【はじめに】 交通事故により外傷性くも膜下出血,多発骨折を呈した60歳女性に対し,職場復帰に向けICFモデルを活用して目標指向的アプローチを行った.主婦業再開と職場復帰が必要な症例に対し,早期より参加向上訓練を行い,入院中からADL自立,主婦役割の再獲得を果たし退院後復職が可能となった経過を報告する.【症例紹介】 本症例は60歳女性.交通事故により外傷性くも膜下出血・左鎖骨端骨折・右母指末節骨骨折・右足関節外果骨折を呈し,高血圧・子宮筋腫の既往歴がある.背景因子は,病前は独歩でADL・ASL自立,長男・長女夫婦と同居,週5日ドラッグストアに勤務し,本人・家族ともに復職を希望していた.入院時,参加は主婦業・パート勤務は非実施,院内では自室にて読書をして余暇を過ごしていた.活動は,車いすレベルで入浴以外のADLを修正自立,歩行レベルではPTB装具・U字型歩行器を使用しADLに軽介助~見守りを要していた.家事は座位で一部介助,仕事は非実施であった.麻痺や高次脳機能障害はなく,立位での複合動作に軽介助,床からの立ち上がりに中等度介助を要し,基本動作は修正自立,歩行はU字型歩行器とPTB使用にて監視レベルであった.ROMは左肩関節屈曲150°,外転140°,足関節背屈5°,MMTは左上肢3,右足関節2~3で足関節外果下部・足背部に荷重・伸張時痛が認められた.【説明と同意】 事前に本報告の説明を行い,本人・家族の同意を得た上で報告する.【経過】 [目標]初期評価で得た情報をICFモデルで構造化し,以下の目標を設定した.3ヶ月の入院期間で在宅生活・主婦業を再開し職場へ復帰する.日常の移動手段として自転車使用を再獲得し,病前同様の生活を送る.[課題]多発骨折による機能制限のためセルフケアや就労に関する動作遂行に介助を要する.在宅生活や主婦業,パート勤務への参加が制約されている.[アプローチ]在宅での主婦業再開と復職に向け,早期からの機能向上訓練と同時に参加向上訓練としての外出外泊訓練を繰り返し,階段昇降や買い物等,徐々に高いレベルの課題を与えていった.実際場面でADL動作を行い,家事や仕事の関連動作も本人が自主的に実施するようPT・OT介入時に確認と促しを行った.[退院時評価]外泊時に家事全てを自立にてこなし,就労は業務動作が独立レベルとなった.そして本人と職場店長,MSWらと情報を共有し復職計画を立て職場に出向いた.院内では外の散歩や自主訓練をして活動的に過ごした.ROMや筋力等の機能は改善され疼痛は消失した.【考察】 初期の生活機能は主婦業・就労が非実施であり,受傷部の可動域制限や筋力低下,疼痛のため車椅子レベルにて修正自立でADL遂行,できるレベルではPTB装具を用いた歩行でADLに軽介助を要し,仕事の関連動作は立ち仕事全般に介助を要した.家庭の経済的理由から働く必要があり,入院時座位レベルで目的動作が修正自立していたことから歩行自立により病前同様の生活が可能であると考え,主目標は入院期間を3ヶ月とし,ADL・ASL自立で在宅生活と主婦業を再開,職場復帰することとした.復職に必要な動作を評価し,機能向上訓練の中に関連動作を取り入れ,併行して歩行レベルでのADL訓練やレベルに応じた課題を与えながら外出泊訓練を実施することで早期から参加向上を意識した介入を行った.主婦とパートを兼任できる高い耐久性の獲得が必須であると考え,本人主体で1日のスケジュールを立てることで自主的な活動を促し,歩行能力向上に伴って物持ちや長時間の屋外歩行等の課題を取り入れた.リハ介入時に声かけによる確認と課題修正を行った.以上により入院2ヶ月目には自発的な運動習慣が身に付き,ADL自立にて自宅退院が可能なレベルとなった.パート勤務に関しては軽介助~見守りを要したため,1か月間集中的なリハビリを行う方針をたて,退院時には全動作自立となった.多発骨折を呈した症例に対し早期から参加向上訓練を積極的に実施し,他部門との協業によって退院後の生活の流れに類似させた生活を行う事で早期のADL・ASL自立を獲得し復職に繋げることができた.【理学療法学研究としての意義】 目標指向的に機能向上訓練と参加向上訓練を併行することで機能改善と同時に参加場面の広範化が図られ早期でのADL・ASL自立を獲得し,復職に対する重点的な介入や協業による他職種との連携が高い訓練効果・QOLの獲得を可能にし,退院後そのまま社会参加を達成した.本症例は退院後,主婦業再開と職場復帰を果たし,目標指向や参加への介入の大切さについて報告することができた.

