To clarify the dynamic changes in the membranous lesions of Meniere's disease, the effects of aerobic exercise on different symptoms were analyzed. Out of a total of 351 patients with Meniere's disease who performed aerobic exercises for 6 months or longer who were enrolled, 338 showed improvement of one or more symptoms: disappearance of vertigo in 97.6% of cases; complete resolution of ear fullness in 60.1% of cases; improvement of hearing impairment in 38.5% of cases; complete resolution or marked decrease of tinnitus in 29.6% of cases. On the other hand, complete recovery of hearing was noted in 29.5% of the 78 patients with low-tone loss, 12.5% of the 96 patients with high-tone loss, and only 4.1% of the 170 patients with hearing loss across all frequencies. More than half of the patients with high-tone hearing loss and hearing loss across all frequencies failed to show any change in the degree of hearing loss. Considering the findings of temporal bone pathology, it seems probable that the Reissner's membrane, being ballooned and highly distended by endolymphatic hydrops, disturbs conduction of low-frequency waves by the perilymph, and the membrane, once it loses tension, is not resonant with either high-frequency waves or indeed waves of any frequency. Although aerobic exercise is effective to cure hydrops and improve membranous tension by activating homeostasis, its effect worsens with decreasing membranous tension.

A network system of new coronavirus infection disease (COVID-19) is characterized using Biochemical systems theory (BST). A differential equation model is constructed in the framework of BST and the parameter values in the equations are determined from the infection data reported for the infected, recovered, and dead individuals (from January 15 to April 29, 2020) in Japan. Nondimensional analysis suggests that the time courses of dependent variables related to the infection are governed by two dimensionless parameters: G (the ratio of rate constants) and x20 (the initial value for the ratio of infected individual number and population), and G must be greater than unity in order to certainly decrease the infected individual number to zero. The infection data in the initial stage indicates that if there was no action to protect, the infected individual number exponentially increased because G=0.0839143 < 1; for 120 million of the population in Japan, almost all persons were infected and the dead individual number increased to 16.8 million. Actually, however, the numbers of infected and dead individuals were only 16,305 and 749, respectively, suggesting that the contact rate was significantly reduced in a very short period of time. The analysis also indicates that x2 takes a maximum when x1 (the ratio of uninfected individual number and population) is equal to G. When the nosocomially infected individual number is equal to 10 % of the total number of infected individuals, the infected individual number never becomes zero unless the nosocomial infection is regulated even when the community infection is kept zero; in this case, the nosocomial infection must be aggressively reduced.

青年期における笑いに対する意識や態度の性差について多角的に検討するため、笑いに対する積極性尺度を作成し、ユーモア志向性および笑わせたい相手との関連から考察した。笑いに対する積極性尺度の作成にあたり、18 ~ 31 歳の大学生261 名(男性70 名,女性191 名; M = 19.37,SD =1.37)の回答を分析した結果、最終的な笑いに対する積極性尺度は第1因子から第4 因子の「笑いに対する好感」「笑わせ欲求」「笑いに対する自信」「お笑いへの親しみ」の4因子で構成された。性差を検討したところ、男性の方が「笑わせ欲求」の得点が高く、恋愛関係にある相手を笑わせることを女性よりも重視していることが確認された。一方、女性は男性よりも「笑いに対する好感」の得点が高かったことから、日常的に笑いがある生活をより重視し、コミュニケーションに笑いがあることを望むからこそ、笑わせてくれる相手やよく笑う他者を魅力的だと感じている可能性が示唆された。

Surface-based morphometry (SBM) is extremely useful for estimating the indices of cortical morphology, such as volume, thickness, area, and gyrification, whereas voxel-based morphometry (VBM) is a typical method of gray matter (GM) volumetry that includes cortex measurement. In cases where SBM is used to estimate cortical morphology, it remains controversial as to whether VBM should be used in addition to estimate GM volume. Therefore, this review has two main goals. First, we summarize the differences between the two methods regarding preprocessing, statistical analysis, and reliability. Second, we review studies that estimate cortical morphological changes using VBM and/or SBM and discuss whether using VBM in conjunction with SBM produces additional values. We found cases in which detection of morphological change in either VBM or SBM was superior, and others that showed equivalent performance between the two methods. Therefore, we concluded that using VBM and SBM together can help researchers and clinicians obtain a better understanding of normal neurobiological processes of the brain. Moreover, the use of both methods may improve the accuracy of the detection of morphological changes when comparing the data of patients and controls.In addition, we introduce two other recent methods as future directions for estimating cortical morphological changes: a multi-modal parcellation method using structural and functional images, and a synthetic segmentation method using multi-contrast images (such as T1- and proton density-weighted images).

