The purpose of this study was to investigate, using path analysis, causal relationships among motion factors for achieving a high release velocity in the male discus throw. The throwing motions of 61 male discus throwers were analyzed using three-dimensional motion analysis. Variables such as release velocity, velocities gained by each body segment, body segment velocity, body angle and angular velocity were obtained. The path model indicating the causal relationships among these factors was constructed by path analysis. The main results were as follows: Influences of velocities gained by each body segment on release velocity were largest for the arm, followed in order by the trunk and legs. Motion factors such as weight shift, acquisition of the velocity of the center of gravity, sweeping the legs, rotations of the hip and shoulder, twisting and untwisting of the trunk, acquisition of the velocity of the right knee and extension of the left knee had direct or indirect influences on the velocities gained by each body segment. Motion factors in the path model revealed causal relationships along the time sequence of the throwing motion. In addition, the path model in this study indicated the cause and effect structure of the throwing motion by which Japanese male discus throwers were able to achieve a high release velocity. The results of this study can be utilized for technical coaching of the discus throw based on causal relationships.

We initiated physical therapy for a female patient with Alzheimer’s disease who suffered a left trochanteric fracture that caused a short and unstable left stance phase in gait. Initially, we performed muscle stretching and strengthening, and weight-bearing exercises in the sitting and standing positions, but the patient could not follow the therapy and did not achieve the target strength because of attention deficit due to Alzheimer’s disease. However, the patient was capable of independent activities of daily living (ADLs) such as dressing and grooming. Therefore, we initiated an exercise therapy which involved putting on and taking off a sock. After the therapy was started, the strength of the patient’s left gluteus maximus and external and internal oblique muscles improved and the instability in her gait stance decreased. This experience shows the necessity of selecting a therapy focused not only on the impairments but also on the patient’s abilities to perform ADL.

Modern day helicopters are frequently required to operate close to non-uniform ground surfaces. During operations above a moving ship deck, it can be foreseen that the hovering performance of the rotor has very different behavior compared with the cases of hovering above stationary flat ground plane. In this report, the hovering performance of the rotor above the moving ground in full ground effect is experimentally studied in order to provide qualified database for the validation of analytical study on dynamic ground effect. The moving ground is simulated by means of harmonic rolling and heaving motions of ground effect plate with variable amplitude and frequency. The thrust, torque and blade flap angle of the rotor were measured at rotor height from 0.4 to 1.5. It is shown that motion types of the ground and the amplitude and frequency of the moving ground have important effects on the variable loads in and out of the rotor disk in dynamic ground effect.

竹久夢二の絵画小説「岬」は、関東大震災の発生により一時連載が中断された後、震災情報が高い価値を持つ情報秩序のもとで再び新聞連載が継続される。その結果、休載期間の前後の言説に変質を抱え込み、「視線」と関わる表象が言説と視覚像の双方で増殖する。夢二の震災スケッチにも配慮しつつ、「岬」に頻出する視線の表象と罹災社会の視線の特性の関係を記述し、一度は断裂した物語の継続に作用した力を考察する。

100歳以上の大腿骨近位部骨折の術後成績を検討した.症例は2001~2017年に手術を施行した100歳以上の大腿骨近位部骨折11患者13例で,年齢は100~103歳,平均100.9歳,性別は男性2名,女性9名,骨折型は大腿骨頚部骨折4例,転子部骨折6例,転子下骨折2例,頚基部骨折1例であった.手術法は全例内固定術を行い,うち頚部骨折1例は偽関節のため再手術(人工骨頭置換術)を施行した.歩行再獲得率は63.6%(7/11例)で,周術期の合併症は術前肺炎1例,術後肺炎1例,術後肺炎+心不全1例であったが,全例が改善して自宅退院・施設入所・転院となった.術前評価にて手術が禁忌となるような既往症がなければ,100歳以上の患者でも積極的に手術を行うべきであると考える.

右頭頂後頭葉の脳梗塞で着衣障害を呈した80歳代男性に対し,徴候の要因を検討する目的で,開眼と閉眼の2条件で着衣動作の分析を行った.開眼条件では,はじめの袖通し工程で,袖に腕を通すことができない徴候を認め,他の工程は問題なかった.一方,閉眼条件では,徴候を認めず,着衣可能であった.その他の評価から,前開き型に限定していること,はじめの袖通し工程で出現すること,人形に着せた場合も同様の徴候を認めることが明らかとなった.加えて,視覚的同定の問題ではない構成行為の障害も認めた.以上より,触・圧覚情報が乏しいはじめの袖通し工程で,視覚情報からの処理に基づく構成行為の問題に起因した徴候と推察された.

