Influenza vaccination can trigger various adverse reactions, and thrombocytopenia is also rarely reported. Although patients with mild thrombocytopenia are sometimes asymptomatic, severe thrombocytopenia can cause severe bleeding. We herein report a rare case of severe thrombocytopenia that occurred within one day of influenza vaccination and diffuse alveolar hemorrhage (DAH) leading to acute respiratory failure. The patient was treated with glucocorticoid pulse therapy, intravenous immunoglobulin, and temporary mechanical ventilation, and eventually he made a full recovery. Vaccine-related thrombocytopenia and DAH should be considered adverse reactions, even if they develop very soon after vaccination.

Although neonicotinoid pesticides are expected to have harmful influence on mammals, there is little animal experimental data to support the effect and mechanisms. Since acetylcholine causes the release of dopamine, neonicotinoids may confer a risk of developmental disorders via a disturbance in the monoamine systems. Male mice were peripubertally administered dinotefuran (DIN) referring to no observed effect level (NOEL) and performed behavioral and immunohistological analyses. In an open field test, the total locomotor activity was increased in a dose-dependent manner. The immunoreactivity of tyrosine hydroxylase in the substantia nigra was increased in DIN-exposed mice. These results suggest that exposure to DIN in peripubertal male mice causes hyperactivity and a disturbance of dopaminergic signaling.

The main objective of this lecture is to present a background understanding for the current endeavor to change the course of the humanospheric evolution. In order for every human being to lead a happy life, physical conditions such as adequate supply of fresh water and foods, no shortage of materials that are essential for our modern living, sufficient control over poisons and diseases that infect us and our environments, and securing enough energy sources that last forever are prerequisite. Obviously, we need to develop new technologies, new societal systems, and new lifestyles to achieve these. However, it should be emphatically stressed that these conditions and the three new subjects (technology, social system, and lifestyle) are only desirable as far as they make it possible to provide everybody with a happy life. The evolution of global material cycles, current status of material and energy exchange between the biosphere and the humanosphere, and four rules of ecology are discussed in this context. On the analogy of human relationship between young and olds, the reduction of the exchange between young humanosphere and old global life support system is proposed to be the most urgent target.

In 2007, probe electrospray ionization/mass spectrometry (PESI/MS) was developed. In this technique, the needle is moved down along a vertical axis and the tip of the needle touched to the sample. After capturing the sample at the needle tip, the needle is then moved up and a high voltage is applied to the needle at the highest position to generate electrospray. Due to the discontinuous sampling followed by the generation of spontaneous electrospray, sequential and exhaustive electrospray takes place depending on the surface activity of the analytes. As modified versions of PESI, dipping PESI (dPESI), sheath-flow PESI (sfPESI) and adjustable sfPESI (ad-sfPESI) have been developed. These methods are complementary to each other and they can be applicable to surface and bulk analysis of various biological samples. In this article, the characteristics of these methods and their applications to real samples will be reviewed.

Objective: To assess the effect of manual lymph drainage (MLD) on pain in Japanese patients up to 10 days after a total knee arthroplasty (TKA).Methods: This study was a randomized controlled trial performed at a University Medical Center. Patients who underwent unilateral TKA and received once daily MLD for 20 minutes prior to standard physical therapy up to 10 days after TKA were investigated. Pain at rest, knee extension muscle contraction, and maximum load were assessed using the visual analog scale (mm) before surgery, after drain removal, and after the fifth MLD. As secondary outcomes, the circumference, range of motion, muscle strength, walking speed, and walking rate were evaluated.Result: Forty-one patients aged 45-85 participated in this study, 21 of whom were assigned to the intervention (MLD group) and 20 who were not (control group). Ten days after TKA, no significant difference was evident between the MLD and control groups for resting pain [4.5 mm (1.6-10.8) vs 7.0 mm (1.8-25.5), respectively, p=0.17], pain during knee extension muscle contraction [12.3 mm (4.5-24.8) vs 20.8 mm (6.4-31.8), p=0.41], and pain at maximum load [13.0 mm (8.3-39.8) vs 16.0 mm (4.6-32.5), p=0.73]. There were no significant differences between groups in terms of secondary outcomes.Conclusion: This study shows that MLD up to 10 days after TKA does not affect pain.

