症例は71歳の女性.心窩部痛を契機に上部消化管内視鏡検査で十二指腸第II部に変形・狭窄および易出血病変を認めた.腹部造影CTでは膵頭部に径50mm大の充実性腫瘤を認め,門脈・脾動脈および十二指腸への浸潤が疑われた.ERPで主膵管は膵頭部で途絶していた.膵液細胞診はadenocarcinomaであり,門脈および脾動脈浸潤を伴う切除可能境界膵頭部癌(cT4, N1, M0, cStage IVa)と診断.門脈および脾動脈切除・再建を伴う膵頭十二指腸切除術を施行した.病理組織所見では,腺管構造の不整を伴う高分化型腺癌が主体で,腫瘍間質の線維化が強く,門脈浸潤を認めた.術後補助療法として放射線化学療法(GEM+RT)およびS-1による6ヶ月間の維持療法を施行した.術後の栄養状態とQOLは良好で,7年7ヶ月経過した現在,無再発生存中である.

A relatively high incidence of chronic limb pain, frequently complicated by violent, tremulous involuntary movements, has been noted in Japanese girls following human papillomavirus (HPV) vaccination. The average incubation period after the first dose of the vaccine was 5.47±5.00 months. Frequent manifestations included headaches, general fatigue, coldness of the legs, limb pain and weakness. The skin temperature examined in the girls with limb symptoms exhibited a slight decrease in the fingers and a moderate decrease in the toes. Digital plethysmograms revealed a reduced height of the waves, especially in the toes. The limb symptoms of the affected girls were compatible with the diagnostic criteria for complex regional pain syndrome (CRPS). The Schellong test identified a significant number of patients with orthostatic hypotension and a few patients with postural orthostatic tachycardia syndrome. Electron–microscopic examinations of the intradermal nerves showed an abnormal pathology in the unmyelinated fibers in two of the three girls examined. The symptoms observed in this study can be explained by abnormal peripheral sympathetic responses. The most common previous diagnosis in the studied girls was psychosomatic disease. Additionally delayed manifestation of cognitive dysfunction in the post–vaccinated girls has been paid much attention: memory loss, difficulty in reading textbooks and/or calculation.

与えられた文書集合を特徴付ける単語を選出することは,様々に応用できる有用技術である。「文書集合を特徴付ける」を,「文書集合中に特異的に多く現れる」と解釈し,これを捉えるために,文書集合D中の単語wに対し,以下の確率値に基づく重み付けを提案する。すなわち,全文書D_0中の単語数をN,wのD_0中での頻度をK,Dの単語数をn,wのD中での頻度をkとしたとき,「N個の玉の中にK個の赤い玉があるとき,任意に取り出したn個の玉の中に赤い玉がk個以上含まれる確率」が小さいほど,wに大きな重みを与えるのである。この指標の有効性を,5指標に関する比較実験により示し,併せて上記の確率の効率的計算方法を述べる.This paper proposes a method of selecting "characteristic words" from a document set. The selection is done by using the weight that is assigned to each word in the document set. The weight is calculated by using the hypergeometric distribution. A comparative evaluation of five methods of word weighting (including tf-idf and SMART) revealed that the proposed method is superior to existing methods. An effiecient method of calculating the hypergeometric probability is also shown.

Basel regulatory framework, one of CAR (capital adequacy ratio) regulations, is said to make markets destabilized in a previous study. But the previous study included some inappropriate assumptions. So, this study assessed this destabilizing effects with a new model. In my model, FCN agents and 2 kinds of portfolio agents, CAR regulated ones and not regulated ones, were included. Using this model, some simulations were run. As results, the simulations revealed some facts: 1. Asset management using portfolio stabilizes markets and the stabilizing effect are significant if there are a lot of markets included in the portfolio; 2. CAR regulation destabilizes markets and vanish the stabilizing effects of portfolio. In addition, the results of my simulations suggest that CAR regulation does not only raise the chance of price crashes but also depress whole price.

