当院の1ヶ月外来患者数964名のうち、イボの患者は40名、約4%。10歳未満が27%、20歳未満が17%であわせて50%弱である。治療内容は液体窒素による凍結療法が62%、モノクロロ酢酸が23%、サリチル酸と凍結療法が15%であった。治療期間を見てみると、1ヶ月以内が29%、3ヶ月以内が15%、1年以上は8%であった。<br>イボ治療は1960年代に凍結療法、1970年代にブレオ局注、グルタールアルデヒド、5FU外用、DNCB感作療法などがあり、1980年代に入って、レーザー治療などが導入されている。「痛くない」治療は、1970年代から検討されはじめているといってよい。その中にはグルタール、モノクロロ酢酸、5FU、DNCBを用いた接触免疫療法がある。<br>従来当院で行っていたグルタールは取り扱いに注意を要するため代わる治療として、モノクロロ酢酸を使用して週1回通院治療をして比較的良好な結果を得ている。腐食後厚くなった皮膚を削いだ時の疼痛以外大きなトラブルはない。「痛くない」治療としては最近ビタミンD3外用なども行われている。(オンラインのみ掲載)

本書は、「女性兵士問題」「国家と『性暴力』問題」「女性性器手術問題」という、フェミニズムにおいて近年クローズアップされてきた問題を論じた、9人の執筆陣からなる論文集である。第I部から第III部のそれぞれに男性の執筆者が1人ずつ含まれることで、議論により一層の広がりが与えられているように思える。<BR>まず第I部で、軍隊内で男女にまったく対等な処遇を行うかどうかという「女性兵士問題」を題材として、フェミニズムが「暴力」とどのような関わりをもつべきかが議論される。続いて第II部では、「従軍慰安婦問題」と「夫婦間強姦」を題材として、近代国民国家においては十分保障されてこなかった「性暴力を受けない権利」をめぐる議論が展開される。さらに第III部で、「女性性器手術問題」を題材として、「第一世界」のフェミニストたちが「第三世界」の女性の経験を「性暴力」の被害として規定すること自体の「暴力」性についての議論が行われる。最後に第IV部で、編者による議論の総括が行われる。<BR>一見しただけでは無関係にも思えるこれらの問題の背後には、共通するより大きな問いが存在している。すなわち、グローバル化が進行しつつある現代において、フェミニズムは、「性」の違いによって「暴力」に関する異なる経験を強いてきた近代国民国家 (=「ネーション」) とどう関わっていくべきかという問いである。しかし、これに答えるのはそうたやすいことではない。もし、フェミニズムが国民国家の枠を越えて「性」と「暴力」の問題に取り組むべきであるとするならば、他国に暴力を行使する軍隊の存在を前提としてそこでの男女の機会均等を主張することは慎まねばならないし、たとえ国家によって合法化・正当化された営みであっても女性の人権侵害と見なされるならば「国家批判」や異文化への「介入」も必要となってくる。しかし他方で、国家を越えた問題設定は国内での性差別を不可視化させる危険性を伴うし、女性の人権のうち国民国家の枠によってこそ保障されうる側面や、異文化への「介入」にともなう「暴力」性をどう考えるのかという問題も起こってくる。<BR>本書には、この問いに対する明確な回答は記されていない。編者がわれわれ読者に求めているのは、本書から唯一の正答を見つけだすことではなく、むしろ本書が提供してくれる議論を足がかりとして、女性あるいは男性として今後国家とどのように関わっていくべきなのかを1人1人が考えていくことなのであろう。