Background:Although advances in cardiac surgery have led to an increased number of survivors with congenital heart disease (CHD), epidemiological data regarding the pregnancies and deliveries of patients with repaired CHD are scarce.Methods and Results:In this study, we retrospectively reviewed the clinical outcomes of pregnancies and deliveries of women with repaired CHD. Overall, 131 women with repaired CHD were enrolled and there were 269 gestations. All patients were classified as New York Heart Association (NYHA) Class I or II. The prevalence of cesarean sections was higher in patients with (CyCHD) than without (AcyCHD) a past history of cyanosis (51% vs. 19%, respectively; P<0.01). There were 228 offspring from 269 gestations and the most prevalent neonatal complication was premature birth (10%), which was more frequent in the CyCHD than AcyCHD group (15.7% vs. 5.6%, respectively; P<0.01). Five maternal cardiac complications during delivery were observed only in the CyCHD group (8%); these were classified as NYHA Class II and none was fatal.Conclusions:Delivery was successful in most women with repaired CHD who were classified as NYHA Class I or II, although some with CyCHD and NYHA Class II required more attention. Cesarean sections were more common in the CyCHD than AcyCHD group, and CyCHD may be a potential risk for preterm deliveries.

Aim: The aim was to investigate the respective associations between lifestyle and proteinuria and the estimated glomerular filtration rate (eGFR).Methods: The lifestyle habits of 25,493 middle-aged participants were investigated in a cross-sectional study to find habits that are associated with a low eGFR (<60 mL/min/1.73 m2) and/or the presence of proteinuria. The lifestyle habits of the participants were evaluated using a questionnaire. Unhealthy lifestyle habits were defined as follows: 1. obesity, 2. being a current/former smoker, 3. eating irregular meals, 4. having less than 5 hours sleep, 5. exercising less than once a week, and 6. drinking more than once a week. The associations among unhealthy habits, eGFR, and proteinuria were evaluated using multivariate analysis.Results: The following lifestyle factors were significantly and independently associated with proteinuria: obesity (odds ratio (OR): 1.18, 95%C.I: 1.04-1.34), being a current/former smoker (OR: 1.26, 95%C.I: 1.11-1.42), eating irregular meals (OR: 1.40, 95%C.I: 1.22-1.61), sleeping less than 5 hours (OR: 1.38, 95%C.I: 1.15-1.65), and exercising less than once a week (OR: 1.18, 95%C.I: 1.05-1.33). In contrast, the following unhealthy lifestyle factors were not clearly associated with a low eGFR: obesity (OR: 1.05, 95%C.I: 0.95-1.17), being a current/former smoker (OR: 0.76, 95%C.I: 0.69-0.84), eating irregular meals (OR: 0.91, 95%C.I: 0.79-1.04), sleeping less than 5 hours (OR: 1.02, 95%C.I: 0.85-1.22), and exercising less than once a week (OR: 0.91, 95%C.I: 0.83-0.99).Conclusion: Associations between proteinuria and unhealthy lifestyle habits were observed in our cross-sectional study. Unhealthy lifestyles should be monitored during the management of CKD patients with proteinuria.

Duchenne muscular dystrophy (DMD) is X-linked recessive myopathy caused by mutations in the dystrophin gene. Although conventional treatments have improved their prognosis, inevitable progressive cardiomyopathy is still the leading cause of death in patients with DMD. To explore novel therapeutic options, a suitable animal model with heart involvement has been warranted.We have generated a rat model with an out-of-frame mutation in the dystrophin gene using CRISPR/Cas9 genome editing (DMD rats). The aim of this study was to evaluate their cardiac functions and pathologies to provide baseline data for future experiments developing treatment options for DMD.In comparison with age-matched wild rats, 6-month-old DMD rats showed no significant differences by echocardiographic evaluations. However, 10-month-old DMD rats showed significant deterioration in left ventricular (LV) fractional shortening (P = 0.024), and in tissue Doppler peak systolic velocity (Sa) at the LV lateral wall (P = 0.041) as well as at the right ventricular (RV) free-wall (P = 0.004). These functional findings were consistent with the fibrotic distributions by histological analysis.Although the cardiac phenotype was milder than anticipated, DMD rats showed similar distributions and progression of heart involvement to those of patients with DMD. This animal may be a useful model with which to develop effective drugs and to understand the underlying mechanisms of progressive heart failure in patients with DMD.

Oral administration of the perilla leaf extract (PLE) to mice inhibits inflammation, allergic response, and tumor necrosis factor-α production. We also found that PLE suppressed the tumor necrosis factor-α (TNF-α) production in vitro. Using the inhibitory activity of TNF-α production in vitro as the index for isolation, we searched the active constituents from PLE and isolated luteolin, rosmarinic acid and caffeic acid as active components. Among the isolated compounds, only luteolin showed in vivo activity: inhibition of serum tumor necrosis factor-α production, inhibition of arachidonic acid-induced ear edema, inhibition of 12-O-tetradecanoylphorbol-13-acetate-induced ear edema and inhibition of oxazolone-induced allergic edema. These results suggest that luteolin is a genuinely active constituent which is accountable for the oral effects of perilla.

