Erectile dysfunction following radical prostatectomy (RP) is still a significant burden as a post-operative morbidity, despite advances in nerve-sparing techniques and penile (erectile function) rehabilitation (PR) programs. We assessed the effects of stimulation with the masturbation device "EGG" on enhancement of erectile response along with administration of phospho diesterase type 5 inhibitor. We also studied the change of self-esteem and motivation for continuation of PR after stimulation with EGG. Eight nonresponders for PDE5-I who underwent retropubic RP were enrolled. Patients' median age was 71.5 years old. No patients received adjuvant therapy for prostate cancer. The patients' erectile response in the penile rehabilitation session (masturbation) with PDE5-I+manual stimulation and PDE5-I+stimulation with EGG were evaluated by erection hardness score (EHS). Changes of self-esteem and motivation for penile rehabilitation were assessed by the self-esteem subscale of the Self-Esteem and Relationship (SEAR) questionnaire and one original question, respectively. PDE5-I + stimulation with EGG significantly enhanced EHS compared to PDE5-I+manual stimulation in the eight patients (p=0.027). Transformed score of self-esteem subscale score of SEAR questionnaire was significantly increased in the PR session with EGG compared to the PR session with manual stimulation (p=0.043). Six patients who showed a better erectile response with EGG retained motivation for continuation of PR. PDE5-I+stimulation with EGG improved the erectile response in post-RP patients. EGG as a masturbation device may have a potential for contribution to successful PR.

1920 年アイルランド統治法の施行によって、北アイルランド政府・議会は独自の立法権限、財政権限などを獲得し、イギリスは実態として「連邦制国家」となった。法施行直後は、これを機能させようとした勢力もいたが、第二次世界大戦を経て、1920 年法は形骸化するに至る。本研究は、この間の経緯を、財政権限委譲論議を中心に分析することで、イギリスおよびイギリス帝国における統合と分離の力学を明らかにする。

概要:ビデオゲームエージェント(COM)の振る舞いのデザインにおいて,『強い』COM の自律的獲得は「熟達者に勝つ」という目標を達成しつつある.一方で,獲得されたCOM の振る舞いは,過度に最適化され機械的に感じるという課題が浮上している.この課題を解決するため,著者らは,『人間の行動原則』を課した強化学習や経路探索により,人間らしいCOM を自律的に構成するフレームワークについて提案してきた.しかし,それらのCOM が本当に人間らしいと解釈されるかどうかの検証が不十分であった.本論文では,自動獲得されたCOM の振る舞いについて主観評価実験を実施する.

1.「大分果研4号」は、1995年に大分県柑橘試験場津久見分場(現大分県農林水産研究センター果樹研究所津久見試験地)において高糖系温州「大津八号」にタンゴール「天草」の花粉を交配して育成した早生カンキツで、2007年3月に種苗法に基づき品種登録を申請し、2009年3月6日に品種登録された。2.樹勢は中程度で、樹姿は開張性である。枝梢の太さは細く、長さは短く、密度はやや密である。結実し始めると樹勢が落ち着き、枝に発生していた短いトゲはほぼ消失する。3.単生花序を形成し、花弁は5枚で白色の紡錘形。花糸の分離程度は一部合一で、花糸の数は少なく、花粉が少しある。4.果実の形は扁球形で、果形指数は115。果頂部の形は陥没しており、凹環は不明瞭。果梗部は球面で、放射条溝は無である。果実の重さは170g程度で、果皮の色は濃橙。油胞の大きさは中で、果面の粗滑は滑〜中、果皮の厚さは中で、はく皮は易である。じょうのう膜は軟らかく、さじょうの形・大きさはともに中である。5.果汁は多く、12月中旬の果実品質は、果汁歩合85%、糖度(Brix)12.0、クエン酸0.82%と、酸切れの早い品種である。香気は中程度のオレンジ香がある。種子は中程度入るが、他家受粉がなければ少となる。6.収穫適期は、果皮が完全着色となり、クエン酸が0.8%程度となる11月下旬頃から12月中旬までである。7.着花は良好で、隔年結果性は低い。また、果皮が弱く、果肉が軟らかいので、収穫時期の果実の取り扱いには注意が必要である。8.温州ミカンの栽培可能な地域であれば栽培は可能であるが、減酸が早く年内出荷が可能な早生カンキツとしての特性を十分に発揮させるためには、比較的土壌が浅く、排水、日照が良好な緩傾斜地での栽培が望ましい。

The J-integral-based technique for measuring strain-softening curves is applied to determine the fracture toughness of granite. The measuring method has been recently proposed by Li for concrete. This method provides information on the fracture process zone formation and can be used for fracture toughness determination in strain-softening materials. CT specimens of three different sizes, 1.5inch, 2.5inch and 6inch CT were tested to examine the specimen size effect of measured strain-softening curves. Strain-softening curves measured on 2.5inch CT specimens are shown to be in good agreement with that determined from 6 inch CT specimens, indicating the possibility of determining the valid fracture toughness value by means of the J-integral-based technique. The test results for 1.5inch CT specimens, however, yield a lower fracture toughness value than that obtained on the other specimen sizes, suggesting the need of detailed investigations on specimen size requirements for the reliable use of subsized specimens. Finally, the strain-softening curves obtained by the J-based technique are compared with existing data determined by uniaxial tensile tests on several types of granite.

