In this paper, we made improvements and evaluated our proposed model for non-reference speech intelligibility estimation on reverberant speech, attempting to improve the estimation accuracy significantly. Our proposed method consists of a DNN for speech enhancement and a separate DNN for intelligibility estimation. The latter uses features obtained from enhanced and degraded speech to estimate intelligibility. Although previous studies have effectively estimated intelligibility for speech degraded by additive noise using similar models, they did not consider the degradation of distortion caused by reverberation. They also did not quantify the effect of various speech enhancement DNN models, the structure of the intelligibility prediction DNN, and the selection of parameters during feature calculation on estimation accuracy. Accordingly, we compared two top-of-the-line speech enhancement DNN models and used their output to train intelligibility prediction DNNs for reverberant speech while also varying the parameters used in the feature calculation. Consequently, the linear correlation coefficient between subjective and estimated intelligibility came to 0.801 with the best combination.

The interleukin 6 (IL-6) family of cytokines is defined by the usage of gp130, a common β-receptor signaling subunit, which promotes a variety of signals. They induce many biological functions on many cell types, including immune and inflammatory cells. They also exhibit hormone-like features, which are involved in homeostatic processes. Signal transducer and activator of transcription 3 (STAT3) is a significant signaling molecule fundamental in regulating IL-6/gp130 and is highly implicated in pathological conditions; therefore, STAT3 activation is tightly regulated through various mechanisms and at multiple levels. There is a large amount of information about STAT3-interacting proteins, which positively or negatively regulate STAT3 activity. This review is focused on IL-6-mediated signal transduction and the introduction of novel STAT3-binding partners. The review will help develop new strategies for clinically controlling the functions of IL-6/STAT3.

Magnetic fusion reactors need a superconducting coil system with the huge magnetic energy of approximately 100 GJ. A number of superconducting fusion experimental devices have been constructed since 1970. Accumulated technologies and experiences for the past 35 years now allow us to design the actual reactors. However, there still remain some engineering problems in order to design the commercial reactors that will need a low construction cost, a high field and high reliability. It is necessary to develop high-strength supporting materials, next-generation superconductors and high-current-capacity conductors.

Cette étude s'intéresse à la réception du Symbolisme en Wallonie et à la variante régionaliste qui s'y est développée. Le Symbolisme est l'un des mouvements artistiques qui a rencontré en Belgique le plus de succès en tant qu'emblème d'une identité « belge » définie comme le résultat d'une hybridation des cultures latine et germanique. Pourtant, Albert Mockel, l'initiateur wallon de cette esthétique d'origine française, est également impliqué dans le régionalisme wallon, qui s'oppose à cette conception. Certes, à partir des années 1890, la revue la Wallonie (1886-1892) dirigée par Mockel commence à perdre le rôle moteur qu'elle jouait jusque-là dans le champ littéraire en Belgique. Le symbolisme belge connaît alors un succès international grâce à l'exotisme « nordique/germanique » que l'on trouve chez les écrivains flamands d'expression française à Bruxelles. Néanmoins, ce passage d'un modèle à l'autre est révélateur du processus qui a engendré le « symbolisme wallon », également issu de la culture nordique/germanique. Mon propos s'appuie principalement sur l'analyse des oeuvres d'un écrivain symboliste wallon méconnu, Hector Chainaye (1865-1913), pour montrer comment son esthétique animiste a pu favoriser l'émergence du symbolisme de type nordique, sans tomber pour autant dans le régionalisme folklorique qui a fait plus tard le succès du « Symbolisme » dans le champ littéraire parisien. De plus, l'esthétique de Chainaye, résumée par la formule « l'âme des choses », entretient des similitudes étroites avec la série de dessins du même nom dus à l'artiste symboliste belge, Xavier Mellery, bien que l'on ignore si tous deux se sont rencontrés. L'esthétique du « symbolisme wallon » fut en fait léguée au « symbolisme belge » autour de Bruxelles à partir des années 1890. L'entreprise de création d'un symbolisme wallon apparaît ainsi comme l'une des sources essentielles ou comme un modèle du symbolisme belge, dont Chainaye pourrait bien être considéré comme le précurseur.

【目的】通所リハに通う要支援・要介護高齢者を対象に,低栄養と関連する要因を口腔・嚥下機能,食事摂取頻度に焦点をあて,低栄養の人数割合,口腔・嚥下機能,食事摂取頻度の調査,検討した研究である.これらの結果をもとに適切な栄養管理方法について考える.【対象者および方法】2017 年11 月から2018 年6 月まで,通所リハを利用している65 歳以上の地域在住高齢者で要支援1,2,要介護1~3 の118 名を対象とした.また,研究方法の理解が可能な者で,研究に同意を得られたものとした.研究デザインは横断研究である.調査項目は,栄養状態(Mini Nutritional Assessment Short-Form,BMI),口腔機能[ 最大舌圧値,摂食状況のレベル(FILS),オーラルディアドコキネシス(OD),咀嚼能力の評価],嚥下機能[改訂水飲みテスト(MWST),EAT-10,水分の一回嚥下量],食事摂取頻度(食品多様性スコア,食習慣)とし,栄養状態の評価より低栄養群,低栄養のリスク群および良好群の3 群に分け,統計学的解析を行った.【結果および考察】低栄養群が23 名(19.4%),低栄養リスク群が63 名(53.4%),栄養状態良好群が32 名(27.2%)であった.口腔・嚥下機能では,栄養状態と舌圧,水分の一回嚥下量,EAT-10 が関連していた(p<0.05).食品摂取多様性スコアでは,低栄養群で栄養状態良好群と比較して低い傾向がみられた(p=0.059).低栄養群では食品摂取の多様性スコアの点数が低く,特にたんぱく質の多い食品の摂取が少ないことがわかった.食物摂取頻度では,栄養状態と鶏肉および麺類に有意な関連がみられ(p<0.05),いずれも栄養状態良好群に比べて,低栄養群が少なかった.低栄養群は口腔・嚥下機能の項目および食品摂取の多様性スコアと関連がみられたことから,口腔・嚥下機能の障害が食事に影響していることが考えられる.【結論】低栄養は口腔・嚥下機能低下,食事摂取頻度に関連していた.要支援・介護になる前の段階から,口腔・嚥下機能低下予防を含めた低栄養対策を行うことが重要と考えられた.

うつ病治療において認知行動療法(CBT)が保険適応となり,改めて非薬物療法的アプローチが注目されている。本稿ではうつ病を例にとり,非薬物療法的アプローチの国内外のガイドラインにおける位置づけと,実臨床への応用の可能性について言及した。国内外のうつ病治療ガイドラインを概観すると,薬物療法は中等症例以上では推奨されているが,軽症例では次善の策に位置しており,むしろ当事者を適切な方向に導くような非薬物療法的アプローチが推奨されている。さらには当事者の背景や希望・嗜好などによって治療アプローチが決められることが望ましいとされる。その決め方としては,当事者と治療者がともに治療方針を決定するShared Decision Making (SDM)が推奨される。当事者が「安心・満足・納得」し,その結果レジリエンス(自己回復力)を刺激するようなアプローチこそ意義があると考える。今後,当事者を安心させる支持的精神療法の重要性も認識しつつ,より多くの非薬物療法的アプローチをわれわれ臨床医が治療選択肢に加えられるよう努めなければならないだろう。
