“LANTUS® XR Inj. Solostar® (450 units / 1.5 mL)” is a preparation that tripled the insulin concentration of existing LANTUS® Inj. Solostar®. As the volume per insulin unit is reduced to one third, the recommended blank beating units before dosing have been increased to 3 units. However, some patients perform 2-unit blank beating as they use the existing injector. A retrospective investigation identified erroneous operation of blank beating in 9.5% of outpatients who had been prescribed LANTUS® XR Inj. Solostar® at Nagoya Ekisaikai Hospital from April to May 2017. In this study, we investigated if 2-unit blank beating with LANTUS® XR Inj. Solostar® is suitable to confirm needle hole clogging and injection of content by naked-eye observation and the amount emitted. Injection of content was confirmed at the second and subsequent 2-unit beatings by naked-eye observation. On the other hand, the emission weighing test with a new needle and syringe barrel revealed that the amount discharged was increased with 2-unit beating and reached the theoretical value at the fourth beating. Based on these results, we concluded that 3-unit blank beating is preferable for LANTUS® XR Inj. Solostar®; however, three successive 2-unit blank beatings are helpful to confirm correct installation of the needle and normal movement of the injector.

Recently we find that the studies of skills in social work are on the increase. But we have not much knowledge about it which has important, essential impact in social wroker' s activities. To practice social work effectively, social worker needs generic / spesific knowledge and mastery of interventions and values. He or she selects useful knowledge and intervention, being guided values of clients and professional values. Then under the contract between a client / clients and social worker, he or she prorvides social services to people and improves their life situation, changes systems / environments of them, understanding causes and backgrounds of their life problems. But guided values and knowledge, slills of social worker function these whole processes of practicing. In this meaning, skill is a central part of social work pracice. We recognize that social work skill depends on social worker's abilities and aciviteis as a profession, also we need to develop a unique whole system of skills in our ptactice. In this paper, we disscuss and analyze, make it clearly from reviewing some research of the pioneers ; (1) what social wrok skill means, (2) what and how position and function of skills in social work practice are, (3) what role of skill is, in case of sescientific practice and creating scietific social wrok with an uniqeness, also (4) what and how relation of a speciality of social work uniqueness (the nature of social work) and skill is, etc.. There, we find several important points and tasks about research of them. There is deferential part in use of term and difintion of skill between pioneers. But the term and defintion of social work skill is very important. They symbolize whole social wrok idntity (uniqueness), especially professional identity, and scientific identity and originality, social meaning of itself. Additioning to say, there are somoe discreminations of concepts and construcions of skills of each pioneers, some are clear and elaborate, some are loose, unclear and limitted. Those indicate it's unmatured level of study. So we need more research of social work skill and must relate it to methodological perspectives under the uniqueness and originality of social work. Finally, We suggest more ideas of how to research skill. Especially we streess that we build up and refine feedback-loop procseess system of research of skill ; "Teaching knowledge, values, skills-Learning-Scientific Practice-Analyzing / verifying-Scientific Aproach of Doing / Practicing (Creating / Recreating and Constructing / Reconstructing new knowledge, values, skills)-Teaching...".

Until the time arrives when stroke patients truly recognize themselves as handicapped (living with their disabilities), it is well known that they usually pass through the five stages of grief, namely : denial, anger, bargaining, depression and acceptance. It is difficult to know which stage a patient is at, but we have found that a patient's writing, their letters, the occasional poem, etc are all of great use to identify the stage. Patients who are satisfied with their present lives are more able to accept their situation and therefore have fewer tendencies toward suicidal thoughts. The richness of their lives helps them to overcome their disabilities and enables them to return back to society. We should not force our patients, especially those in the denial or confusion stage, to accept their disabilities and rather must try to listen sincerely about their suffering and their stories. Most patients with brain strokes are encouraged to do their best “ganbare" by medical staff such as nurses, physical therapists, occupational therapists, speech language hearing therapists and doctors, and their family members and their friends. Usually, they are pleased to hear these words. However, some patients may perceive such encouragement as stressful, but if we know how to use these words in a supportive, positive manner, then they can be useful words in the care of stroke patients.

