止血困難な気道出血に対し, 高出力半導体(GaAlAs)レーザーを用いることにより止血し得た症例を経験したので報告する。症例は85歳男性, 食事中に突然新鮮血を吐き, 意識不明状態で救急車にて搬送された。二次救命処置後, 気管内挿管チューブから血性痰が吸引されたが, 緊急上部消化管内視鏡検査でDieulafoy潰瘍からの出血を認めたため吐血の誤嚥と考えられた。しかし胸部大動脈瘤の病歴, および血痰の持続から, 気道出血の可能性を考え気管支内視鏡検査を施行した。気管および左中間幹に出血源を認めたためエピネフリン散布, トロンビン散布, バルーンカテーテルによる圧迫を5日, 計10回にわたり試みたが止血は不十分であった。今回, この気道出血に対し, 高出力半導体(GaAlAs)レーザーシステム(DIOMED 25, OLYMPUS)を使用し, 経内視鏡的に止血に成功し得た。

われわれは学校や職場において,さまざまな評価を受けている.入学試験や採用試験などテストによって,一人ひとりのその後の人生を左右する評価がなされることもある.心理データの中でも特にテストの結果について,データを読み解く能力,心理データ・リテラシーが,テスト作成者,テスト利用者,受験者には必要である.ここでは,テストに求められる公平性,妥当性,信頼性の観点からテスト・データを読み解く上での留意点について示す.

議会の機能に関する研究では,政策アウトプットにおける議会の影響を扱う研究が多く,議会のパフォーマンスを対象とし,かつ議会間の差を検討する研究が少ない。また議会の機能を説明する変数としては,議会の党派性や首長と議会の会派構成の違いなどが要因として用いられており,アクターの心理的変数,ならびにそれを集団的に捉えた政治文化的変数を用いる研究が少ない。そこで本稿では,地方分権が進展する中,議会にも政策立案機能が求められていることを背景とし,都道府県間の議員提案による政策条例数をパフォーマンス指標として取り上げ,その規定要因として議会内の立法型役割意識の程度,首長―議会間での是々非々意識といった政治文化変数を用い,他の要因も含めて分析を行った。その結果,2つの政治文化変数が政策条例を促進する要因として有意であることがわかり,また鳥取県の事例研究からも同様の結果を得ることができた。

茨城県における児童, 生徒の食物摂取の実態を把握し, 栄養指導の基礎資料を得ることを目的として, 昭和52年5月と11月の2回, 小学校89校, 中学校30校の児童・生徒計7,961名を対象に, 金, 土, 日曜日の3日間食事調査を実施し, 合わせて食品摂取と体位, 体力との関連についても検討した。牛乳, 乳製品, 緑黄色野菜に関する結果は次のとおりである。<br>(1) 家庭における1人1日当たり摂取量を5月, 11月のそれぞれ土, 日曜日の計4日の平均値でみると, 牛乳は児童55.8g, 生徒73.3g, 乳製品は児童4.9g, 生徒5.6g, 緑黄色野菜は児童23.4g, 生徒23.0gであった。<br>(2) 学校給食 (金曜日昼) を除く3日間の家庭食についてみてみる。<br>1) 食事別では, 牛乳は間食で最も多くとられ, 夕食で最も少なく, 緑黄色野菜は夕食で最も多く, 間食で最も少なかった。乳製品は間食で2/3が占められ, 朝, 昼, 夕食の摂取量は極めて少量であった。<br>2) 生徒と児童の比較では, 牛乳は生徒のほうが摂取量が多かったが, 乳製品, 緑黄色野菜は有意差がなかった。<br>3) 男子と女子の比較では, 牛乳は男子のほうが, 乳製品は女子のほうが摂取量が多かった。緑黄色野菜は有意差がなかった。<br>4) 5月と11月の比較では, 緑黄色野菜は11月のほうが多く摂取されていた。牛乳, 乳製品は有意差がなかった。<br>5) 牛乳, 乳製品および緑黄色野菜の摂取量にかなりの地域差がみられた。<br>6)摂取量の分布をみると, 3日間家庭で全く摂取しなかったものの割合は, 牛乳は児童53.7%, 生徒45.6%, 乳製品は児童79.6%, 生徒77.8%, 緑黄色野菜は児童16.7%, 生徒13.7%であった (5月)。<br>7) 各食品群の種類別では, 乳製品はヨーグルト類とアイスクリームが, 緑黄色野菜はにんじん, ほうれん草, ピーマンが約2/3を占めていた。<br>(3) 学校給食は家庭での昼 (土, 日曜日の平均) に比べ, 摂取量は, 牛乳は児童16.3倍, 生徒13.6倍, 乳製品は児童14.8倍, 生徒15.1倍, 緑黄色野菜は児童, 生徒ともに3.6倍であり, 金曜日の1日総摂取量のうち学校給食に由来する摂取量の割合は, 牛乳は児童79.0%, 生徒72.6%, 乳製品は児童62.5%, 生徒73.2%, 緑黄色野菜は児童41.8%, 生徒42.3%であった (5月, 11月の平均)。<br>(4) 牛乳, 乳製品, 緑黄色野菜の摂取量はそれぞれ独立して体位, 体力に関連するというよりはむしろこれらの総合的な摂取状況が他の因子とともに体位, 体力に影響することが示唆された。