Inflammatory colorectal polyps (ICRPs) in miniature dachshunds (MDs) are speculated to be a breed-specific inflammatory bowel disease (IBD). Leucine-rich alpha-2 glycoprotein (LRG) has been identified as a novel biomarker of human IBD. The aim of this study was to examine LRG gene expression in the polypoid lesions of ICRPs. Polypoid lesion specimens were collected from 24 MDs with ICRPs. Nonpolypoid colonic mucosa was collected from 18 MDs with ICRPs and 10 controls. The gene expression of LRG, interleukin (IL)-1β, IL-6, tumor necrosis factor-α, and IL-22 was examined. The expression of LRG gene was significantly increased in the polypoid lesions of ICRPs and correlated with that of the four cytokines. In conclusion, the LRG gene was expressed within the polypoid lesions of ICRPs and might be associated with local cytokine expression.

「小型溝構造液水冷却燃焼器の研究」は宇宙開発事業団によって進められている推力10トン級 液酸・液水エンジン,LE-5の開発に必要な基礎資料を得るための,詳細な液水冷却特性に関するデータ,及び広範な燃焼条件に対する燃焼特性を把握することを目的とするものである。昭和52年度に冷却用液水供給装置および供試燃焼器一号機の製作を行い,53年度より航空宇宙技術研究所角田支所において燃焼試験を開始した。また,これと同時に液水冷却特性の解析上不可欠となる燃焼性能,及び燃焼ガス側熱負荷の詳細なデータを得るために,多分割型環状水冷却燃焼器を用いた水冷却燃焼試験を行なった。これらの累積燃焼試験時間は約140回,4200秒におよんだ。ところで,実際の液酸・液水エンジンでは燃焼器の冷却に用いられた液水が噴射器から燃焼室内に噴射されて,液酸と燃焼する再生冷却方式をとるのが普通である。しかし,本研究においては液水冷却特性に関して広い範囲の冷却条件でデータを得ることを目的としているため,冷却用液水の流量,供給圧力,温度等を燃焼用液水とは独立に変えられるように別系統とした“独立冷却方式”でも試験が行なえるように計画した。さらに独立冷却燃焼試験のデータを踏まえて,完全再生冷却燃焼試験の実証を行ない,設計点では冷却能力にまだ余裕のあること,特性速度効率で98%以上の値が得られることを確認した。このように小推力,F=3KN(300kgf),燃焼器で再生冷却燃焼が可能になったことは,燃焼器構造が従来の管構造燃焼器とは違って,銅製の溝構造(いわゆるSlotted wall)燃焼室としたことによるものである。これは冷却通路となる多数の溝を長手方向の設けたもので,スペース・シャトル主エンジン(SSME)等の高圧高性能エンジンに用いられている高熱負荷燃焼器と原理的に同じものであり,我国においては未経験のものである。これによって,冷却特性を明らかにするために必要な各部の温度,圧力測定が容易になるとともに,将来の高圧エンジンへの発展性をも考慮した詳細な試験データの取得が出来たものと考える。本報告で行なう試作1号機によって得られた主な結果を簡単に述べる。冷却特性については,①小推力溝構造液水冷却燃焼器により,設計点(燃焼圧力P=3.48MPa,混合比O/F=5.5)での完全再生冷却方式による燃焼が可能であり,冷却能力としては十分な余裕のあることが分かった。しかしながら,冷却条件を広範囲に変えて行なった独立冷却燃焼試験の結果から見て, ②従来より提唱されている殆んど全ての設計式が溝構造燃焼器における熱設計式としては不適当であり,かつ設計上危険性の高いことが分った。 ③試験後,供試燃焼器の切断検査を行なった結果,溝構造燃焼器をロー付接合によって製作する本方式には多くの問題点があり,新しい製作法の開発が望まれる。特に各冷却流路間の抵抗値の不均一さ,及び内筒の変形による内外筒接合部の剥離に問題がある。次に燃焼特性については ④混合比,水素噴射温度を広く変化させて特性速度効率におよぼす影響を調べた。その結果これらの効率への寄与は,著者らが以前に得た液体酸素・常温ガス水素燃焼の場合と同様に,ほぼ噴射速度比(水素噴射速度と液酸噴射速度の比)によって表わされることが分った。さらに再生冷却燃焼時に低混合比(O/F < 3)で約100Hzの低周波振動燃焼を起したが,それ以外の試験範囲ではほぼ安定な燃焼が行なわれた。

