要 旨 帯下異常の主因となる感染性の膣炎のうち, 最多の細菌性膣症の有病率は10∼40%1) , カンジダ外陰膣炎の生涯罹患率は75%2) と高頻度であるが, 帯下異常を主訴にプライマリケア医を受診する頻度は少なく3) , 患者・医療者双方に心理的障壁がある. 一方で, 帯下異常の主因を占める感染性の膣炎は診断・治療に関するエビデンスが集積されており, 膣鏡がなくとも膣【扁】平上皮を含む膣分泌物が採取できれば可能なアプローチも存在する. 本稿では疫学データや国内外の診療ガイドラインをレビューし膣鏡や経膣エコーのない施設でも実現可能な「帯下異常」へのアプローチを紹介する. なお妊娠中の治療および, 不正性器出血については紙面の都合上割愛しており, 詳細は成書をご参照頂きたい.

コードレビューは重要な品質保証活動であるが,現実には予算や納期の都合により網羅的な実施が難しい場合も多い.そこで本論文では,限られた予算内で“どのモジュールを重点的にレビューすれば最も高い費用対効果を期待できるか”という問題に着目し,各モジュールでのフォールト潜在の可能性,レビューコスト及び他のモジュールとの結合関係を総合的に考慮した0-1計画問題として定式化する手法を提案している.そして,六種類のオープンソースソフトウェアに対するシミュレーション実験を行い,提案法の有効性について論じている.

The aim of the present study was to investigate whether personal hamstring muscular activation increased by landing with the trunk bent forward. First, 14 healthy female university students were instructed to perform normal drop landing from a 40 cm-high box on both legs. Second, subjects were directed to perform drop landing with the trunk bent forward. Using images obtained from a high-speed video camera, the upper center of mass was calculated in the sagittal plane for determining the differences in these landings. For 0.1 second after toe contact, the muscular activation of quadriceps femoris and hamstring muscles was analyzed using an electromyogram (EMG), and these findings were compared between the 2 types of landings. There was no significant difference in the activation of quadriceps femoris, but the activation tended to decrease during landing with the trunk bent forward. However, the activation of hamstring muscles increased significantly during landing with the trunk bent forward compared with that during normal landing. During landing with the trunk bent forward, the activation of hamstring muscles increased, and the activation of quadriceps femoris tended to decrease. Because contraction of hamstring muscles decreases tension in the anterior cruciate ligament, the findings of this study may help in the prevention of anterior cruciate ligament (ACL) injury.

ITプロジェクトの成功に寄与するプロジェクトマネージャー(以下PM)のマネジメント・コンピテンシー(高業績者の行動特性)について,日本国内の情報通信・IT開発プロジェクトのPM約200名に対し,プロジェクトマネジメント知識体系による42のプロセスを自己評価する質問紙により調査した.得られた結果を因子分析し「リスクマネジメント」「進捗マネジメント」等の9因子を抽出した.これらの因子指数を直近プロジェクトの成果の成功群と不成功群で比較した後,パス解析により成功・不成功に対する因果関係を調べた.その結果,プロジェクトの時間的な成功に「引渡成果物マネジメント」の,コストの成功に「要求事項マネジメント」のコンピテンシーが影響することがわかったが,2つのコンピテンシーには「進捗マネジメント」のコンピテンシーが間接的に影響していることも確認できた.以上よりこの3つのマネジメント・コンピテンシーのITプロジェクトに対する重要性が示された.

遺伝子のメチル化は, シトシンとりわけCpG配列に特異的に起こるDNA修飾である. 遺伝子のメチル化は遺伝子変異, 欠失とならび, 癌抑制遺伝子不活化の第三の機構として重要である. 異常メチル化により, 細胞周期調節遺伝子やアポトーシス関連遺伝子, DNA修復酵素など様々な遺伝子が不活化を受ける. メチル化の網羅的解析から, 一部の腫瘍ではCpG island methylator phenotype (CIMP) を示し, 癌においてメチル化の制御機構が破綻していることが示唆される. メチル化の標的遺伝子が明らかになるにつれ, 異常メチル化を腫瘍マーカーや抗癌剤感受性の指標として用いる試みもなされている. また, メチル化を阻害することにより不活化されている遺伝子を再発現させ, 癌細胞に分化やアポトーシスを誘導することが可能になりつつある.