ヒノキおよびスギ人工林化による土壌の養分特性の変化を明らかにすることを目的に,落葉広葉樹天然林伐採後に人工造林されたヒノキおよびスギ林と,天然更新した広葉樹林が同一斜面上に成立する調査地において,A_0層の乾重および養分含有量,鉱質土壌の化学的性質を調査した。針葉樹林の斜面上部にはヒノキが(ヒノキ林),斜面下部にはスギが(スギ林)それぞれ植栽されている。本研究では,斜面位置ごとに調査結果を広葉樹林と針葉樹林において比較した。ヒノキ人工林化によって土壌深0-30cmにおける交換性塩基量に違いは見られなかった。スギ人工林化によって土壌深0-30cmにおける交換性塩基量は隣接する広葉樹林に比べて多く,特に交換性Ca量は約1.9倍であった。土壌深0-30cmにおける全C,N量,CECは斜面上部のヒノキ林では広葉樹林に比べそれぞれ約0.6,0.6,0.8倍であったが,斜面下部のスギ林では広葉樹林とほぼ同じであった。A_0層量は斜面位置にかかわらず,広葉樹林で約7.1Mg/ha,ヒノキ林とスギ林では共に約9.5Mg/haであった。本調査地のA_0層のC/N比は広葉樹林に比べて針葉樹林で高かった。各調査区のA_0層中の元素含有量と土壌深0-30cmの全C,N量および交換性K,Ca,Mg,Na量の関係について検討したところ,Caについてのみ有意な正の相関関係がみられた。スギ人工林化によってA_0層中に蓄積されたCaの影響を受けて交換性Ca量が増加していると推察された。

In this paper, urban societies of Okinawa, particularly those around U.S. military bases, are viewed as colonial cities, and their special composition and the mobilization and deployment of Okinawa's population after the "reversion to Japanese administration" are examined. Here, Okinawa's society is seen from the viewpoint of postcolonialism study, applying it to the sociological analysis of power relationships in colonial cities. U.S. military bases are an important factor that define the postcolonial characteristics of Okinawa's society. This paper examines how the politics of Okinawa City (Goeku:Koza) blends in with the practices of postcolonialism, and how they exert a synergistic effect in hiding colonial characteristics. Okinawa City is located in central Okinawa where U.S. military bases and urban areas are squeezed together, adjoining to each other. As an example, this paper looks into the Shirakawa Flea Market, which was formed and has been expanding in the forest of Kadena Ammunition Storage Area, and the inner-city issues concerning the city center of Okinawa City. Through these, this paper describes the relationship between postcolonialism and recomposition of urban spaces.

Cet article se propose de réduire le décalage entre les recherches spécialisées et l'image commune concernant la pensée pédagogique de J. -J. Rousseau. On dit souvent que Rousseau, confiant en "la bonnté originelle" de l'enfant, se fait l'apôtre d'une "éducation négative", qui exclut les pressions et les directive par les adultes. D'autre part, des spécialistes insistent sur le fait que la volonté de l'élève est sous l'emprise du gouverneur omnipuissant dans l'<I>Emile</I>. Nous pensons que cette lecture commune surestime l'argument de "la bonté originelle". Pour démontrer cet argument, Rousseau distingue "l'amour de soi" absolument bon et naturel d'avec "l'amour-propre", forme modifiée ou dépravée du premier. On interprète souvent "l'amour de soi" comme le principe de toute léducation d'Emile. Il faut remarquer cependant que "l'amour-propre" peut être investi de valeurs positives chez Rousseau. En ôtant toutes les relations artificielles acquise dans la société pour introduire "l'amour-de soi" comme une forme spéciale de "l'amour-propre", Rousseau critique les jus-naturealistes modernes qui approuvent les passions interéssées et les théologiens qui nient l'amour-propre par l'invocation du péché originel; les uns et les autres coucluent, par une route opposée, que la société telle qu'elle est est nécessaire, soit pour satisfaire les désirs humains, soit pour maintenir l'ordre social. Cette critique qui se fonde sur "la bonté originelle", n'est qu'un <I>préparatif</I> pour "la vraie éducation" qui envisage de faire former uc nouveau par l'élève des relations convenables avec les autres êtres selon le développement de ses facultés.

This study reviewed an over one-year follow-up results of 18 cases who underwent posterior lumbar interbody fusion with Brantigan I/F cage. The clinical success was defined according to Hirabayashi's scale. 9 cases were categorized as excellent, 4 cases as good, 2 cases as fair, and the remaining 3 cases as poor. Plane X-ray radiographs revealed bone union in 18 out of 20 levels (union rate 90.0%), and non-union in only one level. The remaining cases could be concluded as neither union nor non-union because of interrueted bone trabeculae. Assessment results of X-ray radiographs were most or less satisfactory and correction loss was not observed.<br>X-rays of some patients treated for degenerative spondylolisthesis showed carbon fiber cages buried deep in the upper vertebral bodies. A reaming device could be slipped onto the edges of the lower vertebral bodies, so pre-shaving them would be important. It is important that the autologous bone graft harvested from the lamina by laminectomy be unrelated with any donor site pain from the ilium. However it should be borne in mind that of MOB patients have poor bone quality after the autologous bone graft, the possibility of non-union is high. We concluded that the Brantigan I/F cage is an useful implant, but certain characteristics should be clarified for clinical and union success.