A population of <i>Polygala tatarinowii </i>Regel, an endangered annual herb in Japan, was found near Abukuma Limestone Cave, Tamura City, Fukushima Prefecture, Japan. It could be one of the largest population of the species in Japan, where more than a thousand plants grew on dry meadows beneath limestone bluff or on the flower garden surrounding the Limestone Cave.

In the study of the Liao 遼 Dynasty's governance of the Yanyun 燕雲 region one important historical source is an account concerning tax reductions implemented by Liu Liufu 劉六符 in the Yanyun region contained in Lu You's 陸游 notebook entitled Laoxuean Biji 老学庵筆記. Although the research to date has questioned the reliability of this account, it has failed to explain why Lu You would have included such a dubious account among the historical records he copied into his notebook. In this article, the author, through a comprehensive study of the historical records related to the Liao Dynasty contained in Laoxuean Biji and another Lu You notebook entitled Jiashi Jiuwen 家世旧聞, attempts to clarify the origins of the tax reduction account. As the title, Old Recollections About Family History, implies, Jiashi Jiuwen is a collection of narrative passed on to Lu You from his grandfather Lu Dian 陸佃 and father Lu Zai 陸宰. Some of the volume's information pertaining to the Liao was provided by Lu Dian's recollections concerning his experiences while dispatched to the Liao court. Due to the fact that Lu You wrote a postscript to Lu Dian's accounts of those experiences entitled Shilao Lu 使遼録, the author concludes that the information in Jiashi Jiuwen must have extracted from Shilao Lu, which has been lost. Returning to the tax reduction account in Lu You's Laoxuean Biji, the author points to the personage of one Liu Xiao 劉霄, who was the grandson of the same Liu Liufu who enacted the tax reduction and an acquaintance of Lu Dian while the latter was at the Liao court. Liu Xiao was probably the person who informed Lu Dian of his grandfather's accomplishment in the Yanyun region. Finally, the author takes up the authenticity of the "tax reduction account." By refuting its part about Liao Emperor Xing following any advice given by Liu Liufu during his negotiations with the Song Dynasty concerning increases in annual payments, Qing Period bibliographical scholar Bi Yuan 畢沅 denied the reliability of the "tax reduction account" in its entirety. However, the author refers to other sources in concluding that there are other parts of the account that ring true, meaning that the account can be used as a historical source in combination with other evidence. For example, the account is an excellent source for understanding that internal politics and diplomatic policy of the Liao Dynasty were linked organically to winning the consent of the Han-Chinese people of Yanyun. Furthermore, the account is extremely valuable for observing not only the Liao Dynasty's governance of Han-Chinese regions and the international relations surrounding it, but also its character as a nomadic state.

急速に進む高齢化社会をむかえいわゆる超高齢者も急増しておりこの年代の介護が重要な問題となっている. 超高齢者の実態調査の一環として超高齢者の食事摂取状況を検討した. まず1995年百寿者と米寿者の食事摂取状況を同年の対照群 (平成6年度国民栄養調査成績) と比較した. ついで年代変化を見る目的で過去に報告された1981年百寿者との比較も行った.<br>1995年百寿者と米寿群における栄養摂取量は対照群の60%, 75%と低下した. しかし栄養良好な百寿者の栄養摂取量は米寿群と同等であった. 日常生活活動度が良好な百寿者では栄養摂取量が多く, また認知機能良好な百寿者でも栄養摂取量が多かった. 食品群摂取を百寿者と対照群で比較すると乳類, 菓子砂糖類, 果実類の摂取は対照群と同等であったが, 穀類, 肉類, 魚類, 油脂類は低下していた. 同様の傾向は1981年百寿者でも認められ, 超高齢者では柔らかく甘い食品が好まれることがうかがわれた. 単位体重あたりの栄養摂取量は一般栄養所要量を越えており, 栄養良好な百寿者では不良群に比較して多かった. 1981年と1995年百寿者での栄養量摂取を比較すると1995年群で多かった. 食品群摂取では穀類, 豆類, 卵類, 藻類は1995年群で少なかったが, 他の食品群は多く摂取していた. これは年代による食習慣の違いによるものと考えられた.

