The Paralympic Games are currently in the public spotlight not only as a sporting spectacle but also as a vehicle for empowering individuals with disabilities in society. However, previous research indicates that the Paralympic Games currently cater for a limited variety of impairments, which perhaps reflects a limitation of the Games in this respect. The aim of this study was to determine whether this premise is valid by considering the empowerment of individuals in society via the Paralympic Games, focusing on groups including both mobility and hearing impairment. Quantitative data were recovered from a questionnaire survey disseminated to lower secondary departments of special needs education schools for the physically challenged (n=44, collection rate: 80.0%), lower secondary departments of special needs education schools for the deaf (n=119, collection rate: 85.6%) and a regular lower secondary school (n=145, collection rate: 83.8%) used as a control group in prefecture “A” via postal mail. Results were generated using an analysis of variance approach and χ2 test. The data indicated that the Paralympic Games as a vehicle of empowerment is only valid for those with mobility impairment, and not for those with hearing impairment. This suggests that empowerment through the Paralympic Games may only be effective for individuals with impairments that qualify them for entry into the Paralympic Games, while excluding those with impairments that do not. Ancillary findings of this study suggest that the majority of those with disabilities were uncertain about hearing impairment and the eligibility criteria of the Paralympic Games. It is also suggested that the Olympic Games have some positive effects in terms of empowering individuals with disabilities in society.

The ingestion of raw seafood infected with nematode larvae of the Anisakidae family can lead to gastric anisakiasis. The majority of the infections involve a single larva, however, there have been instances of multiple infection. The incidence rate and the characteristics of multiple infection by Anisakis remain poorly understood. We herein present a case of parasitization by multiple Anisakis larvae and describe 14 cases of multiple parasitization representing the largest reported case series to date. Endoscopists should therefore be aware of the potential for multiple infection by Anisakis and the need for a thorough inspection of all parts of the stomach when encountering such cases.

A psychoacoustical experiment was conducted to examine the basic characteristics of auditory presence as the first step to obtain a full understanding of the multi-modal sensation of presence. Twenty-three sounds were binaurally recorded using a dummy head and reproduced to four subjects by headphones. The experiment was carried out using Scheffé's paired comparison method modified by Ura using a scale of five categories. Analysis of variance revealed that the auditory presence as the main effect was highly significant. As a result of scaling, the sounds of moving sound sources were evaluated as having a higher presence than the sounds of stationary sound sources. As for the latter, sounds with larger sound images were rated as having a relatively higher presence. The effects other than the main effect and the interactions between them, however, were also significant. In the interactions, the individual differences for the main effect tended to be explained as the difference in weights of the subjects on the psychological factors involved in the auditory presence.

Difference of onset of increase of PRL content in the anterior pituitary gland and plasma PRL concentration during the late stage of chicken embryogenesis is well known. To investigate the disagreement, changes in PRL content and PRL mRNA levels, and the effects of vasoactive intestinal polypeptides (VIP) on PRL release and PRL mRNA expression were examined using western blot analysis and real-time PCR quantification. Changes in SPRL content were strongly correlated with PRL mRNA levels. The increase in PRL content on day 17 of incubation may be caused by the increase in PRL mRNA levels on day 16 of incubation. Additionally, the effects of VIP on PRL release from the embryonic anterior pituitary gland were not observed until day 18 of embryogenesis. These results suggest that increased levels of PRL mRNA and PRL content in the anterior pituitary gland are closely correlated. However, the increased expression of PRL mRNA observed on day 17 and the initiation of PRL release from the anterior pituitary gland on day 19 were differentially regulated. According to the results of western blot analysis, the proportion of glycosylated PRL (G-PRL) and non-glycosylated PRL (NG-PRL) in the anterior pituitary gland at the end stage of development differed from the proportion of PRL released from the anterior pituitary gland. According to the results of two-dimensional western blot analysis, no isoforms with different isoelectric points were detected in the culture medium on days 19 and 20. These data suggest that the peptide chains of G-PRL and NG-PRL were not modified. In conclusion, the differentiation of PRL-producing cells and the maturation of the hypothalamus and anterior pituitary gland were completed at the end stage of incubation, and that different factors regulated the initiation of PRL mRNA expression before day 18 of incubation.

