We investigate regional characteristics of future changes in snowfall in Japan under two emission scenarios—RCP2.6 and RCP8.5—using a high-resolution regional climate model with 5 km grid spacing and discuss the influence of changes in atmospheric circulation. The high-resolution model can simulate details of changes in distributions of total snowfall in Japan. Under RCP2.6, the annual total snowfall decreases in most parts of Japan except for Japan's northern island, Hokkaido. In Hokkaido, the winter snowfall increases even under RCP8.5, especially in January and February. The snowfall peak is delayed from early December to late January in Hokkaido. Along the Sea of Japan in eastern Japan, the winter-total snowfall decreases even if the winter mean temperature is below 0°C in the future climate. The different snowfall changes in Hokkaido and on the Sea of Japan side of eastern Japan are caused by precipitation changes in each region. Future changes in atmospheric circulation related to the Aleutian low cause the enhancement and the inhibition of winter precipitation in Hokkaido and the Sea of Japan side of eastern Japan, respectively, contributing to changes in the regional characteristics of snowfall and snow cover in addition to moistening due to atmospheric and ocean warming.

To discuss the feasibility of the Himawari follow-on program, impacts of a hyperspectral sounder on a geostationary satellite (GeoHSS) is assessed using an observing system simulation experiment. Hypothetical GeoHSS observations are simulated by using an accurate reanalysis dataset for a heavy rainfall event in western Japan in 2018. The global data assimilation experiment demonstrates that the assimilation of clear-sky radiances of the GeoHSS improves the forecasts of the representative meteorological field and slightly reduces the typhoon position error. The regional data assimilation experiment shows that assimilating temperature and relative humidity profiles derived from the GeoHSS improves the heavy rainfall in the Chugoku region of western Japan as a result of enhanced southwesterly moisture flow off the northwestern coast of the Kyushu Island. These results suggest that the GeoHSS provides valuable information on frequently available vertically resolved temperature and humidity and thus improves the forecasts of severe events.

During the early summer of 2020, a stagnation of the Meiyu-Baiu front brought torrential rainfall over East Asia. Meanwhile, the anticyclone was much enhanced over the subtropical western Pacific (SWP), which contributed abundant moisture to the Meiyu-Baiu rainband along the western rim of the anticyclone. Based on the sensitivity experiments of the linear baroclinic model by prescribing the observed diabatic heating anomalies, a combination of anomalous convection over the Indian Ocean and reduced rainfall over the western Pacific can account for the maintenance of zonally elongated SWP anticyclone. Interestingly, this period corresponded to the developing stage of La Niña, while the convective activities were notably suppressed over the warmed western Pacific. The sensitivity experiments to SST anomalies using the atmospheric general circulation model shows that the attenuated convection over the western Pacific can be ascribed to the warmed Indian Ocean associated with an atmospheric Kelvin wave wedge from the Indian Ocean. Overall, the suppressant SST effect of the Indian Ocean opposes and is greater than that of tropical Pacific. We issue a caveat regarding the additivity of the remote influence across the maritime continent. These results have important implications for the predictability of early summer rainfall over East Asia.

Background: Cardiac troponin I (cTnI) is currently considered to be the most sensitive and specific biochemical marker of acute coronary syndrome and acute myocardial infarction. However, few reports have described the use of cTnI assays for evaluating abnormal hemodynamic load in children with congenital heart disease (CHD). It was hypothesized that significant hemodynamic overload due to a left-to-right shunt induces myocardial injury. Methods and Results: A highly sensitive cTnI assay was used to measure the serum cTnI levels in 30 children with atrial septal defect (ASD), 32 children with ventricular septal defect (VSD), and 350 healthy children. Cardiac catheterization was performed in the children with ASD and VSD to determine the ratio of pulmonary to systemic blood flow, the ratio of pulmonary to systemic arterial pressure (Pp/Ps), the pulmonary vascular resistance index, and the right and left ventricular end-diastolic volume. Serum cTnI levels in both the ASD and VSD children were significantly higher than those in healthy children (P<0.05 and P<0.01, respectively). Furthermore, serum cTnI levels significantly correlated with Pp/Ps (r=0.745, P<0.001) in VSD children. Conclusions: Significant volume and pressure overload due to a left-to-right shunt induce myocardial injury and might eventually cause irreversible myocardial remodeling in children with CHD. The serum cTnI level is a useful biomarker for evaluating myocardial damage associated with pulmonary hypertension in VSD children. (Circ J 2011; 75: 2213-2219)

