【目的】嚥下障害の合併症の一つに窒息があるが,これまで院内発生例に関する報告は少ない.急性期病院における窒息事例を解析検討することで,今後のリスク管理に生かせる因子を明らかにする.【対象と方法】20xx 年4 月から3 年間,当院の入院患者で医療安全部にレベル3b 以上の窒息事例として報告があった例を診療録,看護記録より後方視的に検討した.対照群は同期間の入院患者で平均年齢,入院科を窒息群と合致させ,他は無作為に抽出した130 名とした.【結果】窒息事例は5 例,入院科は整形外科2 例,内科3 例であった.年齢分布は68 歳から94 歳,平均83.4 歳であった.窒息は既往症に嚥下機能低下をきたす可能性のある疾患を有し(5 例中4 例),入院前の食事摂取が自立していた患者(全例)で起こっていた.ADL(歩行,排泄,日常生活の自立度)は自立と介助の境界レベルの患者が多かった.窒息イベントは入院早期に起こり(入院後平均9.2 日),発生時の食事形態は常食もしくは軟菜であり,嚥下調整食で窒息した例はなかった.窒息群では5例中4例(80%)で利尿薬の内服があり,対照群での内服割合130 例中31 例(23.8%)より有意に高かった.この期間の65 歳以上に限定した入院患者数は11,381 名で,65 歳以上の入院患者が院内で窒息を起こす確率は0.04%,おおよそ2,500 人に1 人の割合であった.【結論】院内発生の窒息事例は経口摂取自立者で,既往に嚥下障害リスクを有し,常食もしくはそれに近い食形態の物を食べている,入院早期の高齢者患者に起こりやすい.

昆虫による皮膚疾患には刺咬症,体液による接触皮膚炎,真性皮膚寄生などがある.刺咬症は吸血時に口器から注入する唾液や,毒針(毛)から注入する毒に対するアレルギー反応で,口器や針の物理的刺激や唾液や毒の薬理作用が複合して発症する.原因となった虫が特定できない場合が多く,皮疹の性状,分布,発症時期,場所などの臨床症状から原因虫を推定して診断し,昆虫の生態を理解したうえで生活指導をする必要がある.

1995年と96年の公務災害に申請された針刺し・切創事故をEPINet日本語版を用いて解析を行った.それに基づき携帯型針捨て容器の導入, 病棟で使用する滅菌処置セットに滅菌済みの膿盆を組み込むこと, ゴム栓よりの真空採血用にルアーアダプターを採用などの改善を行った.原因器材としてディスポの注射器針, 翼状針, 留置針, 真空採血針の全体に占める割合は2年平均14件全体の65%から6件30%に減少した.携帯型針捨て容器はコスト的にも100床あたり月5000円程度で一般病院でもまず試みうる対策と考えた.

要旨:ハイヒール用インソールの使用が,ハイヒール着用時の前足部荷重圧と運動機能へ与える影響を解明することを目的とした。ハイヒールを常用しない健常女性8名を対象とし,ヒール高9㎝のハイヒールに対して,ハイヒール用インソール(リアライン^Ⓡ・インソール・フェム,以下フェム)の有無で片脚立位時間,片脚連続ジャンプ回数,片脚立位時における前足部最大圧および前足部平均圧,階段昇降時間を比較した。片脚立位持前足部最大圧が統計学的有意に減少した。フェム使用による主観的な不快感を訴えたものはいなかった。フェムの使用がハイヒール着用時の前足部の疼痛減少につながる可能性を示した。

In recent years, there has been an increase in social interest in maltreatment for children, but there are not many studies on maltreatment in facilities such as kindergartens and nurseries. The purpose of this study was to explore organizational factors related to maltreatment in early childhood education and care facilities. We conducted an online survey of 200 kindergartens or nurseries workers (56 men, 144 women, average age 41.84 years) regarding “maltreatment”, “teamwork” that meant harmony in the organization, and “openness of the organization”. As a result of analysis of variance with “teamwork” and “openness” as independent variables, and “maltreatment” as a dependent variable, a significant main effect of “teamwork” was detected. It suggested that higher teamwork produced lower maltreatment. Based on the results, discussions were made on the mechanism by which maltreatment occurs in the facility and future research issues.

Study on the dietary habits and life of zen monks at zen monastery Empuku-ji was conducted to clarify the health conditiouns zen monks. As giho Nishikata,great master of Empuku-ji once said. "temples and monasteries were said to be 100 years behind the time, but now they are in the van of the time,"the rusults of our study reveal the value of zen diet which has beendeveloped with much wisdom. The results obtained can be summarised as follows; 1) the energy consumption of zen monks was quite high both on routine labor days and the days called ohzesshin. This high energy consumption is one characteristic of the dairy life of the zen monks at the zen monastery studied. 2)The dietary habits at a zen monastery closely resembled those of vegan vegetarians. The deietary inteke of zen monks from their regular meals was not suffcient for their necessary dietary allowance. 3) In addition to their regular simple means, they have a special meal almost once in every five days. This shows thier traditional dietary pattern of having meals at a zen monastery,which has been succeeded over 700 years.4) The health conditions of zen monks were good. 5) Most of their guest meals and meals for special occasions had a large number of dishes with a small quantity for each. These vegetarian deiets were of high nutritive value,and a variety of foods were assorted in each dish to give relish to its color, savor anr aroma. 6)Zen vegetarian diet was characteristically composed of tactful combination of so called " five cooking materials" , "five colors", "five savors", and that are "hot and cold" and "hand and soft".

The purpose of this research was to estimate the difficulty levels of tasks in arithmetic calculations in order to investigate the optimum arrangement of the tasks for teaching. The six forms of tests were constructed so that each form should include items of the calculation tasks for each grade at the elementary school level, and they were administered to 2822 pupils. By means of the two-parameter logistic model, the item parameters were estimated for the 221 items contained in the six forms of tests. The rusults showed that the model was satisfactorily fitted to the data. Furthermore, inappropriateness of conventional order of calculation tasks in teaching was also discussed.

われわれは学校や職場において,さまざまな評価を受けている.入学試験や採用試験などテストによって,一人ひとりのその後の人生を左右する評価がなされることもある.心理データの中でも特にテストの結果について,データを読み解く能力,心理データ・リテラシーが,テスト作成者,テスト利用者,受験者には必要である.ここでは,テストに求められる公平性,妥当性,信頼性の観点からテスト・データを読み解く上での留意点について示す.
