Background:The optimal cutoff values of the brachial-ankle pulse wave velocity (baPWV) for predicting cardiovascular disease (CVD) were examined in patients with hypertension.Methods and Results:A total of 7,656 participants were followed prospectively. The hazard ratio for the development of CVD increased significantly as the baPWV increased, independent of conventional risk factors. The receiver-operating characteristic curve analysis showed that the optimal cutoff values for predicting CVD was 18.3 m/s. This cutoff value significantly predicted THE incidence of CVD.Conclusions:The present analysis suggests that the optimal cutoff value for CVD in patients with hypertension is 18.3 m/s.

Pancreatic islets express high levels of the orphan G-protein coupled receptor C5C (GPRC5C), the function of which remains to be established. Here we have examined the role of GPRC5C in the regulation of insulin secretion and β-cell survival and proliferation using human and mouse pancreatic islets. The expression of GPRC5C was analysed by RNA-sequencing, qPCR, western blotting and confocal microscopy. Insulin secretion and cell viability were determined by RIA and MTS assays, respectively. GPRC5C mRNA expression and protein level were reduced in the islets from type-2 diabetic donors. RNA sequencing in human islets revealed GPRC5C expression correlated with the expression of genes controlling apoptosis, cell survival and proliferation. A reduction in Gprc5c mRNA and protein expression was observed in islets isolated from old mice (>46 weeks of age) compared to that in islets from newborn (<3 weeks) mice. Down-regulation of Gprc5c led to both moderately reduced glucose-stimulated insulin release and also reduced cAMP content in mouse islets. Potentiation of glucose-stimulated insulin secretion concomitant with enhanced islet cAMP level by all-trans retinoic acid (ATRA) was attenuated upon Gprc5c-KD. ATRA also increased [Ca+2]i in Huh7-cells. Gprc5c over expression in Huh7 cells was associated with increased ERK1/2 activity. Gprc5c-KD in clonal MIN6c4 cells reduced cell proliferation and in murine islets increased apoptosis and the sensitivity of primary islet cells to a cocktail of pro-apoptotic cytokines. Our results demonstrate that agents activating GPRC5C represent a novel modality for the treatment and/or prevention of diabetes by restoring and/or maintaining functional β-cell mass.

Background:Some disagreements surround the effects of calcium-channel blockers (CCBs) on the risk of dementia. The purpose of this study was to investigate the protective effects of CCBs on dementia among elderly hypertensive Koreans.Methods and Results:We conducted a large population-based cohort study using the senior cohort database of the Korean National Health Insurance Service (2002–2013). Subjects were elderly hypertensive Koreans older than 60 years of age. A total of 18,423 patients (CCB user group: 13,692 patients; non-CCB antihypertensive user group: 4,731 patients) were statistically analyzed using the Cox proportional hazard regression model to estimate the adjusted hazard ratio (aHR) and confidence intervals (CIs) of dementia associated with CCB use. There were 2,881 cases (21.0%) of dementia in the CCB user group and 1,124 cases (23.8%) in the non-user group. CCB use significantly reduced the risk of total dementia (aHR 0.81, 95% CI 0.75–0.87, P<0.0001), Alzheimer’s dementia (aHR 0.80, 95% CI 0.72–0.88, P<0.0001), and vascular dementia (aHR 0.81, 95% CI 0.70–0.94, P=0.0067).Conclusions:CCB use had a protective effect on the risk of dementia among elderly hypertensive Koreans.

