The positive relationship between volume and outcome in hepatobiliary surgery has been demonstrated for many years. As for other complex surgical procedures, both improved short- and long-term outcomes have been associated with a higher volume of procedures. However, whether the centralization of complex hepatobiliary procedures makes full sense because it should be associated with higher quality of care, as reported in the literature, precise criteria on what to centralize, where to centralize, and who should be entitled to perform complex procedures are still missing. Indeed, despite the generalized consensus on centralization in hepatobiliary surgery, this topic remains very complex because many determinants are involved in such a centralization process, of which some of them cannot be easily controlled. In the context of different health systems worldwide, such as national health systems and private insurance, there are different stakeholders that demand different needs: politicians, patients, surgeons, institutions and medical associations do not always have the same needs. Starting from a review of the literature on centralization in hepatobiliary surgery, we will propose some guidelines that, while not data-driven due to low evidence in the literature, will be based on good clinical practice.

In order to construct three-dimensional shape models of large-scale architectural structures using a laser range finder, a number of range images are normally taken from various viewpoints, and these images are aligned using post-processing procedures such as the ICP algorithm. However, in general, before applying the ICP algorithm, these range images must be registered to roughly correct positions by a human operator in order to converge to precise positions. In addition, range images must be made to sufficiently overlap each other by taking dense images from close viewpoints. On the other hand, if the positions of the laser range finder at viewpoints can be identified pre-cisely, local range images can be directly converted to the global coordinate system through a simple transformation calculation. The present paper proposes a new measurement system for large-scale architectural structures using a group of multiple robots and an on-board laser range finder. Each measurement position is identified by a highly precise positioning technique called Cooperative Positioning System (CPS), which utilizes the characteristics of the multiple-robot system. The proposed system can construct 3D shapes of large-scale architectural structures without any post-processing procedure such as the ICP algorithm or dense range measurements. Measurement experiments in unknown and large indoor/outdoor environments are successfully carried out using the newly developed measurement system consisting of three mobile robots named CPS-V.

PAX6とは? 昆虫・両生類・爬虫類・魚類・鳥類・そしてヒトを含む哺乳類に至るまで,地球上に存在する多くの生物が類似した機能を持つ脳・感覚器・内臓・肢体を有する。近年になって,それらの形成に関与する遺伝子の多くが種の間で高度に保存されていることが解明されている。 1992年,PAX6遺伝子変異が先天無虹彩症の発生に関与することがグレイサーら1)によって報告された。マウスの小眼球症の発生にもかかわるこの遺伝子はウニなどの下等生物からからヒトに至るまでさまざまな種の間でその塩基配列が保存されており2),その発見以来,発生学において最も注目される遺伝子の1つとしてさまざまな研究の対象となっている。

目的:院外心肺停止症例における声門上気道デバイス使用下での呼気終末二酸化炭素(end-tidal CO2,以下EtCO2)測定の臨床的意義を明らかにする。対象と方法:2011年10月1日から2012年9月30日までの1年間,新潟市における院外心肺停止症例でラリンゲルチューブ(laryngel tube,以下LT)が挿入された例を対象とした。救急車内収容時にEtCO2の測定を行い,測定時の心電図波形,自己心拍再開の有無などについて前方視的に検討した。結果:院外心肺停止症例数は869例,うちLT挿入例でEtCO2の測定を実施したのは160例。全測定例のEtCO2値(mmHg)は中央値で19,測定時の心電図波形別ではVF・PEA 群(n=35);25,Asystole群(n=125);17と,VF・PEA群で有意に高値であった(p<0.01)。自己心拍再開群(n=8)では53で,非自己心拍再開群(n=152)の18に比べて有意に高値であった(p<0.01)。結論:声門上気道デバイス使用下の院外心肺停止例においてEtCO2値は,ROSCの有無,心電図波形の有無と関連していた。声門上気道デバイス下でもEtCO2値は病態を反映し,EtCO2値が高値を示す場合は,ROSCの可能性が高いことが示唆された。

The purpose of this study was to profile cytokine storms (cytokine release syndrome) in the LPS-induced disseminated intravascular coagulation (DIC)-cynomolgus monkey model by measuring changes in 22 cytokines using Luminex. In this study, increases were noted in 20 cytokines, excluding IL-4 and IL-17A. Specifically, IL-6, IL-8, G-CSF and TNF-α, pro-inflammatory cytokines, and IL-10, an anti-inflammatory cytokine, as well as MCP-1, markedly increased by 10,000 pg/mL or more. In addition to the marked increases in the pro-inflammatory cytokines IL-6 and G-CSF, the concentrations of IL-5, IL-18, IFN-γ, VEGF and IL-15 increased continuously. Also, in addition to the marked increases in the pro-inflammatory cytokine IL-8 as well as in MCP-1, the concentrations of IL-1ra, IL-2, IL-1β, IL-12/23 (p40), GM-CSF and TGF-α gradually decreased after initially increasing. On the other hand, in addition to the marked increases in the pro-inflammatory cytokine TNF-α and anti-inflammatory cytokine IL-10, MIP-1β and MIP-1α transiently increased and then rapidly disappeared from serum. IL-13 increased at 6 hr after administration only. Since the behavior of cytokines in this monkey model was similar to those noted in DIC in humans, this model will be useful for evaluating the efficacy of anti-DIC drugs. In addition, this model will also be useful for assessing the risk of cytokine storm development, which is a serious adverse effect of certain types of antibody drugs and CAR-T cell-based therapies.

1999年の台風18号は山口県の周防灘沿岸に甚大な高潮被害を及ぼした. 山口県ではこれを契機に護岸高の設計基準の見直しを行なった. この中で, 過去に山口県に大きな被害を及ぼした台風を対象に, 進行コース, 気圧深度, 潮位偏差の観測結果から統計的評価および沿岸部の地形や土地利用などの地域特性を含めた検討を行い, 山口県の高潮防災の基準となるモデル台風としてT9918号を選定した. そして, これを基に周防灘における新たな防護基準を定めた. この結果, 設計潮位については最大90cmの嵩上げが必要との結論を得た. 本論文は, 筆者らが行ったこの検討の内容についてまとめたものである.
