1992年1月から2002年12月までのステロイド投与患者に対する腹部救急手術18例を対象とした。ステロイド投与適応疾患は関節リウマチ7例, 進行性全身性硬化症+皮膚筋炎1例, 全身性紅斑性狼瘡1例, 悪性リンパ腫2例, 骨髄性白血病2例, 再生不良性貧血1例, 潰瘍性大腸炎2例, 急性呼吸循環不全1例, 脳外科手術後1例であった。1日平均ステロイド投与量はprednisoloneに換算して54.5mg, 平均投与期間は41.9ヵ月, 平均総投与量は10.9gであった。術後合併症は, 14例 (77.8%) にみられ, 創部感染, 縫合不全, MOF, DICなどであった。在院死亡を6例 (33.3%) に認めた。ステロイド投与患者では合併症を起こしやすいことから, 原疾患の活動性, ステロイド投与歴を十分把握した上で腹部救急疾患の治療を行うことが重要と思われた。

Cardiovascular disease remains the leading cause of death worldwide and developing novel therapies to treat and cure the disease remains a high priority in the healthcare research community. Adult stem cells were successful in entering numerous clinical trials over the past 15 years in attempts to regenerate the heart. First-generation adult stem cell therapies for myocardial regeneration were highly promising in small animal models but realized benefits in humans were far more modest. Consequently, second-generation therapeutic approaches in early implementation phases have focused on enhancing cellular properties with higher survival and regenerative potential. Genetic programming dictates cellular fate, so understanding genetic composition and responses at the gene level to influence the outcome of the cell is essential for successful outcomes in regenerative medicine. Genetic editing is at the forefront of scientific innovation and as basic scientific research continues to expand upon understanding eukaryotic regenerative themes, a clearer vision of the possible future therapeutic approaches can be realized. Ultimately, enhancing biology and manipulating evolutional selection of cellular properties will be critical to achieving clinically relevant and biologically meaningful cardiac regeneration.

本研究では,肺胞上皮に沈着したナノ粒子がどれだけ上皮内に取り込まれるのかについて,ヒトII型肺胞上皮細胞(A549)と修飾基の異なる3種類(カルボキシ修飾,アミノ修飾,修飾基なし)の蛍光ナノ粒子を用いて,「表面修飾」,及び「表面電位」に着目し,共焦点レーザー走査型顕微鏡によるイメージング,およびフローサイトメーターによる粒子を取り込んだ細胞数の定量を行った.その結果,表面電位と細胞への取り込み量の依存性に関しては明らかにできなかったものの,曝露時間が長い程取り込み量は増加し,特にカルボキシ修飾粒子で顕著であった.

本研究は,ICU において医師・看護師が終末期医療へのシフトをどのような情報・状況をもとに判断しているのかを明らかにすることを目的とした.ICU に勤務する医師・看護師各4名に半構成的面接を行い,質的帰納的に分析を行った.その結果,28 コード,9サブカテゴリーから《救命の困難さ》と《治療継続の不適切さ》の2カテゴリーが抽出された.《救命の困難さ》は,【臓器機能の悪化】【治療に対する反応の低下】で構成され,これ以上の治療を継続しても患者の命を救うことが困難であるという情報である.《治療継続の不適切さ》は,【本人および家族の自己決定】【その人らしさの消失】【家族内の患者の居場所の欠如】などで構成され,これ以上治療を継続することは患者の利益にならないことを示す情報である.医師は《救命の困難さ》に重点を置いて終末期医療へのシフトを判断するが,看護師は《救命の困難さ》だけでなく《治療継続の不適切さ》を重視して終末期医療へのシフトを判断していた.この医師と看護師の判断の特徴の背景には,職務上の役割の違いがあることが示唆された.

