Health and productivity management is a method of strategic of employees' healthcare to achieve greater wellbeing and improve performance. The research focuses on the implementation of health and productivity management system in Japan and its effects on Human Resource Management from the insurer's viewpoint. The health insurer reviewed in the study provides medical insurance to employees in small enterprises. The insurer promotes health and productivity management aimed at reducing medical costs and claims. This ultimately has a positive effect on insurance premiums when insurers pass on the benefits to small enterprises. This research revealed that a collaborative approach between insurers and enterprises may be beneficial for promoting employees' healthcare in small enterprises. The study demonstrates that when the management is better aware of healthcare, Human Resource Management is effective and can have a positive impact on productivity.The study and the medical insurance considered are limited to Hiroshima Prefecture's situation. It does not consider the actual cases of health and productivity management in small enterprises in the main paper under limitations. This leaves many challenges for the future.

日本の少子高齢化は世界に類を見ない速度で進展しており,我が国の医療費は増加の一途をたどっている.このような状況が続けば,日本の医療保険制度の維持ができなくなる.また,日本では生活習慣病の急増を受け,「第4次国民健康づくり対策」が策定されている.現在,高血圧症や糖尿病といった生活習慣病が増加しており,そのような生活習慣病を早期発見する機会である特定健康診査受診率が伸び悩んでいることが課題となっている.そこで,本研究では,国保データベース(KDB)を用いて,まず,生活習慣病と健診回数の関係を示し,健診の効果を示唆する.また,生活環境との関連を調べるため,地域の現況や特性を考慮した分析を行い,健康課題・健診率の地域差を明らかにし,地域別の健康課題や健康な地域の特性について把握する.

This study analyzed factors affecting plasma lamotrigine (LTG) concentrations in patients with bipolar disorder. The mean concentration/dose (C/D) ratio of LTG was significantly higher in patients co-treated with LTG and valproic acid (VPA) [3.72 (μg/mL)/(mg/kg/day), n = 9] than in those receiving LTG monotherapy [1.76 (μg/mL)/(mg/kg/day), n = 9; P < 0.01]. LTG monotherapy patients were genotyped for UDP-glucuronosyltransferase 2B7 (UGT2B7) *1/*1 (n = 5) and *1/*2(n = 4), whereas VPA co-administration patients were genotyped for UGT2B7*1/*1 (n = 6), *1/*2 (n = 2) and *2/*2 (n = 1). *There were no significant differences in mean C/D ratios between patients carrying the UGT2B7*1/*1 genotype and the 1/*2 or *2/*2 genotype regardless of pharmacotherapy (P ≥ 0.150). In the LTG monotherapy group, the mean C/D ratio was similar in smoking patients [1.52 (μg/mL)/(mg/kg/day), n = 3] and non-smoking patients [1.88 (μg/mL)/(mg/kg/day), n = 6; P = 0.393]. In the VPA co-administration group, however, the mean C/D ratio in smoking patients [2.62 (μg/mL)/(mg/kg/day), n = 4] was significantly lower than that in non-smoking patients [4.61 (μg/mL)/(mg/kg/day), n = 5; P < 0.01]. In vitro studies with HepG2 cells indicated that benzo[a]pyrene, one of the polycyclic aromatic hydrocarbons contained in tobacco smoke, as well as 3-methylcholanthrene efficiently induced expression of UGT1A4 mRNA but not UGT2B7 mRNA and that VPA potently enhanced their induction. These results suggest that concomitant VPA administration and/or smoking may influence LTG concentrations in patients with bipolar disorder.

【目的】看護系大学共用試験(CBT)用に作成した老年看護学の出題90項目を古典的テスト理論(CTT),および項目反応理論(IRT)を用い正答率,および項目識別度,項目困難度などの特性によりメタ評価した.【対象と方法】便宜的標本抽出により国公私立看護系23大学の臨地実習前の3年次生730名を対象として,紙筆試験を実施した.分析は正答率,IT双列相関,項目困難度(2PL model),項目識別度,因子負荷量を求め,項目特性曲線等を描いてすべての項目特性を評価した.【結果】老年看護学の平均正答率は65.8~69.3%で,他科目との相関は薬理学r=0.30~0.41, 解剖学・病理学r=0.28~0.38であった(p<0.01).項目困難度(-5.851~4.068),および項目識別度(0.292~2.218)とも幅が広かった.情報量曲線により各項目の特性が示された.90項目中3 項目の項目識別度が低かった.【結論】90項目の問題中87項目はCBTでの利用が可能と考えられた.しかし,このうち71.1%は項目困難度が負の易しい問題であったため,能力水準の高いレベルの受検者の識別に課題が残った.

