【背景】Stevens-Johnson syndrome(SJS)およびtoxic epidermal necrolysis(TEN)の治療としてはステロイド薬の全身投与のほか,免疫グロブリン大量療法や血漿交換療法が試みられている.【目的】SJSおよびTENの治療の現状とその効果を評価する.【方法】2000年から2007年の8年間に横浜市立大学附属2病院皮膚科で経験したSJS 27例とTEN 19例についてその臨床的特徴および治療法を検討した.【結果】TENの1例を除き全例でステロイド薬の全身投与が行われていた.ステロイドパルス療法が選択された症例はSJSの8例,TENの9例であり,免疫グロブリン大量療法や血漿交換療法が併用された症例はSJSで3例,TENで8例あった.死亡率はSJSが3.7%(1例),TENが21.1%(4例)であった.TENの死亡例2例は敗血症を合併し死亡した.【結語】感染症管理の難しさが浮き彫りとなったが,適切なステロイド薬の投与はSJSおよびTENに有効な治療法であると考えられた.

要旨 脳梗塞に対するtPA を用いた血栓溶解療法は,発症から4.5 時間を超えて行った場合,脳出血を合併するリスクが高くなる.脳出血合併症を防止する治療の開発は,予後の改善と,tPA 療法の治療可能時間域の延長をもたらす可能性がある.我々は,血管リモデリングに関与する血管内皮増殖因子(VEGF)やアンギオポイエチン1(Ang1)を標的とした血管保護療法の可能性について検討し,ラット脳塞栓モデルにおいて,① VEGF 抑制薬,および②組み換えAng1 の投与が脳出血合併症を防止し,予後を改善することを明らかにした.その後,知的財産権の確保,および米国ベンチャー企業との産学連携を行い,現在,臨床試験の実現を目指している.これまでの経験から,アカデミア研究者が,創薬研究の「死の谷」を乗り越えるためには,①動物実験の質の改善,②知的財産権の確保,③産学連携の推進が重要であると考えられた.

We analyze user behavior on SoundCloud.com, a web-based music distribution site, using the methodology of computer-mediated discourse analysis. The broad goal is to infer patterns of use that can inform the development of methods to facilitate communication among online users. In order to achieve this goal, we analyze SoundCloud comments and identify site design features that facilitate users' actions, using speech act analysis and dynamic topic analysis. We address two research questions: What are the most common communicative acts on the music distribution site, and which commenting function of the site contributes most to facilitating interactive commenting? Results of speech act analysis show that 56% of comments are REACT which means site users mostly react to the song in short, simple comments. Additionally the users tend to be more interactive when using the timed comment function, which appear below the song waveform, provided by the site. Dynamic topic analysis reveals that the structure of regular comments, made on the song as a whole, shows prompt focused such as blog comment, and structure of timed comments shows sequential threads are more interactive as is the case in chatting. These findings indicate that comments which focus on particular part of the song have possibility to facilitate user's interactive comments.

This paper describes a GNSS (Global Navigation Satellite System) precise point positioning (PPP) technique that can be applied to mobile robots in urban environments. The multipath signals, which from invisible satellites in urban area, cause serious effect on the GNSS positioning. Our proposed technique mitigates GNSS multipath signals by means of an omnidirectional infrared (IR) camera that can eliminate the need for invisible satellites by using IR images. With an IR camera, the sky appears distinctively dark. This facilitates the detection of the borderline between the sky and the surrounding buildings, which are captured in white, because of the difference in the atmospheric transmittance rates between visible light and IR rays. Positioning evaluation was carried out only with visible satellites that have less multipath errors. The evaluation results confirm the effectiveness of the proposed technique and the feasibility of its highly accurate positioning.

高齢者サロンにおいて,寝たきりを予防するための取り組みとして平成21年1月から標語を記したかるた(寝たきり予防健康いろはかるた)を会員間で意見を出し合い完成させた。このかるた作りが高齢者の行動変容に及ぼす効果について検討することを目的にサロン会員17名を対象とした質問紙調査を行った。回答を得た会員は13人であり回収率76.5%であった。作成したかるたの標語の内容は,「骨折予防」に関連したものが17句でもっとも多く,ついで「栄養・運動・休養」に関連するもの11句,「気持ちの持ちよう」に関するもの10句であった。かるたづくりに参加しての意見としては,「人の役に立ちたい」,「かるたはわかりやすい」,「注意喚起した」,「年齢を自覚」,「知恵を出し合った」,「楽しかった」,「感心した」があった。かるた作りに関連した保健行動の変容として,新たに始めた行動,再び始めた行動止めた行動は,かるたの標語を参考にしたものが多かった。これらより,寝たきり予防の取り組みとして「かるた作り」を高齢者サロンの活動として行ったことは評価できると考える。今後,「寝たきり予防健康いろはかるた」の活用方法に関する検討や高齢者の保健行動に関する継続した調査が必要と考える。

The purpose of this study was to examine whether the salon program for elderly can improve individual level social capital indicators corresponding to measures of social networks, religious group involvement, social support, social trust and social norm. The salon program started in 2007. We had before-and-after survey in 2006 and 2008. We analyzed 1606 samples who participated in these surveys. Some of individual-level social capital indicators were improved in Salon program participants compared with non-participants. These results suggest that our intervention program can enhance individual-level social capital.

[Purpose] To assess the evidence for the effectiveness of yoga exercises in the management of headaches. [Subjects and Methods] A search was conducted of six electronic databases to identify randomized controlled trials (RCTs) reporting the effects of yogic intervention on headaches published in any language before January 2015. Quality assessment was conducted using the Cochrane risk of bias tool. [Results] One potential trial was identified and included in this review. The quality critical appraisal indicated a moderate risk of bias. The available data could only be included as a narrative description. Headache intensity and frequency, anxiety and depression scores, and symptomatic medication use were significantly lower in the yoga group compared to the control group. [Conclusion] There is evidence from one RCT that yoga exercises may be beneficial for headaches. However, the findings should be interpreted with caution due to the small number of RCTs. Therefore, further rigorous methodological and high quality RCTs are required to investigate the hypothesis that yoga exercises alleviate headaches, and to confirm and further comprehend the effects of standardized yoga programs on headaches.

The aerial 3D laser scanner is needed for scanning the areas that cannot be observed from the ground. Since the laser scanning takes time, the obtained range data is distorted due to the sensor motion while scanning. This paper presents a rectification method for the distorted range data by aligning each scan line to the 3D data obtained from the ground. To avoid the instability and ambiguity of the line-based alignment, the parameters to be optimized are selected alternately, and the smoothness constraint is introduced by assuming that the sensor motion is smooth. The experimental results show that the proposed method has the good accuracy simulation and actual data.

We investigated method to osmose water and seasoner to dried soybeans fast by pulsed electric field, in order to make soybeans a processed food fast. By applying the pulsed electric field to the dried soybeans in water, osmosis time of water to the soybean became approximately half. Then the emission of the discharge was observed on dried soybean. The color of coffee permeated more into the soybean treated than no-treated by the pulsed electric field.

言語能力の起源と進化を考察する際、現在の機能とメカニズムとともにその歴史性の解明も重視されなくてはならない。他の生物現象同様、ヒトの脳とそれが可能にする言語能力も妥協の産物である。言語が現れるにいたった過程と、言語獲得によって失ったものや新たに対処しなければならなくなった課題の解明は、言語のより深い理解につがなるはずである。本稿では、言語獲得と関連が高いと考えられる現代人的な脳の使い方と生き方が、人類進化の歴史の中で、いつごろ、どのように現れてきたのかについて生物人類学的知見をもとに概観する。
