医療従事者の働き方改革は,医療従事者の健康や幸福だけでなく,医療安全の向上にも寄与すると推測される。本研究は,コメディカルの労働量と医療安全に関するアウトカムとの関係を明らかにすることを目的とした。1964年1月から2018年8月までに出版された医中誌Web収載の文献と,2008年8月から2018年8月までに出版されたPubMed収載の文献を対象に,システマティックレビューを行った。その結果,看護師の労働量に関する文献を26件,薬剤師の労働量に関する文献を2件,その他の文献を7件得た。それらの文献には,システマティックレビューが7件,対照群のある観察研究が28件含まれ,無作為化比較試験は無かった。コメディカルの労働量の多さが患者の死亡率に負の影響を与えるという明確なエビデンスは得られなかった。一方で,看護師の労働量が多いと,与薬エラーや集中治療室における院内感染症が増加する可能性が示唆された。

Illegal cannabis (Cannabis sativa L.) cultivation is still a social problem worldwide. Fifty inquiries on cannabis that Research Center for Medicinal Plant Resources (Tsukuba Division) received between January 1, 2000 and March 31, 2009 were itemized in to 8 categories; 1: seed identification, 2: plant identification, 3: indoor cultivation, 4: outdoor cultivation, 5: germination and growth characteristics, 6: expected amount of cannabis products derived from illegal cannabis plant, 7: non-narcotic cannabis and 8: usage of medicinal cannabis. Top three inquiries were 1: seed identification (16 cases), 3: indoor cultivation (10 cases) and 4: outdoor cultivation (6 cases). Characteristics of cannabis, namely seed morphology, germination and growth characteristics, and distinction from kenaf (Hibiscus cannabinus L.) that is frequently misjudged as cannabis, were studied to contribute for prevention of illegal cannabis cultivation.

近年,下肢装具,特にAFOに関して多くの装具が開発されている.また,脳卒中片麻痺におけるニューロリハビリテーションの重要性が指摘されている.さらに学習理論とロボット技術を応用した新しい装具療法が提案されている状況をふまえて,現状の装具療法をリハ医療の急性期,回復期,生活期に分けて問題点を整理し,今後の課題について論述した.

近年,メタヒューリスティクスは組合せ最適化問題を解く手法として多くの研究が行われている.最近の研究では,コンサルタント誘導型探索(CGS)と呼ばれる新しいメタヒューリスティクスが提案されている.本研究では,CGSを用いた巡回セールスマン問題(TSP)に対する並列アルゴリズムを提案する.アルゴリズムの並列化では,CGSにおける仮想人間をそれぞれの計算機の各プロ セッサコアに割り当てることで効率良く解の探索を行う.また,仮想人間の集団を複数のサブ集団に分割し,各サブ集団どうしで仮想人間の移住を行う島モデルをCGSに取り入れる.10台の計算機を用いた性能評価実験を行い,都市数が5,000のTSPLIBのベンチマーク問題例に対して5%未満の誤差率を達成することを示す.

An 81-year-old man who had been diagnosed as having situs inversus totalis and had suffered from repeated episodes of sigmoid volvulus was admitted with a history of right upper quadrant abdominal pain. Physical examination showed no evidence of peritoneal irritation. A plain radiograph of the abdomen showed a markedly dilated sigmoid colon with an inverted U-shaped appearance. Abdominal CT showed situs inversus totalis, no free air, no ascites, and a whorled appearance of the sigmoid mesentery, with dilated bowel loops. Based on these findings, the patient was diagnosed as having recurrence of sigmoid volvulus. Colonoscopy performed for repositioning showed converging mucosa signifying the distal point of the torsional obstruction, and a dilated section of the bowel with gas and feces proximal to the obstruction in the sigmoid colon. After endoscopic decompression, colonoscopy showed no evidence of mucosal ischemia. We treated this case successfully as we would have a case of sigmoid volvulus without situs inversus.

〔目的〕回復期の片麻痺患者に対してロボット型短下肢装具(R-AFO)を用いたリハビリの効果を検討すること.〔対象と方法〕対象は24名の脳血管障害片麻痺患者.介入期間は10日間で,評価は介入前,介入後の2回実施した.R-AFO群は,通常の理学療法練習60分と,R-AFOを使用した起立や歩行練習を20分の計80分間,非実施群は,通常の理学療法練習を80分間行った.〔結果〕通常群よりもR-AFO群で有意に効果が認められた項目は,歩行速度,麻痺側片脚支持時間,片脚支持時間の左右対称性割合,機能的自立度評価法(FIM)であった.〔結語〕R-AFO装着下での麻痺側に荷重を促す歩行訓練を反復して行ったことで,麻痺側片脚支持時間の割合の増大による,歩行左右対称性の改善効果を有する可能性を示唆した.

〔目的〕新型コロナウイルス感染症の感染拡大に伴い,学外での見学実習が不可能となったため,緊急措置として実施した学内の代替実習の教育効果を検証した.〔対象と方法〕理学療法学科1年生102名,臨床実習指導者の外部講師8名を対象に,主観的学習達成度と習得内容をアンケート調査した.〔結果〕情意領域,認知領域,技能領域での主観的学習達成度は,全て肯定的回答であった.得られた経験は,情意領域全般,幅広い分野の業務内容,動画を通した患者の接し方,症状,理学療法であった.得られなかった経験は,実際の患者とのコミュニケーション,現場実習の緊張感や雰囲気,詳細な患者状態であった.〔結語〕臨床実習を想定した学内での教育の質を向上させることが重要である.

【⽬的】“吹き流し変形”と称する両側変形性膝関節症の症例に対し,保存療法にて有益な結果を得たのでここに報告する。【症例】症例は左膝が内反,右膝が外反変形を呈す両側変形性膝関節症の70 代⼥性であった。 著者らは,治療開始より3 ヵ⽉間の運動療法とADL 指導の実施後,近年,変形性膝関節症患者にも治療効果の期待されている多⾎⼩板⾎漿(platelet-rich plasma:PRP)療法を併⽤し,治療を⾏った。治療のアウトカムとして,従来の理学療法評価に加え,患者⽴脚型評価であるKnee injury and Osteoarthritis Outcome Score(以下,KOOS)を採⽤した。⾝体機能,動作能⼒ともに向上を⽰した。KOOS でも改善を認め,それに基づくOMERACT-OARSI のResponder 基準でも効果ありと⽰された。【結論】本症例報告を通して得られた知⾒は,今後,変形性膝関節症に対する効果的な運動療法を展開するための⼀つの⼿段としての可能性を⽰唆する。

Chasteberry, the fruit of Vitex agnus-castus, has been used in Europe from ancient times for medicinal purposes, and is described in De Materia Medica by an ancient Greece physician, Pedanius Dioscorides. Currently, chasteberry is listed in European pharmacopoeia and its medicinal products are widely distributed in the European market. A Japanese regulation on the marketing authorization application for pharmaceutical products including those with European herbal drugs as their active ingredients was promulgated by the Japanese Ministry of Health, Labour and Welfare in March 2007, and a pharmaceutical product containing chasteberry has been put on the market recently. Since there are several crude drugs derived from the plants belonging to the same genus as chasteberry, the morphological investigation of chasteberry has been performed in order to differentiate it from other crude drugs of the same genus.
