In the present paper, WCA (Wants Chain Analysis) is proposed by extending CVCA (Customer Value Chain Analysis). CVCA is a method for visualizing relationship among stakeholders for analyzing existing social systems and designing new social systems. WCA is a method for visualizing wants and needs of stakeholders in CVCA utilizing Maslow's hierarchy of needs. In WCA it is shown that people's wants or needs are finally realized through the looped and linear chain of relationship among stakeholders. With various examples, effectiveness of WCA is shown by describing that characteristics of business/social systems structure can be clearly visualized using WCA. It is described that WCA is useful for analyzing social systems. “Think of others” should be more and more important for humans' satisfaction and happiness in the near future. WCA can be a tool for clarifying what is for others.

5歳、雌のミニチュア・ダックスフントが腹囲膨満および呼吸困難を主訴に来院した。身体検査、エックス線検査および超音波検査により腹腔内腫瘤と胸水の貯留が認められた。腫瘤と胸水の細胞診により、腫瘤は胸腔内転移を伴う卵巣由来の悪性腫瘍であることが示唆された。手術により腹腔内腫瘤は摘出され、病理組織学的に卵巣の乳頭状腺癌と診断された。術後、肺転移に伴う胸水の制御を目的にパクリタキセルと白金製剤併用の化学療法が合計9回行われ、手術から623日目に死亡した。剖検により肺は腫瘍組織に侵されていたことが確認された。パクリタキセルと白金製剤併用の化学療法は、犬の転移性卵巣癌症例に対する化学療法の選択肢の一つになりうるかもしれない。

重症筋無力症と巨大食道症を伴った胸腺腫と診断された12歳、避妊メスのゴールデンレトリバーに対して4週間ピリドスチグミンの投与後、胸腺腫に対して12Gy の1回照射を行った。照射1ヵ月後、巨大食道症の消失、2ヵ月後、抗アセチルコリンレセプター抗体価の減少が認められ、更に胸部X線検査で腫瘤は消失した。ピリドスチグミン投与は照射後5ヵ月間継続した。照射1年半後、腫瘤は認められず重症筋無力症の症状も認められない。更に放射線障害は見られなかった。以上のことから重症筋無力症を伴った胸腺腫に対して放射線単回照射とコリンエステラーゼ阻害薬の併用療法の有効性が示唆された。

症例は73歳女性. 69歳のとき腎硬化症による慢性腎不全のため, 腹膜透析を導入した. 73歳のときランソプラゾールの内服を開始し, 3か月後, 3回目の腹膜炎で入院となった. セファゾリンとセフタジジムを経静脈, その後腹腔内投与に切り替え計2週間投与し, 軽快退院した. 入院中に軟便であったが便Clostridium difficile抗原は陰性であった. 退院後から水様~泥状便が持続し, 外来にて便Clostridium difficile抗原陽性となり, 偽膜性腸炎の診断でメトロニダゾールを14日間内服した. その後も下痢は持続し, 食思不振, 炎症反応高値を認めたため, 精査加療目的に当科入院となった. 絶食・中心静脈栄養を開始した. 大腸内視鏡検査を施行し, 肉眼で上行結腸に炎症瘢痕を認めた以外は異常所見を認めなかったが, 直腸, 盲腸, および結腸の粘膜生検で粘膜表層上皮下にリンパ球浸潤を認め, Masson Trichrome染色で特徴的なcollagen bandの形成を認めたため, collagenous colitis (CC) と診断した. CCは慢性水様性下痢の原因として知られ, 本邦での報告数はまだ少ないが, 年々増加傾向にある. 原因は不明だが, 薬剤や自己免疫の関与などが考えられている. 本例では被疑薬であるランソプラゾールを休薬し下痢が消失したことから, その関与が考えられた. 透析患者は消化管出血のリスクが高いためプロトンポンプインヒビター (proton pump inhibitor : PPI) を内服していることが多いが, 文献上, 国内外において腹膜透析患者におけるCCの報告はなく, 慢性下痢の鑑別疾患の一つとして念頭におくことが重要であると考え, 報告した.

【目的】下部消化管穿孔の合併症と予後の調査,及び重症化の危険因子に関する検討を行う。【方法】2006年4月から2008年3月までの2年間に,下部消化管穿孔に対する緊急手術後ICU管理を行った50例を対象とした。術後急性期の臓器障害の合併頻度と28日死亡率を後向きに調査した。また重症化の危険因子解明のため,ICU-free days(IFD)をエンドポイントとしてSpearman順位相関分析と多重ロジスティック回帰解析を行った。【結果】下部消化管穿孔術後急性期の臓器障害はショック(40%)が最も多く,次いで播種性血管内凝固(24%)の合併頻度が多かった。また,28日死亡率は6.0%であった。重症化の危険因子としては,Sequential Organ Failure Assessment(SOFA)スコア(オッズ比1.85,P=0.025)と血中の白血球減少(オッズ比20.6,P=0.016)が有意にIFDを減少させる独立危険因子であった。【結論】下部消化管穿孔術後急性期はショックと凝固障害の合併が多い。ICU入室時のSOFAスコア,血中の白血球減少が下部消化管穿孔の重症化に関係することが示唆された。

電解複合研磨は, 昭和51年に開発した金属表面処理技術で, ステンレス鋼はもとより, アルミ, チタン, 銅などの非鉄金属まで, 加工変質層を伴わずに200~300Åまでの高品位鏡面を, 安定した品質で加工できる新しい技術である。従来の研磨法では, 限界にきているULSI製造プロセスのウルトラクリーン化や, 超, 極高真空システムの高性能化など, 先端ハイテク分野へ貢献しうる電解複合研磨技術について紹介する。

The present study attempts to construct an Experiential Time Perspective Scale, which measures not only the experience of personal future, but also those of the present and the past and, it will measure, at the same time, two aspects of future, namely, hopefulness and goal-directedness. This scale was administered to 460 university and professional school students. The results showed rather high internal consistency and test-retest reliability. Correlational analyses with Time Attitude Scale (Nuttin & Lens, 1985) showed convergent and discriminant validity in each subscale, except for the dimension of hopefulness. However, the different relationships of subscales with fear for the future and self-esteem confirmed the validity of this scale including that of hopefulness.

