common complications, even in patients without cardiac murmurs or valvular disease.

This study aimed to investigate the distribution and epidemiological relatedness of methicillin-resistant Staphylococcus aureus (MRSA) isolates from companion dogs, owners, and residential environments of 72 households. Sampling was performed twice from January to June 2018 and a total of 2,592 specimens were collected. The specimens collected from each household were streaked on CHROM agar Staph aureus and the colonies grown on the medium were further identified using a mass spectrometry microbial identification system. Antimicrobial susceptibility testing, Panton-Valentine-Leukocidin (PVL) gene PCR, staphylococcal cassette chromosome mec (SCCmec) typing, Staphylococcus aureus Protein A (spa) typing, pulsed-field gel electrophoresis (PFGE), and multi-locus sequence typing (MLST) were conducted to evaluate the phenotypic and genotypic characteristics of the MRSA isolates. A total of 65 S. aureus strains (2.5%) were isolated and 49 (1.9%) of 65 strains were MRSA displaying cefoxitin-resistance with mecA carriage. MRSA strains were isolated from dogs (n=6, 9.2%), owners (n=27, 41.5%), and residential environments (n=16, 24.6%), respectively. Overall prevalence of non-duplicated MRSA was 16.7% (12/72 households) at household level. ST72-SCCmec IVc MRSA clones predominantly appeared in MRSA-positive families. Furthermore, PFGE analyses showed that ST72-SCCmec IVc-t324 is shared between dog owners and dogs. To our knowledge, this is the first study to report the sharing of ST72 MRSA between dogs and their owners.