これまで遺伝素因によるところが大きいと考えられてきた「太りやすい」体質の原因に,栄養環境を含めた環境因子が少なからず影響を与えていることが明らかとなりつつある.胎児期~新生児期は器官が形成される可塑性の高い時期であり,この時期の栄養状態や摂取する食品成分により成人期の肥満や生活習慣病の易感受性が決定される可能性がある.ここでは,代謝疾患である生活習慣病のエピジェネティクス制御に関する最近の研究の動向を概説し,今後を展望する.

遺伝子が転写されるためには転写因子の存在のほか, 転写因子と結合する部位がクロマチンとして結合可能な立体構造をとる必要がある. このクロマチンの構造はエピジェネティックなメカニズムつまりDNAメチル化とヒストン修飾により調節されている. 従来, 遺伝子解析研究といえばDNA配列の個人差を研究する遺伝子配列解析と組織や細胞のmRNAを定量する遺伝子発現解析の2通りの研究手法が主流であった. エピジェネティック研究はこの2つの研究手法の中間に位置づけられる研究分野である. 従来の遺伝子解析手法では得られなかったアレルギー性炎症や組織リモデリングに関わる機序解明が期待できる.

The Japan Atomic Energy Agency (JAEA) has conducted activities in Japan since March 2009 in order to establish the manufacturing procedure for the ITER toroidal field (TF) coil structures. A TF coil structure consists of a TF coil case and components. The activities include ensuring that the structural materials and welding procedure comply with the Japan Society of Mechanical Engineers (JSME) code for fusion devices, and demonstrating the manufacturing method and procedures using full-scale segments of the TF coil structure. From the results of these activities, the JAEA confirmed that the quality control method of actual series TF coil structures complies with the JSME code. Therefore, the quality of structural materials and weld joints using gas tungsten arc welding (GTAW) satisfy the ITER requirements. In addition, the JAEA obtained knowledge regarding the welding deformation of the actual TF coil structures. This paper describes the results of these compliance and development activities for ITER TF coil structures.

This essay tries to demonstrate that there were two contrasting behaviour patterns manifest by parties faced with conflicts and disputes in ancient Greece. In one those concerned attempted to settle their differences by referring to their shared assumption of their situations and relationships involving surrounding third parties. I call this the type of sharing assumption. In the other pattern, they defied the claims of opposing party with their own contradicting interpretation of their relationships. I call it the type of defying assumption. In this latter type they often failed to reach a settlement.The method employed in this paper is to analyze the dispute scenario with particular reference to a Greek word, peitho, which is the central notion in Greek rhetoric (e.g., Plato's Gorgias), and especially the differences in implication between the middle voice peithomaj, and the active peitho.My analysis starts with the quarrel between Agamemnon and Achilleus which extends all through the Iliad. In Book 1 all the instances but one (1, 132) are in the middle voice and illustrate the assumption shared by the two parties and surrounding Achaians of their relationships in terms of the distribution of spoils. By contrast, the instances of the active peitho in Books 1 and 9 well illustrate the situation in which one party defies the assumptions of the other. Thus, I argue that their behaviour patterns provide illustrations for the two types of Greek attitude towards dispute resolution.Further references can be drawn from other scenes in Greek literature which contains countless examples of disputes and conflicts. From them I only take a few illuminating examples. I suggest that the famous carpet scene in Aeschylus' Agamemnon offers us a subtle example of the first type in a sharp contrast to the scene in the Odyssey Book 23 where Odysseus and Penelope confront each other. By contrast, the scene of Melian Dialogue in Thucydides Book 5 provides an illustration of the situation in which each party is determined to destroy the other's claim.Recent studies on Greek law have stressed context, discourse and structure for settling disputes rather than focussing on (written and unwritten) law. Indeed, it is said that Greek rhetoric contributes to magnifying the 'otherness' of Greek law. As a result I suggest that it is necessary to look at the structural patterns in which ancient Greeks dealt with conflicts and disputes. I hope that this essay will make a contribution to the understanding of Greek law and rhetoric in a much wider context.

