新しい原理にもとづく身体動作支援ツールも射程に捉え,全身に張力を伝播する機能をどのように拡張することで,身体のある部分から身体全体へ,身体全体から部分へという関係に作用して,運動機能の向上へつなげることができるのかという問題に取り組む.そこで,身体内部で生じている筋膜の作用を身体表面に外化し,その機能を操作・計測可能とする独自の方法を考案する.そして,①実験用張力伝播スーツの開発方法を明らかにし,②張力伝播が効果的な運動の抽出とそのメカニズムの解明を行い,その成果に基づいて,③運動用張力伝播スーツの開発と効果検証を行う.

Adult autosomal dominant polycystic kidney disease (ADPKD) is a common hereditary disorder responsible for 8% to 10% of cases of end-stage chronic renal failure. The extrarenal complications of ADPKD kidney include cardiovascular disorders (such as mitral valve prolapse syndrome, bicuspid aortic valve, aortic coarctation, aortic aneurysm, and aortic dissection), intracranial aneurysms, diverticulum of colon, hepatic cysts, inguinal hernia, and urinary calculus. Acute aortic dissection (AAD) associated with ADPKD is life-threatening. There are few reports of ADPKD with AAD. To the best of our knowledge, only 9 cases have been found, including at autopsy, in Japan. We have treated 121 patients with AAD in the last 5 years, and 3 of these 121 patients also had ADPKD in the intensive care unit of Nippon Medical School. We report clinical features of cases of ADPKD with AAD on the basis of 12 cases, including 9 previously reported cases and our 3 cases. AAD requires urgent antihypertensive therapy, which often aggravates renal function. Priority should be given to antihypertensive therapy in emergent cases. In fact, 2 of our patients required hemodialysis owing to antihypertensive therapy for AAD. Close cooperation between cardiologists and nephrologists is essential.

音楽アウトリーチ活動の有効性は実施する音楽家の手法、資質に依存される。クラシック音楽家の養成過程を検証することで、社会情勢の変化に即した音楽家養成と、地域文化振興に有意義な音楽家の必要性が明らかになる。またアウトリーチ活動を媒体とし大学と地域の連携は音楽家、大学、地域の再生に繋がり、地域文化振興に有益である。

We experienced a serious interaction between miconazole gel and warfarin. The patient had just undergone an aortic and mitral value replacement and thus was medicated with warfarin at a dosage according to the International Normalized Ratio (INR) of about 2.0-2.5. He was prescribed miconazole gel 400 mg/day for thrush induced by steroid for asthma. About two weeks later, he showed bleeding in the mouth and underwent a medical examination. His INR was higher than the upper of measurement limit and so he was immediately hospitalized. Vitamin K was given, miconazole gel and warfarin ware stopped. Two weeks later, his INR was less than 3.0 and thus the warfarin was restarted. But his warfarin dose was less than one third of dosethat had been given before hospitalization.The interaction between miconazole gel and warfarin that we experienced by our patient while was as serious as the reports from abroad.