韓国南東部の梁山断層は,ほぼ北北東-南南西方向に約200kmにわたって走り,顕著な破砕帯を伴っている.この断層系において活断層変位地形を野外調査し,その主断層についてトレンチ掘削を実施した.この断層は河成段丘面群とその構成層を変位させ,東側の相対的隆起を伴う右横ずれの活断層であることを確認した(岡田ほか,1994).蔚山郡彦陽南方では,大規模な宅地開発が進められているので,その用地を利用して,主断層に伴われる地殻運動やそれ並行すると推定した副断層について多くのトレンチ調査を行った。こうした調査から,次のような事柄が判明した.彦陽南方の台地(高位面)は北東流していたかつての酌川川が形成した扇状地であり,初生的には北東へ傾いていたはずであるが,トレンチ地点付近では西方へ逆傾斜している.掘削調査の結果,高位面を構成する礫層が撓曲変形を受けていることが判明した.この高位面の撓曲による上下変位量は約5mである.いくつかの断層は認められたが,地表面まで切断するものは見当たらなかった.梁山断層の平均変位速度や高位段丘の形成時期を解明する必要があるので,梁山断層が通過する彦陽地域から太和江沿いに河成段丘面を追跡し,海成段丘面との関係を調べ,段丘面編年に関する資料を得るように努めた.それらの結果は,次のように要約される.河成段丘面はfH面群(fH1面・fH2面)・fH面群(fH1面・fM2面)・fL面群(fL1面〜fL3面)に,海成段丘面はm1面〜m3面に区分できる.海成段丘の旧汀線高度は,それぞれ53.3m・18.7m・3.4mである.fH面群やfM面群には赤色風化殻が形成されており,とくにfH面群で顕著である.fL面群の構成層は新鮮でほとんど風化していない.各河成段丘面は滑らかに蔚山湾周辺まで連続する.蔚山湾周辺では,fH2面は+10mの位置へ,fH1面は数mの位置へと連続してゆく.fL1面は下流部で沖積面下に埋没し,蔚山湾周辺での推定高度は-10mである.こうした資料からみて,fM1面が最終間氷期直前の氷期に,m2面が最終間氷期に形成された可能性が高い.fH面群はそれ以前の海面低下期に,m1面は最終間氷期以前の高海面期に形成されたと推定できる.南北〜北北西-南南東走向の蔚山断層系(延長約40km)は慶州市付近で梁山断層系に会合するが,この中央部に沿っても活断層変位地形の存在と,段丘堆積物を変位させる断層露頭が確認された.この特徴や関連現象について調べ,次のような事柄が判明した.蔚山断層系の断層線は著しく弯曲している.断層露頭表現や地形面の変形状態とから考えると,この断層の活動様式は典型的な逆断層である.第四紀後期に形成された地形面や堆積物が明瞭に変位を受けているので,蔚山断層は明らかに活断層である.この断層は高位段丘面を15m,中位段丘面を5m,上下方向へ変位させており,累積的な変位が認められる.断層崖や段丘面の変位方向からみて,東側の山地域が少なくとも第四紀の中ごろから継続的に隆起している.蔚山断層に沿って,明瞭な断層露頭が2ヶ所で観察された.末方里集落東方にある寺谷池北岸では,破砕した花崗岩が地形面を構成する礫層に,走向:ほぼ南北で,傾斜:25-30°Eの衝上面をもって接している.露頭上部では,上盤の花崗岩を被覆する礫層と砂礫層・腐植質層が急斜・逆転している.数本の断層が伴われ,幅数10cmの断層帯となっている.開谷里集落北東方の淵安川河床でも,やや風化した礫層の上に花崗岩が衝上している.末方里集落東方では,中位面を構成するシルト質層が液状化作用を受けて変形し,堆積直後の大地震発生を示唆する.その再来時間については,堆積物や地形面の年代解明を現在行っており,それらの結果を待って評価したい.こうした南北方向の逆断層性活断層の存在は,当域もほぼ東西方向の広域応力場に置かれていることを示唆する.これは北北東-南南西方向の梁山断層系が右ずれを示すこととも符号し,同じ応力場にあることを意味する.また,浦項市付近には,海成中新統が分布していることから,中新世以降の梁山断層の運動像を解明するために,地質調査を実施した.

This study investigated the impact of observation operators on low-level wind speed analysis. An evaluation of wind speeds retrieved by variational multiple-Doppler analyses using radial velocities (Vr) based on the formats of both a Plan Position Indicator (PPI) (hereafter, PPI-VAR) and a Constant Altitude Plan Position Indicator (CAPPI) (hereafter, CAPPI-VAR) was performed for comparison with wind speeds observed by a wind profiler during the warm season of three consecutive years. The statistical analysis showed that PPI-VAR was more accurate than CAPPI-VAR at 500 m above ground level (AGL). The error of CAPPI-VAR at 500 m AGL was caused by a representative error of CAPPI-formatted Vr derived from a certain radar whose beam height was far from the analysis level, and this error became more obvious the greater the vertical difference in wind speed across the analysis level. CAPPI-VAR uses CAPPI-formatted Vr from each radar equally; thus, the representative error might cause performance degradation of CAPPI-VAR at 500 m AGL. Conversely, PPI-VAR uses PPI-formatted Vr from each radar with appropriate weighting based on the beam height distance from the analysis level. PPI-VAR showed better results at 500 m AGL because the observation grid points were dense around 500 m AGL.

