在宅療養における老年糖尿病患者の家族に対し,食品摂取バランスに着目し,できるだけ負担感が少なく,視覚に訴える食事指導方法として 「栄養バランス表」 を用いた食事指導を試みた。 より効果的なフィードバック方法として 「ひとことメモ」 欄を設け,キーパーソンである家族に繰り返し説明し,励ましと助言を与え,食事療法継続の意欲を継続させることにも心掛けた。 その結果,血糖コントロールに有効性のある可能性が示唆された。

22,000 years ago, about 100 km^3 of magma erupted from the northrn end of Kagoshima Bay, southern Kyushu. The eruption produced 5 units of pyroclastic deposits; (1) 98 km^3 of airfall pumice (Osumi pumicde fall, OS), (2) 13 km^3 of oxidized, fine-grained Tsumaya pyroclastic flow (TSU), (3) Kamewarizaka breccia (KM) of the new vent-opening and enlargement stage, (4) 250 km^3 of Ito pyroclastic flow (ITO) at the climactic stage, (5) >50 km^3 of co-ignimbrite ash fall (AT ash). Phenocryst mineral assemblage throughout the whole sequence is ubiquitously plag+qtz+opx(Mg#45-60) +mt+il. One exceptional sample (ITO 11c) carries Fe-rich oliv (Fo 26-28) and cpx beside other phases. Fifty-five new XRF analyses of 10 major and 15 trace elements show that the majority of the erupted magma consisted of a remarkably homogeneous, high-silica rhyolite with SiO_2 74-76.5%(H_2O free and 100% normalized). The maximum fluctuation found both in major and trace elements is ±40%. These variations can be explained by the crystal-liquid separation near the roof of the magma reservoir. Mt-il temperatures and opx-mt-qtz pressures show narrow ranges, i e., 770±20℃ and 3-5 kb, respectively. Although the sample ITO 11c shows similar temperature, its calculated pressure is close to 0 kb. The bulk and mineral chemistry and the temperature-pressure estimation suggest that the magma reservoir was not distinctly zoned but was very homogeneous throughout prior to eruption.

The influence of photoirradiation conditions on dentin bond durability and interfacial characteristics of universal adhesives was investigated. Universal adhesives were applied to the dentin surfaces and photoirradiated with 100 mW/cm2 for 40 s, 200 mW/cm2 for 20 s, and 400 mW/cm2 for 10 s. A resin composite was bonded to dentin to determine shear bond strength after 24 h water storage and 30,000 thermal cycles, and water contact angle of cured adhesive were measured by the sessile drop method. Greater dentin bond strengths after 24 h water storage and 30,000 thermal cycles were achieved under these conditions at light intensity exceeding 200 mW/cm2. Universal adhesives photoirradiated above 200 mW/cm2 exhibited significantly higher water contact angles than those at 100 mW/cm2. The results of this study suggested that the photoirradiation conditions affect the dentin bond durability and interfacial characteristics of universal adhesives even at the same total energy.

Periodontal disease is caused by inflammation induced by Porphyromonas gingivalis (P.g.) lipopolysaccharide (LPS) and involves expression of proinflammatory cytokines such as interleukin (IL)-1, IL-6, tumor necrosis factor-α, and receptor activator of nuclear factor kappa B ligand (RANKL), which are implicated in bone resorption. Low-intensity pulsed ultrasound (LIPUS) is commonly used in the treatment of bone fracture. However, the mechanisms by which LIPUS inhibits LPS-induced inflammatory cytokines are poorly understood. Therefore, we investigated the effects of LIPUS on LPS-induced expression of the proinflammatory cytokines IL-6 and RANKL. MC3T3-E1 cells were incubated in the presence or absence of P.g. LPS and then stimulated with LIPUS for 30 min/day for a maximum of 14 days. LPS increased mRNA and protein expressions of IL-6 and RANKL on day 14. In addition, mRNA expression of COX-2 LPS was higher after 3 and 7 days of LIPUS treatment. PGE2 was induced by LPS after 7 and 14 days of culture. LIPUS suppressed all stimulatory effects of LPS. These results suggest that LIPUS inhibits LPS-induced expression of inflammation cytokines by suppressing PGE2 production and might thus have potential applications in the treatment of periodontitis.