Japanese native chickens (JNCs) comprise approximately 50 breeds, making Japan a diversity hotspot for native chicken breeds. JNCs were established through the repeated introduction of chickens from foreign countries. Jidori, which is the generic name of JNC breeds whose ancestral morphology resembles that of their wild progenitor (red junglefowls), is generally thought to have propagated from north East Asia (Korea and north China) to ancient Japan. However, mitochondrial haplogroup D, which is abundant in Island Southeast Asia (ISEA) as well as the Pacific but relatively rare in other regions, can be observed in some Jidori breeds (e.g., Tosa-Jidori, Tokuji-Jidori) with high frequency, leading to speculation that chickens from ISEA or the Pacific also contributed genetically to JNCs. To test this hypothesis, we sequenced the mitochondrial genomes of Jidori breeds and conducted phylogeographic analysis. Our results indicate that the JNC Haplogroup D belongs to Sub-haplogroup D2, which is currently only observed in Xinjiang, northwest China, and not to Sub-haplogroup D1, which is widely distributed in the ISEA-Pacific region. The other mitochondrial haplogroups of Jidori examined in this study also showed affinity to those of chickens native to north East Asia. Therefore, our findings support the north East Asian origin hypothesis for Jidori.

Previous studies in mammalian obesity models have suggested that central transforming growth factor-β (TGF-β) controls the gene expression of appetite-regulating neuropeptides and peripheral energy metabolism. In the present study, we investigated the possible involvement of central TGF-β/Smad signaling in feeding regulation in chickens. Central administration of TGF-β1 resulted in phosphorylation of Smad2 in the hypothalamus of chicks and suppressed feed intake without changing the gene expression of hypothalamic appetite-regulating neuropeptides (neuropeptide Y, agouti-related protein, proopiomelanocortin, and corticotropin-releasing factor). However, neither fasting nor refeeding induced the phosphorylation of hypothalamic Smad2. These findings suggest that the activation of hypothalamic TGF-β/Smad signaling suppresses feed intake in chicks but it might not occur in response to feeding status.

〔目的〕間質性肺炎増悪が胸郭出口症候群発症の要因になった可能性のある症例に対して行った介入が,症状改善をもたらした例を報告する.〔対象と方法〕間質性肺炎により常時0.5 L/分の在宅酸素療法を行っていたが,呼吸困難感と左上肢の疼痛,感覚障害を呈し, 間質性肺炎増悪による経皮的動脈血酸素飽和度低下で入院となった80代女性を対象とし,左斜角筋群ストレッチ,リラクゼーション,胸郭ストレッチなどの呼吸リハビリテーションを中心に介入を行った.〔結果〕その結果,疼痛消失と感覚障害消失がみられた.〔結語〕呼吸機能に着目して行った介入が,間質性肺炎増悪が発症の要因になった可能性のある胸郭出口症候群の症状を緩和にしたことから,呼吸器疾患による胸郭出口症候群発症の可能性が示唆された.

Digital subtraction angiography (DSA) の三次元画像やslab maximum intensity projection (MIP) 画像に加え, DSAとMRIやCTとのfusion画像により, 血管と脳・神経や骨構造との関係性が明瞭に描出可能となり, 血管機能解剖の理解が容易となった. 本稿では, 各種疾患の具体例を示しながら, われわれが行っている臨床例の実際を紹介したい. これら最新の診断技術と解剖を熟知した血管内治療医が詳細な術前診断を行うことによって病態理解が深まり, 開頭術を含む脳神経外科全体の治療成績が向上することが期待される.

高次脳機能とは, 伝達・表現・理解・遂行するためのヒトの高度な機能の総称である. 脳神経外科手術において高次脳機能を温存するためには, その機能局在および関連する神経ネットワークを知ることが必須である. われわれは白質神経線維の走行を理解するためには白質解剖が最も優れた手法であると考える. 高次脳機能を司る上縦束, 下前頭後頭束, 下縦束, 前頭斜走路, 帯状束は白質解剖で剖出できる. 覚醒下手術中に適切なタスクを用いて, これらの神経線維束が担う高次脳機能関連領域を同定することが可能であり, 同部位を傷害しないことで全身麻酔手術では温存し得なかった高次脳機能を意図的に温存できるようになった.