The Great East Japan Earthquake (GEJE) and resulting tsunami of March 11, 2011 gave rise to devastating damage on the Pacific coast of the Tohoku region. The Tohoku Medical Megabank Project (TMM), which is being conducted by Tohoku University Tohoku Medical Megabank Organization (ToMMo) and Iwate Medical University Iwate Tohoku Medical Megabank Organization (IMM), has been launched to realize creative reconstruction and to solve medical problems in the aftermath of this disaster. We started two prospective cohort studies in Miyagi and Iwate Prefectures: a population-based adult cohort study, the TMM Community-Based Cohort Study (TMM CommCohort Study), which will recruit 80 000 participants, and a birth and three-generation cohort study, the TMM Birth and Three-Generation Cohort Study (TMM BirThree Cohort Study), which will recruit 70 000 participants, including fetuses and their parents, siblings, grandparents, and extended family members. The TMM CommCohort Study will recruit participants from 2013 to 2016 and follow them for at least 5 years. The TMM BirThree Cohort Study will recruit participants from 2013 to 2017 and follow them for at least 4 years. For children, the ToMMo Child Health Study, which adopted a cross-sectional design, was also started in November 2012 in Miyagi Prefecture. An integrated biobank will be constructed based on the two prospective cohort studies, and ToMMo and IMM will investigate the chronic medical impacts of the GEJE. The integrated biobank of TMM consists of health and clinical information, biospecimens, and genome and omics data. The biobank aims to establish a firm basis for personalized healthcare and medicine, mainly for diseases aggravated by the GEJE in the two prefectures. Biospecimens and related information in the biobank will be distributed to the research community. TMM itself will also undertake genomic and omics research. The aims of the genomic studies are: 1) to construct an integrated biobank; 2) to return genomic research results to the participants of the cohort studies, which will lead to the implementation of personalized healthcare and medicine in the affected areas in the near future; and 3) to contribute the development of personalized healthcare and medicine worldwide. Through the activities of TMM, we will clarify how to approach prolonged healthcare problems in areas damaged by large-scale disasters and how useful genomic information is for disease prevention.

The formation of ATP produced from poly(ADP-ribose) [(ADP-R)n] has been suggested to be required to repair damaged DNA. Here we investigate whether this ATP is involved in DNA replication processes during DNA repair. Poly(ADP-ribosyl)ated mid-S phase cell nuclei, which were isolated from synchronized HeLa S3 cells followed by the treatment with a DNA damaging agent, N-methyl-N′-nitro-N-nitrosoguanidine (MNNG), were revealed to retain DNA replication synthesizing activity during preincubation for de-poly(ADP-ribosyl)ation only in the presence of pyrophosphate (PPi) before DNA synthesis was started by adding 3 mM ATP. This DNA replication activity was not maintained in the presence of a potent and specific inhibitor of poly(ADP-ribose) glycohydrolase (PARG), Oenothein B (Oen B) during the preincubation with PPi. In the preincubation with PPi, μM orders of ATP was produced from (ADP-R)n. These results point to an important function of ATP generated from (ADP-R)n in nuclei for the maintenance of replication apparatus during DNA repair.

Differentiated thyroid carcinoma (DTC) is generally indolent in nature and, even though it metastasizes to distant organs, the prognosis is normally excellent. In contrast, the overall survival (OS) of patients with radioactive iodine (RAI)-refractory and progressive metastases is dire, because no effective therapies have been available to control the metastatic lesions. However, recently, administration of tyrosine-kinase inhibitors (TKIs) has become a new line of therapy for RAI-refractory and progressive metastases. Previous studies have reported significant improvement regarding the progression-free survival rates of patients with metastatic lesions. However, TKIs cause various severe adverse events (AEs) that damage patients’ quality of life and can even be life-threatening. Additionally, metastatic lesions may progress significantly after stopping TKI therapy. Therefore, it is difficult to determine who is a candidate for TKI therapy, as well as how and when physicians start and stop the therapy. The present review, created by Committee of pharmacological therapy for thyroid cancer of the Japanese Society of Thyroid Surgery (JSTS) and the Japan Association of Endocrine Surgeons (JAES) describes how to appropriately use TKIs by describing what we do and do not know about treatment using TKIs.

