【目 的】 加熱式タバコの煙や臭いによる頭痛の実態を明らかにするためにアンケート調査を実施した。【方 法】 インターネットでアンケート調査を行った。対象は20~40歳代の頭痛持ちの男女で、加熱式タバコ使用者に限定していない。【結 果】 有効回答は627人で、片頭痛は163人、その他の頭痛は464人だった。片頭痛群はその他の頭痛群と比較して、頭痛が日常生活に及ぼす影響が大きく、紙巻タバコだけでなく加熱式タバコの煙や臭いにより頭痛が引き起こされる人が有意に多かった。片頭痛群のうち、非喫煙者(117人)でも紙巻タバコだけでなく加熱式タバコの煙や臭いにより頭痛が引き起こされる人がいたが、喫煙者(46人)ではさらに多かった。【結 論】 頭痛患者のなかで、片頭痛患者は、喫煙や受動喫煙による紙巻タバコや加熱式タバコの煙や臭いにより頭痛を誘発しやすいことが明らかとなった。紙巻タバコと加熱式タバコを区別せずに禁煙環境を整備する必要がある。

急性心筋梗塞に対する治療法の発展により、その予後は改善傾向であるものの、最近、閉塞血管のない心筋梗塞であるMINOCA(myocardial infarction with non-obstructive coronary arteries)の存在が認識され始めたが、日本においてはまだMINOCAに関する報告がほぼなく、MINOCAの実態は不明である。本研究ではビッグデータを用いて日本におけるMINOCAの診療実態を疫学的に明らかにするとともに、その診断に寄与する血液バイオマーカーの探索を行う。

IT(情報技術)の急速な発展と社会浸透により,私たちの暮らしがより快適なものへと変容している.半導体技術は,このような社会インフラシステムを下支えする根幹技術であり,私たちが今後さらなる利便性を享受していくためには,継続的な半導体技術の発展が不可欠である.本稿では半導体技術の現状を振り返りつつ今後の人工知能を支えるハードウェアプラットフォームについて展望する.

胃排出機能とインスリン皮下吸収の食後早期血糖変動への影響を検討する目的で, 胃排出遅延を認めるインスリン依存型糖尿病 (IDDM) 11例にGastrokineticsを投与し, 明らかな胃排出改善の得られた6例 (I群), 改善のなかった5例 (II群) について食前および食後120分までの血糖 (PG) と血中遊離インスリン (Free-IRI) をPEG抽出法で測定した.胃排出機能はアセトアミノフェンとアイソトープ併用の液食一固形食同時測定法で検討した.I群;ΔFree-IRIには全検査時間を通じ, 胃排出改善前後で有意差なく, ΔPGは食後60分以降, 有意な上昇を認めた.すなわちΣΔFree-IRIには有意な変化はなく (2310±1266→2707±6889μU/ml・min), Σ ΔPGには有意な上昇 (1853±3442→4799±2983mg/dl・min, p<0.02) を認めた.II群;ΔFree-IRI, ΔPGともに胃排出改善前後で有意な変化を認めなかった.IDDMではインスリン皮下吸収に有意な変化を認めない場合, 胃排出機能が食後早期血糖を制御する重要な因子であることが示された.

In this report we presented a case of emphysema of the parotid gland induced by abnormal habit.The patient was a 45-year-old man. In Sept. 1983, he noticed painless swelling of the left cheek. In Feb. 1985, concerned about the swelling which had continued to grow at times, he visited a clinic of an otorhinolaryngologist. However he received no cure because no swelling was found at that time. On Dec. 3, 1986, when he was admitted to a hospital for a physical examination, the swelling of the same region was pointed out by the doctor. And on Dec. 12, he was referred to our university clinic.In the clinical examination, a diffuse swelling of the left cheek was observed. W hen we pressed the bilateral parotid gland regions, we felt as if we had pressed a mass of flour. And foamy saliva was ejected from the parotid papilla with air. CT scan and sialogram revealed air in the bilateral parotid glands and ducts.Clinical diagnosis of emphysema of the bilateral parotid glands was made. This emphysema was considered to be induced by the abnormal habit that he had puffed out his cheeks strongly with his breath many times since he was 11 years old.

