栄養失調とサルコペニアを伴う高齢者に対し,運動療法と栄養療法によるマネジメントの必要性が言われている.66歳男性,肺腺癌のため右肺全摘出後,外来通院していた.混合性換気障害と呼吸器症状が進行し,体重減少および身体活動性の低下を認めるようになったが入院を拒否.サルコペニアが疑われ外来で栄養士と看護師が介入し,栄養状態は改善した.しかし自覚症状は改善せず,理学療法士が介入を開始した.4ヶ月間の介入によりAMC(19.9→20.9 cm),MIP(31→68 cmH2O),握力(20→22 kg),CAT(12→9点)と筋力およびCATの改善を認めた.今回,入院が困難なCOPD症例に対し,外来で多職種が介入することで全身状態を改善させ,入院を回避することができた.栄養失調を伴うサルコペニアが疑われるCOPDに対する,外来における栄養および運動療法の有用性が示された一例と考えられる.

A practical synthesis of 3'-O-benzyl-2'-deoxy-5-trifluoromethyluridine (1), a candidate antitumor agent for clinical testing, was developed from 2'-deoxy-5-iodouridine (3). Benzylation of 2'-deoxy-5-iodo-5'-O-trityluridine (14) with benzyl bromide and sodium hydride in tetrahydrofuran gave the 3'-O-derivative (16). Benzoylation of 16 afforded the N^3-benzoyl derivative (17). Coupling of 17 with trifluoromethylcopper, prepared from bromotrifluoromethane and copper powder in the presence of 4-dimethylaminopyridine, gave the 5-trifluoromethyl derivative (19) minimally contaminated with the 5-pentafluoroethyl compound. Deprotection of 19 furnished 1.

本研究の目的は,床冷房条件の違いが人体に及ぼす影響を被験者実験により明らかにし,床冷房時の至適域を提案することである.実験は,気温と床表面温を組み合わせた条件に,被験者6名を用いておこなった.その結果,以下の知見を得た.1)熱放射と熱伝導の影響を組込んだ伝導修正作用温度は,熱放射と熱伝導の効果を気温換算して的確に表現し得る温熱環境指標であることを示した.2)床冷房の冷却の効果により,全身の温冷感申告値が中立温冷感よりも暑い側を好む傾向を明らかにした.3)投げ足位姿勢での床冷房の至適温熱環境条件は,伝導修正作用温度が25.3〜30.5℃の範囲となることを提案した.

切り餅の食味官能試験方法を確立するため,新潟県作物研究センターの生産力検定試験供試材料を用いて検討した結果を報告する.試料米の調製:供試玄米を粒厚1.85mm以上の整粒に調製した.調製した玄米を搗精歩合89%に搗精し,精白米を10℃で1週間以上貯蔵して,米粒水分の均一化を図った.切り餅製造:家庭用製餅器(SD-3603,ナショナル)を使用し,以下のように行った.規定量の白米を洗米し,15時間吸水させた.脱水後,餅つき機に規定量の蒸し水を注入,うすにもち米を移してセットし,以降,自動操作で餅をついた.ビニールを敷いた型枠に餅を移し,上からさらにビニールをかけて,のし棒で1.5cmの厚さに圧延した.一昼夜放冷後4×6cmに裁断し,切り餅を作成した.官能試験方法:供試点数は基準のわたぼうしを含めて4点とし,熟練した15名以上のパネルで実施した.水温75℃で10分間ゆでた餅を皿に盛り,外観・白さ・コシ・粘り・のび・味・舌触り・総合評価について,11段階で評価した.平均値の有意差検定により,,基準との差で判定を行った.生産年が異なる育成材料および比較に供試したこがねもちの食味評価から,本法による切り餅の食味官能試験方法は有効であると判断された.

The upgrade of JT-60U to the superconducting tokamak “JT-60SA” has been carried out as a combined project of JAEA's program for national use and the Satellite Tokamak Program in collaboration with EU and Japan fusion communities. The JT-60U torus was dismantled so as to install the new JT-60SA torus at the same position in the torus hall. JT-60U used deuterium for 18 years, so the neutron yield reached about 1.5×1020 (n) in total. The dismantling project of JT-60U was the first decommissioning experience of a fusion device with radioactivity in Japan. The project was intended to demonstrate decommissioning technologies and work activities, and to acquire experience and data on the technologies as well as to implement the dismantling project safely. Moreover, all disassembled components were stored with the data such as dose rate, weight and kind of material being recorded, so as to apply the clearance level regulation in the future. The lessons learned from the dismantling project indicated that the cutting technologies and storage management of disassembled components were the key factors in conducting the dismantling project efficiently. After completing the dismantling project, efforts have been made to analyze the data for characterizing disassembling activities, so as to contribute to the estimation of manpower needs and the radioactivity of the disassembled components from other fusion devices.

A questionnaire survey of medical institutions Chiba Prefecture, Japan, specializing in liver diseases was conducted. This study aimed to provide an overview of the current hepatitis virus tests situations and measures taken to link test-positive individuals to specialists. The positivity rate in these institutions was high, with 2.2% for hepatitis C antibody (HCVAb) and 0.9% for hepatitis B surface antigen (HBsAg). Although many institutions (70%) had been employing linking measures, the consultation rate with hepatologists was low, with 7.6% and 14.3% for HCVAb- and for HBsAg-positive cases, respectively. Only half of the institutions disclosed that their measures were working well. These data suggested the importance of improving the system for determining test-positive individuals and promoting hepatologist consultation.

The material plant of a tinny basket excavated from the San’nai-maruyama site (early Jomon period) of Aomori, Aomori Prefecture, was identified as a stem of a monocotyledon in 1993. On the other hand, a further observation of the basket suggested that the studied material did not derive from the basket. Thus, a newly obtained material of the basket was reexamined anatomically and was shown to be the torn inner bark of a Cupressaceae tree. The original material formerly identified as a monocotyledonous stem was re-identified as a petiole of a fern akin to Arachniodes standishii (Dryopteridaceae). Furthermore, a rope excavated from the Sakuramachi site (middle Jomon period) of Oyabe, Toyama Prefecture, was also made of the same material. These results show that the original sample of the basket was a contamination from sedimentary deposits. This is the first report of a basket made of conifer bark and a rope made of fern petioles in the Jomon period of Japan.

背景.炎症性筋線維芽細胞腫inflammatory myofibroblastic tumor(IMT)は稀な低悪性度の軟部腫瘍であり,術前診断しえた例は非常に少ない.症例.39歳,男性.健診での胸部異常陰影の精査目的で当院を紹介受診された.胸部CTでは造影効果を伴う左B8をほぼ閉塞するような境界明瞭な4 cm大の腫瘤影を認めるも,血液検査では炎症反応の亢進や腫瘍マーカー値の上昇を認めなかった.気管支鏡検査では左B8に発赤を伴う腫瘤の露頭を認めた.経気管支針生検transbronchial needle aspiration(TBNA)にてリンパ球などの炎症細胞の浸潤を伴った細胞密度の高い紡錘形細胞の増生を認め,α-smooth muscle actin(αSMA)とanaplastic lymphoma kinase(ALK)陽性であったためIMTと診断した.結論.本症例のように若年者の孤発結節影を認めた場合,IMTを鑑別にあげる必要がある.TBNAとALKの免疫染色は,術前診断に有用な手法であると考えられた.