Oral mucosal lesions associated with foreign body injuries can have various origins, but traumatic lesions of the oral mucosa caused by other organisms are rare. Such cases thus require special knowledge for diagnosis. We report a case of oral stings from spermatophores of <I>Todarodes pacificus</I>, the Pacific squid. The patient was a 31-year-old woman who cooked the internal organs of a raw <I>T. pacificus</I> for lunch. She experienced a sharp pain on the tongue and buccal mucosa when eating the organs. On checking the oral cavity, she identified multiple white objects with a worm-like appearance sticking into the tongue and oral mucosa. Attempts to remove these objects herself were unsuccessful. She then visited the emergency department of our hospital. We examined the oral cavity and found multiple white objects appearing to be parasitic worms sticking into the tongue and oral mucosa. Attempts to remove the objects with forceps were unsuccessful because of tight attachment to the mucosa. Removal was thus achieved by making slight incisions under local anesthesia. The specimens showed a white spinate shape and were about 4mm long. Endoscopic examination of the upper digestive tract after treatment of the oral cavity revealed no additional foreign bodies. The final pathological diagnosis was spermatophores of <I>T. pacificus</I>.

「重要文化財 金沢文庫文書」4149通について、その本文の校訂、年代推定、紙背文書を利用した関連文書群の復元を行い、「重要文化財 称名寺聖教」との接続の関係をあわせて考察し、称名寺収蔵資料群の一群としての金沢文庫古文書の資料的価値を定める努力を行った。その成果は、「金沢文庫文書検索システム」としてデータベースを構築し、インターネットでの公開をめざしたが、接続のための環境整備が調わず、金沢文庫図書室でのスタンドアローンとしての公開となった。データベースでは、古文書本文・書誌情報・画像(古文書表裏)を金沢文庫図書室で公開した。

H28年度において実施した研究の実績概要は以下の通りである。1.高温低圧環境高速X線回折(XRD)装置及び蛍光X線分析装置(XRF)の導入:複数の候補からリガク製XRD装置SmartLab3を選定し、低圧加熱実験可能な電気炉と2θに対して10°以上の範囲の同時測定可能な高速2次元検出器を併用することで、加熱時及び冷却時の構造変化をその場観察できる装置を導入した。また、XRF装置も導入した。2.アルカリ塩化物試薬におけるカチオン依存性の解明:非放射性Csを収着させた風化黒雲母(WB)をモデル土壌とし、CaCl2とKCl単塩をそれぞれ添加した際のXRD、XRF測定を行った。CaCl2添加により700℃低圧加熱で100%のCs除去率を確認した。一方、KCl添加では約50%のCs除去率となった。また、CaCl2添加時はXRDパターンが大きく変化したのに対し、KCl添加ではWBの構造がほぼ保たれた。これらの結果から同じ塩化物でも1価と2価のカチオンではWBからのCs脱離過程が異なることを明らかにした。また、昇温脱離法(TDS)を用いた分析でもCaCl2添加時にCs脱離が促進されることを確認した。X線吸収分光法(XAFS)を用いた加熱中のその場観察の研究ではNaCl-CaCl2混合塩へのCs溶出が観察された。それと共に、600℃付近より高温側と低温側でCs脱離過程が異なる可能性を見いだした。以上の知見に関しては欧文誌に論文発表すると共に、国内外の学会において報告を行った。3.福島汚染土壌による実証試験:福島の帰還困難区域から粘土質汚染土壌を採取し、加熱処理による放射能変化をNaI検出器により調べた。その結果、KCl、MgCl2試薬では低圧加熱の方が大気加熱よりも高い放射能減衰率が得られたがCaCl2試薬では両者で大きな差が見られず750℃大気加熱で約97%の放射能減衰が得られた。

