In Nagoya City University Hospital, a two-week training program for senior students in pharmacy training was carried out up until 1999. The large majority of senior students took a passive role in this pharmacy training course and therefore did not understand that importance of such training. Therefore, the pharmacy training period was extended from two-weeks to four-weeks in 2000. We thus evaluated what differences there were after the training was increased from two-weeks to four-weeks.The four-week training program was found to increase student motivation toward the pharmacy training. Our survey concluded that approximately 69 % of all senior students were interested in pharmacy training and 75% of senior students understood the role of the pharmacist. In addition, 87% of senior students had an improved image of pharmacists after the new pharmacy training. The reason for such an increase was that the senior students gained a better understanding of the reasons for training and also obtained practical experience in pharmaceutical care. These findings suggest that the four-week training program with self-learning exercises was useful for normally passive senior students. It also enabled hospital pharmacists to make an even greater contribution to the pharmacy training program.

最近,乳房切断術後,直ちに広背筋皮弁や腹直筋皮弁など自家組織を用いて乳房再建術を行う一期的乳房再建術が普及しつつあるが,予後に関する報告を見ない.今回,これら自家組織を用いて乳房再建を行った83例と乳房切断術のみを行った153例の生存率と非再発生存率を比較検討した.その結果,一変量解析で5年生存率および非再発生存率は乳房再建症例でそれぞれ92%, 90%,非乳房再建症例でそれぞれ92%, 86%であり,いずれも両群間に有意差を認めなかった.更に多変量解析で両群症例の背景因子を補正し検討したが,両群間に有意差を認めなかった.従って,一期的乳房再建術の有無は乳癌の予後に影響を与えないことが示唆された.

植物の香りは人間生活の中に深く入り込み,様々な場面で暮らしを豊かにしている.植物の芳香成分の中でも,特にモノテルペンと総称される揮発性有機化合物は,ハーブや花の複雑な芳香成分の主要なグループを形成する.我々は,近年発展が著しい植物分子生物学の知見を応用し,ハーブの一種であるシソ由来のリモネン合成酵素遺伝子を,芳香成分を作る樹木であるユーカリに導入した.その結果,ユーカリオイルの芳香成分の蓄積量を最高で約5倍に増加させることに成功した.本稿では,こうした香りのエンジニアリングにおいて基礎研究が極めて重要な役割を果たしたこともあわせて報告する.

We report a case of the Far-Out Syndrome that occurred in a female patient with isthemic spondylolisthesis and a 30% slip.<br>This syndrome was described as alar transverse process impingment of the L5 spinal nerve by Wiltse in 1984. The patient's chief complaint was bilateral leg pain and we were not able to visualize an abnormal lesion in the spinal canal with myelography, CTM or MRI. At surgery, it was necessary for L5 spinal nerve decompression to be carried out far enough laterally. L5 spinal root block is a important diagnostic tool in this syndrome.

Light transmission property of plain woven fabrics was studied in order to perform the fabric design considering the transmission. A light transmission measurement apparatus which could change incidence and receiving direction of light was developed and the change of the transmission factor by light direction was measured for plain woven fabrics. Because structures of the fabrics are anisotropy, the light transmission property showed anisotropy. The anisotropy of transmission factor was remarkable in large incidence angle, and the minimum or maximum values were showed every Pai/4 cycle. An approximation curve of anisotropy was proposed and the anisotropic light transmission factor was estimated from a measurement at front of a fabric and structure of the fabric based on crimp theory of Peirce.

2011年1~3月の間に当院に入院した非代償性心不全患者で, これまでに3回以上の心不全入院歴があるか, または強心薬依存状態でフロセミド40mg/日相当以上, あるいは複数の利尿薬を内服していた65歳以上の高齢者心不全増悪症例計18例 (平均81歳) に対するバソプレシンV2受容体, トルバプタンの有用性を検討した. トルバプタンの経口投与開始24時間後には, 尿量が有意に増加した. 心エコーから求めた心内圧や脳性ナトリウム利尿ペプチド (brain natriuretic peptide ; BNP) 値は有意に低下した. 一方, 血圧に関しては有意な変化は認めなかった. 血清クレアチニン値に有意な変化はなかったが, 血清ナトリウム濃度は3例で145mEq/Lを超えたため, トルバプタンを中止した. 入退院を繰り返す非代償性心不全において, トルバプタンの追加投与は, 少量の内服でも有効な尿量増加が期待できる.

肺切除術の際に過分葉を認めることが時にあるが,不全分葉であることが多い.今回,上舌区域間に明瞭な過分葉を認めた症例に対して区域切除を行った2例を経験したので報告する.症例1は57歳男性.左S1+2の肺癌に対して,胸腔鏡下上区域切除術を施行した.上舌区域間に明瞭な過分葉を認め,肺動脈は上葉支の腹側を走行し,B1+2+3が独立して分岐する転位気管支を認めた.症例2は80歳女性.左舌区の肺癌に対して,胸腔鏡下左舌区域切除を施行した.左上舌区域間に明瞭な過分葉を認め,共通肺静脈幹を認める以外,分岐異常は認めなかった.術後経過は共に良好で,現在まで無再発生存中である.過分葉を認める肺癌手術症例について,文献的考察を交え報告する.

真珠は、主に炭酸カルシウムとタンパク質からなる多層薄膜構造である。文化財において古墳から出土した真珠は白濁化しており、タンパク質が分解などにより大部分が消失した結果との報告がある(1987 年、「鳥浜貝塚」調査報告)。このことはタンパク質が保存環境により変質する場合があることを示し、タンパク質シートが真珠の結晶層の維持に重要な役割を果たしていると考えられる。現在流通している真珠は加工されているものが多く、その工程の中に行われる溶液に浸漬、加熱乾燥などで真珠層内部の有機物を多く含む稜柱層などの脆弱な箇所から、層われ、ヒビ、剥離などの劣化が起こる場合もある。さらにこれらの劣化現象は保管時の温湿度変化などでさらに顕在化する。これらの真珠の欠陥は真珠の品質に大きく関係しているため、流通の際には注意する必要がある。今回はこれらの劣化現象のうち加工キズの劣化現象について、見え方、構造、原因などで体系化することを目的とした。特に加工キズの中のひびについて断面の構造を観察し、その成因について考察した。また、温度サイクルによる加速実験を行い、どのように顕在化するかを検証した。

Not few investigators have story interest on the relation between food intake and mortality. From the viewpoint of Dr. Kondo, we have also been motivated to survey Yonaguni Island of the Ryukyu Islands.<br>Dr. Kondo suggested that food consumption in this island would be characteristic and biased to the large intake of fish and meat.<br>But nobody can exactly describe the status of food in Yonaguni Island because no dietary survey on its inhabitants has been conducted.<br>We had a happy chance to do make a dietary survey there in October 1973, in jointwork of the Yaeyama Health Center.<br>Subjects; The subjects of this survey were fifty eight families selected at random from three hamlets (Sonai, Kubra and Hikawa)<br>Results; The results are shown below.<br>1. The intake of calories, protein, fat and vitamin C was estimated to be higher than the average of the Japanese National Nutrition Survey of 1971.<br>On the other hand calcium and vitamin B₂ was estimated as fairly lower than national levels.<br>2. In comparison with those surveys, the amount of several food items; fish, meat, green and yellow vegetables in Yonaguni are estimated to be higher than the National Level and Okinawa Island.<br>3. Food intake patterns differ among the three hamlets, which compose the community of an isolated island.<br>4. On the other hand, there is little difference in food preparation among them.<br>5. From above it may be said that the dietary intake of the hamlets is in unity, whereas food preparation is definite as the whole island.