Secretory leukocyte protease inhibitor (SLPI) is a serine protease inhibitor whose expression level is positively correlated with tumor aggressiveness and metastatic potential. However, the mechanism underlying SLPI-induced enhancement of malignant phenotype is not completely understood. The malignancy of cancer cells is highly dependent on cell migration activity. Our previous study revealed that gingival carcinoma Ca9-22 cells, but not colorectal adenocarcinoma HT-29 cells, expressed SLPI. Therefore, we investigated the migration activity of these two cell types to understand the nature of SLPI-mediated tumor aggressiveness and metastatic potential. In vitro wound healing assay indicated that HT-29 cells and SLPI-deleted Ca9-22 cells showed lower migration activity than wild-type Ca9-22 cells, suggesting that SLPI-induced cell migration plays an important role in tumor aggressiveness and metastatic potential. In addition, our gene expression profiling study based on microarray data for the three cell types identified a number of candidates, including LCP1 and GLI, that could be key molecules in the mechanism of SLPI-induced cell migration.

Previous finite element analyses of peri-implant stress assumed a bone-implant contact (BIC) ratio of 100%, even though the BIC ratio is known to be approximately 50% or less. However, the recent development of ultraviolet treatment of titanium immediately before use, known as photofunctionalization, significantly increased the BIC ratio, to 98.2%. We used a unique finite element analysis model that enabled us to examine the effects of different BIC ratios on peri-implant stress. A three-dimensional model was constructed under conditions of vertical or oblique loading, an implant diameter of 3.3, 3.75, or 5.0 mm, and a BIC ratio of 53.0% or 98.2%. Photofunctionalization and larger implant diameters were associated with reduced stress on surrounding tissues. Under vertical loading, photofunctionalization had a greater effect than increased implant diameter on stress reduction. Under oblique loading, increased implant diameter had a greater effect than photofunctionalization on stress reduction.

目的 1998年にたばこ特別税が創設されて以来,2003年,2006年,2010年と過去 3 回のたばこ税•価格の引き上げが実施された。我が国におけるたばこ販売数量および販売代金に関する統計データの年次推移を用いて,過去のたばこ税•価格引き上げの影響を評価する。方法 (社)日本たばこ協会による紙巻たばこ統計データより,平成 2 年~平成22年度(1990~2010年)の年度別販売実績(数量および代金)をそれぞれ,Joinpoint Regression Model に適用し,年次推移を分析した。また,過去三回のたばこ税•価格引き上げの影響を平野らの方法を用いて,たばこ価格引き上げ前の販売数量の減少(税•価格引き上げ以外の要因による減少)を考慮した上で,価格引き上げによる販売数量減少効果を推定した。結果 Joinpoint Regression Model により,1998年度以降たばこ販売数量は減少に転じ,2005年度以降は年率平均 5%で減少傾向にあることがわかった。また,2003年度,2006年度,2010年度のたばこ税•価格引き上げ年度における減少効果はそれぞれ−2.4%,−2.9%,−10.1%(震災影響の補正後)であり,価格弾力性はそれぞれ−0.30,−0.27,−0.28(同補正後)であった。2010年度の大幅値上げ時に販売数量の減少効果がもっとも大きくなった。一方,価格弾力性は2003年度,2006年度とほぼ同レベルで,税•価格を大幅にあげても販売代金および税収への影響は小さいことが示唆された。結論 2010年度におけるたばこ税•価格の大幅引き上げは,たばこ販売数量を大きく減少させたが,価格弾力性は2003年度,2006年度とさほど変わらなかった。今後我が国における喫煙の被害を減少させるためにも,さらに大幅なたばこ価格の引き上げが必要であることが示唆された。

This paper studies the condition of reducing knock intensity which is the pressure oscillation initiated by auto-ignition of the end gas. The knock intensity is thought to be decreased by suppressing the reaction rate of auto-ignition. In this study, the effect of the mixture dilution which decreases the reaction rate on the knock intensity was investigated by using a spark ignition engine. In the case of low dilution, knock was observed when the auto-ignition of the end gas occurred. When the dilution ratio was over 30%, there was the condition that the knock did not occur even if the end gas auto-ignited. The combustion with low knock intensity was observed in either case that dilution gas was inert gas or air. The knock intensity was shown as a function of the maximum temperature and the maximum pressure which affected the reaction rate although the knock intensity was affected by the composition of the mixture, load, ignition timing, auto-ignition timing of the end gas and heat release quantity of the auto-ignited mixture.