The Non-Hydrostatic Regional Climate Model (NHRCM) was used in simulating the present and future rainfall climate over Malaysia under the RCP8.5 scenario in this study. Simulation and projection from 1979 to 2002 for present day and 2070 to 2100 for the end of century were conducted over the Malaysia. The 20 km resolution MRI-AGCM3.2 model simulation from Meteorological Research Institute (MRI) was used as boundary conditions. The objective of this study was to estimate the extreme rainfall projections in Malaysia at 5 km of resolution during the November to February period, representing the northeast monsoon season. Overall, the model was capable to simulate the historical rainfall climatology and distribution, but model tended to underestimate high rainfall frequency and mean rainfall intensity in Malaysia. However, compared with simulations at 25 km, added values have been shown at 5 km resolution. Based on the NHRCM05 simulations, a number of hotspots have been identified with significant projected increases up to 80% for the extreme rainfall indices (R20mm, RX1day, R95pTOT and R99pTOT), 30% increases in mean rainfall intensity (SDII) and 20% for consecutive dry days indices (CDD).

Background:Atrial fibrillation (AF) is an important prognostic determinant in heart failure (HF) with preserved ejection fraction (HFpEF). However, it is unclear which HFpEF phenotypes are affected by AF in terms of long-term clinical outcomes because HFpEF is a heterogeneous syndrome with comorbidities such as coronary artery disease (CAD). In this study we determined the differential prognostic significance of AF in HFpEF patients according to CAD status.Methods and Results:Data for 408 hospitalized HFpEF patients enrolled in the Japanese Heart Failure Syndrome with Preserved Ejection Fraction Nationwide Multicenter Registry were analyzed. Patients were divided into 4 groups according to the presence of AF and CAD. The primary outcome was the composite of all-cause death and HF rehospitalization. The incidence of adverse events was higher in the AF–non-CAD than non-AF–non-CAD group (P=0.004). On multivariable Cox regression analysis with prespecified confounders, AF–non-CAD was significantly associated with an increased risk of adverse events than non-AF–non-CAD (adjusted HR, 1.91; 95% CI: 1.02–3.92) regardless of the type of AF. In contrast, risk was comparable between the AF–CAD and non-AF–CAD groups (adjusted HR, 1.24; 95% CI: 0.64–2.47).Conclusions:In HFpEF patients without CAD, AF was independently related to adverse events, indicating that intensive management of AF would have more beneficial effects particularly in HFpEF patients without CAD.

High-frequency variations are excluded in the merged satellite and in-situ data global daily sea surface temperature (MGDSST) used in weather forecasting in Japan Meteorological Agency. We investigated the importance of temporal resolution on sea surface temperature (SST) when predicting winter precipitation using the Non-Hydrostatic Regional Climate Model. We used seven-day temporal smoothing to investigate the influence of temporal resolution on prediction. The Gaussian filter was used as spatial smoothing for comparison with the influence of spatial resolution. The influence of the temporal resolution of SST on monthly precipitation is smaller than that of spatial resolution. However, the influence of the temporal resolution on daily precipitation is comparable to that of spatial resolution. The temporal resolution of SST greatly affects precipitation, particularly in December, as the variations in SST are largest compared to the rest of the year. Furthermore, the winter monsoon promotes the effect of SST on winter precipitation. Our experiments using seven-day moving average smoothing indicates that the temporal resolution of the SST on precipitation become about 15 %/K under the winter monsoon.

A daily rest period (DRP; rest taken from daily work during a 24 h period), is essential to work recovery. This study examined DRPs' distribution and association with health outcomes among information technology workers recruited from an internet panel (N=1,811). Participants completed a web questionnaire examining psychological distress as a primary outcome, along with non-restorative sleep, fatigue (stress reaction), and work performance. Logistic regression analysis showed elevated psychological distress when DRP was <12 h (OR: 2.54; 95% CI: 1.47–4.42) and <11 h (OR: 2.48, 95% CI: 1.17–5.26), although the 95% CI included 1 after adjusting for age, sex, and working and commuting hours. After the above adjustment, similar associations were found with non-restorative sleep and fatigue, but not work performance, when DRP was <12 h. These findings constitute the first analysis of a dose-response relationship between DRP and subjective health outcomes among white-collar workers.