Great earthquakes have repeatedly occurred along the Nankai Trough, the subduction zone that lies south of Japan’s heavily industrialized southern coastline. While historical records and geological evidence have revealed spatial distribution of paleo-earthquakes, the temporal variation of the rupture zone is still under debate, in part due to its segmented behavior. Here we explore the potential of the sediment records from Lake Hamana and Fuji Five Lakes as new coherent time series of great earthquakes within the framework of the QuakeRecNankai project.We obtained pilot gravity cores form Lake Hamana and the Fuji lakes Motosu, Sai, Kawaguchi, and Yamanaka in 2014. In order to image the lateral changes of the event deposits, we also conducted reflection-seismic survey. Based on these results, potential coring sites were determined and then 3–10 m long piston cores were recovered from several sites in each lake in 2015. The cores consist of 2 m long sections with 1 m overlaps between the sections allowing us to reconstruct continuous records of tsunamis and paleo-earthquakes. In this presentation we introduce the progress of QuakeRecNankai project and discuss the potential of the lakes as Late Pleistocene and Holocene archives of tsunamis and paleo-earthquakes.

[Purpose] This study investigated the effects of a 12-hour neuromuscular electrical stimulation program in the evening hours on upper extremity function in sub-acute stroke patients. [Subjects and Methods] Forty-five subjects were randomized to one of three groups: 12-hour neuromuscular electrical stimulation group (n=15), which received 12 hours of neuromuscular electrical stimulation and conventional rehabilitation for the affected upper extremity; neuromuscular electrical stimulation group (n=15), which received 30 min of neuromuscular electrical stimulation and conventional rehabilitation; and control group (n=15), which received conventional rehabilitation only. The Fugl-Meyer assessment, Action Research Arm Test, and modified Ashworth scale were used to evaluate the effects before and after intervention, and 4 weeks later. [Results] The improvement in the distal (wrist-hand) components of the Fugl-Meyer assessment and Action Research Arm Test in the 12-hour neuromuscular electrical stimulation group was more significant than that in the neuromuscular electrical stimulation group. No significant difference was found between the two groups in the proximal component (shoulder-elbow) of the Fugl-Meyer assessment. [Conclusion] The 12-hour neuromuscular electrical stimulation group achieved better improvement in upper extremity motor function, especially in the wrist-hand function. This alternative therapeutic approach is easily applicable and can be used in stroke patients during rest or sleep.

Chronic Fatigue (CF) is a common reason for consulting a physician due to affecting quality of life, but only a few effective treatments are available. The aim of this study was to examine the effectiveness of subcutaneous injection of the human placental extract (HPE) on medically indescribable cases of CF and safety in a randomized, double-blind, placebo-controlled clinical trial. A total of seventy eight subjects with CF were randomly assigned to either a HPE group or a placebo group. Subjects in the HPE group were treated with HPE three times a week subcutaneously for 6 weeks, whereas those in the placebo group with normal saline. Then, the Fatigue Severity Scale (FSS), Visual Analog Scale (VAS) and Multidimensional Fatigue Inventory (MFI) were measured in both CF group and chronic fatigue syndrome (CFS) and idiopathic chronic fatigue (ICF) subgroup. The FSS, VAS and MFI score at baseline were not different between the HPE and placebo group in total subjects with CF. In CFS group, the FSS (p= 0.0242), VAS (p =0.0009) and MFI (p= 0.0159) scores measured at the end of the study period decreased more in the HPE group than in the placebo group when compared with those at the baseline. There were no significant differences between the HPE group and placebo group in the mean change from baseline in FSS, VAS, and MFI in subjects with ICF during the study period. The subcutaneous injection of HPE was effective in the improvement of CFS.

