【目的】重症患者における早期経腸栄養開始後は便秘・下痢が問題となる。排便量から緩下剤の継続・休止や必要な処置・検査などを決定する排便コントロール基準 (以下、本基準) の効果について検討した。【対象および方法】ICUで経腸栄養を7日以上継続した53症例 (導入前群24例、導入後群29例) を対象とした。「一日排便量≥300g」を下痢、「48時間以上排便がない状態」を便秘と定義し、経腸栄養開始後1週間の排便状況を両群間でレトロスペクティブに比較検討した。【結果】一日排便量の1週間における推移は導入前後で交互作用がみられた。7日間における下痢の頻度は導入前群2.5±0.3日、導入後群2.0±0.3日と有意差はなかったが、便秘の頻度は1.5±0.3日から0.7±0.2日に、便秘または下痢の頻度は4.0±0.3日から2.6±0.3日に有意に減少した。【結語】排便量に従って薬剤投与や浣腸処置の追加を判断できる本基準の導入により、排便量および下痢・便秘の頻度が減少した。本基準の導入により適切な排便コントロールが可能となり、経腸栄養管理を有効かつ安全に実施できると考えられた。

β2アドレナリン交感神経受容体(β2-AR)刺激薬の大部分は未変化のまま腎臓より排泄されるため,ネフロンを通過する過程でなんらかの薬理学的効果を発揮すると思われる.しかしながら,β2-ARの腎機能調節における役割が明らかにされていないため,実際の使用時には,このような薬理効果は考慮されていない.腎臓のβ2-ARは主に近位尿細管上皮細胞と,腎動脈の平滑筋細胞膜に分布している.これらの発現の部位を考えれば,β2-ARは糸球体機能や,ネフロンでのナトリウムと水分バランスに作用していると思われる.実際,β2-AR刺激薬を投与すると腎糸球体濾過率は著しく低下する.一方,β2-AR刺激薬は腎臓での炎症性サイトカイン,例えばTNF-αの産生を阻害する.さらに,β2-AR刺激薬は溶血性尿毒症症候群(HUS)の志賀毒素によるアポトーシスの誘導を抑制することがわかっている.腎臓のβ2-AR機能に関して薬理学的根拠に基づいた理解を進めることは,呼吸器疾患で投与されるβ2-AR刺激薬の腎機能を考慮した適正使用についてや,敗血症とHUSに伴う腎臓の炎症や障害に対する治療について重要かつ新しい情報を提供することになる.

The present article briefly overviews the principal pathway of ethanol metabolism in the liver cell and hormonal regulations of ethanol metabolism, and a relationship between alcohol drinking volume (=alcohol consumption) and the degree of alcohol intoxication. This article also focuses on a possible mechanism of sex difference in rat liver cytosolic alcohol dehydrogenase (ADH) activity, and the effects of bile, bile acids, vegetable oils and unsaturated fatty acids on plasma ethanol levels and gastric emptying in ethanol-administered rats. Finally, the recent findings concernig a relationship between alcohol drinking and human health are described.

We should understand the earthquake potential in and around Quaternary fault zones, in view of recent destructive inland earthquakes at previously unknown active fault zones in Japan. The Senpoku Plain and its surrounding areas are characterized by high seismic activity in northeast Japan, highlighted by four destructive earthquakes, M 6.8 in 2008, M 6.4 in 2003, M 6.5 in 1962, and M 7.0 in 1900, which occurred during the past 100 years, although few geomorphic features indicate active faulting. A comprehensive survey was conducted on the tectonic geomorphology in the area to understand the structural and geomorphic expression of the Ichinoseki–Ishikoshi Flexure Line (IIFL), which suggests Quaternary activity. Geological and geomorphical mapping shows that the IIFL is located between the Kitakami Lowland Fault Zone and the Senpoku Plain. The IIFL extends about 30 km from Isawa to Ishikoshi with a slightly sinuous trace. A high-resolution seismic reflection profile and a gravity profile define the subsurface geometry of the IIFL. The IIFL is interpreted to be a steeply west-dipping reverse fault. The Pliocene Kazawa and Yushima Formations typically dip 40° to 20°E along the IIFL, and are overlain by the Pleistocene Mataki Formation, which becomes thinner toward the fold axis of the IIFL, and their dips decrease progressively upward. This suggests that the Mataki Formation was deposited concurrently with fault activity of the IIFL. Fission-track dating of a tuff layer within the uppermost section of the Kazawa Formation indicates that active reverse faulting of the IIFL began at about 2 Ma. At least 280 m of the tectonic uplift is consumed by active faulting and the average uplift rates are estimated to be 0.14–0.08 mm/yr. Vertical separations of Hh surface are about 15 to 40 m. Heights of fold scarps on L1 surface are about 2 m. Their ages are determined to be 0.4–0.5 Ma for Hh and 24–12 ka for L1, respectively. Therefore, the Quaternary average uplift rates of the IIFL are estimated to be 0.03–0.17 mm/yr. Quaternary activity of the IIFL is weak, but there are differences in the magnitude of dissection in the Iwai Hills between the hanging-wall and the footwall of the IIFL.