Marketing authorization holders do not disclose any information on the pharmaceutical properties of over-the-counter drugs (OTC). When a drug is switched from a prescription drug to OTC, pharmacists can acquire that information from the corresponding ethical drug (ED) through the package insert, interview form, and so on. However, the pharmaceutical equivalence between ED and OTC is unclear. In this study, we examined the drug dissolution behavior of both ED and OTCs containing scopolamine butylbromide. Dissolution tests were performed by the paddle method using Japanese Pharmacopeia (JP) XV test fluids at pH 1.2, 4.0 and 6.8 and water based on the guidelines for bioequivalence studies of generic products. The dissolution profiles of OTCs differed significantly from ED showing a similarity factor (f2) value ranging from 8.9 to 42.9. Time until 85% dissolution ranged from 23 to 95 min and from 17 to 174 min at pH 1.2 and pH 6.8, respectively. Then JP XV disintegration tests were conducted to investigate differences in the disintegration process. The disintegration time of preparations showing delayed dissolution was prolonged compared to that of others, suggesting that the disintegration of the tablet or capsule is one of the important factors affecting the drug dissolution. These differences in the disintegration and drug dissolution might cause differences in the bioavailability of the drug. For patient safety, more detailed product information of OTCs should be supplied by the manufacturer, and not be assumed from that of corresponding ED.

アトピー患者白血球および受動的感作ヒト肺切片からのアナフィラキシー性histamineおよびleukotriene (LT) 遊離に及ぼすtranilastの影響について検討した.1.10^<-5>g/mlのtranilastの反応惹起30min前処理は, 末梢血白血球からのhistamine遊離を抑制しなかったが, peptide LTsの遊離を抑制した.2.10^<-5>および10^<-4>g/mlのtranilastの反応惹起30min前処理は, 感作ヒト肺切片からのhistamine遊離のみならず, LTB_4およびpeptide LTsの遊離をも抑制した.以上の成績から, tranilastの抗喘息効果の一端は, histamineおよびLT遊離を抑制する機構によることが示唆された.

I.症状 思春期あるいは性成熟期の女性にみられる,子宮以外の場所からの周期的な出血と定義できるが,これは月経出血とほぼ同時におこるもののほかに,月経出血をともなわないが周期的に1ヵ月に1回くらいの割合でおこる性器外出血の意味に用いられていることもある。 しかし診断上のまぎらわしさをさけるためには前者のみを代償月経とよぶべきであろう。

Background:The Resolute Integrity coronary drug-eluting stent (DES), the next evolution of the Resolute DES, is designed with thinner stent struts and continuous sinusoidal technology to further enhance performance. This study evaluated the performance of the Resolute Integrity DES compared with the historical performance of Xience V.Methods and Results:We assessed the safety, efficacy, and deliverability of Resolute Integrity compared with the Xience DES, the prespecified historical control, in PROPEL, a large, real-world prospective, single-arm, open-label study. A total of 1,204 subjects were enrolled in PROPEL and treated with the Resolute Integrity DES at 76 sites in Japan. Lesion and procedural success rates were 100% and 99%, respectively. Patients were equally complex in the Resolute Integrity and Xience cohorts. At 1 year in the clinical-only cohort, the primary endpoint target lesion failure was 4.3% (34/800) in the Resolute Integrity cohort compared with 8.5% (97/1,142) in the Xience historical controls (P<0.001 for non-inferiority). Target vessel failure, major adverse cardiac events, target vessel myocardial infarction, and the composite cardiac death and target vessel myocardial infarction were all lower with Resolute Integrity compared with Xience, including in “high risk” patients.Conclusions:In the large, prospective, multicenter PROPEL trial, the performance of the Resolute Integrity DES was non-inferior to that of the benchmark Xience DES used as a historical comparator.

A cohort study was conducted on 27 patients suffering from chronic obstructive pulmonary disease (COPD) and visiting outpatient medical facilities within Osaka Prefecture, to elucidate meteorological factors related to their complaints of dyspnea and disease exacerbation.Measurements in patientsʼ bedrooms made daily between 00:00-06:00 from July 1 to August 31, 2012 showed that the percentage of patients exposed to a wet-bulb globe temperature (WBGT) of ≧ 28°C increased between mid- and late July, reaching a one-day peak of 21%. In addition, 62.5% of patients experienced a WBGT of ≧28 °C at least once during the monitored hours in the same period. The results of applying a logistic regression model to data from June 1 to September 30, 2012 showed that the minimum WBGT in a patientʼs bedroom, even when controlled for daily minimum temperature, has statistically significant association with complaints of dyspnea and bad physical condition. This finding demonstrated that regulating a patientʼs bedroom environment is very important in the summer season.
