(Multiple organ failure) MOF or (Disseminated intravascular coagulation) DIC often becomes a fatal complication for the patients suffering from endotoxin shock. And it is also pointed out that MOF and DIC share many common or simiar clinical features. To clarify the relationship between these two, we surveyed the changes in haematological indices of organ failure patient.DIC was complicated in 10 of 14 patients with endotoxin shock and 9 of these were victimized. In the latter 9 cases, failure of more than three organs occured concomitantly. DIC observed in patients with organ failure is as follows: 10 in 13 cases of lung failure, 3 in 5 cases of heart failure, 10 in 12 cases of renal failure, 3 in 5 cases of liver failure and 3 in 4 cases of gastro-intestinal bleeding. Both organ failure and DIC usually began at the lung in endotoxin shock patients. From the viewpoint of haematology, hypercoagulable tendency preceded the lung failure. For example, decrease in antithrombin III and plasminogen level was observed prior to that in platelet.In endotoxin shock patients, MOF and DIC have close relationship, which suggests that hypercoagulable state induced by endotoxin triggers an organ failure and results in MOF through chain reactions.

The authors propose the idea of ultra-high-frequency rail network timetabling. In such a timetable, the scheduled trains will take very diverse routes on a given rail network to meet the diverse needs of the passengers and to enable ultra-high-frequency train operation. However, at junctions where two or more lines meet, the level crossing interference between different railway tracks will form the bottleneck for the realisation of such high train frequency. The typical example of this can be seen at crossovers between fast and slow lines on the quadruple track railway paired by use. To reduce such level crossing interference, the authors propose the simultaneous crossovers of two trains in both directions at crossover tracks, and the installation of multiple crossover tracks at one junction for realtime selection of the optimal route. In this paper, the authors present the detailed calculation of headways between trains at this kind of crossover tracks, which will form the basis of the authors’ future research into their ideas.

Mt. Bukou, which is located in Chichibu Region, is known as a regional symbolic mountain with a lot of religious events. It has been dramatically changing its landscape since 1970s because of rich limestone resources exploited by several cement companies. This study attempts to manifest the transition of image to its changed landscape in descriptions of student's essays. Obtained images are, (1) Obscure and indirect recognition of the landscape change (2) New movement of formal evaluation of the existing landscape and (3) Deterioration of direct recognition of dignified image and preservation of indirect recognition.

Conclusions were drawn by analyzing the possibility of citizenship education from the standpoint of intrinsic motivation and competency based on the design and implementation of the Fukui Prefecture education program, ‘Climate Change Mystery.’    1) Three elements of intrinsic motivation, ‘competence,’ ‘autonomy’ and ‘relatedness,’ were used when evaluating the target group (302 students) and after the program. The scores of all three elements were higher than before participating in the program. However, particularly noteworthy was that increases in competence and autonomy were greater than relatedness. Furthermore, the lower the score was before the program, the greater the demonstratable change was evident for all three elements afterwards.  2) All 10 elements of competencies showed marked improvement; especially, the ‘strategic,’ ‘anticipatory’ and ‘normative’ elements showed demonstratable development. Those elements were related to the intrinsic motivation elements of competence and autonomy.  3) Several opinions in the student feedback forms demonstrated that these results are typically characteristic of the Fukui Prefecture model. This is evidence that confirms the significance of Japan’s first regional version of the Climate Change Mystery.

Purpose: To elucidate MRI features of primary hepatic mucosa-associated lymphoid tissue (MALT) lymphoma, particularly, the “speckled enhancement” on gadolinium ethoxybenzyl diethylenetriamine pentaacetic acid (Gd-EOB-DTPA)-enhanced MRI.Methods: The institutional review board approved this retrospective observational study and waived informed consent. Using our picture archiving and communication systems and electronic medical records, five patients histopathologically diagnosed as hepatic MALT lymphoma and clinically confirmed as primary lesions who had undergone dynamic contrast-enhanced (DCE)-CT and DCE-MRI with Gd-EOB-DTPA were identified from September 2009 to December 2020. Two radiologists assessed their CT and MRI data in consensus with a pathologist’s advice.Results: Overall, five lesions in five patients were included in this study. Precontrast CT showed hypoattenuation in all lesions. In the arterial phase of DCE-CT, four lesions (80%) showed hyperattenuation, whereas all lesions showed iso- to hypoattenuation in the delayed phase. A vessel penetration sign was also observed in all lesions. On MRI, all lesions showed hypointensity on T1-weighted images, hyperintensity on T2-weighted images, and restricted diffusion on diffusion-weighted images. Both DCE-CT and DCE-MRI with Gd-EOB-DTPA showed similar enhancement patterns, except for the hepatocyte phase. Notably, however, four out of five lesions showed characteristic “speckled enhancement” that refers to punctate positive enhancements within the low signal lesions on the hepatobiliary phase of Gd-EOB-DTPA-enhanced MRI pathologically confirmed to be hepatocyte clusters that remained in the tumor.Conclusion: Primary hepatic MALT lymphomas were characterized by arterial phase enhancement, restricted diffusion, vessel penetration sign, and more specifically “speckled enhancement” in the hepatobiliary phase of DCE-MRI with Gd-EOB-DTPA.
