PURPOSE : This study aims to develop a scale to measure childbirth experience quantitatively and to evaluate reliability and validity of the scale. METHODS : This is a cohort study examining relationship between childbirth experience and following physical and psychological states of mother and child. All women who gave birth at five study centers (four birthing homes and one maternity hospital) from May 2002 to August 2003 were asked to participate in the study. A total of 2314 women were recruited and 1453 of eligible women agreed to participate. Based on the literature review and the written notes by women after childbirth, a multi-disciplinary group consisting ten specialists generated 45 items for inclusion. RESULTS : After examining the content of the items of factors and correlations between factors, the CBE-scale was presented to consist of four positive factors and 18 items ; such as Happiness (4 items), Body Sense (6 items), Discovery (5 items) and Unaffected Self Expression (3 items). It was shown that the CBE-scale was internal consistency with Cronbach's Alphas and itemtotal correlation. Concurrent validity was discussed using a question of subjective attitude towards childbirth such as "Do you feel that the other women would also have the same childbirth experience as you had?" The prediction validity was confirmed using three questions as follows. "Would you like to be pregnant again and to give a birth?", "Do you enjoy being with your baby?" and "Do you feel like holding your baby in your arm all the time?" These questions were asked repeatedly at four-month-, nine-month-and sixteen-month-visits. Students' T-test and logistic regression analysis were carried out to examine relationship between these 3 items and the scale score. CONCLUSIONS : These findings support that the CBE-scale has reliability and validity. The CBE-scale has potential to measure women's subjective experience of childbirth and contribute to an assessment of the quality of care during childbirth.

本研究の目的は,2004年の卒後医学教育改革前後の医療の質を比較することである.日本の8つの臨床研修指定病院において研修中の医師が本研究に参加した.参加した医師は,外来において頻度の高い疾患(糖尿病, 慢性閉塞性肺疾患,心血管疾患,うつ病)についての臨床シナリオに回答した.回答をエビデンスに基づいた診療の質の基準に照らしあわせて採点し,正答率スコアを算出した.ローテート研修が導入された前後でスコアの変化が生じたかを検証するために,2003年の参加者のスコアと,2008年の参加者のスコアを比較した.2003年では,141名(70.1%)が,2008 年には237名(72.3%)が参加に同意した.交絡因子を調整後も,両年の間にスコアの違いを認めなかった(2003 年からのスコアの変化 = 1.9%. 95% CI -1.8 to 5.8%).教育改革前の研修プログラムがストレート研修の施設ではスコアが 3.1% 改善しており,改革前にローテート研修を採用していた施設の改善度1.4% と比較して有意に高値であった.全般的には,2004 年の医学教育改革前後において,研修医の医療の質は変化していなかった.

日本南岸の黒潮流路の変動特性を知るため,1975年から1995年までの都井岬から房総半島沖までの9点からの黒潮の離岸距離を海上保安庁水路部の海洋速報の黒潮流路の中央点との距離として求め,その時間変動を調べた。その結果1975年に発生した黒潮大蛇行は室戸岬から大王崎にかけて離岸距離が大きく御前崎以東で離岸距離が小さいのに対し,1980年以後の五回の大蛇行は室戸岬から潮岬では離岸距離が小さく御前崎以東で離岸距離が大きくなり,大蛇行の流路のパターンが1980年前後で大きく変化していることが示された。大蛇行期ごとの平均距離をみると,1975年発生の大蛇行は伊豆海嶺の三宅島と八丈島の間のゲート領域を通るのに対し,1980年以降発生の大蛇行は平均距離が伊豆海嶺のゲート部よりも南に位置し,C型流路かゲート部を通る流路の選択を強制されることが示唆された。このため1980年以降発生の大蛇行は流路に及ぼす伊豆海嶺の地形効果が大きく,低気圧渦である大冷水塊を伴う大蛇行が比較的短時間で消滅する可能性が示唆された。九州南の潮位差解析により黒潮の南側分流の流量が大きいと都井岬から室戸岬沖の黒潮離岸距離が大きくなり,御前崎から石廊崎沖では離岸距離が小さくなる傾向が示された。一方北部流量が大きくなると,都井岬から潮岬沖の離岸距離が小さくなり御前崎から野島崎沖で離岸距離が大きくなる傾向がある。また,犬吠埼沖では黒潮の離岸距離が九州南の潮位差と有意な相関を示さない。

This paper addresses ontologies of intentional and unintentional processes. Specifically, a methodology for developing processes ontologies is described. Typically, domain ontologies are developed in an ad-hoc fashion, without the reasons and justifications of the class structure. To resolve this issue, we propose a methodology based on Formal Concept Analysis (FCA) as a way to assist the development of a domain ontology. FCA is an analysis technique for knowledge processing based on applied lattice and order theory. The methodology is illustrated with the development of an explosion ontology.

