Although enormous efforts have been made to develop anti–amyloid therapy against mild to moderate Alzheimer's disease (AD) dementia, no single drug has succeeded in clinical trials. These failures could be attributed to 1) inaccurate clinical diagnosis, and 2) inappropriate timing for intervention. The clinical diagnosis used in previous trials are clinical criteria with low sensitivity and specificity. Thus new criteria that incorporated biomarker results has been established. In addition, various longitudinal studies revealed that Alzheimer's disease develops dementia long after it's pathological starting point where almost 15 years of asymptomatic stage precedes mild cognitive impairment (MCI) due to AD or prodromal AD. This asymptomatic stage is now called “Preclinical AD” and thought to be a promising intervention period for anti–amyloid therapy. We started an observational study in Japan which is funded by the Japan Agency for Medical Research and Development (AMED) recruiting 500 individuals in total, that follows cognition, biomarker samples, and multimodal images for 3 years. Moreover, in the US, there are multiple drug intervention studies for preclinical AD subjects. Hopefully, these efforts could help developing AD prevention.

山東京伝の有力な後援者の一人であった松前藩主松前道広の弟、松前文京(俳号泰郷)の俳諧活動を調査・分析し、その俳諧活動を明らかにした。文京(俳号泰郷)は江戸座俳諧宗匠存義を師とし、弟の松前武広(俳号李井)とともに、大名子弟の俳人柳沢米翁・本多清秋・松平雪川・酒井抱一らと俳諧交友を行っていたことを解明した。これによって文京の江戸座俳諧活動が明らかとなり、山東京伝の係わった江戸文化圏の一端を解明することができた。

本稿は主にJ.A. Smith et al.,(2009)に依拠して,IPAを概説しそれに基づいてIPAの問題点を論ずる。IPAは一人称の経験談を三人称の視点から分析する,現象学,解釈学,そして個性記述学から影響を受けた心理学的方法論である。IPAには様々な批判がなされているが,本稿ではその最大の問題点は二重の解釈といいながら,一人称+三人称の方法論に二人称の方法論が紛れ込んでいる点にあることを論じている。

近年,タブレット端末利用の需要が高まっており,市場も拡大傾向にある.しかし,タブレット端末を使用する際に不便だと感じたことの調査では,文字入力がしにくいという回答が多数存在する.市場で流通している入力装置では,価格や可搬性の低下といった問題があるためAR技術とタブレットPC内臓のカメラを用いた入力装置を提案する.

プリオン病は正常プリオン蛋白が伝播性を有する異常プリオン蛋白に変化し蓄積することにより発症する.孤発性,遺伝性,獲得性の3種類があり,本邦では1999年からの約10年間のサーベイランス調査で1,320名の患者が確認され,硬膜移植CJD例が多いことや遺伝性CJDで本邦に特異的な変異例が多いなどの特徴がある.孤発性古典型では急速進行性の認知症,四肢のミオクローヌス,MRI拡散強調画像で大脳皮質と基底核の高信号,脳波でPSDなどの特徴的な所見をみとめるが,非典型例も少なくない.臨床症状,髄液検査,遺伝子検索により的確な診断をくだすことが,病態の解明,二次感染予防,心理サポートにおいて重要である.

進行性非流暢性失語 (PNFA/nfvPPA) 24 例を対象に, 神経学的所見, 神経心理学的所見, 画像所見, および経過を検討した。その結果, 少なくとも 3 群に分類された。(1) 前頭葉性失語型 (古典的失語症分類ではブローカ失語, 超皮質性運動失語に該当する言語症状を呈する群) , (2) 前部弁蓋部症候群型, (3) 純粋失構音型である。(1) は発症 3 ~5 年以内に, bvFTD と同様の精神症状や行動異常を呈した。(2) は発症早期に流涎や口部顔面失行を呈し, 1 ~3 年以内に嚥下障害を呈した。(3) は失構音のみ進行する場合, 失調症状を伴う場合, 発症から 2 ~5 年以内に, パーキンソニズムや中心回症状を呈し, CBS と診断されるに至った場合など, いくつかの亜型分類がさらに可能であった。これらの群は, 画像診断で明らかな所見が得られない時期でも, 初診時の症候学的検討で, 分類することが可能であると考えられた。

急性一過性の健忘を呈する患者のなかに, 健忘の原因がてんかん性の機序であるような, 一過性てんかん性健忘 (transient epileptic amnesia: TEA) と呼ばれる一群がいる。TEA の患者は, 典型的には発作時でも記憶以外の認知機能がほぼ保たれる。発作時を除けば, 記憶を含む認知機能検査の成績はあまり低下しない。一方で, TEA 患者には, てんかん発作に起因すると思われる, 発作間欠期に生じる特徴的な健忘症状もしばしば認められる。加速的長期健忘 (accelerated long-term forgetting) などと呼ばれる前向性健忘や, 主に自伝的記憶の障害で現れる遠隔記憶障害 (逆向性健忘) である。てんかん性健忘は認知症と誤診されることもあるが, 抗てんかん薬の治療により軽快する疾患であり, 鑑別は重要である。本稿ではてんかん性健忘と診断された症例を紹介し, 特徴的な健忘症状について述べる。

Although Funatsu Denjibei was a trained farmer, he worked at the Komaba Agricultural School, where he managed the farm and lectured students on traditional agricultural techniques. All the other teachers at the Komaba Agricultural School were foreigners and lectured the students on Western Agricultural Science. In this paper, we clarify Funatsu Denjibei’s attitude toward Western Agricultural Science.Funatsu Denjibei contributed to the establishment of modern agricultural science in Japan by imparting empirical knowledge to agriculturists. He kept company with the other foreign teachers and acquired knowledge on Western Agricultural Science from them and the young agriculturists who graduated from the school. Although he used some of this knowledge, it did not influence his agricultural techniques. The agricultural techniques that he advocated had accumulated over years of experience. He attached great importance to them and when the ideas held by other agriculturists ran contrary to his experience, he always retorted and strongly questioned their opinions.

The Experiences Questionnaire measures decentering, the state of observing thoughts and feelings as temporary events in the mind. This study developed the Japanese version of the Experiences Questionnaire (JEQ). The fit indices in confirmatory factor analysis (n=297) suggested an acceptable fit to a model consistent with the original. The correlations (n=411) between the J-EQ and the Acceptance and Action Questionnaire- II, the Affective Control Scale, the Ruminative Response Scale, and the Cognitive Control Scale showed adequate construct validity. Internal consistency (n=411) and test–retest correlations of factors (n=54) indicated good reliability of the J–EQ. The J–EQ can be used to examine the influence of decentering on psychopathology.
