新型コロナウイルス感染症(COVID-19)の流行に伴い,当院PICUでは面会制限と,その対策として遠隔面会を実施した。この影響について質問紙調査を実施し,PICU看護師60名中40名(回収率67%),患者家族51名中25名(回収率49%)から回答を得た。約90%の回答者が面会制限を肯定的に捉えていたが,家族は子どもと会えない不安を,看護師は看護ケアの不十分さを感じていた。遠隔面会は,視覚情報が得られたことや,普段は行えない両親以外の面会ができたことなどから有用であった。一方で,視覚情報の提示だけでは家族の不安軽減には不十分であり,さらなる工夫が必要と考えられた。

肥満は,がん,糖尿病,循環器疾患などの多くの生活習慣病のリスクファクターであることが知られている.しかしながら,世界規模で肥満人口は増加の一途をたどっており,肥満人口の最も多い米国では,成人の3人に1人が肥満(BMI≧30:国際基準)と言われている.我が国でも,成人男性の約3割,成人女性の約2割が肥満(BMI≧25:日本肥満学会基準)に該当し,健康寿命の延伸や生活習慣病の発症ならびに重症化の予防を目指す上で,肥満人口の減少は大きな課題の1つとなっている.肥満病態の形成に最も密接に関わるのが食習慣であり,それゆえ,食事を通じた肥満の予防・改善を目指した研究が盛んに行われている.ローズマリー(Rosmarinus officinalis L.)はシソ科の多年草であり,香辛料やハーブとして用いられているほか,抽出エキスは食品添加物や記憶力を高めるサプリメントとして利用されている.本稿では,Zhaoらによるローズマリーの抗肥満作用とその活性成分に関する研究を紹介する.なお、本稿は下記の文献に基づいて、その研究成果を紹介するものである。1) Zhao Y. et al., J. Advertising calendars were first created at the end of the 19th century and became phenomenally popular in the 1920s. Market players in emerging industries including tobacco, pharmaceuticals, cosmetics, insurance and food extensively used them to expand their sales channels. Illustrations with the elements of advertising, art and a calendar were extremely well received and even spread to Chinese communities overseas. British American Tobacco Co. tried to take every opportunity to force its major competitor Nanyang Brothers Tobacco Co, Ltd. out of the Chinese market and dominate it. To this end, the former invested an enormous amount of money. In the 1920s and 1930s, the two were in an intense advertising war, desperately trying to outperform each other. Amid this competition, illustrators of advertising calendars did their best to draw consumer attention and created innovative images of modern girls. Image advertisements are different from other types in that they mainly consist of illustrations, not text. Thus, illustrators made the most of their creativity to attract consumers. Following the trend of the time, they came up with illustrations of various types of women - playing a sport, smoking, posing sexily, and so on. The images express illustratorsʼ artistic creativity and what companies wanted in advertisements to win a competition. This suggests that images of modern girls were fully commercialized. Advertising calendars have not really been studied. Even though visual materials have been attracting the interest of scholars lately, most studies are on art history, visual culture, modernity in Chinese society, the image of Chinese women, and the identity of women. This paper will discuss how the influence of the competition in the tobacco industry on the development of advertising calendars affected the creation of various images of modern girls, based on 284 advertising calendars for tobacco companies collected through different routes.

長期に渡る対策が功を奏し、手術室での気道トラブルは減少した。しかし、手術室外においては未だ課題が多い。 そこで、気管挿管をめぐる最近の文献を調査し、現状と動向を概説する。集中治療室での気管挿管には、MACOCHA スコアや気管挿管バンドルの使用が勧められている。呼吸不全患者の挿管前の酸素化(前酸素化)には非侵襲的陽圧 換気が有効で、挿管操作中は無呼吸酸素化が推奨される。ビデオ喉頭鏡は声門部の視認性に優れ、小型化、低価格化 により普及しているが、各医療現場では適正使用に向けた教育が重要である。上気道エコーによる気管チューブの位 置確認は感度、特異度共に高く、習得も容易なため普及が望まれる。困難気道管理ガイドラインの登場により、アル ゴリズムに沿った気道管理が実施されている。医療従事者は、“cannot intubate and cannot oxygenate”(CICO)シナリ オを念頭に置き、的確に対処できなければならない。

近年,日本においてスズメ <i>Passer montanus</i> の個体数が減少している可能性が指摘されている.その減少要因について,いくつかの仮説が提示されているが,何が原因かわかっていないのが現状である.減少原因のひとつとして都市環境において,スズメの繁殖がうまくいっていないことが考えられる.そこで,都市部と農村部において,「幼鳥の比率」および,「ひとつがいが面倒をみている巣立ちヒナ数」を比較した.その結果,どちらの値も都市部において低く,予測した通り,スズメの繁殖は,都市部においてうまくいっていないことが示唆された.さらに,後者の値は,都市部において 1羽程度と少なく,都市部では,スズメの増殖率がマイナスになっている可能性も考えられた.都市での繁殖の不振が,スズメの個体数の減少の一要因になっているのかもしれない.

