目的:この論文では疼痛構造化問診票とTMD+OFP診査表(脳神経検査含む)を用いた1人の歯痛を訴える患者の診断過程が述べられている.歯科医療に携わる者にとって歯痛診断は挑戦の連続である.また,その一方で歯痛診断は歯科医にしかできない.よって,他科との共同治療の面からも患者の現症や病歴などの情報としての診断は,その疾患の病態,治療法の知識と理解をも伴った正確なものでなくてはならない.しかしながら,臨床経験の浅い歯科医にとって,あるいは臨床経験豊かな歯科医にとっても複雑な症例を前にしたとき,それは困難な課題として専門家としての決断が揺らぐことも珍しくない.今回著者らは仮説演繹的な臨床診断推論手法と情報収集には疼痛構造化問診票,診査にはTMD+OFP診査表を用いて推論を行いその有効性を試験した.研究の選択:1名の下顎左側臼歯部の歯痛を訴える患者の診断を第1ステップ「包括的病歴採取」,第2ステップ「鑑別診断の列挙」,第3ステップ「鑑別の確認作業」,第4ステップ「鑑別診断の見直し作業」,第5ステップ「最終診断」の5つのステップで行った.また,第1ステップにおいては疼痛構造化問診票を,第3ステップではTMD+OFP診査表を用いた.結果:本症例は確定診断に至り,それによる治療法の選択で完治した.結論:疼痛構造化問診票とTMD+OFP診査表を用いた臨床診断推論により本症例の歯痛診断への有効性を確認した.

I report digenean metacercariae from Staurozoa, which were not previously known as digenean hosts. The host species, Haliclystus tenuis Kishinouye, 1910, was collected from algae in Oshoro Bay, Hokkaido, Japan, and contained metacercariae in the mesoglea. The metacercariae were encysted; cysts were oval, 93 μm long by 64 μm wide in one live individual. For the digenean, I generated partial sequences for the 18S rRNA (1585 bp) and 28S rRNA (1672 bp) genes, and the region spanning the 3′ end of the cytochrome c oxidase subunit gene and the 5′ end of the 16S rRNA gene, including the threonine tRNA gene (868 bp in total). Phylogenetic reconstructions based on combined 18S+28S datasets showed the digenean to belong in Opecoelidae, members of which utilize marine or freshwater teleost fishes as definitive hosts, and placed it in Plagioporinae (sensu lato) clade C within Opecoelidae.

Agriculture on Earth has developed at several areas in various ecosystems during long periods after the end of the final glaciation by replacing natural producers with crops and by making human as a sole cousumer mainly through the method of trial and error. On the contrary, agriculture on Mars should be created rapidly and unmistakably under natural conditions completely different from those on Earth. In addition, the agriculture on Mars should supply clean water and air as well as foods necessary for sustaining the emigrants to Mars. This can be achieved by a strategy consisting of two stages. The first stage is to modify the air, water, and debris inside an artificial dome somewhat similar to those on Earth. The second stage is to establish a sustainable agriculture on Mars. These two stages are discussed rather in detail.

本稿ではPL480タイトルIIによる学校給食贈与の成立過程について日米両政府の公文書に基づき検討し,以下の点を明らかにした.第1に,アメリカ政府は食生活改善のため脱脂粉乳贈与を提案したが,日本政府は粉食奨励による国際収支改善を訴え小麦贈与を認めさせた.第2に,日米二国間の利害だけでなく他国との協定の影響により交渉は難航した.第3に,日米は学童服への綿花贈与でも合意したが加工費負担問題により断念された.第4に,贈与小麦はパン用小麦でないためカナダ産小麦とのブレンドが必要だった.以上の知見は,学校給食によるパン食の普及を通じたアメリカ小麦市場開拓という従来の見解が成り立たないことを示すものである.

Purpose: The hazards of aerosols generated during dental treatments are poorly understood. This study aimed to establish visualization methods, discover conditions for droplets/aerosols generated in simulating dental treatments and identify the conditions for effective suction methods.Methods: The spreading area was evaluated via image analysis of the droplets/aerosols generated by a dental air turbine on a mannequin using a light emitting diode (LED) light source and high-speed camera. The effects of different bur types and treatment sites, reduction effect of intra-oral suction (IOS) and extra-oral suction (EOS) devices, and effect of EOS installation conditions were evaluated.Results: Regarding the bur types, a bud-shaped bur on the air turbine generated the most droplets/aerosols compared with round-shaped, round end-tapered, or needle-tapered burs. Regarding the treatment site, the area of droplets/aerosols produced by an air turbine from the palatal plane of the anterior maxillary teeth was significantly higher. The generated droplet/aerosol area was reduced by 92.1% by using IOS alone and 97.8% by combining IOS and EOS. EOS most effectively aspirated droplets/aerosols when placed close (10 cm) to the mouth in the vertical direction (0°).Conclusions: The droplets/aerosols generated by an air turbine could be visualized using an LED light and a high-speed camera in simulating dental treatments. The bur shape and position of the dental air turbine considerably influenced droplet/aerosol diffusion. The combined use of IOS and EOS at a proper position (close and perpendicular to the mouth) facilitated effective diffusion prevention to protect the dental-care environment.

The aim of this paper is to examine the Deliberative Organ (or Council)for broadcast programmes (Bangumi Shingikai), a statutory advisory bodyestablished by each TV and radio station in accordance with Japan’s broadcastact. Together with Programme Standards, the Deliberative Council is a significantpart of ‘Japan’s Regulation Model for Broadcasting’ that ensures the appropriatenessof the broadcast programmes. Designed to be self-regulatory, thecouncil enables broadcasters and audiences with high esteem to discuss thequality of the programmes and consider whether they are appropriate forbroadcasting. Despite its importance, little research attention has been given tothe practice of this system. In reflecting upon the history of the system and thegender and occupational ratio of committee members, this paper focuses onhow terrestrial commercial broadcasting stations have managed the council.Surveys were also conducted among these stations in 2016, and the results indicate that the system is not fully regarded as a system of self-regulation amongbroadcasters, although it is generally valued from the point of programmeimprovement.

近年考案された注意障害の行動評価尺度 (BAAD,Behavioral Assessment of Attentional Disturbance) は,原則的に作業療法施行中の場面を作業療法士 (OT) が観察してスコア化する.今回,家庭での家族による評価を実施し,注意障害の評価に有用か検討した.脳障害者 (脳卒中,脳外傷など) 53 名を対象とした.OT,家族による評価合計点 (最高18 点) はほぼ一致し,級内相関係数も0.89と高値を示した.項目別に検討すると,6 個中5 項目では64 %以上で両検者のスコアが完全に一致したが,1 項目のみ43 %に留まった.以上より家庭での評価も「注意」障害の検出に有用と考えられた.

スギ花粉の主要抗原 Cry j 1 および Cry j 2 の主要なT細胞エピトープを連結して作成した 7Crp ハイブリットペプチドを発現させたスギ花粉米を開発した. 7Crp ペプチドはスギ抗原特異的 IgE 抗体と反応しないことから, 副作用が起こりにくくスギ花粉米を大量に摂取することが可能である. この米を炊飯加工して, パック米 (80g) としての剤形でスギ花粉症患者に提供したところ, 花粉飛散時を含む 5 ヵ月間の経口摂取を通じてスギ花粉抗原に対する免疫寛容が誘導されていることが示された. 臨床症状に関しては, 鼻症状が軽減化する傾向が観察された.