A new species of red-toothed shrew belonging to the caecutiens-arcticus section of the Sorex minutus group is described from Sado Island, under the name Sorex sadonis, The species is similar to Sorex arcticus KERR, 1792 in having a large skull and massive teeth, but the 2nd lower tooth has a secondary cusp and the lateral pelage is much darker than the ventral. This species is easily distinguished from Sorex shinto shinto THOMAS, 1905 from Honshu, Sorex shinto saevus THOMAS, 1907 from Hokkaido, and Sorex shinto shikokensis ABE, 1967 from Shikoku, Japan, by the much bigger claws of the forefoot and the decidedly darker coloration.

PEG(Parsing Expression Grammar,解析表現文法)は文法規則に曖昧さのない形式文法であり,プログラミング言語の構文記述に有用である.PEGによる文法は再帰下降構文解析器と対応しているため,パーサコンビネータによる簡易な実装によってPEGの文法から実際に動作するパーサが得られる.特にEDSL(Embedded Domain Specific Language)によるパーサコンビネータ実装は,パーサの記述のし易さを確保しつつ,意味規則の記述などの点でホスト言語との接続が簡潔であるという利点がある.また,バックトラックのある再帰下降構文解析器は素朴な実装では入力長に対して最悪指数関数時間を要するが,再帰下降構文解析にメモ化を組み合わせたパックラット構文解析という手法によって入力長に対して線形時間で解析が可能となる.本発表では,C++で記述されたPEGに基づくパーサコンビネータ実装として,Boost.Spirit.X3,cpp-peglib,PEGTL,matcha2の4つの実装の実行性能について比較した結果と,これらの中で最も実行速度の速いmatcha2の実装の詳細について報告する.

水温上昇速度1.5~20℃/minで加熱する場合もしくは沸騰水加熱におけるジャガイモの内部温度および硬さの変化をプログラム計算により予測し,最適加熱時間を算出した。計算の結果,沸騰のみで加熱を行った場合に比べて,水温上昇期を利用することで適度な硬さとするための沸騰継続時間が短縮され,試料形状の違いによる沸騰継続時間の差も小さくなった。水温上昇速度が3℃/minよりも緩慢になると硬化の影響が大きくなり,沸騰継続時間は長くなった。水温上昇が1.5℃/minと緩慢な加熱,もしくは95℃一定温度での加熱により試料内部の硬さは均一となり,煮くずれしにくいことがシミュレーションにより示された。5段階評点法による官能評価を行ったところ,1.5℃/minでの加熱により調製した試料よりも煮くずれがほとんどないと評価された。

The type specimen of <I>Canis hodophilax</I> TEMMINCK, 1839, is a mounted skin'a'and a skull'c'of old male collected from Japan by BÜRGER in the collection of Rijksmuseum van Natuurli jke Historic, Leiden. The skull can be distinguished rather easily from those of related forms by a combination of following characters : remarkably short condylobasal length, well developed auditory meatus external to postglenoidal foramen, emarginated anterior border of pterygoid fossa and well balanced skull jointed with mandible when put them on a horizontal surface. Based on these key characters, specimens from Japan which have been known as"<I>hodophilax</I>"were reexamined and identified as follows : a skull ('b', ad.) in Rijksmuseum, a skin and a skull from Nara (5.5.30.1, _??_ subad.) in British Museum, Nat. Hist., and a mounted skin and a skeleton from Fukushima (M 100, _??_ subad.), a skull from Fukui (M 1185, ad.) and a semifossil skull from Kumamoto (P 9792, old) in National Science Museum, Tokyo, are undoubtedly of <I>hodophilax</I>, but a skeleton ('a', _??_ ad.) in Rijksmuseum may not be of this species but of a Japanese domestic dog.