Tobacco smoking is a worldwide public health problem. Recently, there has been growing interest in smoking cessation. On the other hand, oral mucosal ulcers are common presentations encountered in daily practice. These ulcers may be caused by various etiologies.We report a case of refractory oral mucosal ulcer caused by improper use of nicotine gum. A male in his 50s presented to our department with an intractable ulcer in the oral floor existing for over 3 months. He had smoked 20 cigarettes per day for 25 years. He had given up smoking and been using nicotine gum for 6 months. He used two pieces of nicotine gum (2mg tablet) per day, and the gum was improperly placed at the oral floor for 3 to 4 hours per piece. An excisional biopsy was performed and revealed a mucosal ulcer without atypical cells. He was instructed to stop using nicotine gum at the first visit to our department, and the ulcer healed completely in 4 weeks.

【目的】せん妄を呈した進行がん患者における苦悩の実態の検討.【方法】国内14施設の緩和ケア病棟に入院中または国内10施設の一般病棟に入院し精神腫瘍科が介入中の進行がん患者のうち,せん妄と診断され抗精神病薬の定期投与を受ける患者を前向きに連続サンプリングした.苦悩の有無を緩和ケア専門医が判断し,患者背景,DRS-R-98で評価したせん妄の重症度を比較した.【結果】対象患者818名のうち99名(12.1%)に苦悩を認めた.年齢,39歳以下,認知症の有無に有意差を認めた.治療前のDRS-R-98(15.3±8.1点 vs 17.3±7.8点,p<0.02)は苦悩を伴う群で有意に低く,情動の変容は有意に高かった.【考察】せん妄を呈した進行がん患者で苦悩を伴うものでは年齢が低く,認知症の併存が少なく,せん妄の重症度は低く,情動の変容が強いことが示された.

EIE-core type variable inductor is the equipment of variable inductance, which is controlled by dc current. This device controls the cancellation effect of AC magnetic flux. Accordingly, the device always keeps the flux density on the magnetic path within a saturation flux density on control-mode. And, the device cancels a high harmonic distortion, does not need the gaps, which reduce a distortion. Therefore, the device is simply structured, and is easily to scale up. This paper describes a basic structure, basic characteristics (exciting, distortion, ...etc.), and examples of application for the electric power equipment.

But, his apperans is by a design of KOJIKI that a energy of Izanaki's sword should cover the story of Kuniyuzuri.

in vivoおよびin vitro実験により,小豆ポリフェノール(APP)の抗酸化活性の能力を検討した.マウスに0.05%(w/v)APP入りの飲料(20ml/日)を一週間与えて,その血清,肝臓および腎臓ホモジネートの酸化促進剤に対する影響を検討したところ,どれもコントロールのそれらに比べて酸化を受けにくく,特に肝臓ホモジネートでは有意に酸化抵抗性を示した.また,0.05%(w/v)APPを1週間,事前投与したマウスにガラクトサミンとリポポリサッカライドを腹腔内注射したところ,コントロールのそれに比べて,血清GOT活性の上昇抑制や肝臓の過酸化脂質の生成が有意に抑制された.肝臓のグルタチオン量やGPx活性は,コントロールのそれらよりも有意に高い値を保持していた.これらの結果から,APPは炎症により発生するフリーラジカル・活性酸素を消去し,生体内グルタチオン量とGPx活性を高く保持して,過酸化脂質の生成を抑えることで,結果的に肝臓の炎症拡大を抑制している可能性が推察された.また,APPのDPPHラジカル消去活性におけるIC50は64.2μmol/l(カテキン量として換算)であり,市販のカテキンやビタミンCの1/2量で同じ効果を示した.また,0.05%APPを80μl添加した2.5mlのヒトLDL溶液(タンパク質70μg/ml)は,200μmol/lの硫酸銅溶液による酸化促進に対して抵抗性を示し,APP無添加の場合に比べて,LDL酸化の開始時間を1時間程度遅延させた.これらの結果は,APPには生体の酸化防止効果や肝臓保護作用があることを示唆している.また,小豆の主要なモノマー型ポリフェノールは,カテキン-7β-グルコシドであることを明らかにした.
