<p> 目的:新型コロナウイルス感染症(COVID-19)感染拡大により,摂食嚥下リハビリテーションの対面診療について,慎重な対応が求められた。本研究では,COVID-19感染蔓延下に対面での診療を中断した患者に対してオンライン診療を実施し,その有用性を検討した。</p><p> 方法:対象は,摂食嚥下リハビリテーションを専門とする歯科大学病院附属クリニックを受診する摂食嚥下障害患者であり,緊急事態宣言により対面診療中断となった患者21名とした。緊急事態宣言期間中にオンライン診療での嚥下訓練と食事指導を行い,期間中の肺炎発症,入院の有無,オンライン診療移行前と対面診療再開後での摂食状況(Food Intake LEVEL Scale:FILS),栄養状態を比較し検討した。また,アンケートでの意識調査を行った。</p><p> 結果:オンライン診療中に,FILSが向上した者は3名,低下した者は2名,変化のなかった者は16名であった。発熱を4名に認めたが,いずれも入院にはいたらなかった。体重減少率が3%以上の者はいなかった。アンケート調査では,オンライン診療の効果として,感染リスク低減や安心感が得られたと回答する者が多かった。</p><p> 結論:感染リスクを考慮した摂食嚥下リハビリテーションの診療形態としてオンライン診療は嚥下機能維持,向上に寄与し,また患者不安を低減した。オンライン診療での摂食嚥下リハビリテーションや食事指導は,対面診療を補完する診療形態として有用であることが示された。</p>

わが国の精神科治療は、欧米に比べてはるかにおくれてはいるが、入院中心から地域へと移行しつつある。慢性精神障害者が生活の質を高め、地域の中で適応していくのに必要な生活技能を学習する有用な方法として、生活技能訓練(SST)が治療の中に組み込まれはじめて10年になる。おもに入院患者を対象として実施されているが、その中には治療動機の乏しい患者、SSTの流れについていけないスキルの低い患者、さらにはストレス耐性が非常に低く、症状の増悪をきたしやすい患者なども含まれており、一筋縄ではいかないことがよくある。本稿では、臨床上遭遇するこのような困難なケースに対して、課題設定、フィードバック、モデリング、宿題などの技法をどのように使っていけばよいか、具体的な事例をあげながら筆者の考えを述べたい。

The Totsukawa dialect (TD) shares the accent of the Tokyo type, though TD is located in the area of the accent of the Keihan (Kyoto-Osaka) type. The accent of TD is derived originally from the accent of the Keihan type. Actually, the neighbouring dialects of Tanabe, Hongu, etc. exhibit the accent of the Keihan type clearly, namely they have more accent patterns than TD has.<BR>It can be concluded that the accent of TD has been transformed from the original type of acceet which had more patterns. This conclusion is supported by historical developments and the geographical distribution of dialects in the said area.<BR>As for other linguistic features such as segmental phonemes, morphology and vocabulary, TD is similar to the dialects of Tanabe, Hongu, etc.

本発明の目的は, 鋳造床程度の大きさの純チタンを電解研磨により鏡面にまで仕上げることである.従来から, チタンの電解研磨は, その強い酸化傾向のために困難とされてきたが, 今回非水系の電解液を用い, 被研磨体の形状や電解条件などに工夫を加えた結果, 30cm2程度の大きさの純チタン板を電解研磨により鏡面仕上げすることができた.まず, 小型試片の電解研磨条件を見いだした.しかし, 大型試片の場合には同じ条件では, 研磨面の中程に顕著な梨地を生じ, 均一な研磨はできなかった.梨地部分は, 表面あらさ値が大きく, このままでは生体材料に適さない.このような梨地は電解液面を貫く被研磨体の面積を小さくすることで阻止できることを見いだした.また, 純チタン鋳造体の電解研磨は, 加工材の場合と同一の研磨条件で可能であった.さらに, アルコールを主成分とする電解液の溶媒成分の配合を変えることによって, より安全で, ふかみのある鏡面研磨が可能で, 疲労しにくく, 使用回数などの制限の比較的緩やかな優れた電解液を開発した. 例 エチルアルコール70ml iso-プロピルアルコール30ml 塩化アルミニウム6g 塩化亜鉛25g 電解条件 電圧30V 通電時間6分 電解液温25℃

われわれは,以前に,画像サイズと視域角の両法を拡大するためのホログラム表示技術を提案している.これは,高フレームレートSLMをアナモルフィック光学系により縦長画像として結像し,これをガルバノスキャナで水平方向に走査する.今回は,不要光である共役像,ゼロ次回折光,および高次回折像の除去を行った.不要光の除去にはシングルサイドバンド法を用いた.垂直スリットと水平スリットを,再生光学系の垂直方向と水平方向のフーリエ面にそれぞれ配置した.実験により,不要光のないホログラム表示を実現した.

