Fosphenytoin (FOS) is a phosphate ester prodrug developed as an alternative to intravenous phenytoin for acute treatment of seizures. Although both FOS and phenobarbital (PB) are used for status epilepticus as second-line drugs, there is no data on their comparative efficacy and safety. We retrospectively analyzed data from children treated with FOS or PB for convulsive status epilepticus and acute exacerbation of seizures. Our aim was to compare the efficacy and safety of intravenous FOS with those of intravenous PB for convulsive status epilepticus and acute exacerbation of seizures. Seventy-seven children were included in the study: 54 received FOS, and 23 received PB. The primary end point was recurrence of seizures and drug-related adverse events. The response rate, defined as no recurrence of seizures within 24 hours after termination of seizure, was 92% (50/54) and 95% (22/23) in FOS group and in PB group, respectively. Adverse events occurred in 27% (15/54) of patients in FOS group and in 95% (22/23) of patients in PB group (P < 0.01). Although no serious adverse events occurred in patients in both FOS group and PB group, the incidence of both sedation and disturbance of consciousness were significantly higher in PB group than FOS group. From these results, FOS is recommended as second-line drugs for status epilepticus and acute exacerbation of seizures. However, the optimal serum concentration achievement rate was significantly lower in FOS group than PB group. To maintain optimal serum phenytoin levels (10 - 20 µg/mL), higher doses of FOS might be required.

生物はどうやって温度を感じるのか? 非常にシンプルな問いであるが,わからないことが多い.これまで,さまざまな生物で温度センサー分子が報告されているが,植物では一つも見つかっていなかった.筆者らは,植物細胞で起こる温度依存性の葉緑体配置変化について研究し,最近,青色光受容体フォトトロピンが温度センサー分子であることを発見した.また,この発見を基盤にして,生物の温度感知に関する新しい説を提唱している.本稿では,青色光受容体フォトトロピンによる温度感知のメカニズムを解説するだけでなく,これの解明にたどりつくまでの10年間について記録したいと思う.

Starting to prevail in the end of the 1960's, interdependence studies have been much in fashion throughout the seventies. Almost all phenomena related to international relations were discussed in connection with “interdependence” despite the absence of its substantial clarification. Consequently, there were too many definitions to use it as an analytic concept; and attention shifted from interdependence proper to (1) international regime/order, and (2) policy/adaptation towards a contemporary complex world. First of all, this paper reviews a brief intellectual history of such interdependence studies as the vogue in a scholary world reflecting recent profound changes in the real world.Doubtlessly influenced by leading academic moods in America, interdependence studies became popular in the late 1970's in Japan, but an independent and isolated research on the subject had been conducted years earlier. Taking this study as a model, this paper analyzes interdependence studies as various approaches to understand international society which contains both national and non-national actors interwoven by numerous and complicated transactions, and which creates new types of problems awaiting novel ways of solution.In the course of analysis herein, the meanings of “interdependence” are redefined and compared with one another, and most importantly a theory of interdependent relations is proposed. Specifically, it attempts to explain conflict and cooperation over interests in transactions by the following factors: (1) transactions between actors, (2) actor's internal system, (3) actor's perceived cost and benefit associated with transactions, (4) actor's controllability of transactions, and (5) the international order to regulate transactions.Designing interdependence is needless-to-say extremely difficult. According to the analysis in this paper, however, the necessary fundamental structure of interdependent relations can be at least pointed out. Namely, the most basic structure which differentiates the situation of “interdependence” from other types of situations is multidimentional conflict cum cooperation over complicated transactions cum alternative regimes.

Hyperkalemia is common in patients with renal disease, and is sometimes caused by dietary potassium intake. We aimed to determine and compare the content of potassium in nine brands of glucosamine supplements sold in the Japanese market and via the Internet. The potassium content was 0.165-3 mg per daily dose in Japanese products, which contained glucosamine hydrochloride or N-acetylglucosamine, while the content in foreign products, in which glucosamine was sulfated, was 197-280 mg. Our results show that the potassium content in glucosamine sulfate supplements can correspond to 20% of the maximum daily intake of potassium by patients on hemodialysis, because the products sometimes contain glucosamine as glucosamine sulfate potassium chloride for stabilization. Although it is not permitted to sell glucosamine sulfate as food in Japan, consumers can easily buy foreign products that contain glucosamine sulfate via the Internet, and those products rarely indicate the potassium content. Health professionals should pay attention to patients' use of glucosamine supplements, especially when patients' dietary potassium intake needs to be restricted.

低K血症, 高K血症は日常診療上頻繁に遭遇し, 様々な疾患の診断の手がかりになるとともに, 時に緊急治療の対象にもなるため, 実地医家はその対処法をよく知っておく必要がある. ここでは, 基本的なK調節メカニズムについて述べたあと, K代謝異常の具体的なアプローチ法につき解説したい.
