甲状腺乳頭癌と濾胞癌について, 索状, 充実性, ないしは硬性の低分化構造の有無およびその量より, 分化癌, 低分化構造微少混在癌, 低分化癌に再分類し, その臨床的意義について検討を加えた.乳頭癌において性別, 年齢別, 病期別に生存率を比較すると, 45才以上, 病期III・IVで分化癌に比して微少混在癌の生存率が低値であつたが, 両者には有意な差異は認められなかつた.一方, 低分化癌は乳頭癌と濾胞癌を併せても2例と少数であるが, 2例とも再発から死亡に至つており, これらの点より,「いわゆる低分化癌」の分類には低分化構造の有無のみならず, 量的な問題を含めてさらに検討する必要があると思われた.

Background: The global ROCKET AF study evaluated once-daily rivaroxaban vs. warfarin for stroke and systemic embolism prevention in patients with atrial fibrillation (AF). A separate trial, J-ROCKET AF, compared the safety of a Japan-specific rivaroxaban dose with warfarin administered according to Japanese guidelines in Japanese patients with AF. Methods and Results: J-ROCKET AF was a prospective, randomized, double-blind, phase III trial. Patients (n=1,280) with non-valvular AF at increased risk for stroke were randomized to receive 15mg once-daily rivaroxaban or warfarin dose-adjusted according to Japanese guidelines. The primary objective was to determine non-inferiority of rivaroxaban against warfarin for the principal safety outcome of major and non-major clinically relevant bleeding, in the on-treatment safety population. The primary efficacy endpoint was the composite of stroke and systemic embolism. Non-inferiority of rivaroxaban to warfarin was confirmed; the rate of the principal safety outcome was 18.04% per year in rivaroxaban-treated patients and 16.42% per year in warfarin-treated patients (hazard ratio [HR] 1.11; 95% confidence interval 0.87–1.42; P<0.001 [non-inferiority]). Intracranial hemorrhage rates were 0.8% with rivaroxaban and 1.6% with warfarin. There was a strong trend for a reduction in the rate of stroke/systemic embolism with rivaroxaban vs. warfarin (HR, 0.49; P=0.050). Conclusions: J-ROCKET AF demonstrated the safety of a Japan-specific rivaroxaban dose and supports bridging the global ROCKET AF results into Japanese clinical practice.

学校教育法施行規則等の省令改正を背景に,研究機関毎での研究者総覧構築のニーズが高まっている。研究者向けサイエンス基盤サービスResearchmapには,外部システムに対してデータ提供する機能が用意されている。この機能を利用することで,各研究機関は機関毎の研究者総覧を低コストかつ,短期間で公開することが可能になった。また,研究者は自身の研究活動における情報を高品質でかつ,長期間にわたって安定的に公開することが可能になった。本論文では,同機能を使った研究機関の研究者総覧をクラウド上に構築した際のメリットを提示する。

Purpose: Despite robust empirical and theoretical evidence for higher rates of suicide among lesbian, gay, and bisexual (LGB) youths, little is known about the relationship between suicide and sexual orientation among Asian youths. This study examined differences in prevalence of suicidal ideation and suicide attempts between LGB and heterosexual youths in the cities of Hanoi, Shanghai, and Taipei, China.Methods: The data are from a community-based multi-centre cross-sectional study conducted from 2006 to 2007, with a sample of 17 016 youths aged 15–24 years from Hanoi, Shanghai, and Taipei. Chi-square test and logistic regression were used to evaluate correlates of suicidal ideation and suicide attempts.Results: The overall prevalence of suicidal ideation and suicide attempts in the preceding 12 months in LGB youths were both higher than in heterosexual youth (12.8% vs. 8.1% and 4.0% vs. 2.4%, respectively). Stratified by city, the prevalence of suicidal ideation was lowest in Hanoi (2.2%), followed by Shanghai (8.0%) and Taipei (17.0%). Similar trends were observed in the prevalence of suicide attempts, which was lowest in Hanoi (0.3%), followed by Shanghai (1.2%) and Taipei (2.5%). Of note, however, multivariate logistic regression results revealed that LGB youth were at a higher risk for suicidal ideation than heterosexual youth only in Taipei (odds ratio 1.65).Conclusions: Suicidality is common among Asian youth, with higher prevalence observed in urbanized cities. LGB youths are at greater risk of suicidal ideation than their heterosexual counterparts in Taipei than in the other two examined cities.

Nelle battaglie campali al tempo dell'Ariosto, mentre-come osservava anche Machiavelli-l'artiglieria pesante aveva un ruolo secondario, gli archibugieri spagnoli cominciavano a costituire una minaccia seria, non solo per la cavalleria pesante francese, ma anche per i quadrati di picchieri svizzeri, fino allora considerati invincibili. Questo fatto era di perticolare interesse anche perche, con la sconfitta dei francesi a Pavia (1525), avvenuta 4 mesi prima del ritorno dell'Ariosto dalla Garfagnana, la diplomazia ferrarese era entrata in un periodo difficile, caratterizzato da una instabilita internazionale, che si concluse nel tragico Sacco di Roma (1527) e nelle successive guerre della Lega di Cognac. L' analisi attenta delle tracce di correzioni apportate dall'Ariosto sul Fascicolo I dei Frammenti Autografi (Canto IX) ci induce a credere che il poeta, nell' inventare la storia dell'archibugio di Cimosco, si sia ispirato all'uso di questa arma da parte degli spagnoli in una serie di battaglie contro i francesi, alleati tradizionali degli Estensi. Questa osservazione spiega bene anche la metafora della polvere da sparo usata per descrivere il comportamento di Orlando che abbatte Cimosco (IX, 78). Inoltre il processo di perfezionamento del IX Canto e il successivo ritorno sull' argomento-con la famosa invettiva contro le armi da fuoco dell'XI Canto-lasciano trasparire le tortuose vicende della diplomazia ferrarese fino all'abbandono definitivo della linea filo-francese, con la Pace di Cambrai (1529) e l'incoronazione imperiale di Carlo V (1530) Nella "invettiva" (erroneamente considerata da alcuni studiosi una coraggiosa critica del poeta al proprio signore, che si vantava della sua potente artigliereia) si puo, quindi, rilevare un forte momento creativo dell'Ariosto, insieme all'interesse per gli avvenimenti storici del suo tempo.

この記事は,ここ数年のオープンアクセスの動向について詳述するもので,前後編に分かれる。この前編では,はじめにオープンアクセスの定義と歴史について,特にゴールド・オープンアクセスとグリーン・オープンアクセスについて簡単に説明した。近年研究助成機関と各国政府のオープンアクセス志向が強まり,米国・英国など主要国で助成研究成果論文の無料公開が義務付けられている。なかでも,2013年2月の米国OSTPのメモ,2012年6月の英国のFinch Reportとその影響,ECの動向などについて解説した。さらに,米国では,この義務化に対応するための出版社の取り組みCHORUS,図書館の取り組みSHAREなどの動きがある。 CHORUSはCrossRefのFundRefに支えられている。こうした,義務化を支えるインフラストラクチャーについて述べた。
