Bronchial asthma has been reported to greatly influence the outcome of endoscopic sinus surgery (ESS) in patients with chronic sinusitis. We studied 191 patients undergoing ESS from January 2000 to December 2004, focusing on postoperative improvement in olfactory disorders, recurrence, and incidence of reoperation in 143 patients with follow-up exceeding 6 months. Airway hypersensitivity was observed in 35% of patients undergoing ESS, compared to 75% of asthmatic patients and 81% of those with both asthma and allergic rhinitis who had preoperative olfactory disorders. Olfactory disorders improved in 82% of patients undergoing ESS. Over 50% of relapsed patients had airway hypersensitivity. Asthmatic patients had an especially high relapse 37% (10 of 27 patients) . The outcome of ESS was significantly worse in patients with asthma. We conclude that patients with chronic sinusitis and asthma, who tend to experience more recurrence, require regular examination and early treatment.

Othello is called as one of the Shakespeare's`Four Great Tragedies', and they have been called`character tragedies'which means that the causes of these tragedies depend on their heroes'character. In the back of the tragedy, Othello, there exists the traditional view of the black moor which is seen in Iago's abuse against Othello and the Brabantio's view. The audience in Shakespeare's days must have shared the same prejudice with Iago, and at the same time they must have been perplexed by the heroic character of a black moor whom Desdemona loved, and whom Duke, most of the senators and Cassio praised as noble, valiant and great of heart. This paper discusses why Othello is so easily deceived by the words of his`ancient', Iago. Why didn't Othello believe his most beloved wife, Desdemona, but his subordinate,`ancient', Iago? The paper concludes that the true cause of the tragedy, Othello, is in the white people's consciousness of being white, not in Othello's inferiority complex of being black or his consciousness of being black.

今回訳出したのは、2回目に来日した際の日記のうち九州旅行に関する部分である。この時リヒトホーフェンは研究旅行家として1870年9月9日(旧暦明治3年8月14日)に横浜を出発し、それより西に向い富士山に登り、それから一旦江戸に戻りしばらくそこに滞在した。次いで、東山道、琵琶湖周辺を調査し、さらに進んで中国地方を経て九州へ渉った。特に九州旅行記は苦心の末に成ったと推測され、街道、住民、諸侯及び産業等詳細に記述されている。

A survey to investigate environmental behavior difference between the period of programmedblackout (March of 2011)and one month after the finish of the blackout (May of 2011)hasbeen performed. Three respondent groups and three behavior groups are obtained by clusteranalysis of the questionnaire data of environmental behaviors. The positive people forenvironmental behaviors do almost of all listed behaviors, the moderate people do two thirds ofthe behaviors, and the negative people do one third of the behaviors. It has a tendency that thenegative people think public offices have responsibility for environmental conservation, andhave less knowledge of ecorogical information.

We conducted a workshop that aimed to clarify problems with care workers supporting medication use in nursing homes, to propose measures for solving these problems, and to raise awareness of these problems among care workers. Eighteen care workers from different fee-based elderly nursing homes were enrolled in the workshop, and divided into four groups. The participants in these groups identified the issues based on their experiences regarding medication-related incidents, and discussed related problems and viable measures using the KJ method. The issues identified by each group were “dropping a medication”, “wrong resident”, “refusal to take medication”, and “confusion”. To resolve these problems, the participants recommended: “conducting study sessions or testing of manuals and medication knowledge”, “strengthening monitoring systems”, “enhancing information sharing”, etc. The involvement of pharmacists was hardly mentioned, despite the workshop being designed for “medication assistance”. A post-workshop questionnaire revealed that 88.9% of the participants acknowledged an increased awareness of safe assistance in the use of medication. A follow-up questionnaire, distributed approximately seven months after the workshop, revealed that 82.4% of participants applied the experience and knowledge they learned at the workshop to their work. The workshop seemed to raise awareness and lead to preventive measures for safe medication assistance. Communication between care workers and other health care professionals, such as pharmacists, is important to designing and implementing safe medical care in nursing homes.
