Cat scratch disease (CSD) is an infectious disease caused by Bartonella henselae. Atypical clinical presentations of CSD include prolonged fever and multiple hepatosplenic lesions, although these are rare. Furthermore, multiple renal lesions are extremely rare in CSD. The patient was an 11-year-old Japanese girl who had a prolonged fever of unknown cause after being scratched and bitten by a kitten. Abdominal computed tomography (CT) revealed multiple small, round hypodense lesions in both kidneys and in the spleen. Based on her history and the results of CT, a diagnosis of CSD was made; the diagnosis was confirmed with serological tests, which indicated antibodies against Bartonella henselae. After treatment with azithromycin, her fever immediately improved. Careful history taking and imaging are essential for the diagnosis of atypical CSD. In CT images, not only hepatosplenic lesions but also renal lesions are important features indicative of a diagnosis of atypical CSD. Subsequently, a diagnosis of CSD can be confirmed with specific serological tests. To the best of our knowledge, this is the first reported Japanese case of multiple renal and splenic lesions in a patient with CSD. Although atypical CSD is difficult to diagnose, an early diagnosis is important to prevent invasive examinations.

Self-effacement has become an important topic in cross-cultural social psychology. However, descriptive characteristics of self-effacement have rarely been investigated. Based on representative network survey data, this study examined to whom Japanese people efface themselves. The results indicated as follows: 1) people tend to efface themselves to their neighbors or mere acquaintances more than to their spouses, colleagues, or best friends; 2) the duration of the relationship correlated negatively with self-effacement; 3) the relationship between psychological intimacy and self-effacement shaped reversed-U; and 4) people tend to efface themselves to maintain or attenuate their status difference, i.e., people efface themselves more to colleagues when the people considered those colleagues to be superior to them, and also to best friends when they considered those friends to be inferior to them.

日本語文章推敲支援ツール『推敲』は,日本語文章ファイルを解析して,推敲に役立つ情報を書き手に提供することを目的としたツールである.われわれは,まずこれを,大型計算機上に開発した.しかし,大型機には使用上の制約があったり,ユーザーインタフェースが十分でなかったりして,文章作成のような個人的な仕事を大型機上で行うのは快適ではない.そこで,『推敲』の機能の強化やユーザーインタフェース向上,そして利用者層の拡大を期して,これをパソコン上で実現した. 『推敲』では,日本語文章の解析に文法解析や辞書を用いず,字面解析を行うことによって,素早い処理を可能にしている.文法解析をしないでも,推敲に役立つ情報をかなり抽出することができる.パソコン版『推敲』は,処理速度の速さと,ユーザーインタフェース設計の自由度が大型機と比べて大きいことも相まって,実用的なツールとなっている.また,『推敲』のコマンドを構成しているプリミティブな処理単位を,基本コマンドとしてユーザーに開放し,ユーザーがそれらを組み合わせることによって,多様な処理を行えるようにしている.

We herein report a case of persistent methicillin-resistant Staphylococcus aureus (MRSA) bacteremia that was successfully treated with combination therapy consisting of high-dose daptomycin (DAP, 10 mg/kg) and rifampicin. The patient's condition was complicated with multiple infectious foci, including an iliopsoas abscess and epidural abscess, as well as discitis and spondylitis at the cervical, thoracic and lumbar levels. Monotherapy treatments with vancomycin, linezolid and usual-dose DAP were all ineffective. It has been shown that usual-dose DAP administration may result in the emergence of a resistant strain and treatment failure. We would like to emphasize the importance of high-dose DAP therapy for MRSA bacteremia, a condition with a potentially high mortality rate.
