Background: Several reports have validated the criteria for damage control surgery (DCS). However, although metabolic acidosis and body temperature can be measured quickly, tests for predicting the severity of coagulopathy require special laboratory equipment and take 15 to 30 minutes. Such delays could be life-threatening for patients requiring DCS. The aim of this study was to establish simplified and practical criteria to enable rapid decision-making regarding the need for DCS. Methods: Thirty-four consecutive patients with unstable hemodynamics after initial fluid resuscitation who had undergone DCS for severe abdominal or pelvic injuries were retrospectively analyzed. The patients' characteristics, clinical courses, laboratory data, and outcomes were reviewed using the data contained in their medical records. Results: The overall survival rate was 55.9% (survivors group: n=19; nonsurvivors group: n=15), which was similar to the calculated mean probability of survival (Ps=0.5671). At the start of surgery, the systolic blood pressure (SBP) was less than 90 mm Hg in all cases in which surgery failed, and the mean SBP in the nonsurvivors group (69.6 ± 14.8 mm Hg) was significantly lower than that in the survivors group (93.2 ± 22.9 mm Hg, p=0.006). Except in two cases, the value of the base excess in the nonsurvivors group was less than -7.5 mmol/L, and the mean base excess (-11.5 ± 5.3 mmol/L) in the nonsurvivors group was significantly less than that in the survivors group (-5.5 ± 4.9 mmol/L, p=0.008) at the start of surgery. The core temperature at the start of surgery was less than 35.5°C in all cases in the nonsurvivors group. On the basis of these results, three indicators (SBP less than 90 mm Hg, base excess less than -7.5 mmol/L, and core temperature less than 35.5°C at the start of surgery) were identified. The success rate of DCS in patients who possessed all three indicators (28.6% ) was significantly lower than that in patients who did not possess all three indicators (75.0%; p=0.014). Conclusion: Our results indicate that surgeons should decide to perform DCS when only one or two criteria defined in this study are met and should not wait for all three criteria. Although our proposed criteria are not strict and may broaden the indications for DCS, leading to an increase in the number of DCS procedures, saving the lives of patients who have sustained severe torso trauma must be the priority; 'over-triage' may be acceptable in situations where an appropriate decision-making protocol has been followed.

In this paper, I investigate El Greco's St. Ildefonso, currently placed at the Chapel of the Hospital of Charity in Illescas. This was painted in his most prolific stage, around 1600. Until recently the details of the commission issued to El Greco have not been obvious. Here, I present a tentative plan concerning the ditails of the commission of St Ildefonso, examining the relationship between El Greco and Salazar de Mendoza, a patron of El Greco. First, I focus on how St. Ildefonso is depicted as a Church Father, or author/scholar figure, which differs from the general representation of St. Ildefonso which portrays him receiving the chasuble, or dressed as an archbishop. I demonstrate how that El Greco's Ildefonso resembles the figure of St. Jerome in his study and I examine the description on St. Jerome and St. Ildefonso in Mendoza's work El Glorioso San Ildefonso…(1618), thus adding evidence for the likelihood of an influence from Mendoza. Moreover, I suggest a close connection between Charity and Mendoza. This is evident from the fact that El Greco and Charity both signed a contract for the retable at the Hospital Tavera, of which Mendoza was administrator. The goal of this paper is to present the details of the commission of St. Ildefonso, considering the intervention of Mendoza, an Toledan.

We have investigated trapping effects of monoatomic vacancies Vmono and alloy atoms on hydrogen diffusion at Al(111) surfaces. We performed first-principles calculations based on density functional theory with generalized gradient approximation in order to obtain the hydrogen adsorption energies in the vicinity of monoatomic vacancies and alloy atoms in Al(111) subsurfaces. We considered Si, Cr, Mn, Fe, Cu, Ge, and Zn as alloy atoms. We substituted one Al atom with one monoatomic vacancy or alloy atom, which corresponds to Al0.95X0.05(111) (X=Si, Cr, Mn, Fe, Cu, Ge, Zn, Vmono) surfaces. We found that all of monoatomic vacancies and alloy atoms increase adsorption energies. We also clarified that hydrogen atoms make strong covalent bonds in Cr, Mn, Fe-alloyed Al(111) subsurfaces, while they make only weak ionic bonds in pure and Si, Cu, Ge, Zn-alloyed Al(111) subsurfaces.