本論文では、第1 章で現在日本において観光が国家的な課題となっていることを示すとともに、旅行者の趣味の多様化、個人化によって地域において旅行者と地域住民の直接接触が増加している可能性を示唆した。それゆえに、旅行者と地域住民の関係に関して研究が必要であることを示し、その際に理論的土台として自律的観光の概念が有効であることを示した。ただし、その背景として現代社会では情報化が進展しており、それを踏まえることが自律的観光を考える際に必要であることを示した。また、情報化を背景にして旅行者の趣味の多様化、個人化が進んでいることから、自律的観光を考える際には、旅行者の自律について考えることが必要になることを示した。第2 章では、第1 章で立てた問題を解決するための背景として、1990 年代の旅行情報の利用状況と2000 年代の旅行情報の利用状況についてデータを分析し、現在旅行者はどのような情報環境にあり、旅行行動時の情報収集の方法としてどれほど情報化の影響を受けているかを明らかにした。その結果、20 代男性、20 代女性、30 代男性、40 代男性が、旅行の際にインターネットを多く活用していることが明らかになった。本論文ではこの世代の男性を「旅行情報化世代」と名付けた。第3 章では、第2 章で明らかになった世代がどのような旅行行動をおこなっているのかを明らかにするために、近年新聞や雑誌などで取り上げられているアニメ聖地巡礼についてその誕生と展開経緯を整理するとともに、アニメ聖地巡礼はツーリズムとアニメの潮流の中でどのように位置づけられるのかについて検討をおこなった。その結果、アニメ聖地巡礼は、情報化社会特有の現象である可能性が示唆され、また、アニメ聖地巡礼者がある地域を訪れることによって、その地域の住民との相互作用が生まれ、それが元でまちおこしにまで発展する事例もあることが明らかになった。第4 章では、第3 章で明らかになったアニメ聖地巡礼の特徴が、実際に一つの地域において確認できるかどうかを確認するとともに、新聞や雑誌などには取り上げられてない、アニメ聖地巡礼の特徴を明らかにするために、埼玉県鷲宮町におけるらき☆すた聖地巡礼とその後の展開について、整理をおこなった。その結果、第3 章で明らかになったように、地域住民との相互作用が生まれ、それが元でまちおこしに発展する経緯を詳細に確認した。それだけではなく、アニメ聖地巡礼者は地域住民から「マナーが良い」と認識されていることや、アニメ聖地巡礼者が、当該地域のガイドブックを作製する、ボランティアスタッフとして地域に貢献する、グッズやイベントに企画から参画する、祭りの神輿製作を地域住民と共同でおこなう、などが確認された。つまり、アニメ聖地巡礼者は地域に迷惑をかけないばかりか、地域住民と一緒になって当該地域を活性化する役割を担っていることがわかる。第5 章では、鷲宮神社を訪れる来訪者の中の聖地巡礼者の割合を示すとともに、アニメ聖地巡礼者の動態をアンケート調査などによって明らかにした。その結果、アニメ聖地巡礼者は、「旅行情報化世代」を中心としており、従来の旅行会社に頼った旅行形態は採用せず、自らの興味・関心にしたがって、自分たちで旅をする主体であることがわかった。本論文で展開した議論の結果、情報化社会は観光旅行者が自らの興味・関心に従って旅をすることができる環境を用意し、現在は「旅行情報化世代」がそうした旅を実行していることが明らかになった。それらを踏まえた上で、第6 章では、自律的旅行者のあり方を仮説的に提示した。自律的旅行者は、旅行会社を利用しない、交通手段を選択できるなどの選択の自由をもち、かつ、強力な情報入手能力と、情報発信能力を駆使して、地域を訪れる際には事前に情報を仕入れ、旅行者同士で情報交換をおこない、地域住民に迷惑をかけないような形で行動を取ろうと努力し、旅行を楽しもうとする主体である。また、それは何か外発的な要因によって規定されるのではなく、旅行者が地域住民との気持ちの良い関係や、そこから生まれる心理的報酬を求める内発的動機付けによるものである。第7 章では、今後の展開として、自律的旅行者の萌芽形態である可能性の高いアニメ聖地巡礼者に関して、心理的報酬について明らかにする必要があることを示すとともに、他の地域のアニメ聖地巡礼者も鷲宮町に訪れるアニメ聖地巡礼者と同様の特徴を持つのか、比較・検討を行う必要があることを示した。

日本では、1990年代前半ごろからアニメを観光動機とした旅行行動であるアニメ聖地巡礼が行われ始め、近年ではアニメ聖地巡礼を契機として、地元住民と巡礼者が共同でまちづくりに取り組む事例も見られ始めた。これは、情報技術の発展が旅行者行動に影響を与え、従来型の観光客とは異なる行動を誘発している事例と見ることができよう。本発表では、埼玉県鷲宮町の事例(アニメ「らき☆すた」の聖地とされている。)からアニメ聖地巡礼者の情報環境に着目し、前後を含めたアニメ聖地巡礼行動で、どのような情報収集および情報発信を行っているかを整理する。
