<p> 水産高校には乗船実習等の長期の校外実習があり,その中では授業も行われる。この実習期間中,生徒は自習により学力を維持することになるが,容易なことではない。そこで我々は自習を支援するツールとして映像授業に着目し,水産専門科目の映像授業を開発した。この映像授業の学習効果を測るために,水産高校の分野の異なる2つのコースの生徒を対象に介入実験を行なった。各コースの生徒をくじにより映像授業を受ける群(映像授業群)と教師から直接講義を受ける群(講義授業群)に分け,授業後に記述式のテストを実施した。その結果にt検定を行ったところ,映像授業は講義授業と同等以上の学習効果があることが示された。これにより,開発した映像授業は生徒の自習を支援し,学習効果が高いことが分かった。</p>

In patients with musculoskeletal and central nervous system diseases, even if the sitting and standing postures can be maintained to some extent, the instability of the hip joint and pelvic area may limit the muscle activity when the lower limbs are raised. By presenting electromyographic studies and images of the hip joint and pelvis during the above-mentioned movements, this study introduces points of clinical evaluation and treatment.

質問 ビタカンファーという注射は現在でも意味を持つ薬でしょうか.意味ありとすれば,どんな適応がありますか.(北海道 Y生)答 ビタカンファーの適応症は大別して,強心・血管緊張・呼吸中枢興奮・吃逆制止とされている.その薬理作用を要約すると,小量使用により心拍動は増強され,増量することで呼吸中枢も刺激され,さらに増量すれば血管運動神経をも刺激し,この結果,末梢血管の収縮と血圧の上昇を生ずるとされている.この薬効に関して,研究者間に見解の差が見られる1).

的確な齲蝕予防法を検討する目的で,保健所で行われた健康診査から,1歳6カ月と3歳時の資料(451人分)を用いて,菓子類,飲物の摂取と歯磨き実施とが3歳時の乳歯齲蝕にどのようにかかわっているかを観察し,以下の結果を得た。<BR>1)3歳時に菓子パン,ビスケット,アイスクリーム,ケーキ類,市販のジュース,チョコレート,乳酸飲料,炭酸飲料,スナック菓子,あめなどの菓子,飲物類を摂取するものは齲歯有病率が高い。<BR>2)1歳6カ月時の菓子類摂取では,あめの摂取が最も強く齲歯発生と関係していた。<BR>3)1歳6カ月と3歳の両時点の菓子摂取では,いずれかの時点であめ,チョコレート,ケーキを摂取していたものの齲歯有病率が高かった。<BR>4)歯磨きの実施時刻別に,歯磨き実施と齲歯発生との関係を注意深く観察したが,両者に明らかな関係はみられなかった。<BR>5)因子分析により歯磨き,菓子類,飲物の摂取と齲歯発生との係わりを総合的観察した結果,菓子類の摂取と齲歯発生とは明らかな相関がみられた。しかし,歯磨きと齲歯発生との関係を明らかにすることはできなかった。

Corticosteroids or steroids are useful drug in clinical renal transplantation because of multiple effects in immunosuppression. There are not a few renal transplant recipients who need steroid therapy, although various new immunosuppressive drugs have developed. However, steroid dosage should be kept as low as possible and preferably withdrawn, because adverse effects of steroids are the most problematical in immunosuppressive drugs. The individual difference of efficacy and adverse effects of steroids depends on pharmacodynamics factor. We have been tried to resolve the dual nature of steroid therapy by steroid sensitivities test using peripheral blood mononuclear cell(PBMC). We clarified significant relationship in steroid sensitivity and clinical outcome, relative immunosuppressive efficacy of steroid, availability of methylprednisolone as immunosuppressive drug, utility for selecting patients who can sustain steroid withdrawal, and pharmacodynamic interaction of calcineurin inhibitors and steroids. We describe here not only results of pharmacodynamic research of steroid sensitivity test, but also comprehensive issues, i. e., history of steroid therapy in renal transplantation, action mechanism of steroid and factor of individual difference of steroid sensitivity, pharmacokinetics of steroid, suppression of hypothalamic-pituitary-adrenal system and immune system, adverse effects of steroid, and practice toward optimal steroid therapy for individual renal recipients.

<p>Today changes are so frequent and extensive so that we cannot predict the future. To cope with this situation, we have to team up to solve the problems. But if we team up with 11 best players, the team will not be the best. The best team is realized when every member of a team can adaptably change his role. Such an adaptable team will be the final winner. But our engineering education has focused on how we can grow up best players and we did not pay too much attention to how we can make our team adaptable enough and how we can increase such adaptability of each player. We should make our efforts to grow such adaptable engineers.</p>

Bipolar disorder is one of the mood disorders and is a mental illness that alternates between depression and manic state. It is important that the patients become aware of early symptoms for effective prevention of disease recurrence. In this study, we qualitatively analyzed narratives of patients to investigate how they understand the disease of their own and what they typically experience early in the disease recurrence.Nine patients with bipolar disorders were allowed to talk freely about their disorders. In order to study the disease from the patient's point of view, we used a narrative interview method, and the data were analyzed using SCQRM. Based on our analysis, we developed 33 concepts and ten categories, which were further grouped into three core categories.Through this study, we learned that it is difficult for patients to become aware of their hypomanic state preceding the depression, and to recognize the cause that lead to a change in the disease phase. Further, our study suggests that narrative interviews help patients realize and talk about these aspects that are usually overlooked.