Sixteen cases of testicular torsion were experienced between 1983 and 1988. Diagnosis was confirmed by surgical exploration in all cases. The patients' age was between 10 and 31 years. Fourteen patients (88%) were in their second decade. The left side was involved in 13 cases, the right side in 2 and both sides in 1. Shortness of spermatic cord of involved side was observed in 12 cases (75%). This seemed to be one of the most important signs in differential diagnosis between testicular torsion and other acute scrotum. Prehn's sign was not available. Body temperature was more than 37 degrees C in only 1 case. Urinary tract infection was not noted. Orchiopexy was performed in 12 cases. 9 cases (75%) were treated within 24 hours after onset of the symptom. Two cases (13%) were operated more than 72 hours after onset, but repaired testis was not atrophic one year after surgery.

従来,ケアに対する考えかたは,「気づかい」「世話」「思いやり」など好意的側面が多く,マイナス要因に目を瞑っているように思われる。昨今の介護殺人,介護難民,老老介護は「ケア」におけるドメスティックな問題に止まらず,大きな社会問題となっている。最近ではマスメディアで「無縁社会」として大きく取り上げられている。筆者は「家庭介護」における介助者の視点の観点からケアを考えることで,ケアを外部化せざるを得ない問題点を着地点とした。また,この危機的状況を乗り越えるために,1つの問題点と6つの打開策を定義した。本論は「ケア」の倫理的側面の再認識,共感と傾聴の現実,社会学及び看護学の「感情労働」,臨床心理学の「共感疲労」の思考形式を学際的に捉え,介護において有効性がある定義を提案することにより,介護の現実問題を打破する倫理的試みをした。そして今日の日本社会の闇に一石を投じることで問題を浮き彫りにすることが本論の狙いである。Although many studies on care have focused on its positive implications such as consideration, tender-heartedness, and sensitivity, they, in my view, failed to examine negative factors involved in the practice of care.These negative aspects, typically seen in nursing murder, nursing refugees, and old couple nursing, are now not only domestic but also big social problems. These current problems have urged me to rethink about the ethical aspect of care.Considering that apathy in self-care is one of the most serious problems, I suggest in this paper that six factors are significant in solving the problem: flexibility of caring, the setting of border for close listening, a space appropriate for emotional expression, buffer distance, meta-recognition sympathy, and short-distance reliance.By so doing, I illustrate the importance of ethical value in nursing. I also show an ethical approach to solving the practical problems by examining the connection between compassion and close-listening, "emotionlabor"addressed in sociology and the science of nursing, and compassion fatigue in clinical psychology.Note) I call kaigo "nursing".

Spontaneous vapor explosion can occur when a layer of the high temperature molten material lies on the water pool or on the moisture floor. This is so-called base-triggered vapor explosion. The base-triggered vapor explosion is supposed to occur in the case of a severe accident in a nuclear reactor and in other industrial facilities. It is very important to clarify the base-triggered vapor explosion from the viewpoints of the prediction and the prevention of the vapor explosion. In order to evaluate the heat transfer and fluid dynamic behavior of the base-triggered vapor explosion, the experimental apparatus is designed and constructed. The experiments using U-Alloy95 as a stimulant material are conducted. Consequently, the behavior of the molten material can be observed in detail with this experimental apparatus. The digital auto-correlation method and PIV are also applied to the visual observation data obtained on the experiments in order to evaluate the velocity distribution of the molten material. Based on the velocity, the conversion ratio of kinetic energy in initial thermal energy at the vapor explosion is also evaluated. In addition, the experimental results are compared with the thermal interaction zone (TIZ) theory in order to evaluate the occurrence condition of base-triggered vapor explosion.

カンゾウ(Glycyrrhiza glabra Linn.)は甘味剤, 胃腸薬などとして用いられる重要な生薬で, 日本における消費量は増加傾向にあるが, その大部分を海外, 特に中国からの輸入に頼っている. 2000年の日本のカンゾウ総輸入量は4151tで, このうち中国産は3250tを占め, 中国からの輸入は過去10年で最高となっている(厚生労働省, 2002). その一方で, カンゾウの乱獲による環境破壊が中国西北地域一帯で進行しつつあり, 中国政府は現在まで実施していたカンゾウの輸出許可制度を活用した輸出総量規制, 輸出港の限定による無許可輸出の防止, 輸出許可取得料の値上げ, 生産地に対する管理, 規制などを進めている. そこで, 中国では野生カンゾウの採集に代わり, 栽培によるカンゾウの生産を実施しているが, かならずしも日本薬局方の定めるグリチルリチン酸含有量を満たしていない. 一方, 個人輸入業者が主として販売する未承認医薬品による健康被害があとを絶たないことから, 既存の漢方や生薬においてもより厳密な品質管理が求められる可能性が考えられる.

鈍的腹部外傷による小腸・腸間膜損傷の病型は,破裂・断裂・挫傷の3型に分類される.腸管の破裂や断裂は遅発性のものを除けば診断は比較的容易で,多くは開腹に至り救命しうる.一方,挫傷は症状も軽微で何らの後遺症も残さず治癒することが多い.しかしまれな合併症として遅発性穿孔や腸狭窄をきたす場合がある.今回われわれは外傷性小腸狭窄を呈した2症例を経験し,術前診断が可能であったので文献的考察を加えて報告する.