In preceding report III, the histological results of the Sino-Tawara system were reported as a few specific systems. In this report, the functions of these systems were confirmed physiologically by means of cutting and ligating examinations of these systems. These physiological results were reported by Tateishi. The author carried out the histological aspects in relation between the cut or ligated parts and specific pathways on 18 dogs.The summary of this study are as follows.1) In 9 cases, that confirmed the complete cut of the Systema septo-angularis anterior by histological examinations, the PQ intervals were prolonged prominently. And in some cases that were cut but a small part of this system, the PQ intervals were prolonged in near proportion to the cut extent. From these results, it could be supposed certainly that this system was a main pathway as the Sino-Tawara system.2) In 10 cases, that comfirmed the complete cut of the Systema septo-angularis posterior by histological examinations, the P Q intervals reduced rather than the former or prolonged a little. These results were not agreeable with the author's and past investigators' histological observations. The author asserted the opinions of Robb, Kaylor & Turman in relation between the specific muscular fibers and its function.3) The cut method of the Systema dextroatrii caudalis was difficult technically because it was along the Arteriola sini. But from the facts that the PQ intervals were prolonged by the cut of the right atrial bundle, it could be supported that this system had the function as the Sino-Tawara system.4) In 5 cases, that comfirmed the complete cut of the right atrial bundle, the PQ intervals were prolonged prominently. From these facts, it could be supposed that this bundle was very important as the final pathway connected with the Tawara node.5) In 2 cases cut one-third of the Tawara node, the PQ interval was prolonged prominently. But in this cut extent, the A-V blocks were not observed. The author discussed as to the relationship between its functioin and histological structure.6) The atrial parts, in which were not observed the specific muscular fibers as the Sino-Tawara system by histological examinations, were performed by the cut examinations. From these results, these cut parts were not recognized functionally as the significance as the Sino-Tawara conducting pathways. Particularly, in the Torus Loweri, the author did not observe the important value as the Sino-Tawara system by histological and physiological examinations.

This paper presents how an FEA model using MSC.NASTRAN was developed to simulate dehydration dynamics of a washing machine with spinning basket rotating about vertical axis. Gyroscopic effect, centrifugal forces, and self-balancing mechanism (liquid balancer) associated with rotating structures were considered in the simulation. Using the model, frequency response analysis was performed to simulate response of non-rotating structures in operating test. Static analysis was performed to simulate strain and stress of rotating structures at a discrete operating speed. The model was validated successfully with operating test data measurement. This paper also presents how the operating test was performed to understand dynamics of the dehydration process. The test was performed by installing an unbalance mass to the spinning basket based on manufacturer's design standard.

Our previous study showed that oridonin isolated from <i>Rabdosia rubescens</i> enhanced phagocytosis of apoptotic cells by macrophage-like U937 cells through tumor necrosis factor (TNF) α and interleukin (IL)-1β release. In this study, we further investigated signaling events involved in oridonin-augmented phagocytosis. Phagocytic stimulation was significantly suppressed by inhibitors, including a phosphoinositide 3-kinases (PI3K) inhibitor (wortmannin), a protein kinase C (PKC) inhibitor (stauroporine), and a phospholipase C (PLC) inhibitor (U73122). Exposure of U937 cells to oridonin caused an increase in PKC activity time- dependently, which was prevented by pretreatment with inhibitors of PI3K and PLC. Simultaneously, the activation of protein kinase B (PKB/Akt) and the increased expression of PLCγ2 were also blocked by wortmannin. In addition, an extracellular signal-regulated kinase (ERK) MAPK inhibitor, PD98059, suppressed oridonin-augmented phagocytosis, whereas the p38 MAPK inhibitor (SB203580) and c-Jun N-terminal kinase (JNK) MAPK inhibitor (SP98059) had no inhibitory effect. Furthermore, pretreatment of U937 cells with anti-TNFα and anti-IL-1β antibodies blocked oridonin-induced phagocytic stimulation as well as phosphorylation of ERK, but did not block the activation of PKC, indicating that these signaling events are triggered by oridonin, whereas secreted TNFα or IL-1β only activate the ERK-dependent pathway. Taken together, oridonin is suggested to enhance phagocytosis of apoptotic bodies by activating PI3K, PKC, and ERK-dependent pathways.<br>
