Mycobacterium marinumは,皮膚に結節や潰瘍を形成し,時に深部に進展して関節炎,腱滑膜炎,骨髄炎などを来すことがある.症例はprednisolone10mgを内服中の60歳代女性.手関節の腫脹で発症し,広範な皮膚潰瘍と四肢深部膿瘍を呈した.難治性の皮膚潰瘍をみた場合,抗酸菌感染症も念頭に置く必要がある.

組織学的にサルコイドーシスが疑われた抗酸菌症を経験した.症例)67歳男性.既往:糖尿病.右手背部腫瘤を主訴に来院した.右手背部に2.5×3.0×0.5cmの弾性軟の腫瘤を認め,軽度圧痛を除くと血液検査,X線検査上,有意な所見はみられなかった.MRIでは手背~手関節に伸筋腱腱鞘の腫瘍性肥大と液体貯留を認めた.軟部腫瘍,感染等による肉芽腫を疑い,切開生検を施行した.肉眼的には腱鞘周囲に腫瘍性病変がみられた.病理検査では壊死を伴わない類上皮細胞様の細胞増殖が主体をなしサルコイドーシスと診断された.手術後3週間でMycobacrerium marinumが検出され,抗菌薬治療を開始した.考察)本症例では病理組織学的に腫瘤内に乾酪壊死を検出できなかったので,積極的に抗酸菌症は診断されず,サルコイドーシスの所見と考えられた.しかし抗酸菌症とサルコイドーシスの治療法は全く異なるので鑑別は重要である.診断不確定な腫瘍では適切な病歴聴取,MRIや抗酸菌検査を含めた細菌培養が望まれる.

Advocacy in nursing, is a central concept in terms of protecting the interests and rights of patients as well as establishing and improving the expertise and social status of nurses. In fact, advocacy is a role played not only by nurses, but by all medical professionals. Advocacy is an important concept for nursing with extensive meaning and contents. Consequently, confusion can occur due to the uncertainty advocacy poses for nursing practice. Moreover, advocacy is accompanied by risk and difficulty, and there are no clear guidelines on how to implement it. It is necessary to enhance education and improve the workplace environment to practice advocacy effectively. It is also essential that nurses reconsider their values and how they support patients while collaborating with other medical professionals.

This paper aims to elucidate how the meaning of words would change by the experience in using figurative expressions on the basis of later Wittgenstein's considerations. The various figurative expressions he investigated can be divided into three categories, metaphors in thinking, metaphors in perception, and “the secondary meaning”. I insist that all of these are brought about by the notice or forefeel of resemblances which is also needed in aspect-switching and conclude that the experiences of meaning could be the driving forces for changes of language games.

We describe three short stories around Gödel's incompleteness theorems. Firstly, we show that the second incompleteness theorem is derivable by formalizing Yablo's paradox. Then, we prove a version of the incompleteness theorem which is not formalizable. Finally, we discuss the theorems of arithmetic in nonstandard models of arithmetic and the definability of truth in models of arithmetic within other models.

D. Hilbert raised the problem to prove consistency of formalized mathematics. In this paper I will expound its background and the status quo of the consistency proofs. First let me explain the intended significance of proving consistency, and a background idea to divide mathematical objects in two, i. e., transfinite/finitary objects. Next I will discuss impacts of the incompleteness theorems to the problem and their interpretation. Finally let me give sketches of the practice of the consistency proofs due to G. Gentzen et al. and their significance, and the present state of the consistency proofs.
