Ehlers-Danlos syndrome(以下EDS)は皮膚過伸展,関節過可動性,皮膚と血管の脆弱性を3主徴とする遺伝性結合織疾患である.今回,癒着性イレウスにより多発小腸憩室を形成したEDSの1例を経験したので報告する.症例は50代,男性.開腹術後の癒着性イレウスで入院中,状態悪化し当院紹介.腹部CTで遊離ガスを認め消化管穿孔の診断で緊急手術を施行した.前回の手術創直下に癒着による小腸狭窄あり.狭窄部口側小腸は拡張し多発憩室を認め,そのうち一つは腸間膜に穿通し炎症性に硬化していた.EDSによる組織脆弱性から小腸切除および吻合は不適と判断し,癒着剥離によるイレウス解除術のみ行った.本症例は組織の膨張に対する歯止めの役割をもつIII型コラーゲンの減少が特徴であるEDS IV型と考えられた.そのため,組織脆弱性から閉塞部より口側腸管の内圧上昇によって組織が膨張し,多数憩室を形成したと推測された.

Kernel method is one of the promising approaches to learning with tree-structured data, and various efficient tree kernels have been proposed to capture informative structures in trees. In this paper, we propose a new tree kernel function based on ``subpath sets'' to capture vertical structures in tree-structured data, since tree-structures are often used to code hierarchical information in data. We also propose a simple and efficient algorithm for computing the kernel by extending the Multikey quicksort algorithm used for sorting strings. The time complexity of the algorithm is O((|T_1|+|T_2|)log(|T_1|+|T_2|)) time on average, and the space complexity is O({|T_1|+|T_2|)}, where |T_1| and |T_2| are the numbers of nodes in two trees T_1 and T_2. We apply the proposed kernel to two supervised classification tasks, XML classification in web mining and glycan classification in bioinformatics. The experimental results show that the predictive performance of the proposed kernel is competitive with that of the existing efficient tree kernel proposed by Vishwanathan et al., and is also empirically faster than the existing kernel.

A surge-wave coupling model was developed for numerical analysis of the disaster phenomena in coastal areas. The basic equations of storm surge model are the continuity and momentum equations integrated. The shear stresses the pressure gradients and the radiation stresses are taken into consideration in the momentum equations. The wave fields of a coupling model are calculated by MRI shallow water wave hindcasting model. The developed surge wave coupling model is applied to the estimation of the wave and water level generated by T7010 in the inundation area in the Kochi city.

Neglected tropical diseases (NTDs) are an extremely important issue facing global health care. To improve “access to health” where people are unable to access adequate medical care due to poverty and weak healthcare systems, we have established two consortiums: the NTD drug discovery research consortium, and the pediatric praziquantel consortium. The NTD drug discovery research consortium, which involves six institutions from industry, government, and academia, as well as an international non-profit organization, is committed to developing anti-protozoan active compounds for three NTDs (Leishmaniasis, Chagas disease, and African sleeping sickness). Each participating institute will contribute their efforts to accomplish the following: selection of drug targets based on information technology, and drug discovery by three different approaches (in silico drug discovery, “fragment evolution” which is a unique drug designing method of Astellas Pharma, and phenotypic screening with Astellas' compound library). The consortium has established a brand new database (Integrated Neglected Tropical Disease Database; iNTRODB), and has selected target proteins for the in silico and fragment evolution drug discovery approaches. Thus far, we have identified a number of promising compounds that inhibit the target protein, and we are currently trying to improve the anti-protozoan activity of these compounds. The pediatric praziquantel consortium was founded in July 2012 to develop and register a new praziquantel pediatric formulation for the treatment of schistosomiasis. Astellas Pharma has been a core member in this consortium since its establishment, and has provided expertise and technology in the area of pediatric formulation development and clinical development.