The 1st Mitsubishi Experimental Tank was built in 1908 at Akunoura, Nagasaki by Denny Brothers Co., England, This tank was closed in 1943 and a new one was built at Urakami, Nagasaki. The new tank started its work from Jan., 1944. But this was severely destructed by the Atomic Bomb on 9. Aug., 1945, just before its completion. After the war, we continued enthusiastic endeavours for the restoration of the tank. And last autumn, this new experimental tank was completed. The tank is 285m long, on which 7m paraffin modelship and 500mm dia. model propeller are used as standard size ones, At one end of the tank, 30HP-wave maker is installed which can produce regular waves, max. 8m long 40cm high. The towing carriage is moved by four 25HP-motors and regulated by electronic automatic speed regulator, The speed is preset before start and after acceleration, preset constant speed is automatically kept within ± 0.2% variation, max. towing speed is 9m/s.All facilities including carriages, dynamometers, model shaping machine, etc. are designed and manufactured by us. On 15.-16. Oct. last year, the Japanese towing tank conference was held at this tank commemorating its completion. Hereunder, the author wishes to give some details of this new tank.

Particulate matter (PM) adversely affects human health. Even though PM tends to consist of higher concentrations outdoors, indoor PM concentrations should not be overlooked, given that people spend most of their time indoors. When there is no strong indoor PM source, indoor concentrations change within a certain ratio following the diffusion of outdoor concentrations indoors. The Indoor/Outdoor (I/O) concentration ratio is affected by static building properties such as airtightness, ventilation system type, and the system's air filter performance. However, natural ventilation can have a dynamic influence on the I/O ratio because of significant changes in ventilation rates and flow patterns within the building. This study measured PM concentrations for particles ranging from 0.3 to 10 μm in size. The study was conducted in a room of an office building in Tokyo, over a three-week period. The size ranges were divided into 16 channels and 10-min averages of the number of particles at each channel were measured using an optical particle counter. Occupants in the room utilized natural ventilation by manually opening a window. The window was opened and closed twelve times during the entirety of the measurements. Because outdoor PM consisted of higher concentrations than that found indoors, the natural ventilation lead to larger Indoor/Outdoor (I/O) concentration ratios. The PM concentrations were observed to increase and decrease just after the window was opened and closed, respectively. These concentration changes were more remarkable for PM consisting of larger particle sizes. Where PM measured 10 μm, concentrations increased 8 times after opening the window and decreased 0.5 times after closing the window on an average hourly basis. Particle-size is a factor for concentration changes caused by natural ventilation as discussed using concentration prediction based on the mass balance equation of indoor PM. As a result, the dependency was explained quantitatively in terms of the difference in the gravity-settlement velocity of the particles. Using ventilation parameters estimated in this analysis, the I/O ratio of PM concentration as a function of the particle size was predicted. During natural ventilation, indoor PM consisting of particles smaller than 1 μm can have the equivalent concentration of outdoor PM. However, particle size increase leads to a larger gravity-settlement velocity and lower I/O ratio. Therefore, particles measuring 10 μm are predicted to have an I/O ratio of 40% in accordance with the room analyzed in this study.

チェルノブイリ事故が発生してから今年で10年の歳月が過ぎようとしている。事故直後から旧ソ連は近隣諸国や米国との間で多彩な研究協力を行い,この事故による放射線被曝の健康影響に関する共同研究の成果が公表されている。チェルノブイリ事故に関するわが国と旧ソ連との研究協力は必ずしも敏速かつ円滑に発足したわけではなく,研究協力が開始されるまでに数年を要した。以来,今日まで旧ソ連の体制の変化を含め紆余曲折を経て,政府および民間レベルでの国際研究協力が多様な形態で続けられている。健康影響についての評価は,今後の調査研究に待つところが多いと考えられ,特に低線量,低線量率被曝の影響という観点から貴重な成果が期待される。研究協力により得られた経験を基に,今後のこの国際研究に関し,協力のあり方について考えたい。

Unintended accelerations (UA) occur in vehicles unexpectedly and uncontrollably. Every year, numerous critical injury and death cases related to UA occur. Recently, researches related to the development of automated driving systems are attracting a lot of attention among researchers and vehicle manufacturers. Additionally, silent vehicles such as electric power and hybrid vehicle are increasing. However, there are negative effects on driver safety. Recognition time (RT) and recognition velocity (RV) are some of the important factors lead to avoiding crashes. Early detections give the drivers more time in decision-making toward the UA hazards. In this study, RT and RV toward UAs under various environments were analysed. The findings highlighted that RTs toward UAs were longer in no engine sound environment. Drivers were unable to respond to UA earlier in silent vehicles compared to conventional vehicles. The findings provide insights that can be useful in developing automation vehicle and silent vehicle.