We employ the independent component analysis (ICA) for multiple-site grand-averaged auditory-evoked-potentials and show its feasibility to interpret the data physiologically. We first simulate near field potential at auditory cortex to examine how ensemble firing activities and synaptic activities generate the potential: both arise biphasic potential and firing activities are followed by synaptic activities by a few millisecond. We then implement ICA to our multiple-site auditory-evoked middle latency responses (MLR). The simulation and previous reports suggest that ICA successfully extract from the data three significant components, that are cortical firing activities, cortical synaptic activities, and far activities. ICA also shows how these compo-nents contribute to MLR waveforms and suggests how typical MLRs are generated: MLR only with Na-Pa-Nb appears when both synaptic activities and firing activities are sufficiently large: the No-Po-Na-Pa-Nh complex appears when both are not so large; and the NA-PA-NB-PB-NC complex appears when both are sufficiently small.

A method of deducing the I-131 concentration in a radioactive plume from the time history of peak count rates determined from pulse height spectra obtained from an NaI(Tl) scintillation detector employed as a detector of a monitoring post was presented. The contribution to the count rates from I-131 accumulated around the monitoring post was subtracted in accordance with the time history, taking into consideration the cumulative attachment and its decay. The concentrations of I-131 in the plumes were estimated from the count rates using the calculated response of the NaI(Tl) detector with egs5 for a model of a plume uniformly containing I-131. This method was applied to the data from the monitoring posts at Nuclear Science Research Institutes of Japan Atomic Energy Agency (JAEA). The estimated time history variation of I-131 concentrations in plumes was in fair agreement with those measured directly by an air sampling method. The difference was less than a factor of 4 for plumes that arrived on March 15 and March 21, indicating relatively high I-131 concentrations among the plumes studied in this work.

人間ドックでの甲状腺超音波検査での甲状腺がん検診とTSH測定による甲状腺機能亢進症および(潜在性)甲状腺機能低下症発見の対費用効果を調べた。1例の甲状腺がん発見のための費用は342万円で1例の甲状腺機能異常症発見の費用は12万5千円であり,がん検診よりもTSH測定による機能異常症検診の方が有効と考えられた。

Hypothesis: Simotinib Hydrochloride (SIM6802), which is a new epidermal growth factor receptor-tyrosine kinases inhibitor (EGFR-TKI), is often prescribed for cancer patients with comorbidities and has serious adverse effects gastrointestinal physiology. The drug-drug interactions (DDI) between simotinib and other drugs in combination and the underlying mechanism of its gastrointestinal toxicity remains unclear. We hypothesized that the DDI and the gastrointestinal toxicity of simotinib was related to its effects on the permeability of intestine. Methods: To determine the intestinal absorption ability, pharmacokinetic studies and in situ loop assay were used. The intestinal permeability was measured by Caco-2 transwell model. Real time PCR and western blots were applied to detecting the expression changes of cell junction genes. Results: Our research demonstrated that simotinib upregulated the absorption of cefaclor, valaciclovir and acyclovir. The increase of non-selective absorption was caused by the low expression of cell junction gene afadin-6 and the increasing of paracellular permeability in intestinal epithelial cells after simotinib treatment. Conclusion: These findings revealed that simotinib upregulated intestinal absorption by increasing the paracellular permeability of intestinal epithelial cells. Our research provides theoretical bases for better formulation of EGFR-TKIs to alleviate adverse gastrointestinal effects and also provides guidance for clinical administration of simotinib.

Large strides have been made in primary health care and services in Jinzhai County, Anhui, sincefourth-grade health care facilities at hamlet and village levels were improved under the guidance ofthe public health organization of the county.Worthy of special note are the establishment of the health compensation system for women andinfants, introduction of premarital education programs for the young, regular checkups for pregnantwomen, regular health screenings and vaccination for infants under the age of 7. The results havebeen outstanding.Mortality rates of pregnant women, nursing mothers and babies during the perinatal period have dropped remarkably. The incidence of tetanus among the neonates has been reduced to zero.Health inquiries have found there is a high rate of cancer of the thyroid gland. As preventive measures, iodine preparations are given to primary and middle school children. Thyroidectomy has been performed on some malignant cases.Meanwhile, the spread of a clean water supply system is a major task grappled with by community health officials. Although the service area is limited yet, tap water has become available. The living environment in Jinzhai County overall has been improved to a level comparable with the PHC (?) standards recommended by the World Health Organization.Under the slogan “Creating Better Health for All the People, ” continuous effort is being madewith 2000 as the target year.

The present investigation has been made in order to determine the influence of induction-hardening on the fatigue strength of shrink-fitted specimens. Cyclic direct stress fatigue tests were made on shrink-fitted specimens 20mm in diameter of NiCrMo steel.The results obtained were summarized as follows:1. The reduction ratio 2.3 was obtained by comparing the endurance limit for a shrink-fitted specimen with that of a plain specimen.2. The endurance limit of a shrink-fitted specimen was increased by about 2.1 times by induction-hardening.3. The fatigue strength of the induction-hardened specimen with a shink-fitted member may be estimated approximately from the modified Goodman diagram, in which the residual compressive stress on the surface layer of specimen is assumed to be mean stress.