The presence of circulating tumor cells (CTCs) serves as a prognostic marker and indicator of disease relapse, as well as a means of evaluating treatment efficacy in human and canine lymphoma patients. As an extension of our previous study for the construction of clinically useful GeneScan system, we utilized the GeneScan system for detecting CTCs in canine lymphoma patients. Samples from the primary lesion and peripheral blood mononuclear cells (PBMCs) were obtained from 32 dogs with lymphoma at initial diagnosis. All samples were subjected to polymerase chain reaction (PCR) for antigen receptor gene rearrangements (PARR) followed by GeneScan analysis. Common clonal rearrangements with identical amplified fragments were detected in both the primary lesion and PBMCs in 19 of the 32 dogs (59.4%). However, the detection rate of CTCs varied among the anatomical classification of lymphoma studied. GeneScan analysis following PARR would facilitate studies on determining the clinical significance of CTCs in canine lymphoma patients.

Background:In the multinational, double-blind, double-dummy ENGAGE AF-TIMI 48 phase 3 study, once-daily edoxaban was non-inferior to warfarin for prevention of stroke or systemic embolism event (SEE) in patients with non-valvular atrial fibrillation (AF). Here, we evaluated the efficacy and safety of edoxaban in patients from East Asia.Methods and Results:Patients aged ≥21 years with documented AF and CHADS score ≥2 were randomized to receive once-daily edoxaban higher-dose (60 mg) or lower-dose (30 mg) regimen or warfarin dose-adjusted to an international normalized ratio of 2.0–3.0. Patients with a creatinine clearance of 30–50 ml/min, weighing ≤60 kg, or receiving strong p-glycoprotein inhibitors at randomization or during the study received a 50% dose reduction of edoxaban or matched placebo. This prespecified subanalysis included 1,943 patients from Japan, China, Taiwan, and South Korea. The annualized rate of stroke/SEE for higher-dose edoxaban was 1.35% vs. 2.62% for warfarin (hazard ratio [HR], 0.53; 95% confidence interval [CI]: 0.31–0.90, P=0.02) and 2.52% for lower-dose edoxaban (HR, 0.98; 95% CI: 0.63–1.54, P=0.93). Compared with warfarin (4.80%), major bleeding was significantly reduced for the higher-dose (2.86%; HR, 0.61; 95% CI: 0.41–0.89, P=0.011) and lower-dose regimens (1.59%; HR, 0.34; 95% CI: 0.21–0.54, P<0.001).Conclusions:Once-daily edoxaban provided similar efficacy to warfarin while reducing major bleeding risk in the East Asian population.

The present study aims to outline the genetic makeup of the current population of the town of Yanga (Veracruz State, Mexico), the first Latin American settlement founded by African slaves in Mexico. For this purpose, we carried out the genetic characterization of 60 individuals from Yanga, analysing 15 autosomal short tandem repeats (STRs) and interpreting the results in the context of the admixed population known as Mexican mestizos. The genetic contribution from the three most important human groups in the current admixed Yanga population was calculated using Structure software. We detected a high percentage of Amerindian (48%) and European inheritance (44.7%), and a much less important African contribution (7.3%). These results were then compared with 10 other Mexican mestizo populations. The results fit the tri-hybrid model for admixture characterized by a high genetic contribution from Europeans and Africans in the north—though the African influence is lower—and a decreasing contribution from these two populations to the south and southeast. Conversely, the Amerindian component presents maximum values in the south and minimum values in the north. The Amerindian and European genetic traces are related to their ancestral settlements, but the African contribution can be explained by other parameters. To understand the current African genetic traces, we have to assume that there was a redistribution of these population groups and an important admixture phenomenon which led to the dilution of the African ancestral genetic pool. Furthermore, admixture was favoured by conditions that allowed individuals who intermarried to ascend in social status. These reasons would explain why despite the fact that Yanga was founded by black slaves, high levels of African ancestry are not found in the current population.