The representation of knowledge of manufacturing processes plays a key role in the reuse and sharing of knowledge in areas such as product design and process planning. One common approach for knowledge representation is ontologies. Ontologies are formal models that use mathematical logic to disambiguate and define classes of things. The reasons behind this are twofold. First, ontologies have the ability to be integrated with automated reasoning applications. Second, ontologies are also useful for enabling knowledge sharing between different knowledge-based applications. However, in the absence of systematic methods for their design, most ontologies are developed in an ad-hoc manner. This paper presents a methodology for developing manufacturing process ontologies, which combines formal concept analysis with a set of criteria for characterizing classes of processes. The application of the proposed methodology is illustrated with a case study on the development of an ontology for machining processes.

筆者は島根県農林水産部水産課所属の漁業取締船「せいふう」に乗船中、監視業務のなかでしばしばクジラ類を目撃する機会を得た。クジラ類が海岸に漂着したり、河口や湾口等に迷いこんだりする現象はマスコミに取り上げられてよく耳にするが、海洋を遊泳している情報はきわめて少ないように思える。そういったことから、これまでに得られた事例を記録・整理して報告することにした。

1992年3月上旬,駿河湾口東部および同湾奥西部沿岸の2係留観測地点で,約5.5日間に約5.5℃に及ぶ顕著な水温急上昇が観測された。この水温急上昇は同湾沖の黒潮系暖水の流入による影響であり,さらに流入した暖水は同湾沿岸を反時計回りに伝播することが分った。両係留観測地点の水温急上昇の時間差と距離から求めた暖水の伝播速度は0.79m s^-1である。この値は暖水の移動を沿岸密度流の先端部の速度(Kubokawa and Hanawa, 1984)と見なして計算した値0.93ms^-1におおむね等しい。同様な現象は相模湾でも発生することが知られていて,それは急潮と呼ばれていることから,今回観測した水温急上昇は駿河湾の急潮と呼ぶことが出来る。また,今回の急潮をもたらした黒潮系暖水の流入の原因は黒潮の接岸であり,それには石廊崎沖での黒潮の小さな蛇行の発生が関係している可能性が考えられる。なお,同湾内各地の水位変動には急潮に伴う水温急上昇の効果はほとんど現れなかった。In early March 1992, a steep temperature rise of about 5.5℃ for 5.5 days was observed in the eastern part of the mouth of Suruga Bay and the western part of the bay head. The warm water in the eastern part of the bay mouth was influenced by an intrusion of the Kuroshio warm water. The intruded warm water moved cyclonically along the bay coast. The propagation speed of the warm water estimated from the time lag of the steep temperature rise and the distance between two mooring stations is about 0.79 m s^-1. The estimated speed is similar to that of the coastal density current in a rotating fluid, 0.93 m s^-1, theoretically derived by the same method as Kubokawa and Hanawa (1984). As a similar event that previously occurred in Sagami Bay was called kyucho, the steep temperature rise in this case is also called kyucho. The intrusion of the Kuroshio warm water into Suruga Bay as kyucho is related to a northward shift of the Kuroshio path. This northward shift seems to be related to the occurrence of a small scale meander of the Kuroshio off Irosaki. The temperature change induced by kyucho, has a little effect on the sea level change in the bay.

This study into the Japanese blogosphere examined the sources used by Japanese A-list political blogs, the sources' influence on the attitude of the A-list bloggers, and the influence of A-list blogs on the attitude of other bloggers. A content analysis of 47 A-list political blogs during Japan's 2007 Upper House election campaign found that A-list blogs considerably depended on mainstream media as sources. 44.8% of these mainstreams sources were accounted for by newspapers, with online news making up another 39.3%. The analysis also found upon an examination of opinions regarding the Liberal Democratic Party that few news items in mainstream media expressed political opinions, whereas most blog items did. The low degree of coincidence between the political opinions of A-list blogs and those in mainstream media suggests there was no influence from these sources on the attitude of A-list bloggers. On the other hand, there was a statistically significant coincidence between the political opinions of A-list blogs and the ones of blogs that cited A-list blogs. Nevertheless, this importance of this coincidence should not be over-emphasized, as the majority of blogs analyzed expressed negative opinions regarding the Liberal Democratic Party in any case.