目的本報告の目的は,東京都における2011年の麻しんの流行状況を把握し,麻しん排除に向けて,今後の対策へ反映させることである。方法感染症発生動向調査事業によって2011年に都内で報告された麻しん患者の発生届と麻しん調査票の内容を対象とした。年齢階級別,遺伝子型別,ワクチン接種歴別の発生状況,発生届の取り下げ状況,遺伝子検査結果別の IgM 抗体検査結果と検体採取日の関係等について分析を行った。結果 2011年の麻しん患者報告数は178件で,13週から24週にかけて一つのピークを形成し,この期間に128件(71.9%)の報告が集中していた。1~4 歳が40件(22.5%)で最大で,20歳代と30歳代がそれぞれ34件(19.1%)であった。麻しん患者が 2 人以上発生した施設数は 6 か所,1 事例の患者数は 4 人以下にとどまり,散発例がほとんどであった。1~4 歳の年齢層を除くすべての年齢層で,接種歴が無いか不明が過半数を占めた。PCR 遺伝子検査で検出された麻しんウイルスの遺伝子型は主に D4 型,D9 型で,2008年まで国内流行株の中心だった,いわゆる土着株である D5 型ウイルスはみられなかった。結論 2011年の麻しんの流行は,海外から持ち込まれたウイルス株によるものであった。2007年,2008年のような大流行に至らなかったのは,発生後の関係機関の対応が迅速かつ適切になったことと感受性者が減少したことが影響していると思われる。麻しん排除に向けて,引き続き麻しんワクチン接種率の向上に努めること,麻しん疑い患者に確実に遺伝子検査を実施し,的確なサーベイランスを実施することが重要と考えられた。

Twelve distributional patterns of fluorescent body parts, including complete non-fluorescence, were found in diverse taxonomic groups of small hydromedusae (≤21 mm in umbrellar diameter) collected in 2008-2010 in Tanabe Bay and a freshwater pond in Tanabe city, Wakayama Prefecture, Japan, and in Suma harbor, Kobe city, Hyogo Prefecture. Only three of the green fluorescence patterns have been described until now. Particular fluorescence patterns may have originated convergently more than once within this taxonomic group. In Liriope tetraphylla the distribution of fluorescence changes during development, and the eggs of Clytia languida also display fluorescence.

目的愛媛県東部地域(以下東予地域)に発生した成人麻疹流行を分析し,保健所における今後の感染症対策のあり方を検討した。方法東予地域では,2002年10月~2003年 7 月の間に成人麻疹(18歳以上の麻疹)および麻疹(17歳以下の麻疹)の流行が発生したが,流行期間中,感染症発生動向調査だけでは把握が困難と判断されたので愛媛県医師会の協力により全数把握調査を行った。また麻疹を診察した医師に患者病状調査票による情報提供を依頼した。さらに成人麻疹多発の原因究明を目的に患者の検体のウィルス検査および遺伝子解析を行った。成績 2002年10月~2003年 7 月の間に,麻疹200人,成人麻疹112人,計312人の麻疹患者が報告され,県全体に占める割合は麻疹89.7%,成人麻疹94.1%,全体で91.2%であり東予地域に限定した流行であった。さらに週毎の発生数の推移から成人麻疹発生から麻疹が流行した事例であった。患者疫学調査の結果,ワクチン接種歴無しの割合は麻疹84.1%,成人麻疹59.3%,全体で73.7%であり,接種歴有りの割合は麻疹11.4%,成人麻疹21.9%,全体で15.8%であった。そしてウィルス遺伝子型は全例で中国や韓国の流行株である H1 型であり,H1 型を原因とした成人麻疹の流行としては国内初の事例であった。また東予地域での小児科定点の麻疹患者報告数は全数把握の32.0%であり,基幹定点の成人麻疹患者報告数は全数把握の11.6%に止まった。結論東予地域では患者発生の極めて少ない状況が数年来続いたので,ワクチン未接種でも感染を免れた成人や小児(とくに年長児)および,ワクチン既接種者でも不顕感染による追加免疫がないために免疫力の低下した者(二次性ワクチン効果不全)が混在したことで成人麻疹の流行が発生したと考えられた。今回の結果より,乳幼児のワクチン接種率の向上と追加接種による学童や若年者への対策が必要である。また麻疹のように感染力が強く局地的に流行する感染症の場合,通常の定点報告では流行を見逃し対応が遅れる可能性が高いため,患者発生状況の的確な把握には,定点数の拡充および地元医師会を中心にした医療機関と保健所の平素からの積極的な情報交換が必要と考えられた。