Blood vessels can be regarded as autonomous organs. The endothelial cells on the vessel surface serve as mechanosensors or mechanoreceptors for the flow velocity and turbulence of the blood flow in terms of wall shear stress (WSS), thereby monitoring changes in the flow velocity. Accordingly, the endothelial cells regulate the flow velocity by releasing numerous mediators. Such regulatory systems also trigger atherosclerosis, where the WSS decreases or fluctuates to maintain the flow velocity or local WSS. As occurrences of abdominal aortic aneurysms and aortic dissection are closely related to atherosclerosis, understanding the hemodynamics of the abdominal aorta is necessary to obtain useful information concerning the pathogenesis, diagnosis, and interventions. 4D flow MRI is beneficial for measuring the hemodynamics through comprehensive retrospective flowmetry of the entire spatio-temporal distributions of the flow vectors. This section focuses on abdominal aortic aneurysms and aortic dissection as representative examples of abdominal aortic diseases. Their hemodynamic characteristics and how hemodynamics is involved in their progression are described, and how 4D flow MRI can contribute to their assessment is also explained.

We developed a new inhalation exposure method to evaluate effects of synthetic cannabimimetics that are being distributed as new, unregulated drugs in the Tokyo area. We selected the commercial product “SOUTOU” containing AB-CHMINACA and 5F-AMB as the test drug and dried marshmallow (Althaea officinalis) leaves as the negative control. A half cigarette packed with dried marshmallow leaves or SOUTOU was ignited, then mainstream smoke from each was delivered to five mice in an exposure box. After the cigarettes were fully consumed, neurobehavioral observations and a catalepsy test were performed at 15, 30 and 60 min after exposure. The effluent air from the exposure box was poured into impingers containing acetonitrile (first impinger) and dimethyl sulfoxide (second impinger). The resulting solutions were analyzed to assess decomposition of the synthetic cannabimimetics. Mice exposed to SOUTOU smoke showed many excitement behaviors and some suppressive behaviors at 15, 30 and 60 min. These clearly included cannabimimetic specific pharmacological actions. Negative control mice also showed some suppressive behaviors at 15 min but these were attenuated at later times, nearly disappearing at 60 min. In addition, the behavioral effects observed in controls were less pronounced than those in SOUTOU exposed mice. The inhalation exposure method developed in our study would be effective for determining cannabinoid specific pharmacological effects of illegal drugs, as well as for assessing the presence of active compound(s) by comparing the test substance with a negative control.

本研究の目的は,現場のリスクマネジメント担当者の満足感と役割獲得構造の分析を手がかりの一端として,権威勾配が最少になるように意図した,組織横断的なリスクマネジメント・システムの意義と課題を検討することにある.このシステムの下で,既存の部門内に閉じられた官僚制的な組織管理下では対応できなかった病院全体に及ぶ業務改善に,比較的容易に取り組めたことから,われわれは,このシステムが有機的に機能する手応えをもった.そこで,体制がスタートして1年半が経過した2002年3月,副看護師長の現場担当リスクマネジャー31名に,自由記載のアンケート調査を実施し,その内容をK-J法により構造化した.その結果,病院組織の中では決して高くはない地位にある副看護師長の現場RMが,整備されたリスクマネジメント・システムと上司である看護師長や総括リスクマネジャーの適切なサポートにより,実践を通してRM役割を獲得していくことが確認された.以上より,部門の壁と縦の権威を乗り越えて,組織横断的に機能するリスクマネジメント・システムは,従来の官僚制的組織運営の弱点を補完する機能の一端を担う可能性が示唆された.このような機能の出現は,本来あるべきチーム医療を促進し,病院組織のあり方を刷新する原動力として期待される.