This study developed a Vitality Scale (VS), and examined its reliability and validity with 518 participants including undergraduates and adults. Factor analysis revealed that the VS consisted of six factors: vigor, flexibility, social withdrawal (reversed scoring), activeness, mental toughness, and positiveness. Six subscales were developed according the factor analytic results, which had sufficient internal consistency (α=.74–.82). Correlational validity was demonstrated in terms of the relationships of the VS with sleep and eating. These results indicate that each subscale was useful to measure an individual's vitality. Males obtained higher mean scores than females did on flexibility, social withdrawal (reversed) and activeness. The mean of adults' scores was higher than undergraduates' scores on the most of vitality subscales.

Background:Recently, identification of independent risk factors for ischemic stroke in Japanese non-valvular atrial fibrillation (NVAF) patients was made by analyzing the 5 major Japanese registries: J-RHYTHM Registry, Fushimi AF Registry, Shinken Database, Keio interhospital Cardiovascular Studies, and the Hokuriku-Plus AF Registry.Methods and Results:The predictive value of the risk scheme in Japanese NVAF patients was assessed. Of 16,918 patients, 12,289 NVAF patients were analyzed (mean follow up, 649±181 days). Hazard ratios (HRs) of each significant, independent risk factor were determined by using adjusted Cox-hazard proportional analysis. Scoring system for ischemic stroke was created by transforming HR logarithmically and was estimated by c-statistic. During the 21,820 person-years follow up, 241 ischemic stroke events occurred. Significant risk factors were: being elderly (aged 75–84 years [E], HR=1.74), extreme elderly (≥85 years [EE], HR=2.41), having hypertension (H, HR=1.60), previous stroke (S, HR=2.75), type of AF (persistent/permanent) (T, HR=1.59), and low body mass index <18.5 kg/m2(L, HR=1.55) after adjusting for oral anticoagulant treatment. The score was assigned as follows: 1 point to H, E, L, and T, and 2 points to EE and S (HELT-E2S2score). The C-statistic, using this score, was 0.681 (95% confidence interval [CI]=0.647–0.714), which was significantly higher than those using CHADS2(0.647; 95% CI=0.614–0.681, P=0.027 for comparison) and CHA2DS2-VASc scores (0.641; 95% CI=0.608–0.673, P=0.008).Conclusions:The HELT-E2S2score may be useful for identifying Japanese NVAF patients at risk of ischemic stroke.

In order to recover trace of platinum (Pt) from hydrochloric acid (HCl) leaching solution of Cobalt-Rich Crust (CRC), the absorption properties of four kinds of seaweeds (Undaria pinnatifida, Laminaria angustata, Enteromorpha linza and Gloiopeltis tenax) and activated carbon were studied. At first, their adsorption abilities for Pt ion as well as selective adsorption properties for other coexisting metal ions contained in CRC (including iron (Fe), manganese (Mn), cobalt (Co), nickel (Ni) and copper (Cu)) were investigated by using artificial solutions. Finally, the application to CRC HCl leaching solution was also carried out. The results showed that all seaweeds achieved their best Pt adsorption capacity only at pH 2, whereas activated carbon possessed a high Pt adsorption ability over a wide pH range from pH 0 to 6 as well as in HCl solution up to 3 mol/L. Regarding the CRC leaching solution obtained by 0.5 mol/L HCl and 1 vol.% hydrogen peroxide (H2O2), 46.9% and 100% Pt was recovered by Enteromorpha linza and activated carbon, respectively. It was found that seaweed and activated carbon are both effective adsorbents for recovering Pt from HCl leaching solution of CRC.