Oncolytic virotherapy is a new strategy for cancer treatment for humans and dogs. Reovirus has been proven to be a potent oncolytic virus in human medicine. Our laboratory has previously reported that canine mast cell tumor and canine lymphoma were susceptible to reovirus. In this study, canine solid tumor cell lines (mammary gland tumor, osteosarcoma and malignant melanoma) were tested to determine their susceptibility towards reovirus. We demonstrated that reovirus induces more than 50% cell death in three canine mammary gland tumors and one canine malignant melanoma cell line. The reovirus-induced cell death occurred via the activation of caspase 3. Ras activation has been shown to be one of the important mechanisms of reovirus-susceptibility in human cancers. However, Ras activation was not related to the reovirus-susceptibility in canine solid tumor cell lines, which was similar to reports in canine mast cell tumor and canine lymphoma. The results of this study highly suggest that canine mammary gland tumor and canine malignant melanoma are also potential candidates for reovirus therapy in veterinary oncology.

Objective: Transradial approach (TRA) is increasingly used as a viable alternative to the traditional transfemoral approach (TFA) in neuroendovascular therapy (NET) owing to its potential anatomical benefits and lower puncture-site complication rates. However, the real-world challenges of implementing TRA-NET have not been thoroughly studied, particularly those related to guide catheter (GC) placement. In this study, we aimed to explore the feasibility and challenges of TRA-NET, with a specific focus on GC placement.Methods: This retrospective observational study included patients who underwent NET at our institution between December 2019 and May 2022. Procedural success was defined as the successful placement of a GC in the target vessel. Cases in which a Simmons-shaped GC was used or the approach was changed to TFA were classified as difficult. Safety was assessed based on the rate of severe puncture-site complications requiring either blood transfusion or surgical intervention.Results: Among the 310 patients who underwent NET during the study period, 222 (71.6%) with a median age of 74 years were selected for TRA-NET. The target vessel was in the left anterior circulation (LtAC) in 101 (45.5%) patients, and 8-F GCs were the most frequently used (40.1%). TRA-NET achieved a 95.0% success rate, with a switch to TFA required in 5.0% of the cases. Procedural challenges occurred in 42 (18.9%) patients, primarily in those with LtAC lesions. Specifically, a type III aortic arch (p <0.0001) and age ≥80 years (p = 0.01) were significantly associated with procedural difficulties. Radial artery evaluation was confirmed in 66 cases (29.7%), revealing one instance (1.5%) of radial artery occlusion. No severe puncture-site complications were observed.Conclusion: TRA-NET may provide substantial therapeutic benefits without significant limitations in device use. However, it may be challenging, particularly in older patients and those with a type III aortic arch with LtAC lesions. Consequently, careful selection of the approach route is imperative.

A 19-year-old Japanese man was hospitalized for cardiogenic shock 28 days after receiving a second dose of the coronavirus disease 2019 (COVID-19) mRNA-1273 vaccine. He had had a high fever for three days with vomiting and abdominal pain before arriving at our hospital. The patient visited a local hospital and was diagnosed with heart failure and acute appendicitis. An endomyocardial biopsy specimen showed myocarditis. Thereafter, Impella CP left ventricular assist device implantation and venoarterial peripheral extracorporeal membranous oxygenation were initiated immediately along with inotropic support and steroid pulse therapy. Given these findings, he was finally diagnosed with multiple inflammatory syndrome and fulminant myocarditis.

A 30-year-old man admitted with renal dysfunction (serum creatinine, 8.19 mg/dL) was diagnosed with immunoglobulin A nephritis through a renal biopsy. He was treated with intravenous methylprednisolone pulse therapy and urgent hemodialysis, and eventually, he underwent maintenance hemodialysis. On day 108, he developed amnesia. Magnetic resonance imaging revealed bilateral basal ganglia lesions. Wernicke encephalopathy (WE) was diagnosed based on decreased serum thiamine concentration (12.8 μg/dL; reference range, 24-66 μg/dL). Thiamine replacement therapy was initiated, but the Wernicke-Korsakoff syndrome persisted. Careful monitoring of thiamine is required in patients undergoing dialysis. In addition, patients with WE may exhibit bilateral basal ganglia lesions.

