"Gravel bar and Inner-bank pond called Oppori in Japanese are important flood landforms for research of alluvial plain and prevention of disasters of the Kizu and Uji Rivers, because they are indicators of the point of bank breaking and historical flooding. The author studies their location, present land use, origin and relationship between landforms and flooding. Inner-bank ponds had been about eighty during the past one hundred years, but are only three in April 2007, because of decrease by reclamation. Gravel bar were formed by debris flow or sandy mud flow, and are classified into A, B and C types in descending orders from their freshness and land use. Relationship between gravel bar, inner-bank pond and historical flooding in Seikacho, Kyotanabe City and Uji City were made clear since the eighteenth century. The author reports a rare case of left recurrent nerve paralysis in an infant aged 1 year and 10 months and male. He refers to cardiac diseases as its chief causes. The main cause is the compression of the nerve by the aorta and left pulmonary artery. Concerning other etiologic cardiac diseases he further states that in the stenosis of the mitral valves or their insufficiency, the dilatation of the 2nd cardiac arch is remarkable in X-ray findings.

【目的】地域交流サロンに参加する高齢者の生きがい意識の実態および生きがい意識に影響を及ぼす要因を明らかにすることを目的とする.【方法】A地区の地域交流サロンに参加する高齢者40名に対し,平成30年 8 ~ 9 月に,年齢,性別,同居者・配偶者の有無,相談できる友人の有無,地域交流サロンへの参加状況,家族・他者との交流頻度,主観的健康度を含み,そして生きがい意識を表するIkigai-9を用いた質問紙調査を行った.Ikigai-9は生きがいを感じている精神状態(生きがい意識)を測定する 9 項目による質問紙であり,合計得点が高いほど生きがい意識が高い,すなわち生きがいを実感しているとされている.収集したデータから,各項目における生きがい意識得点の平均の比較を行った.【結果】 対象者の平均年齢は83.6±6.9歳で,生きがい意識得点の平均は28.6±7.1点であった.各項目における比較では,「相談できる友人」という項目で,友人がいる群がいない群より平均得点が高かった(P=0.04).その他の項目では有意差はみられなかった.【結論】本研究において,A地区の地域交流サロンに参加する高齢者の生きがい意識の実態,および生きがい意識は相談できる友人の有無と関連があることが確認された.Objective: The purpose of this study is to clarify the actual state of feeling that life is worth living among older people who participate in community-based interactions and to detail the factors related to this feeling.Methods: We conducted a questionnaire survey among older people( n=40) participating in communitybased interactions in one area of Japan. The questionnaire contained questions about age, availability of a partner and friends for consultation, subjective health, and Ikigai-9 scale. Ikigai-9 is a 9-item questionnaire that measures the mental state of feeling that life is worth living, where a higher total score indicates a higher feeling of life worth. We compared the average points of life worth in each item.Results: The average age of the subjects was 83.6 ± 6.9 years, and the average score of life worth was 28.6 ± 7.1. Comparison of each item revealed that the average score was higher for the group who had "friends available for consultation" than for the group without friends( P=0.04). No significant differences were detected for the other items.Conclusion: In this study, we were able to c clarify the actual state of feeling that life is worth living among older people who participated in the community-based interactions and to explore the relationship between their feeling that life was worth living and friends for consultation.

[目的]本研究の目的は,多裂筋のトレーニングによって腰部の脊柱安定化が得られ坐位姿勢のバランスが改善されるかを検討することである。[対象]対象は健常者21名であった。[方法]週3回,4週間の多裂筋の筋機能トレーニングを行わせ,トレーニング前後でトレーニング時の腰部の前弯,後弯コントロールを示す圧変動幅,安静坐位・最大側方リーチ時の圧中心軌跡,坐位保持課題遂行時の3つの体幹筋の最大等尺性収縮時の筋放電に対するパーセンテージ(以下%MVC)を測定した。[結果]圧変動幅は,トレーニング前と比較してトレーニング後に有意に減少した。安静坐位・最大側方リーチ時の圧中心軌跡,%MVCはトレーニング前後で有意差はみられなかった。[結論]脊柱安定性に寄与する多裂筋の機能が向上し椎体間の固定性が高まることが,必ずしも坐位バランスに影響を与えるものではないことが明らかとなった。

56歳女性. 右耳痛・右顔面痛を主訴に当院耳鼻咽喉科を受診, その後右耳介及びその周囲に水疱形成がみられ, 急性呼吸不全を呈して当科に紹介された. 血中抗帯状疱疹ウイルス抗体価は1,024倍と上昇, 神経学的に多発性脳神経麻痺を呈し, 胸部X線写真上右下肺野に浸潤影が見られた. 多発性脳神経麻痺を合併した Ramsey Hunt 症候群と診断, 急性呼吸不全の原因は反回神経麻痺による中枢部気道閉塞と嚥下性肺炎によるものと考えられた. 抗生剤と抗ウイルス剤 (アシクロビル) の併用により著明な改善がみられた. 文献的検索では, 下部脳神経麻痺を合併した Ramsey Hunt 症候群の報告は比較的少なく, さらに, 呼吸器合併症を呈した症例は稀であった.