A 12-year-old female American shorthair cat presented with a one-month history of hematuria and general lethargy. Abdominal ultrasonography revealed complete thickening of the left uterine wall. At a diagnostic laparotomy, a large mass arising from the left uterine horn was discovered, and ovariohysterectomy was performed. Histological diagnosis revealed a T-cell high-grade lymphoma of the uterus. After the ovariohysterectomy, the patient achieved complete remission and maintained by combination chemotherapy from 14 days after surgery. However, relapse occurred in the urinary bladder wall on day 287, and the patient died of postrenal acute renal failure on day 310. This is the first report of a feline case of primary uterine lymphoma that was treated with ovariohysterectomy followed by systemic chemotherapy.

The black flower color of dahlias (Dahlia variabilis) has been suggested to be attributed to a high accumulation of cyanidin (Cy)-based anthocyanins. A possible explanation for this effect is that Cy-based anthocyanins in dahlias contribute more to the black flower color than pelargonidin (Pg)-based anthocyanins by lowering petal lightness (L*) and chroma (C*), but no obvious evidence has been reported. In this study, four major anthocyanins accumulated in dahlia petals, 3,5-diglucoside (3,5diG) and 3-(6''-malonylglucoside)-5-glucoside (3MG5G) of Pg and Cy, were purified and their colors were evaluated in vitro at various pHs (3.0, 4.0, 4.5, 5.0, 5.5, 6.0, or 7.0) and various concentrations (0.25, 0.5, 1.0, 2.0, or 3.0 mg·mL−1 at pH 5.0 or pH 3.0). The color of solution of purified anthocyanins varied depending on pH. At pH 5.0, which is approximately the same as pH of dahlia petals, and at pH 3.0, at which anthocyanins are relatively stable, the L* and C* of Cy 3,5diG were similar to or higher than those of Pg 3,5diG, suggesting that Cy 3,5diG did not contribute more to the black flower coloring than Pg 3,5diG. On the other hand, the L* and C* of Cy 3MG5G were significantly lower than those of Pg 3MG5G, particularly above 2.0 mg·mL−1, suggesting that Cy 3MG5G contributed more than Pg 3MG5G. A similar tendency was observed in the color measurement of mixed anthocyanins in various proportion of Pg and Cy. The L* and C* of Pg 3MG5G were much higher than those of the other three anthocyanins; therefore, its color was considered to be the farthest from black among the four anthocyanins. The accumulated amount of 3MG5G-type anthocyanins was much higher than that of 3,5diG-type anthocyanins in all nine cultivars, although the proportion of Pg- and Cy-based anthocyanins varied among the cultivars. Considering these results, it was suggested that because 3MG5G-type anthocyanins predominantly accumulate in petals, and Cy 3MG5G has a significantly higher contribution to lowering L* and C* than Pg 3MG5G, the high accumulation of Cy-based anthocyanins is critical for the black flower coloring of dahlias. The contribution of each anthocyanin is considered to depend on the structure; therefore, identifying the anthocyanin with the highest contribution to lowering L* and C* may enable the production of black flowers in various species through the high accumulation of the anthocyanin in petals.

Knowledge of stature is necessary for evaluating nutritional status and for correcting certain functional parameters. Measuring stature is difficult or impossible in bedridden or wheelchair-bound persons and may also be diminished by disorders of the spinal column or extremities. The purpose of this work is to develop estimation equations for young adult athletes for their subsequent application to disabled persons. The main sample comprised 445 male and 401 female sportspersons. Cross validation was also performed on 100 males and 101 females. All were Caucasian, the males being over 21 and the females over 18, and all practiced some kind of sport. The following variables were included: stature, sitting height, arm span, and lengths of upper arm, forearm, hand, thigh, lower leg, and foot. Simple and multiple regression analyses were performed using stature as a dependent variable and the others as predictive variables. The best equation for males (R2=0.978; RMSE=1.41 cm; PE=1.54 cm) was stature: 1.346+1.023 * lower leg+0.957 * sitting height+0.530 * thigh+0.493 * upper arm+0.228 * forearm. For females (R2=0.959; RMSE=1.57 cm; PE=1.25 cm) it was stature: 1.772+0.159 * arm span+0.957 * sitting height+0.424 * thigh+0.966 * lower leg. Alternative equations were developed for when a particular variable cannot be included for reasons of mobility, technical difficulty, or segment loss.