Socio-economic residential segregation is an important issue of social concern and academic interest. Using population census data, we analyze the changes in residential segregation by finer occupational groups at the neighborhood level and their local spatial distribution in Tokyo from 1980 to 2005. This period was characterized by increasing economic disparities in Japan. We find that: 1) Multiple segregation indices provide evidence of some level of residential segregation by occupational groups at the neighborhood level in Tokyo. The level of residential segregation is higher for both ends of the occupational hierarchy than it is for other occupational groups. 2) While the overall level of residential segregation has continually declined, this does not necessarily translate into desegregation between opposite social groups. Furthermore, there are different patterns of changes in residential segregation, even between white- and gray-collar workers. Therefore, using finer or larger occupational groups leads to different insights on the changes in socio-spatial segregation. For the highest occupational group (managerial workers), the level of residential segregation from the lowest group was growing. However, segregation also increased from other occupational groups, except for a short period immediately following the collapse of the bubble economy in the early 1990s. Managerial workers were even more spatially concentrated in central areas of Tokyo, which were already highly concentrated.

The surface temperature was projected to increase from 4 to 5°C in most regions around Japan in winter at the end of the 21st century, according to the Non-Hydrostatic Regional Climate Model (NHRCM) under the Representative Concentration Pathway 8.5 scenario. The melting of sea ice in the Sea of Okhotsk significantly affected the temperature around Hokkaido Prefecture, raising it by more than 8°C in some places. The temperature also rose by more than 8°C in some areas in Honshu where the atmosphere was not susceptible to sea ice. The reduction in snow-covered areas due to global warming raised the temperature further and induced changes in local wind, such as airflows over mountains and wind blowing from the sea. These changes raised the seasonal average temperature and caused the temperature to rise by over 8°C. The dynamical downscaling method played a significant role in projecting such small-scale features in the future climate.

Background: The effect of delayed ambulation on the outcome of coronary artery bypass grafting (CABG) remains to be clarified.Methods and Results: The long-term and in-hospital outcomes of 887 patients who underwent isolated CABG (455 off-pump cases, 135 urgent cases) were evaluated, with a focus on the timing of first ambulation. In-hospital mortality cases were excluded. Early ambulation (first ambulation within 3 days after operation) was achieved in 339 (38%) patients. In the multivariable logistic regression analysis, longer operation time and urgent case, EuroSCORE II, re-thoracotomy, and respiratory time were associated with delayed (≥4 days) ambulation. Delayed ambulation was associated with a high incidence of postoperative complications, such as pneumonia, and stroke (P<0.01). Following discharge, 22.2% of patients experienced major cardiac events and 13.8% died during the follow-up period (median follow-up 60 months). Cox hazards analysis revealed that delayed ambulation was associated with long-term adverse events (hazard ratio 1.04 per day, P<0.001). With adjustment for preoperative factors, the estimated future risk of adverse events was found to be increased day-by-day during the delay until initial ambulation.Conclusions: In isolated CABG patients, delayed ambulation was associated with poor outcomes, even in the long-term period. The results support the current guideline recommending early ambulation protocol after cardiac surgery.

Overwork-related disorders, such as cerebrovascular/cardiovascular diseases (CCVD) and mental disorders due to overwork, are a major occupational and public health issue worldwide, particularly in East Asian countries. This report discusses the recent trend of overwork-related disorders in Japan from the perspective of workers' compensated occupational diseases, as well as the development of a national policy for preventive measures against overwork-related disorders in Japan. Recently, the number of claimed and compensated cases of occupational mental disorders has increased substantially, particularly among young workers, as compared to those of occupational CCVD. In response to these situations and action from society, the Japanese Government passed the "Act on Promotion of Preventive Measures against Karoshi and Other Overwork-Related Health Disorders" in June 2014 to develop a national initiative towards the prevention of overwork-related disorders. Changes in the trend of overwork-related disorders in Japan under a legal foundation and an initiative by the central government should be closely monitored so that other countries can benefit from the experiences.

Acute eosinophilic pneumonia (AEP) is an acute respiratory illness with diffuse pulmonary infiltrates and pulmonary eosinophilia. While the etiology of AEP remains unclear, a relationship between cigarette smoking and AEP onset has been suggested. The use of heated tobacco products (HTPs) has been growing, but the impact of these products on our health is not fully understood. We herein report a case of AEP that developed after switching from conventional cigarette to HTP smoking. The patient's condition improved after the cessation of HTP smoking and corticosteroid treatment initiation. In cases of AEP, physicians should consider HTPs use as a possible cause.
