Since cytokine release syndrome is considered to be associated with severe cases of COVID-19, steroids are expected to be effective for its treatment. We aimed to investigate the use of steroids and its impact. We conducted a retrospective chart review and analysis of 226 consecutive hospitalized patients with confirmed COVID-19. Patients were divided into those who received steroids (steroid group) and those who did not (no steroid group). Inverse weighted probability weighted analysis was performed to assess the effect of steroids for in-hospital mortality. The steroid group had higher rates of preexisting hypertension and peripheral vascular disease than no steroid group and also had higher lactate dehydrogenase, d-dimer, and inflammatory makers compared to no steroid group (all P<0.05). The steroid group had significantly higher rates of multifocal pneumonia than no steroid group at admission (75.4% versus 50.3%, P=0.001). Notably, steroid group had higher rates of bacterial infection (25% versus 13.1%, P=0.041) and fungal infection (12.7% versus 0.7%, P<0.001) during hospital course. After adjustment, steroid did not decrease or increase in-hospital mortality (OR [95% CI]: 1.02 [0.60-1.73, P=0.94]). Steroid did not decrease the in-hospital mortality rate. There were increased bacterial and fungal infections with steroid use.

麻酔科医からすれば, 無痛分娩は麻酔行為であるから, その担当者は当然麻酔科医と思うかもしれない. しかし実際に産科麻酔を実践するには, 麻酔科学の基礎知識や麻酔技術はいうまでもなく, 麻酔が母親の出産体験や, その後の家族の育児にどのようにかかわるのか, 自然の分娩経過にどのような影響を及ぼすのか, あるいは担当産科医の分娩管理にどのように影響を及ぼすのかを考慮し, 家族, 産科医, 新生児科医, 助産師など妊婦を取り巻く人々と良好なコミュニケーションをとりながら, 妊婦にいかに安全で快適な環境を提供できるかを考えていく必要がある. そのためには産科麻酔科医というサブスペシャルティーの確立が望まれる.

The effects of low-dose alcohol on experimental animals are unclear. This study examined plasma metabolites in senescence-accelerated mice 8 (SAMP8) given low-dose ethanol, and compared them with aging progress and skeletal muscle strength. Male SAMP8 mice (10-wk-old) were given drinking water containing 0% (control), 1%, 2%, or 5% (v/v) ethanol for 14 wk. Compared with the control group, only mice who consumed 1% ethanol experienced a lower senescence score at 18 and 23 wk, as well as an increased limb grip strength at 21 wk. Plasma metabolites of control, 1% and 2% ethanol groups were analyzed by capillary electrophoresis–time-of-flight mass spectrometry (CE-TOF/MS). Among the 7 metabolites affected by ethanol, notewhorthy is the positive association of the ethanol levels in drinking water with the levels of α-ketoglutarate (antioxidant and anti-inflammatory metabolite) and hippurate (antioxidant and microbial co-metabolite) (p<0.05). Intriguingly, the levels of 2-hydroxyisobutyrate (the biomarker of energy metabolism and microbial co-metabolite) were higher in the 1% ethanol group (p<0.05), but not in the 2% ethanol group as compared to the control. Furthermore, the levels of some of the metabolites affected were correlated with some variables in the grading score of senescence and muscle strength. This study provides a novel insight into how low-dose ethanol in SAMP8 mice modulates the levels of circulating metabolites relating to chronic disease risk.

To investigate changes in serum and hepatic levels of amino acids in ALD and to provide novel evidence and approaches for the prevention and treatment of ALD. Twenty specific pathogen-free SD male rats were devided into two groups, ten for the control group, and ten for the model group. Serum biochemical markers, including alanine aminotransferase, aspartate aminotransferase, laminin and hyaluronidase were measured. Histological analysis of liver tissues was performed. Serum and liver amino acids levels were quantitatively determined by ultra-high-performance liquid chromatography-tandem quadrupole mass spectrometry (UPLC-TQMS)-based targeted metabolomics. Compared with the normal group, ALD rats showed an obvious increase in the levels of β-alanine, alanine, serine, ornithine, tyrosine and the tyrosine ratio, while there was a decrease in arginine levels, the BTR ratio and Fischer’s ratio in serum. Additionally, ALD rats exhibited a significant increase in the levels of cysteine and putrescine, while there was a decrease in sarcosine, β-alanine, serine, proline, valine, threonine, ornithine, lysine, histidine, tyrosine, symmetric dimethylarginine, methionine, isoleucine and methionine-sulfoxide levels in liver tissues compared with the normal group. The serum and liver amino acids showed significant changes in ALD rats and can be considered as potential specific diagnostic biomarkers for ALD.

腎性骨異栄養症(ROD)は腎性骨症,透析骨症とも呼称され,当初は骨の病変に限定されていたが,最近になり全身症状や生命予後に対する効果が重視されるようになり大きく概念が転換した.即ちRODは,慢性腎臓病に伴う血管石灰化に代表される軟部組織異所性石灰化を含む骨ミネラル代謝異常(chronic kidney disease-mineral and bone disorder: CKD-MBD)の一部分症で,全身性疾患としてとらえるべきであると変わってきた.したがってRODの治療においては生命予後を最も重視して予防・治療する必要がある.

口腔・鼻腔から出力される音声は必ず超分節的特徴を具えている。音声の最終出力の音声プランが描かれるとき,分節素の検索と配置の構成とともに,プロソディックワード(韻律語)の設定が,音韻単位でまとめられていく。その単位の末端が分節素で,そしてモーラ,音節,フット,プロソディックワードが各単位を構成し最終的には文ということになる。プロソディックワードの構成要素である各単位で,言語の普遍性に基づいた音韻制約や規則が展開され,経済性や忠実性等に根差した分節素のまとまりの発音が一体化し,プロソデックな側面が形成される。具体例としてアクセントやイントネーションの高低や強弱や長短の変化が設定される。形態素や語同士の連結で,アクセントの位置や連結部の発音が決定される。また分節素の環境からの関連で,同化や弱化が生じる。英語と日本語ではその音節構造とリズムの類型が違い,そのプロソディックな様相が全く異なる。もし言語障害の要因が発話産出過程のある個所の弱点だとすると,要因が同一であっても言語間でその症状が異なって表出する。

A transparent boundary condition was developed for calculating water waves propagating to a distant area by using a particle method. In front of the boundary, the incident waves were analyzed by Fourier analysis, and the particles on the transparent boundary were forced to move to absorb the incident waves. The characteristics of this study are to introduce the technique of wave analysis used for the wave tanks to the particle simulation and to reduce the calculation cost by employing an inflow and outflow boundary. Water waves were calculated in two wave periods by the developed transparent boundary condition. As the results, it was shown that this transparent boundary transmitted the incident waves without the reflection waves. It was also shown that the calculation cost was smaller than that of the ordinary high viscosity boundary condition.