α1-酸性糖蛋白 (α1-AG)は急性相反応蛋白の一種であり,猫において炎症や腫瘍を検出するスクリーニング項目として注目を集めつつある。今回新たに開発した測定試薬を用い免疫比濁(TIA)法による猫血中α1-AGを測定した結果,同時再現性と希釈直線性はともに良好で,その測定値は一元放射免疫拡散法による測定値と高い相関を示した。この試薬を用いたTIA法で日本獣医生命科学大学付属動物医療センターに来院した238頭の猫の血中α1-AG濃度を測定したところ,α1-AGが上昇していた125頭のうち血清アミロイドA(SAA)の上昇を伴っていたものは62頭で,乖離が見られることからα1-AGとSAAの同時評価が病期判定や鑑別診断に結びつく可能性が示唆された。また,TIA法による猫α1-AGの測定は大規模な検体処理に適し,検査項目としてのα1-AG測定の普及に寄与することが期待される。

情報通信技術(ICT)の急速な進歩を背景に、コンテンツ産業は、日本の持続的成長・発展を支える基盤産業としての貢献が期待されている。そのため、同産業に対して何らかの成長阻害要因が予見される場合は、相応の政策的対処を検討し、望ましい産業組織を育成する必要がある。近年のインターネットプロトコル(IP)技術の高度化は、ネットワーク産業とコンテンツ産業を垂直統合した新しいプレイヤーを生みつつあり、これら新型プレイヤーは、独占力のレバレッジを通じて競争政策上の新たな課題をもたらす懸念がある。具体的には、モバイルブロードバンド(BB)が主流になるにつれ、プロバイダ市場の寡占化が進み、コンテンツ・ディストリビューションの効率的確保が阻害される可能性がある。そのため、モバイル BB 事業者に対し一定の規律を与えることは、BB 市場の効率性改善の観点のみならず、コンテンツ産業育成の観点からも重要である。米国のネット中立性規制は、その際、良い参照例となろう。

Intercomparison of in situ gamma spectrometry was organized at a site contaminated by the radioactive fallout that originated from the Fukushima Dai-ichi Nuclear Power Plant accident. This intercomparison was conducted by eight teams from four different institutions, which have contributed to the government-led project to construct distribution maps of radionuclides deposited on the ground soil. The resultant 134Cs and 137Cs inventories evaluated by the participants agreed within 6% of the coefficient of variation, after correction for inhomogeneous distribution of the dose rate in air. The evaluated 40K inventories agreed within 4% of the coefficient of variation. The authors estimated that these results were in good agreement for creating distribution maps of the radionuclide inventory in the ground soil.

[Purpose] This study aimed to examine the effects of self-mobilization techniques for the sciatic nerves on the quality of life in patients with chronic low back pain in the lower limbs accompanied by radiating pain. [Subjects and Methods] The subjects were divided into two groups: a group receiving of lumbar segmental stabilization exercise training including sciatic nerve mobilization techniques, which included 8 males and 7 females, and a group receiving lumbar segmental stabilization exercise training, which included 8 males and 7 females. [Results] There were statistically significant differences in comparison of measurement results between the groups before and after the intervention. [Conclusion] Application of mobilization techniques for the sciatic nerves may promote healing of the soft tissues by stimulating the functions of the nervous system to improve nervous system adaptability and decrease sensitivity, helping to alleviate the symptoms.

外科の臨床医として,外科医のアドヴァンテージを最大限に生かした臨床に直結する取り組みを行ってきた.様々な分野におけるパラダイムシフトとわれわれの歩みをふり返り,若い外科医へのメッセージとなりえるか,「外科の美学」という観点から考証した.1970年代の生化学的な手法に始まり,遺伝子工学の勃興時代,ポストゲノム,など相次ぐ研究のパラダイムシフトの中で,これらに即応し,2007年から,臨床検体と情報を基にした,「遺伝子発現解析を活用した個別癌医療の実現と抗癌剤開発の加速」を開始した.2012年には,福島の震災復興事業の一つとして,「福島医薬品関連産業支援拠点化事業」と継続的に発展している.福島県は,医療立県構想のもと,「臨床材料と臨床情報」を産業界で活用できる技術と体制の確立を目指してきた.医産連携「福島モデル」は,薬剤支援領域と医療機器支援領域の二本立てになっている.