This paper focuses on bibliographical information concerning the Pishamen yigui 毘沙門儀軌 translated by Amoghavajra 不空. Previous studies have considered that this work is of non-Indic origin. The Pishamen yigui consists of three parts: Yiguibu 儀軌部, Yuanqibu 縁起部, and Siji 私記. Since the contents of these three parts are not connected to each other, they seem to have developed separately. Moreover, this text is not contained in the catalogue (表制集) of texts translated by Amoghavajra. Therefore, the translator (or compiler) of this Yigui should not be accepted as Amoghavajra.The text of this Yigui contained in the presently used canons is derived from a version preserved in the Hasedera in Japan. This is most likely derived from a text found in the Kozanji edited by Jijun 慈順.

夏の暑い環境をより快適に過ごすために,冷涼感性や,皮膚からの汗をすばやく吸って,そして急速に乾燥させる吸汗速乾性を有する衣料は,近年夏物衣料としては必須アイテムになりつつある。本稿では人体から発生する水蒸気や汗の移動により,人体をより快適な状態に保つことを目的とした衣服における繊維素材の実例を紹介する。

To clarify the associated factors for negative response to sumatriptan nasal spray in patients with cluster headache, we investigated the involvement of clinical information, such as the characteristics of headaches, before commencing sumatriptan nasal spray treatment. There were 18 male patients and 4 female patients. A total of 17 responders and 5 non-responders to sumatriptan nasal spray participated in the present study. Three factors for negative response to sumatriptan nasal spray, “young age of onset”, “psychiatric disorder”, and “the headache is not in the orbit,” were found. Oxygen inhalation and/or subcutaneous injection were effective for nonresponsive cases. Therefore, these factors are considered to be useful for predicting therapy before applying sumatriptan nasal spray.

【目的】一時的回腸人工肛門に対して人工肛門閉鎖術を施行した患者における器械吻合と手縫い吻合との吻合方法により合併症発生率に違いがあるかを検討した.【対象と方法】2002年から2006年までに回腸人工肛門閉鎖が行われた129例を手縫い吻合(以下,手縫い群)57例,器械吻合(以下,器械群)72例に分け,手術時間,在院日数,創感染,イレウスに関して検討した.【結果】手術時間は両群に有意差を認めなかった(手縫い群:63.8±14.4分vs器械群:61.7±19.0分,p=0.225).器械群では有意に術後在院日数が減少した(手縫い群:10.0±6.1日vs器械群7.1±1.2日,p=0.00004).創感染は両群に有意差を認めなかった(p=1.000).イレウスは器械群で有意に減少した(手縫い群:14/57vs器械群:3/72,p=0.001).【結語】器械吻合によるストマ閉鎖術は術後イレウスを減少させると考えられた.

71歳, 男性。平成13年11月, タバコの火にて右栂指に熱傷を受傷。平成15年6月頃より潰瘍化し, 近医にてbasic-FGF製剤などにより加療を受けるも改善せず, 同年12月当科に紹介された。右担指に5×2.5cm大の潰瘍が存在し, 辺縁は堤防状に隆起し, 浸軟していた。病理組織検査により基底細胞癌と有棘細胞癌のそれぞれの特徴を持った部分が混在し, それらの間に移行部がみられた。また, サイトケラチンを抗原とするKレ1抗体を用いた免疫組織学染色において染色性の違いを認めた。以上, 自験例をBasosquamous cell carcinomaと診断した。腫瘍辺縁より1cm離して腫瘍を摘出し, 植皮術を行った。症例の概要を記載するとともにBSCCの位置づけ, 本邦の報告例などについて, 若干の文献的考察を行った。

It is theoretically well known that when the two bodies are running in the same direction and in a pair in water and/or on water surface, sometimes their total wave making resistance becomes smaller than that of the individual body due to the mutual interference of the waves caused.The proposed semi-submerged ship of the least wave resistance consists of the submerged main hull of the so-called Rankine's ovoid and the surface piercing bridge having sectional area of square cosine curve.Theoretically and experimentally, the combined hull form as mentioned above thus proves its capability of reducing the wave resistance tremendously in a certain specific speed range, i. e., the dimensions of bridge can be chosen so that the waves caused by the submerged hull running near surface may be diminished by the waves of the bridge at the specific speed.Furthermore, it is investigated that the upper limit of the optimum speed range of this type of vessel is between 0.35 and 0.40 in Froude's number.