Background:It is disputed whether the risk of cardiogenic embolism varies with type of atrial fibrillation (AF). Although several studies have found that the risk of cardiogenic embolism was similar among paroxysmal and persistent/permanent AF, a few studies have found that patients with paroxysmal AF had a lower rate of stroke and systemic embolism than those with persistent/permanent AF. In the present study, post-hoc analysis of the J-RHYTHM Registry was done to compare the risk of thromboembolic events among 3 types of non-valvular AF (NVAF).Methods and Results:A total of 7,406 NVAF patients were followed up prospectively for 2 years. At baseline, warfarin was used for 78.6%, 90.0%, and 91.8% of patients with paroxysmal, persistent, and permanent AF, respectively. There were 126 thromboembolic events during the follow-up period. The crude event rate was 2-fold higher among the patients with permanent NVAF (2.29%) than among those with paroxysmal (1.16%) or persistent (1.20%) NVAF (P=0.001). After adjusting for warfarin use and CHA2DS2-VASc score components, however, the hazard ratio for thromboembolism did not differ between paroxysmal (reference) and permanent NVAF (1.007; 95% confidence interval: 0.955–1.061).Conclusions:The crude rate of thromboembolic events was higher in permanent NVAF than in paroxysmal NVAF, but after adjusting for warfarin use and CHA2DS2-VASc score components, paroxysmal and permanent NVAF patients had similar risk of thromboembolism.

[Purpose] The purposes of this study were to investigate the immediate and delayed effects of two directions of Kinesio taping (KT) on maximal isometric strength of the wrist and finger muscles of healthy adults and compare their differences. [Subjects] Nineteen healthy junior college students participated in this study. [Methods] The inhibition and facilitation KT techniques were separately used to tape on the dominant and non-dominant forearms of the participants, respectively. Maximal isometric strength of wrist extension, middle finger extension, and grip of both hands were measured before taping, immediately after taping, and after 24 h of taping (with the tape in situ). [Results] Compared with the baseline, the average maximal isometric strength of middle finger extensors increased considerably after application of the facilitation technique. No significant time effect was observed for measurement of middle finger extension strength on the dominant side or for wrist extension and grip strength on both sides. Significant differences between both taping techniques were observed for wrist and middle finger extension strength immediately after taping, and for middle finger extension after 24 h of taping. [Conclusion] The results suggest that the application direction of KT may have different effects on isometric muscle strength. Future studies involving a larger sample of subjects and a sham condition are warranted to confirm our findings.

[Purpose] This study examined the effect of taping the quadriceps femoris muscles of healthy college students on anaerobic muscle power (AMP) and the anaerobic threshold (AT) to analyze the functional value of taping on athletic performance capacity. [Subjects] Thirty male healthy college students were enrolled in this study. [Methods] The study was performed from April to May 2010 (for a month). Kinesio tapes, 5 cm in width, were applied to quardriceps femoris muscles of participants. AMP and AT tests were conducted a sports science research laboratory. The paired t-test was conducted to examine the significance of differences between before and after taping. [Results] Mean power, peak power, mean power/kg, peak power/kg showed significant increases after taping. The induction time of AT, and, VE, VO2, HR, VO2max at AT showed no significant improvements after taping, but RPE showed a significant improvement. [Conclusion] Taping of the quadriceps femoris muscles of healthy college students affected AMP but not AT suggesting that taping influences athletic performance capacity.

We investigated mitochondrial DNA haplogroups of four Jomon individuals from the Sanganji shell mound in Fukushima, Tohoku district, Japan. Partial nucleotide sequences of the coding and control region of mitochondrial DNA were determined. The success rate of sequencing increased when we analyzed short DNA sequences. We identified haplogroups from all four samples that were analyzed; haplogroup frequencies were 50% (n = 2) for N9b and 50% (n = 2) for M7a2. Haplogroup N9b has been previously observed in high frequencies in the other Tohoku Jomon, Hokkaido Jomon, Okhotsk, and Ainu peoples, whereas its frequency was reported to be low in the Kanto Jomon and the modern mainland Japanese. Sub-haplogroup M7a2 has previously been reported in the Hokkaido Jomon, Okhotsk, and modern Udegey (southern Siberia) peoples, but not in the Kanto Jomon, Ainu, or Ryukyuan peoples. Principal component analysis and phylogenetic network analysis revealed that, based on haplogroup frequencies, the Tohoku Jomon was genetically closer to the Hokkaido Jomon and Udegey people, than to the Kanto Jomon or mainland modern Japanese. The available evidence suggests genetic differences between the Tohoku and Kanto regions in the Jomon period, and greater genetic similarity between the Tohoku Jomon and the other investigated ancient (Hokkaido Jomon, Okhotsk) and modern (Siberian, Udegey in particular) populations. At the same time, the Tohoku and Hokkaido Jomon seem to differ in sub-haplotype representations, suggesting complexity in Jomon population structure and history.