A 13-year-old castrated male Labrador retriever dog presented with a mass caudal to the first molar of his left mandible. Although the tumor was excised, a recurrent tumor was detected one month later and resected. Both tumors displayed invasive growth and were composed of neoplastic proliferation arranged in irregular lobules, nests and cords continuous with mucosal epithelium. The most prominent feature of the tumors was the presence of many proliferating spindle cells admixed with palisading basal-like cells, acanthocytes and stellate cells. In immunohistochemical examinations, the spindle cells were found to be positive for vimentin; cytokeratin AE1/AE3, 5/6, 14 and 19; and p63. The other neoplastic cells were positive for all of these markers shown above except vimentin. Based on these findings, the tumors were diagnosed as spindle cell ameloblastic carcinoma.

工学の技術を医療診断と治療に生かす方向の一つとして、超小形ロボットを使って、生体情報をモニタリングしたり、体内病巣を治療したりする、いわゆる低侵襲医療が活発化してきている。これは、人形ロボットと同様、工学の幅広い分野に関連している。五感の役割をする各種マイクロセンサ技術、センサ情報の信号処理技術、自立移動を実現するドライブ技術、超小形バッテリや燃料発電及び生体発電などのエネルギー技術、生体表面に優しい材料技術、ユビキタス無線通信技術、そして、これらを実装するLSI技術やフィルム基板実装技術などである。ここでは、実用化の先陣を切っているカプセル内視鏡と生体センサの動向を紹介する。また、これらマイクロロボットや生体センサの頭脳として開発した、私たちの体内コンピュータLSIを一例として紹介し、その応用例と発展の展望を述べる。

世界的に1990年代からインターネットとPCが急速に普及し始め,産業活動はもちろんのこと一般社会にもその活用が広がりつつある。そのひとつのビジネス形態が,電子商取引である。各国の政府機関等では,これらの情報技術(Information Technology: IT)を積極的に政策に取り込み,自国のIT政策として展開している。本稿では,最初に英国のIT政策の経緯について記述する。そして,包括的なIT政策であるUK onlineについて概観する。さらに,内閣府に設置されたIT推進機関であるOffice of e-Envoy,Office of e-Envoyに代わる新しい組織である電子政府局,UK Onlineに代わる新しいオンライン・サービスであるDirect.govについて記述する。最後に,英国のITを含む科学技術政策を推進している貿易産業省の「5か年プログラム」について記述する。

世界的に1990年代からインターネットとPCが急速に普及し始め,産業活動はもちろんのこと一般社会にもその活用が広がりつつある。そのひとつのビジネス形態が,電子商取引である。各国の政府機関等では,これらの情報技術(Information Technology: IT)を積極的に政策に取り込み,自国のIT政策として展開している。本稿では,まず,1993年に発足したクリントン政権から現政権のブッシュ政権までの情報技術政策の経緯について記述する。そして,ITによって生まれた新しい経済現象であるデジタル・エコノミーについて概観する。さらに,情報セキュリティに影響を与える国土安全省の新設,電子商取引,電子政府について記述する。最後に,コンピュータ・システム関連のR&Dプログラムであるネットワーク情報技術研究開発プログラムについて記述する。

An intermolecular benzylic C–H bond amination utilizing the combination of chloramine-T and I2 without the aid of transition-metal catalysts has been developed. The reaction was found applicable to a variety of benzene-substituted alkanes, as well as to an adamantane derivative to give N-alkylated p-tosylamides in good yields.