Ausgehend von der Fragestellung und Diskussion H. Herzmanns (1975) und basierend auf dem kritischen Überblick über die Forschungsgeschichte W. Haugs (1985) wird im vorliegenden Aufsatz die Liebesthematik im "Tristan“ Gottfrieds von Straßburg untersucht.Mit einer auf den ersten Blick primitiven Fragestellung: Warum verlassen Tristan und Isolde die Minnehöhle? behauptet Herzmann, vom erzähltechnischen Standpunkt her überzeugend, daß die Rückkehr des Liebespaares aus der Minnegrotte in die Hofwelt zwar kein positives Zugeständnis an die höfischen und christlichen Werte sein sollte, aber als ein Abfall von der idealen Welt, als ein Versagen des Liebespaares durch ihre Unreife zu verstehen ist. Bei Gottfried werden die Zentralbegriffe des höfischen Denkens wie minne, fröude, tugent, êre in doppeldeutiger Weise verwendet, wodurch eben die unangemessen polarisierten Ansichten, die Haug kritisch zusammenfassend "sexuellen Sündenfall oder erotische Utopie“ nennt, in der Forschungsgeschichte entstanden sind. Um diese Unzugänglichkeit des Romans zu überwinden, gibt die mystische Thematik der ewigen minne nach dem Tod eine glückliche Interpretationsmöglichkeit, aber diese Deutungsrichtung braucht dabei gerade die Voraussetzung, daß die Liebe von Tristan und Isolde an sich nicht unmittelbar als die wahre Liebe verstanden werden sollte, wie Herzmann in der Minnegrotte-Szene untersucht hat.Im vorliegenden Aufsatz wird diese Problematik in der Liebesthematik über den Anfang der Liebe von Tristan und Isolde untersucht. Ihre Liebe besteht im Konflikt zwischen der vom Minnetrank verursachten absoluten erotischen Verbindung, also einem möglichen Weg zu der wahren Liebe in der wirklichen Welt einerseits und ihrer Unreife andererseits, die das Liebespaar bei dem Versuch der wahren Liebe versagen läßt. Schon aus der Vorgeschichte ihrer Liebe zu entnehmen ist diese Unreife bzw. die Unzulänglichkeit, die das Verlassen der Minnegrotte, den inneren Zerfall des Liebesparadieses vorbereitet.Aber die Unmöglichkeit der wahren Liebe in dieser Welt selbst ist nicht Gottfrieds Motiv, sondern seine Liebesthematik wendet sich vielmehr an die unreifen Menschen, die sich nach der absoluten Verwirklichung der wahren Liebe sehnend in der wirklichen unvollkommenen Liebe irren. Den Realitäten dieser Menschen entspricht die Doppeldeutigkeit der Begriffe Gottfrieds, und deswegen ist größeres Augenmerk zu lenken auf den irrenden Weg zur Katastrophe als auf den inneren Zerfall des Liebesparadieses selbst, um den Roman Gottfrieds produktiver interpretieren zu koönnen.

In an effort to determine the mechanism causing motion sickness, various studies have been conducted on the autonomic and central nervous systems. Recently, it was reported that the level of antidiuretic hormone (ADH) rises when motion sickness appears, and the possible involvement of hormones in the manifestation of motion sickness has become a focus of attention.We induced motion sickness by Colioris stimulation in healthy adult volunteers, and examined hypothalamus-pituitary-adrenal hormones and neuropeptides before and after stimulation and during the recovery process.In the high susceptibility group blood levels of ADH, ACTH, prolactin, cortisol, β-endorphin and MET-enkephalin and urinary levels of ADH immediately after stimulation were significantly higher than those in the low susceptibility group. These levels returned to normal after 30 minutes of rest. In the high susceptibility group the motion sickness score immediately after stimulation showed a significant correlations with the blood levels of ADH, ACTH and β-endorphin.These results indicate that ADH, ACTH, β-endorphin and MET-enkephalin are involved in the nausea and vomiting of motion sickness. Moreover, it seems that the mechanism and effects of these hormones may differ from each other. Since the urinary level of ADH is very closely correlated with its blood level, the use of urinary ADH testing is advocated as a simple method with no discomfort.

『毘沙門天王経』は、不空三蔵によって漢訳された経典である。この経典は、義浄訳の『金光明最勝王経』中「四天王護国品」の内容と一部、対応箇所がみられる。<br> 前稿において、これら両経典の対応箇所について取り上げ、構造部分を対比し考察をおこなった。<br> 本稿では、前回ふれられなかった、両経典の内容の部分について比較検証した。既に両経典がほぼ対応関係にあることは指摘しているが、改めて『毘沙門天王経』は、「四天王護国品」よりも内容が整理され、手法などが詳細に説かれている。<br> そして、「四天王護国品」では、あくまで多聞天の持つ如意宝珠陀羅尼とその功徳を説く内容となっているが、『毘沙門天王経』では毘沙門天の心真言つまり毘沙門天自身とその功徳を説いている経典といえよう。
