We herein report an unusual case of spontaneous parathyroid gland rupture. A man was admitted with respiratory distress in September 2010. He had been receiving hemodialysis since 1995. He was diagnosed secondary hyperparathyroidism in 2006 and began receiving cinacalcet therapy in 2009. His intact parathyroid hormone (iPTH) level decreased, and massive traumatic bleeding occurred, following which rupture of the parathyroid gland was detected during surgery. The ruptured gland showed nodular hyperplasia. Previous reports have indicated that parathyroid bleeding is associated with glandular hypertrophy. This is the first report of parathyroid apoplexy occurring after suppression of elevated parathyroid function caused by cinacalcet therapy.

症例は73歳, 女性. 2008年11月から, 息苦しさ, 顔面, 下肢の浮腫が出現した. 入院時心電図は心拍数42/分, 2対1房室ブロック, QRS幅90ms, 胸部写真にてCTR 55%, 肺血管陰影の増強あり, 正常冠動脈, 左室収縮能は良好で, 徐脈によるうっ血性不全と診断した. 血清カリウム値2.6mEq/Lと低下していたため, カリウム製剤の投与を開始した. 入院直後のホルター心電図では, 完全房室ブロック, Mobitz II型第2度房室ブロックも認めた. 第2病日には第2度以上の房室ブロックは消失した. 第5病日には血清カリウム値は正常化し, 以後低カリウム血症の再燃はなかった. 低カリウム血症をきたす内分泌・腎疾患を認めず, 原因は不明であった. 電気生理学的検査にて洞調律時に分裂ヒス電位を認め, ピルジカイニド25mg投与後に2対1ヒス束内ブロックとなり, ヒス束内伝導障害を認めた. 房室結節伝導は正常であった, ペースメーカーの適応については, 房室ブロックに対する低カリウム血症の関与の有無にかかわらず, ヒス束内伝導障害があるためclass IIaの適応と考え, 永久的ペースメーカーの植え込みを行った. 本例は, もともとヒス束内伝導障害があるうえに, 低カリウム血症によりヒス束の不応期の延長あるいは第4相の過分極をきたし房室ブロックをきたしたと推測した.

低Ca血症に伴う症状は四肢のしびれ感やちくちく感といった軽い知覚異常からテタニーや痙攣といった神経・筋症状まで様々である.また先天性PTH不応症のように特徴的な体型から疑診される.したがって患者からのちょっとした症状や徴候からも低Ca血症を疑い,積極的に血中のCa, P,アルブミン濃度を測定し,心要であれば血中intact PTHを測定することが,本症を見逃さないコツである.

Since Aristotle, people have believed that metaphors and similes express the same type of figurative meaning, despite the fact that they are expressed with different sentence patterns. In contrast, recent psycholinguistic models have suggested that metaphors and similes may promote different comprehension processes. In this study, we investigated the neural substrates involved in the comprehension of metaphor and simile using functional magnetic resonance imaging (fMRI) to evaluate whether simile comprehension differs from metaphor comprehension or not. In the metaphor and simile sentence conditions, higher activation was seen in the left inferior frontal gyrus. This result suggests that the activation in both metaphor and simile conditions indicates similar patterns in the left frontal region. The results also suggest that similes elicit higher levels of activation in the medial frontal region which might be related to inference processes, whereas metaphors elicit more right-sided prefrontal activation which might be related to figurative language comprehension.
