The Studies of British sports history mention the reform of sports and recreational activities for urban residents by middle-class people trying to promote rational recreation. However, it has not been clear how the sports environment needed for sports and recreational activities was developed. This study focuses on the Metropolitan Public Gardens Association (MPGA), which established urban parks and recreation grounds for city residents in late 19th century London. Specifically, this study will clarify the actual situation of one of its major projects, the conversion of a disused burial grounds into an urban park and recreation grounds. The primary source material used in this study was the MPGA's Annual Reports (1884-1904). The results showed that the MPGA was not only developing the disused burial grounds as parks, but also carrying out actions to make them more comfortable and accessible areas for recreation, and to promote exercise opportunities people including school children. Such projects were implemented especially in poverty affected areas.

【目的】脊髄小脳失調症(spinocerebellar ataxia: 以下、SCA)患者個々のバランス能力を評価するために、Balance Evaluation Systems Test(以下、BESTest)が使用されることがある。近年、理学療法の優先順位を明確にできるKeyformが注目されている。Keyformは評価指標における項目毎の難易度を数値化および序列化することで、難易度を可視化することができる。そこで本研究では、SCA患者におけるBESTest各セクションのKeyformを開発することを目的とした。【方法】SCA患者述べ81名を対象とし、BESTestを評価した。一次元性および内的信頼性を確認し、Rasch分析にてBESTest各セクションの項目別適合度指標と難易度を求めたのち、Keyformを作図した。【結果】Section IおよびIIは一次元性を確認できず、内的信頼性はSection V以外で優れた信頼性を得られた。また、2項目の適合度が不良であったため、一次元性を確認できなかったSectionの二つと適合度が不良であった2項目を解析から除外したうえでKeyformを開発した。【結論】SCA患者を対象として開発した本Keyformは、BESTestにおけるバランス課題の難易度を可視化できることから、個々のSCA患者に対して理学療法の目標設定と治療計画を適切に行う上で、利用価値のあるツールになると考えている。

The capacity to be alone (CBA) is a concept proposed by Winnicott. A scale for assessing CBA that can measure anxiety in both solitude and individuation was developed, and the relationship between acquiring CBA and the internal object was investigated. University students (N = 220) responded to a questionnaire. Factor analysis of their responses using principle factor method and Varimax rotation indicated that “the Scale of Anxiety about Being Alone” consisted of two factors: “Avoidance of intimacy” “Anxiety about solitude”.Cronbach’s alpha coefficients of the two factors were indicative of sufficient reliability (.91 and .90, respectively). Moreover the difference in the inner object among the following groups: “High-CBA”, “Anxiety about solitude”, “Anxiety about intimacy” and “Low-CBA”, divided by each factor score was analyzed using a one-way analysis of variance. The results indicated that “High-CBA” was characterized by an inconsistent object rather than by a good, or a bad object. The group with anxiety about solitude, which mostly tended to imagine a good object, also tended to recollect an inconsistent object. This result suggested they were vulnerable to object constancy.

The predictors of pacing capture threshold (PCT) exacerbation after leadless pacemaker implantation remain unknown. We analyzed the predictors of poor PCT by identifying risk factors using multivariate logistic regression analysis for 211 patients with leadless pacemaker implantation. Twenty patients met the criteria for elevated PCT levels and were categorized in the poor PCT group. Multivariate analyses revealed that PCT (P < 0.0001) and pacing impedance (P = 0.03) were independent predictors of PCT exacerbation. Elevated PCT levels and low pacing impedance during leadless pacemaker implantation were potential risk factors for the replacement of leadless pacemakers after the procedure.

経肛門的直腸異物は,性的嗜好などが原因で肛門から異物が挿入され,抜去不可能となったものである.治療は一般的に内視鏡や経肛門的摘出術が,第一選択とされるが,難渋する場合には開腹手術への移行が必要になる. 今回,われわれは摘出の際の器具を工夫することで経肛門的に摘出し得た巨大直腸異物を経験したので報告する.

実装科学とは、エビデンスに基づく介入を「どのように」すれば実装できるのか、という問いに対する知識体系を構築する学問領域といえる。本稿では、事業場における健康づくりの仮想的な事例をもとに、実装科学が実践にどのように役立つのかを解説する。つぎに、保健医療の分野で用いられる実装研究の要素について解説する。最後に、産業保健の領域で健康増進対策実施を促進するための、実装研究、あるいはその要素を持つ介入研究の紹介を行う。

睡眠はsleep homeostasisと,中枢の時計遺伝子の支配を受ける概日リズム(circadian-rhythm)により制御されている.末梢臓器である腸管も時計遺伝子による制御を受け,腸内細菌の組成と機能には概日リズムが認められる.時差症候群や睡眠時間制限などによる睡眠障害は,腸内細菌の概日リズムに変調をもたらし,dysbiosisや腸管バリア機能低下を惹起し,宿主のエネルギー代謝異常症の原因となる.規則正しい摂食は腸内細菌の概日リズムを回復させ,中枢時計との同調を促し,睡眠障害の治療となる可能性がある.またプレ・プロバイオティクスなど腸内細菌を介した睡眠障害の治療も期待されている.