Objectives: The possible effects and benefits of oral rehydration solution (ORS) on chronic dehydration after total proctocolectomy. Methods: To evaluate the effect of ORS on the renin-angiotensin system after remnant proctocolectomy in patients with ulcerative colitis (UC), we selected 20 patients after remnant proctocolectomy, ileal J pouch-anal anastomosis, and construction of a diverting ileostomy for UC. Patients were randomly divided into two groups, A (n=9) or B (n=11), 2 weeks after the surgery. In group A, ORS (1000 mL/day) was given for the first 7 days and mineral water (1000 mL/day) for the next 7 days. In group B, mineral water (1000 mL/day) was given for the first 7 days and ORS (1000 mL/day) for next 7 days. Plasma levels of renin, aldosterone and excretion of sodium in urine were evaluated at days 0, 7, and 14. We defined day 0 as the day of beginning this study. Results: Mean plasma renin levels on day 0 were six to eight times greater than the upper normal limit. In group A, ORS lowered plasma renin levels. In group B, plasma levels of renin and aldosterone after ORS were lower than those at days 0 and 7. Conclusions: ORS corrected increased plasma levels of renin and aldosterone to within the normal range in patients after proctocolectomy.

タイルドディスプレイは,複数のディスプレイを格子状に並べることで大画面を低価格で実現する手法である.本研究では,モバイルディスプレイを用いてタイルドディスプレイを実現することによって,安価で持ち運びが容易なモバイルとしてのタイルドディスプレイを実現する.タイルドディスプレイの実現にはFPGAを用い,モバイル化するにあたってディスプレイの回転や縦長表示などスマートフォンから出力する場合の固有の問題点を解決していく.また,歩きながらの使用を可能にすることで,モバイルタイルドディスプレイを動くデジタルサイネージ(もしくは電子サンドイッチマン)として使用することも視野に入れている.

循環器疾患の発症に栄養は深く関わっている。栄養と血圧の関係を明らかにするための国際共同研究 (INTERMAP) の実施にあたっては, 国際的に標準化された主要栄養素, 脂肪酸, アミノ酸, 微量栄養素に関する食品成分表が必要である。我々は, 国際比較可能な栄養データを得られる食品成分表を得ることを目的に, 24時間思いだし法の実測値を用いて日本標準食品成分表の標準化を行った。

Classes were carried out for beginner learners of Japanese to check the effect that the differences in slide presentation has on the learners' understanding and thinking processes. They were taught using two kinds of methods; one is by presenting the illustration in the slide gradually, the other is showing it all at once. The results of the questionnaire given to the learners suggested that it might promote the understanding and thinking of the learners in the case of the former in the relatively more difficult grammar section, while the latter promotes understanding and thinking in the relatively easier grammar section.

アメリカ・ポピュラー・ミュージックのルーツを一般的によく言われる南部の黒人音楽であるブルースやゴスペルなどではなく、それとは別のカリブのフォーク・ミュージックへと辿る。1950年代、カリプソというカリブの音楽のブームがアメリカで巻き起こり、それがアメリカのポピュラー・ミュージックに大きな影響を及ぼす。しかしそれ以前にこの地域の音楽がすでに何らかの形で影響を与えていたのではないかを探るためにカリブ海の島の中から英語圏のジャマイカ、バハマとキューバを取りあげ、ニュー・オーリンズという他の町とは異なった当時の国際都市を通してどのようにアメリカへ影響を与えたかを考える。アメリカ・ポピュラー音楽の持つ屈託のない明るさという側面を作り上げた1つの要素はカリブの黒人のフォーク・ミュージックに由来し、その側面への影響力は南部よりこちらの方が大きかったことを議論する。

Corsica, French island in the Mediterranean Sea, was a transhumance based pastoral society of sheep and goat, however swine also was an important animal as a key of understanding traditional Corsican mountainous villages and their people called paese and paisani. Traditionally Corsican swine were divided into two types. Mannarinu was that which was fed in each household in paese. And Porcu di furestu or Porcu di banda was that which was raised collectively by the people called purcaghju, swine shepherd. Purcaghju has also a role of swine breeder, sells new born piglets to paisani and takes charge of feeding mannarini from sheep and goat shepherd or paisani. Tumbera, killing the swine is a symbolic and ritual activity of paese just before Christmas. However, Corsicans did not eat pork meat in their daily life traditionally. The pork gained by tumbera was almost transformed into preserved products for example ham and sausage. In the latter of the 20th century, two mainstreams about pork culinary and swine raising appeared in the island. The one is disappearance of a tradition caused by urbanization of eating life and decrease of purcaghji. And the other is several movements for official recognitions of “Porcu Nustrale”, Corsican original swine race and “AOC”, controlled designation of origin of several pork preserved products, coppa, lonzu and prisuttu.