[Purpose] Gait-training robots have been developed for stroke patients with gait disturbance. It is important to survey the views of physiatrists and physical therapists on the characteristics of these devices during their development. [Subjects and Methods] A total of 100 physiatrists and 100 physical therapists from 38 hospitals participated in our questionnaire survey. [Results] The most common answers about the merits of gait-training robots concern improving the treatment effects (28.5%), followed by standardizing treatment (19%), motivating patients about treatment (17%), and improving patients’ self-esteem (14%). The subacute period (1–3 months post-stroke onset) was most often chosen as the ideal period (47.3%) for the use of these devices, and a functional ambulation classification of 0–2 was the most selected response for the optimal patient status (27%). The preferred model was the treadmill type (47.5%) over the overground walking type (40%). The most favored commercial price was $50,000–$100,000 (38.3%). The most selected optimal duration for robot-assisted gait therapy was 30–45 min (47%), followed by 15–30 min (29%), 45–60 min (18%), ≥ 60 min (5%), and < 15 min (1%). [Conclusion] Our study findings could guide the future designs of more effective gait-training robots for stroke patients.

As a result of The Great East Japan Earthquake, approximately 28 million tons of disaster wastes (such as disaster debris and tsunami deposits) were generated in the three prefectures of Iwate, Miyagi, and Fukushima, mainly due to the tsunami. The waste treatment on this massive scale would normally take more than a decade. For the local recovery and reconstruction, disaster waste treatment was the first step, and its early treatment was essential. Two months after the disaster, the government prepared a master plan for completing the treatment by the end of March 2014, approximately three years after the earthquake, focusing on recycling of the waste. Due to the enormous amount of waste and its complicated physical properties, disaster waste treatment posed a number of problems. However, the construction industry and related parties worked on the task with a sense of mission, completing the treatment in Iwate and Miyagi prefectures by the end of March 2014, as specified by the national government, and achieving a recycling ratio of nearly 90% based on thorough recycling and reuse. Materials recycled through treatment were used for embankment and backfilling, etc. in public reconstruction projects.

Sixteen plant species were tested for their potential to remove radioactive cesium from contaminated soil with the Fukushima Daiichi nuclear disaster. There was a large difference of transfer factor (TF) of radiocesium (137Cs) from the soil to plants. Among the examined plants, Hollyhock belonging to Malvaceae showed the highest TF but the efficiency of phytoremediation, such as total 137Cs absorption of a plant or 137Cs removal from unit area, was not proportionally high. According to the evaluation criteria (uptake of 137Cs per plant and/or area), Kochia was shown to be the best plant species among 16 plants investigated here. Analysis of correlation showed that there was positive relationship between the total 137Cs absorption and plant biomass, and contribution rate of plant biomass in total 137Cs uptake by plant was scored to be 91.3%. These results indicate that selection of plant species that shows high biomass on site is important for 137Cs-targeted phytoremediation rather than the plants’ ability in 137Cs-uptake. On the other hand, our results also suggested that the TF value should be considered in order to evaluate the plant ability for 137Cs-targeted phytoremediation. From this point of view, the present study suggests that field-grown Kochia could be used as a potential candidate plant for phytoremediation of 137Cs from soil.

The improved catalytic activity of enzymes is required in various fields. Enzymes have conventionally been improved by the screening of bacteria possessing mutant enzymes. However, the screening conditions are limited since screening requires the growth of bacteria. Here, we report the development of a protein microarray for the analysis of enzymatic activity. A his-tagged enzyme is synthesized in situ and immobilized on the microarray, which is composed of microreactors with a diameter and depth of 4 μm and a density of 1.0 x 106 reactors/cm2. β-glucosidase, synthesized in situ using a cell-free synthesis system, was immobilized on the microreactor array chip and its catalytic activity was observed. This enzyme-immobilized microarray is expected to enable the rapid and quantitative screening of enzymes.