Hydrogen-assisted crack growth of pre-strained twinning-induced plasticity (TWIP) steel was investigated using artificial defects (micro-drilled holes), which acted as artificial crack initiation sites. Hydrogen was introduced into the specimens by electrochemical hydrogen charging during slow strain rate tensile test. The quasi-cleavage crack propagation observed was due to repeated crack initiation near the crack tip and subsequent coalescence. Crack initiation near the crack tip occurred after plastic deformation of the crack tip, and pre-straining facilitated plasticity-driven crack initiation. The early stage of plasticity-driven crack growth was sensitive to the crack length and remote stress level. Accordingly, the crack growth rate in the early stage increased with the increase in the initial defect size. In the following stage of the crack growth, the crack growth rate exhibited a complicated trend with respect to the crack length, which is possibly due to the plastic-wake-altered stress field around the crack tip, which depends on the initial defect size.

A variety of bond portfolio optimization problems of institutional investors are formulated as linear and/or bilinear fractional programming problems and algorithms to solve this class of problems are discussed. Our objective is to optimize certain index of returns subject to constraints on such factors as the amount of cash flow, average maturity and average risk, etc. The resulting objective functions and constraints are either linear, bilinear or bilinear fractional functions. T. he authors devised a special purpose algorithm for obtaining a local optimal solution of this nonconvex optimization problem containing more than 200 variables. Though it need not generate a global optimum, it is efficient enough to meet users' requirement.

Xanthine and hypoxanthine are intermediate metabolites of uric acid and a source of reactive oxidative species (ROS) by xanthine oxidoreductase (XOR), suggesting that facilitating their elimination is beneficial. Since they are reabsorbed in renal proximal tubules, we investigated their reabsorption mechanism by focusing on the renal uric acid transporters URAT1 and GLUT9, and examined the effect of clinically used URAT1 inhibitor on their renal clearance when their plasma concentration is increased by XOR inhibitor. Uptake study for [3H]xanthine and [3H]hypoxanthine was performed using URAT1- and GLUT9-expressing Xenopus oocytes. Transcellular transport study for [3H]xanthine was carried out using Madin–Darby canine kidney (MDCK)II cells co-expressing URAT1 and GLUT9. In in vivo pharmacokinetic study, renal clearance of xanthine was estimated based on plasma concentration and urinary recovery. Uptake by URAT1- and GLUT9-expressing oocytes demonstrated that xanthine is a substrate of URAT1 and GLUT9, while hypoxanthine is not. Transcellular transport of xanthine in MDCKII cells co-expressing URAT1 and GLUT9 was significantly higher than those in mock cells and cells expressing URAT1 or GLUT9 alone. Furthermore, dotinurad, a URAT1 inhibitor, increased renal clearance of xanthine in rats treated with topiroxostat to inhibit XOR. It was suggested that xanthine is reabsorbed in the same manner as uric acid through URAT1 and GLUT9, while hypoxanthine is not. Accordingly, it is expected that treatment with XOR and URAT1 inhibitors will effectively decrease purine pools in the body and prevent cell injury due to ROS generated during XOR-mediated reactions.

Objective: This study aimed to investigate the age at which scoliosis progresses to a severe condition and identify the factors related to severe scoliosis in patients with cerebral palsy.Patients and Methods: This retrospective study included 51 patients aged ≥15 years. The Cobb angle was measured over time using radiographs. Patients were divided into the following groups according to their final Cobb angle: <60°, 60°–100°, and ≥100°. The age at which the Cobb angle was ≥20° in the patients was compared among the groups. Moreover, the age at which a significant difference in the Cobb angle occurred in the groups was considered the age at which the scoliosis worsened. Association of the final Cobb angle with factors such as the location of curve, Gross Motor Functional Classification System (GMFCS), capability of turning over, orthosis use, hip dislocation, tracheotomy, and gastric fistula was examined.Results: The mean age at which the Cobb angle was ≥20° was significantly lower in the ≥100° group. From 9 years of age, a significant difference was noted in the Cobb angle between the <60° group and ≥100° group. Between 13–19 years, a significant difference in the Cobb angle was observed among the three groups. Furthermore, GMFCS, capability of turning over, hip dislocation, and gastric fistula were the factors showing a significant difference among the three groups.Conclusion: Scoliosis progressed to the severe form (Cobb angle ≥100°) at 9 years of age. Moreover, scoliosis is aggravated during the growth period.