Objective: We describe 3 cases with folding deformation of a PRECISE (Cordis, Miami, FL, USA) stent in carotid artery stenting (CAS).Case Presentations: The 3 cases with cervical carotid stenosis consisted of 3 males around 80 years old and included 2 symptomatic lesions. During CAS, distal embolic protection was established using a Mo.Ma (Medtronic, Minneapolis, MN, USA) along with a filter device in 2 cases and an Optimo (Tokai Medical Products, Aichi, Japan) along with a filter device in 1 case. For the filter device, either FilterWire EZ (Boston Scientific, Natick, MA, USA) or Spider FX (Covidien, Irvine, CA, USA) was employed. In all cases, a PRECISE stent was deployed after pre-dilation performed using a percutaneous transluminal angioplasty (PTA) balloon with the diameter of 2.5 to 3 mm. Post-dilation was performed after the stent deployment using a PTA balloon whose diameter was about 80% of that of the normal distal internal carotid artery. In all cases, cone-beam CT taken after the deployment of a stent showed folding deformation of the stent. In 2 cases, heavily calcified plaque hampered self-expansion of the stent, which resulted in the stent deformation. On the other hand, in the remaining 1 case, a distal shaft of the Mo.Ma caused the stent deformation, which was likely accelerated by head rotation and cervical compression that was performed to resolve difficulties for a filter retrieval device to pass through the stent, and post-dilation after the stenting.Conclusion: Heavily calcified plaque and a distal shaft of a Mo.Ma would result in stent deformation.

持続する上室性頻拍においてP波とQRS波の出現タイミングは一定であり, 通常その関係が変化することはない. 頻拍中, 陰性P波の出現時相が, さまざまに変化したため, P波がQRS波に重なり, QRS波高が減少した房室結節リエントリー性頻拍(AVNRT)の稀な1例を経験したので報告する.症例は71歳, 女性. 月に1~2回の頻拍発作を認め入院. 心臓電気生理学的検査よりAVNRTと診断された. 入院前に記録された頻拍時の心電図では, 頻拍周期540~440ms, QRS波の直後に深い陰性P波を認めた. 頻拍周期の短縮に伴い逆行性P波は早い時相に移動し, QRS波形を変形させた. 軽度の前方移動ではS波を, より高度の前方移動ではQ波を, 中等度の前方移動ではS波とQ波を同時に形成し, QRS波形をさまざまに変形させた. QRS波高も同時に減少したが(陥没現象), 陰性P波の“重なり”効果によると推察された.

本稿は個々の漁村の研究を通じて水産に関する地理学的研究をこころみる際に,どのような配慮がなされるべきかを,主としてわが国の場合を中心に論述したものである. 漁撈行為=狭義の漁業も,人類の最も初源的な行為の一つであるから,人類の自然に対する根元的な反応・適応のあるべき姿を学びとることが出来ると思われているので,伝統的な農業や狩猟業と同様に,すぐれて文化的な現象と考えられる.したがって,従来の伝統的な研究視角としての経済地理学的視角・集落地理学的視角のほかに,文化地理学的視角が必要であると考える立場が支配的となって来た.このような考え方をとる代表的な研究者の一人,斎藤毅の所説に触発され,斎藤論文の批判的な評価をおこないつつ,筆者自身の考え方を披瀝してみた. 結論的にいうならば,陸上と海域の双方を生産と生活の場とし,双方の生態系を監視する立場を保持し続けて来た沿岸住民の生活の場としての臨海集落の研究が,まず何よりも重要な課題となって登場して来るであろう.このような臨海集落は,わが国の場合,旧藩時代以来,伝統的に漁浦とよびならわされて来た小さな村落社会=生活体を形成し,現在の漁業協同組合を構成するような生活体に,系譜的につながっているケースが多い.したがって,この漁浦は,斎藤の指摘するような地理学と水産学の双方の境界領域に形成される一種のフロンティアとして,水産地理学樹立のための研究対象に位置付けられるであろう.何故ならば,斎藤も指摘するように,現実に即し,地域に即したきめ細かい水産地理学的な研究には,前述のような経済地理的・集落地理学的・文化地理学的といった限定された一つの研究視角によらない総合的・包括的にとらえるべき領域,すなわち,水産誌・地域漁業政策論的なもののウエイトが高まって来るにつれ,各々の漁村とその背域の地域(沿岸部海域と沿岸陸上域の双方)の実態の正しい把握が必要となるからである. さらにまた,200カイリ時代の到来によって形成された,あたらしい世界的漁業の秩序=水産究間秩序に対応するためと,沿岸域(陸上)の正しい利用の仕方を理解する時の出発点と考えられるからである.