高圧力下の固体の水への溶解度を (1) 高圧溶解度測定法 (2) 有機化合物の溶解度 (3) 無機化合物の溶解度に分けて、 データも含めて紹介した。 熱力学的には圧力と体積は相補的関係にあり、 高圧力下の溶解度から体積の情報が得られる。 逆に体積の情報から高圧力下の溶解度が推定できうることおよびそこでの注意点を示した。

The upper mantle earthquakes occurring within the Philippine Sea plate subducting beneath the Kanto district, Japan, were investigated for their hypocentral distribution and focal mechanisms. Earthquakes located above the double seismic plane, which is formed in conjunction with the subducting motion of the Pacific plate, were picked up on a graphic display with an “on-map selection” method. Hypocentral parameters were re-determined with removing stations far from the epicenter and with introducing station corrections to the observed arrival time data. Based on the relocated hypocenters, the focal mechanism solutions were also re-determined. Examining the hypocentral distribution in detail, we found that the earthquakes occurring within the Philippine Sea plate are classified into three groups and that their focal regions form a triple-layered structure. The earthquakes in Group I, which are located at the uppermost part of the slab, were found to occur in the oceanic crust of the Philippine Sea plate from their waveform analysis. The focal mechanism analysis showed remarkable difference between the earthquakes in Groups II and III. The down-dip compression type events are predominant for the earthquakes in Group II, while most of the focal mechanisms in Group III showed the earthquakes are occurring under the down-dip extension. We proposed a model to explain the earthquake mechanism within the Philippine Sea plate, where the “forced unbending” by the underlying Pacific plate causes compressional and extensional stress fields in the upper and lower portion of the slab, respectively.

In this study, we examined the antimicrobial susceptibility of the enterococci isolated from dogs and cats in Japan during 2011–2012. Fecal samples were collected from 84 dogs and 16 cats that underwent antibiotic treatment. Enterococci were detected in 70 of 84 dogs (83.3%) and 7 of 16 cats (43.8%). The most prevalent Enterococcus species was Enterococcus faecalis (64.9%); Enterococccus faecium and Enterococcus durans were also isolated from 14 of 77 (18.2%) and 5 of 77 (6.5%) of these animals, respectively. The most active resistance was observed for erythromycin (44.2%) and oxytetracycline (44.2%), and there was considerable resistance to lincomycin (41.6%), gentamicin (31.2%) and kanamycin (31.2%). Compared with the results of a similar study conducted in 2006 and 2007, enterococci susceptibility to enrofloxacin and ampicillin had significantly increased. Enterococcus gallinarum harboring vanC1 and Enterococcus casseliflavus harboring vanC2/3 were isolated from 4 of 77 enterococcal isolates. However, no enterococcal isolates were resistant to vancomycin. Multidrug resistance was found for as few as two and as many as nine antimicrobials regardless of the class. These results demonstrate that dogs and cats treated with antibiotics are commonly colonized with antimicrobial-resistant enterococci.

Lymphocytic-plasmacytic colitis (LPC) is a common form of inflammatory bowel disease (IBD) affecting the canine large intestine. Cytokines are thought to be involved in the pathogenesis of IBD. However, to date, few studies have investigated cytokine mRNA expression in dogs with LPC. In this study, we investigated mRNA transcription levels of T helper cell cytokines, such as IFN-γ, IL-4, IL-17 and IL-10 and pro-inflammatory cytokines, such as IL-1β, IL-6, TNF-α, IL-8, IL-12 and IL-23, in colonic mucosa from LPC dogs by quantitative real-time RT-PCR. No significant differences were detected in cytokine mRNA expressions between dogs with LPC and controls, except for IL-23p19. Dogs with LPC failed to express a predominant cytokine profile in inflamed colonic mucosa as opposed to human IBD.

Metallic materials as represented by steels mostly contain various precipitates such as nitrides, carbides and inter-metallic compounds in addition to non-metallic inclusions, oxides and sulfides. These are present as an inevitable former or by design. Those varieties, size and distribution types are very wide then one of important structural factors that affects on various properties of materials.Therefore, accurate analysis of them is considerable significance to control target properties of materials reproducibly. In the steel research field, study of analytical methods for inclusion in steels has been actively conducting since 1960’s. Especially in chemical analysis method, systematization and standardization like JIS has been promoted by corroborated researches in an ISIJ. Previously established methods and detail of analysis of chemical states have been described in publications edited by ISIJ and comprehensive review paper has been issued by Takayama. In this short article, direct observation methods with microscopy, rapid analysis categorized as instrumental analyses, non-destructive inspections and extraction method for non-metallic inclusions are briefly reviewed.