We herein report two cases of drug-induced liver injury (DILI) due to mosapride. Case 1: A 78-year-old man was admitted with elevated transaminase levels. The cessation of mosapride led to the improvement of elevated liver enzyme levels. Case 2: A 54-year-old man was admitted with jaundice. Mosapride was discontinued immediately, and methylprednisolone was administered for acute liver failure. The patient's data showed improvement, and he was discharged on Day 32. In both cases, mosapride gave a positive response to a drug-induced lymphocyte stimulation test (DLST), and the patient's score based on the criteria for DILI was "highly probable".

Tuberculosis is an important health problem in developing countries, with varying clinical presentations depending on the organs/systems involved. Tuberculosis is mostly seen in immuno-compromised individuals, such as those with acquired immune deficiency syndrome or malignancies. Here we report a case of a spleen tuberculoma in a 29-year-old male patient with no known immune deficiency. He first presented with abdominal pain, and subsequent ultrosonographic examination revealed a splenic lesion of 10 cm in diameter. A computerized tomography scan of the abdomen confirmed the presence of a solitary, hypodense, septated cystic lesion. Lack of evidence supporting the presence of a splenic infection or a primary/metastatic malignancy prompted explorative surgery where a septated abscess formation was discovered and splenectomy was performed. Histopathological examination revealed granulomatous inflammatory changes with Langerhans-type giant cells, which are consistent with tuberculosis. For a period of two months, antituberculosis therapy with four drugs, isoniazid, rifampicin, pyrazinamide, and ethambutol, was carried out. Pyrazinamide and ethambutol were quitted at the end of two months. Theraphy with isoniazid and rifampicin was planned for an additional 10 months. We would like to call attention to yet another atypical presentation of extrapulmonary tuberculosis.

In unserer Klinik begegnen wir häufig Wachstumsanomalien. Ms wir jedoch die -Wachstumsanomalien mit dem normalen japanichen Körperbau vergleichen wollten, gerieten wir in grosse Verlegenheit, da sich über diesen so gut wie keine Literatur vorfindet. Lebensjahre rasch, dann bis zum etwa 20. Lebensjahre langsam zu. Aber das Längenwachstum ist im allgemeinen etwas schneller vollendet als das Breitenwachstum.2) Was die gefundenen Masse bei den beiden Geschlechtern anbetrifft, so sieht man, dass die bei den männlichen Individuen im allgemeinen grösser als die bei den weiblichen sind.Bezüglich der Bauchlänge und der Beckenmasse übertreffen jedoch die weiblichen Individuen die männlichen.Das Wachsturn des weiblichen Unterleibes und Beckens geht sehr schnell vor sich und ist frühzeitig, nämlich im 15. Lebensjahre, schon abgeschlossen.3) Aus den Beobachtungen über den Blutdruck and die Körpermasse ergibt sich, dass der maximale Blutdruck fast iinmer mit den Körpermassen parallel geht.

Owing to recent progress in structural biology, the structures of a number of proteins have been resolved and deposited into databases such as the Protein Data Bank (PDB). Many proteins function as molecular machines or show allosteric effects, which require integrated communication between different parts of the protein structure;however, the general principles underlying such communication are not yet clear. All-atom molecular dynamics simulation is a powerful tool for tracking molecular motion, but is still far too computationally costly to be applied widely. In this paper, we present a simple method for assessing the properties of mechanical communications using coarse-grained steered molecular dynamics simulations with elastic network models. The method was tested by screening for a certain mechanical property among protein structures in the PDB.
